El treball presentat fa l'estudi i l'anàlisi per a la implementació d'un sistema de gestió integrat ERP (Enterprise Resource Planning) a l'empresa TEXFIL, centrat únicament en solucions empresarials estàndards de paquets integrats per a la planificació dels recursos de l'empresa. Tot i que al llarg del document es fa esment de diferents fabricants de sistemes ERP, nosaltres, a petició del nostre client, ens hem centrat bàsicament en el sistema SAP R/3.

BACKGROUND: The AO comprehensive pediatric longbone fracture classification system describes the localization and morphology of fractures, and considers severity in 3 categories: (1) simple, (2) wedge, and (3) complex. We evaluated the reliability and accuracy of surgeons in using this rating system. MATERIAL AND METHODS: In a first validation phase, 5 experienced pediatric (orthopedic) surgeons reviewed radiographs of 267 prospectively collected pediatric fractures (agreement study A). In a second study (B), 70 surgeons of various levels of experience in 15 clinics classified 275 fractures via internet. Simple fractures comprised about 90%, 99% and 100% of diaphyseal (D), metaphyseal (M), and epiphyseal (E) fractures, respectively. RESULTS: Kappa coefficients for severity coding in D fractures were 0.82 and 0.51 in studies A and B, respectively. The median accuracy of surgeons in classifying simple fractures was above 97% in both studies but was lower, 85% (46-100), for wedge or complex D fractures. INTERPRETATION: While reliability and accuracy estimates were satisfactory as a whole, the ratings of some individual surgeons were inadequate. Our findings suggest that the classification of fracture severity in children should be done in only two categories that distinguish between simple and wedge/complex fractures.

OBJECTIVES: Although endogenous nitric oxide (NO) is an excitatory mediator in the central nervous system, inhaled NO is not considered to cause neurologic side effects because of its short half-life. This study was motivated by a recent case report about neurologic symptoms and our own observation of severe electroencephalogram (EEG) abnormalities during NO inhalation. DESIGN: Blind, retrospective analyses of EEGs which were registered before, during, and after NO inhalation. EEG was classified in a 5-point rating system by an independent electroencephalographer who was blinded to the patients' clinical histories. Comparisons were made with the previous evaluation documented at recording. Other EEG-influencing parameters such as oxygen saturation, hemodynamics, electrolytes, and pH were evaluated. SETTING: Pediatric intensive care unit of a tertiary care university children's hospital. PATIENTS: Eleven ventilated, long-term paralyzed, sedated children (1 mo to 14 yrs) who had EEG or clinical assessment before NO treatment and EEG during NO inhalation. They were divided into two groups according to the NO-indication (e.g., congenital heart defect, acute respiratory distress syndrome). MEASUREMENTS AND MAIN RESULTS: All 11 patients had an abnormal EEG during NO inhalation. EEG-controls without NO showed remarkable improvement. EEG abnormalities were background slowing, low voltage, suppression burst (n = 2), and sharp waves (n = 2) independent of patients' age, NO-indication, and other EEG-influencing parameters. CONCLUSIONS: These preliminary data suggest the occurrence of EEG-abnormalities after application of inhaled NO in critically ill children. We found no correlation with other potential EEG-influencing parameters, although clinical state, medication, or hypoxemia might contribute. Comprehensive, prospective, clinical assessment regarding a causal relationship between NO-inhalation and EEG-abnormalities and their clinical importance is needed.

INTRODUCTION: Ankle arthrodesis in an intervention that may be performed using a variety of surgical approaches. The joint may be stabilized using diferent systems, with or without bone graft. MATERIAL AND METHODS: Retrospective study that collected the results of 40 patients who underwent ankle arthrodesis; the complications that arose were analyzed, results according to the AOFAS clinical rating system, global patient satisfaction and the relationship with the articular approach route, the !xation system or the usage of grafts.RESULTS: 55% of the patients presented one or more complication that motivated a total of 12 re-interventions in 10 patients (excludes removal of osteosynthesis material). The absence of consolidation, which required a new arthrodesis was more frequent in patients where osteosynthesis was not used (two out of three cases) than in those using screws (four out of 24 cases), while there was no register of this in 13 patients in which intramedullary pin. The use of the distal extreme of the fibula or the iliac crest as bone graft has failed to improve the consolidation. Arthroscopic approaches avoid the appearance of soft tissue complications, although the consolidation disorders were more frequent. The average score on the AOFAS clinical rating system was 56.4±21.2 and 55% of the patients were satisfied. CONCLUSIONS: Although the case histories fail to provide significant results, it is a demanding intervention, with an elevated complication rate. Intramedullary pins seem to favor improved consolidation.

This study examined the validity and reliability of the French version of two observer-rated measures developed to assess cognitive errors (cognitive errors rating system [CERS]) [6] and coping action patterns (coping action patterns rating system [CAPRS]) [22,24]. The CE measures 14 cognitive errors, broken down according to their valence positive or negative (see the definitions by A.T. Beck), and the CAP measures 12 coping categories, based on an comprehensive review literature, each broken down into three levels of action (affective, behavioural, cognitive). Thirty (N = 30) subjects recruited in a community sample participated in the study. They were interviewed according to a standardized clinical protocol: these interviews were transcribed and analysed with both observer-rated systems. Results showed that the inter-rater reliability of the two measures is good and that their internal validity is satisfactory, due to a non-significant canonical correlation between CAP and CE. With regard to discriminant validity, we found a non-significant canonical correlation between CAPRS and CISS, one of most widely used self-report questionnaire measuring coping. The same can be said for the correlation with a self-report questionnaire measuring symptoms (SCL-90-R). These results confirm the absence of confounds in the assessment of cognitive errors and of coping as assessed by these observer-rated scales and add an argument in favour of the French validation of the CE-CAP rating scales. We carried out a retrospective review of 155 patients with lumbar spinal stenosis who had been treated surgically and followed up regularly: 77 were evaluated at a mean of 6.5 years (5 to 8) after surgery by two independent observers. The outcome was assessed using the scoring system of Roland and Morris, and the rating system of Prolo, Oklund and Butcher. Instability was determined according to the criteria described by White and Panjabi. A significant decrease in low back pain and disability was seen. An excellent or good outcome was noted in 79% of patients; 9% showed secondary radiological instability. Surgical decompression is a safe and efficient procedure. In the absence of preoperative radiological evidence of instability, fusion is not required.

This study reviewed the subjective, clinical and radiological outcome of 71 patients (84 feet) treated by scarf osteotomy for hallux valgus deformity at our institution from 1995 to 1998 with an average follow-up time of 22 months (range, 17 to 48 months). At the time of follow-up, 39% of the patients were very satisfied, 50% were satisfied and 11% were not satisfied. The mean AOFAS score raised significantly from 43 points (14-68) preoperatively to 82 points (39 to 100) at follow-up (p < 0.001). The radiological angles including M1-M2, M1-P1, M1-M5 and DMAA improved significantly (p < 0.001). Among the 16 complications recorded, seven (8%) were minor and nine (11%) required an additional procedure. The scarf osteotomy of the first metatarsal coupled with a lateral soft-tissue release and, in three-quarters of our cases, with a basal closing wedge varisation osteotomy of the first phalanx, resulted in overall high satisfaction rate as well as significant clinical and radiological improvements in our series. Nevertheless, the range of motion of the first MP joint remained low: 30 degrees to 74 degrees in 52 patients (62%) and <30 degrees in four patients (5%). Furthermore, the mobility of the first ray as well as the consequences of the procedure in the sagittal plane need to be assessed more accurately, and this may be achieved by incorporating measurement of the plantar pressures in the forefoot area into the global rating system.

A large percentage of bridges in the state of Iowa are classified as structurally or fiinctionally deficient. These bridges annually compete for a share of Iowa's limited transportation budget. To avoid an increase in the number of deficient bridges, the state of Iowa decided to implement a comprehensive Bridge Management System (BMS) and selected the Pontis BMS software as a bridge management tool. This program will be used to provide a selection of maintenance, repair, and replacement strategies for the bridge networks to achieve an efficient and possibly optimal allocation of resources. The Pontis BMS software uses a new rating system to evaluate extensive and detailed inspection data gathered for all bridge elements. To manually collect these data would be a highly time-consuming job. The objective of this work was to develop an automated-computerized methodology for an integrated data base that includes the rating conditions as defined in the Pontis program. Several of the available techniques that can be used to capture inspection data were reviewed, and the most suitable method was selected. To accomplish the objectives of this work, two userfriendly programs were developed. One program is used in the field to collect inspection data following a step-by-step procedure without the need to refer to the Pontis user's manuals. The other program is used in the office to read the inspection data and prepare input files for the Pontis BMS software. These two programs require users to have very limited knowledge of computers. On-line help screens as well as options for preparing, viewing, and printing inspection reports are also available. The developed data collection software will improve and expedite the process of conducting bridge inspections and preparing the required input files for the Pontis program. In addition, it will eliminate the need for large storage areas and will simplify retrieval of inspection data. Furthermore, the approach developed herein will facilitate transferring these captured data electronically between offices within the Iowa DOT and across the state.

The work of the Department of Natural Resources impacts the lives of all Iowans. Iowans deserve a clean environment and quality natural areas for public use and enjoyment. This report reflects the progress made during fiscal year 2013 (FY13) toward our goals and provides information regarding the condition of our state’s natural resources and the effectiveness of our programs. In FY13, we continued to improve collaboration with other executive branch agencies. The DNR and DOT work very closely on the issuance of permits needed for road and bridge constructions, but recently we have also been working together to meet the administrative needs of the agencies. The DNR is working closely with the DOT to adopt an Electronic Records Management System used by the DOT. This system will improve accessibility to public documents and reduce the amount of paper files retained in storage. The DNR also continues to improve collaboration with other agencies, such as the Iowa Economic Development Authority as we work closely with them on business development in the state. The DNR strives to continually improve our customer service and how we can meet Iowan’s needs. As an example, the online reservation system for campground reservations has grown over the past eight years so that now 88% of the camping reservations are made online. The DNR continues to improve our online presence and accessibility. In FY13 the Iowa Legislature approved paying off the State’s bond debt used to construct Honey Creek Resort State Park. By removing this debt, the DNR will be able to focus more on the future of the Resort, rather than the past debt. Finally, in August of 2012, the DNR was faced with a tragic accident, where a seasonal parks employee died after rolling a mower into a lake. This incident has caused us to establish a Safety Program at the DNR and to review all of our departmental safety trainings, programs, and equipment. By focusing on our employee’s safety and well being, it is another way that we can demonstrate that at the DNR, our employees are our greatest asset.

The work described in this report documents the activities performed for the evaluation, development, and enhancement of the Iowa Department of Transportation (DOT) pavement condition information as part of their pavement management system operation. The study covers all of the Iowa DOT’s interstate and primary National Highway System (NHS) and non-NHS system. A new pavement condition rating system that provides a consistent, unified approach in rating pavements in Iowa is being proposed. The proposed 100-scale system is based on five individual indices derived from specific distress data and pavement properties, and an overall pavement condition index, PCI-2, that combines individual indices using weighting factors. The different indices cover cracking, ride, rutting, faulting, and friction. The Cracking Index is formed by combining cracking data (transverse, longitudinal, wheel-path, and alligator cracking indices). Ride, rutting, and faulting indices utilize the International Roughness Index (IRI), rut depth, and fault height, respectively.

Tämän työn tarkoituksena on käytännöllisen suositusjärjestelmäratkaisun kehittäminen verkkokauppaympäristöön olemassaolevaa teoriatietoa käyttäen. Työn ensimmäisessä osiossa tarkastellaan ensin tapoja lähdetiedon keräämiseksi järjestelmää varten. Tämän jälkeen käydään läpi eri menetelmiä suosituksen toteuttamiseksi. Lisäksi tutustutaan yleisiin ongelmiin eri menetelmien kanssa. Seuraavaksi tutkitaan miten järjestelmän käyttämään suositustietoa voidaan ryhmitellä. Tämänjälkeen arvioidaan esitettyjä menetelmiä yleisesti tunnettujen kriteerien perusteella. Suositusjärjestelmän toteutustyö on kuvattuna työn toisessa osiossa. Toteutettu ohjelmisto on asennettu kahteen erilliseen toimintaympäristöön.

Tässä diplomityössä tarkastellaan sähköisen liiketoiminnan mukanaan tuomia mahdollisuuksia yrityksen hankintatoimen prosessien tehostamiseksi. Työ keskittyy erityisesti raskaassa konepajateollisuudessa toimivan yrityksen, Metso Paperin, hankinnan tietojärjestelmien integroimiseen. Pääasiallinen tutkimusmenetelmä on kirjallisuustutkimus, jota lisäksi täydennetään empiirisesti tutkittujen case-tapauksien avulla, joissa tarkastellaan sähköisen hankintatoimen käytännön toteutusta Metso Paperin ja sen alihankkijan, Toijala Worksin välillä. Diplomityön tavoitteena on esittää yrityksen hankintaverkoston ja siihen kuuluvien alihankkijoiden toiminnan uudelleen organisoiminen ja hallitseminen, minkä lisäksi työssä käsitellään myös hankintastrategian merkitystä yrityksen liiketoiminnan kannalta. Työn tietotekninen osuus keskittyy sähköisen liiketoiminnan edellyttämän tietojärjestelmän suunnittelemiseen, mutta ei sen käytännön toteuttamiseen. Työn tärkein tulos on sähköisenliiketoiminnan aloittamisen mahdollistava kaksivaiheinen tiedonsiirtomalli, jonka ensimmäinen vaihe perustuu jo olemassa olevien tietojärjestelmien, eli Extranet- ja Notes -työpöytien, hyödyntämiseen uudella, aiempaa tehokkaammalla tavalla. Mallin toinen vaihe jakautuu yrityksen sisäisen ja ulkoisen tiedonsiirtoon. Toisen vaiheen keskeinen tulos on ratkaisuehdotus sisäisten ja ulkoisten tietojärjestelmien toimintojen ja tietojen saumattomaksi integroimiseksi toisiinsa, eli tutkimuskohteena olevan yrityksen hankintaverkostoon ja sen erityispiirteisiin sopiva sähköisen kaupankäynnin sovellutus.

Tämän projektin tarkoituksena oli kehittää paperinvalmistuslinjan multimediapohjaista tietotukijärjestelmää. Kirjallisuusosassa on todettu, että nykyisessä työympäristössä tarvitaan entistä enemmän tietotaitoa työkyvyn ylläpitämiseksi. Työntekijöiden osaaminen on avainasemassa kilpailukyvyn säilyttämisessä. Esille nousee myös suurten ikäluokkien eläköityminen ja osaavan työvoiman riittäminen. Näistä syistä erilaiset koulutukset ovat tällä hetkellä erityisen tärkeitä. Verkko-opetusvälineet ovat taloudellisesti kannattavia erityisesti suurille kohderyhmille ja sopivat hyvin paperiteollisuuden käyttöön. Parhaimmillaan ne ovat myös hyvin tehokkaita opetusvälineitä. Kokeellisessa osan alussa nähtiin tärkeäksi järjestelmän käytettävyyden ja kiinnostavuuden parantaminen ja siksi siihen päätettiin tehdä kokonaan uusi käyttöliittymä ja selkeämpi rakenne. Järjestelmän sisältö rakennettiin uudelleen hyväksikäyttäen vanhaa materiaalia soveltuvin osin. Järjestelmään laadittiin perussisältösivujen lisäksi useita johdantoja, joista saa nopeasti käsityksen tietystä prosessin osasta. Järjestelmään lisättiin myös useita multimediaelementtejä, kuten uusia kuvia, animaatioita ja videoita. Järjestelmään lisättiin myös hakutoiminto, sanasto ja käyttöohje. Näillä uudistuksilla pyrittiin parantamaan järjestelmän käytettävyyttä erityisesti prosessityöntekijöiden perehdytyksen ja työnopastuksen apuvälineenä.
