As we face a difficult economic climate, in which inequalities may worsen, the PHA faces many challenges in its efforts to improve the health of the population. One such challenge is the issue of obesity. Recently, in the Draft Programme for Government and, again today, in anticipation of the publication of the Consultation on the Review of Health and Social Care Services in Northern Ireland, the specific issue of obesity has been highlighted in the media.The PHA is committed to playing a lead role in tackling this major health issue and has been systematically examining the evidence of best practice and effectiveness to ensure that investment and working in partnership will bring clear benefits. A welcome consequence of any success would be a reduction in the impact of the physical, and emotional costs of obesity related ill-health to individuals - and the financial costs to an overstretched healthcare system.A multi-facetted approach to tackling obesity is required for Northern Ireland. This will mean working across government departments, looking at relevant legislation, taxation, food standards and labelling, as well as supporting a raft of programmes within education, workplace, and at the local community level."The prevalence of overweight and obesity has risen dramatically in recent years in Northern Ireland and is now the norm to be overweight, rather than the exception. The Northern Ireland Health and Social Wellbeing Survey (2010-11) indicated that 36% of adults are overweight and a further 23% are obese; this means that approximately 3 in 5 adults in Northern Ireland carry excess weight. A similar proportion of males and females were obese (23%) however males were more likely to be overweight (44%) than females (30%).Data from the Northern Ireland Health and Wellbeing Survey (2010-11) reported that 27% of children aged 2-15 years are obese or overweight. The findings presented here are based on the guidelines put forward by the International Obesity Task Force. Using this approach, 8% of children were assessed as obese, with similar results for boys (8%) and girls (9%). Obesity has serious implications for health and wellbeing and is associated with an increased risk of heart disease and stroke, type 2 diabetes, some cancers, respiratory problems and joint pain.Evidence indicates that being obese can reduce life expectancy by up to 9 years; and it can impact on emotional and psychological well-being and self-esteem, especially among young people.Obesity also impacts on wider society through economic costs, loss of productivity and increased demands on our health and social care system. It is estimated that obesity in Northern Ireland is resulting in 260,000 working days lost each year with a cost to the local economy of £500 million.The good news is that the intentional loss of significant weight (approx 10kg) in overweight and obese adults has been shown to confer significant health benefits, decreased morbidity and may also reduce obesity-related mortality.Key programmes and interventions are undertaken by the PHA in order to prevent and reduce overweight and obesity. The programmes/interventions are supported by significant ongoing work at local level. Examples include:the promotion of breastfeeding; local programmes to increase awareness of good nutrition and develop cooking skills, for example 'Cook It!'; promotion of more active lifestyles, for example, Walking for Health' and 'Teenage Kicks'; development of community allotment schemes; programmes for primary school children, for example Skip2bfit and Eat, Taste and Grow; and sports and other recreation, for example 'Active Belfast'. The PHA's multi media campaign 'It all adds up!' to encourage children to become more active and understand the importance of keeping fit, in a fun and exciting way, ran until October 2011. It encouraged parents and carers to go to the website www.getalifegetactive.com and download the PHA logbook It all adds up! to plan activities as a family. The logbook helped children and parents plan and keep track of their participation in physical activity at school, home and in the community. PHA is currently developing a public information campaign and other supportive work to increase public awareness of obesity as well as to provide advice and support for those who want to make real changes. The campaign development is well underway and is anticipated for launch in late Spring 2012. Like many common health problems, people living in disadvantaged circumstances suffer most and the PHA is committed to tackling this aspect of health inequality. The good news is that even a modest weight loss, of 1-1 Â_ stones, can help to reduce the risk of many of the health problems resulting from being overweight or obese. Information on losing weight through healthier eating and being more active can be found on the PHA websites - www.enjoyhealthyeating.info and www.getalifegetactive.com . These websites provide help and advice for anyone who wants to improve their eating habits and fitness levels, by making small, sustainable, healthy changes to their lifestyle. 1 Insect pests, biological invasions and climate change are considered to representmajor threats to biodiversity, ecosystem functioning, agriculture and forestry.Deriving hypothesis of contemporary and/or future potential distributions of insectpests and invasive species is becoming an important tool for predicting the spatialstructure of potential threats.2 The western corn rootworm (WCR) Diabrotica virgifera virgifera LeConte is apest of maize in North America that has invaded Europe in recent years, resultingin economic costs in terms of maize yields in both continents. The present studyaimed to estimate the dynamics of potential areas of invasion by the WCR under aclimate change scenario in the Northern Hemisphere. The areas at risk under thisscenario were assessed by comparing, using complementary approaches, the spatialprojections of current and future areas of climatic favourability of the WCR. Spatialhypothesis were generated with respect to the presence records in the native rangeof the WCR and physiological thresholds from previous empirical studies.3 We used a previously developed protocol specifically designed to estimatethe climatic favourability of the WCR. We selected the most biologicallyrelevant climatic predictors and then used multidimensional envelope (MDE) andMahalanobis distances (MD) approaches to derive potential distributions for currentand future climatic conditions.4 The results obtained showed a northward advancement of the upper physiologicallimit as a result of climate change, which might increase the strength of outbreaksat higher latitudes. In addition, both MDE and MD outputs predict the stability ofclimatic favourability for the WCR in the core of the already invaded area in Europe,which suggests that this zone would continue to experience damage from this pestin Europe.

Few subjects have caught the attention of the entire world as much as those dealing with natural hazards. The first decade of this new millennium provides a litany of tragic examples of various hazards that turned into disasters affecting millions of individuals around the globe. The human losses (some 225,000 people) associated with the 2004 Indian Ocean earthquake and tsunami, the economic costs (approximately 200 billion USD) of the 2011 Tohoku Japan earthquake, tsunami and reactor event, and the collective social impacts of human tragedies experienced during Hurricane Katrina in 2005 all provide repetitive reminders that we humans are temporary guests occupying a very active and angry planet. Any examples may have been cited here to stress the point that natural events on Earth may, and often do, lead to disasters and catastrophes when humans place themselves into situations of high risk. Few subjects share the true interdisciplinary dependency that characterizes the field of natural hazards. From geology and geophysics to engineering and emergency response to social psychology and economics, the study of natural hazards draws input from an impressive suite of unique and previously independent specializations. Natural hazards provide a common platform to reduce disciplinary boundaries and facilitate a beneficial synergy in the provision of timely and useful information and action on this critical subject matter. As social norms change regarding the concept of acceptable risk and human migration leads to an explosion in the number of megacities, coastal over-crowding and unmanaged habitation in precarious environments such as mountainous slopes, the vulnerability of people and their susceptibility to natural hazards increases dramatically. Coupled with the concerns of changing climates, escalating recovery costs, a growing divergence between more developed and less developed countries, the subject of natural hazards remains on the forefront of issues that affect all people, nations, and environments all the time.This treatise provides a compendium of critical, timely and very detailed information and essential facts regarding the basic attributes of natural hazards and concomitant disasters. The Encyclopedia of Natural Hazards effectively captures and integrates contributions from an international portfolio of almost 300 specialists whose range of expertise addresses over 330 topics pertinent to the field of natural hazards. The article analyses the functioning of the different systems employed for compensating authors for the right of public lending in libraries. Following a historical review, the text explores the principal aspects covered by standards: libraries included, works affected, methods for calculating the compensations, economic costs of the systems and beneficiaries. The main conclusions make reference to the diversity of legislative approaches in different countries and to the probable increase in the number of countries that will be applying this type of norm in the coming years

Abstract The giant hogweed (Heracleum mantegazzianum) has successfully invaded 19 European countries as well as parts of North America. It has become a problematic species due to its ability to displace native flora and to cause public health hazards. Applying population genetics to species invasion can help reconstruct invasion history and may promote more efficient management practice. We thus analysed levels of genetic variation and population genetic structure of H. mantegazzianum in an invaded area of the western Swiss Alps as well as in its native range (the Caucasus), using eight nuclear microsatellite loci together with plastid DNA markers and sequences. On both nuclear and plastid genomes, native populations exhibited significantly higher levels of genetic diversity compared to invasive populations, confirming an important founder event during the invasion process. Invasive populations were also significantly more differentiated than native populations. Bayesian clustering analysis identified five clusters in the native range that corresponded to geographically and ecologically separated groups. In the invaded range, 10 clusters occurred. Unlike native populations, invasive clusters were characterized by a mosaic pattern in the landscape, possibly caused by anthropogenic dispersal of the species via roads and direct collection for ornamental purposes. Lastly, our analyses revealed four main divergent groups in the western Swiss Alps, likely as a consequence of multiple independent establishments of H. mantegazzianum.

The goal of this research was to recommend specific changes relating to service by intercity buses and third level air carriers to propose an appropriate state role in the implementation of changes. Changes contemplated in this research were to be directed to the increased use of intercity buses and third level air carriers in such manner as to exert a benefit to user as well as the general public. Specific objectives of the project included: (1) to analyze the potential for a system of express intercity bus routes in Iowa; (2) to estimate the demand for third level air carrier services in cities having populations under 50,000; (3) to forecast the relationship between economic costs and benefits from an intercity bus system emphasizing express routes between major population centers and supporting a subsystem of local and intraregional public transportation; (4) to estimate the economic feasibility of expanded third level air carrier service with emphasis upon those routes proposed as worth of further evaluation; and (5) to provide guidance for establishing the need for state and local subsidies to institute a system of express intercity buses and to expand third level air carrier services. In addition to passenger movements, enhancing package freight shipments was also to be considered in the evaluation of proposed improvements.

L'entrada en vigor de la Directiva 1999/31/CE limita l'entrada de residus biodegradables a l'abocador i promou la gestió d'aquests residus per altres vies. El compostatge és una alternativa a la deposició en abocador que respecta el cicle natural de la matèria. A Catalunya, aquesta tècnica s'ha vist potenciada pels plans de gestió de residus PRECAT201 i PINFRECAT202. L'objecte d'aquest estudi és avaluar l'eficiència de la recollida selectiva al municipi de l'Esquirol; i comparar i avaluar diferents alternatives de gestió de la FORM3 dins i fora del municipi. La metodologia de l'estudi ha consistit en analitzar les dades de la recollida selectiva de l'ARC4, tant del municipi de l'Esquirol com de la resta de municipis del Collsacabra, i calcular els costos econòmics de la gestió de la FORM en el supòsit de tractament mitjançant l'autocompostatge, una planta de compostatge a petita escala o el tractament en una planta de compostatge convencional. Els resultats de l'estudi indiquen que la recollida selectiva del municipi de l'Esquirol obté uns valors per sobre dels objectius del PROGREMIC5, amb l'excepció de la FORM. Per tal de millorar la qualitat i la quantitat de la FORM recollida, l'estudi proposa en una primera fase: implantar el sistema de recollida selectiva porta a porta d'aquesta fracció i de la fracció resta; i en una segona fase: tractar la FORM en una petita planta de compostatge amb el sistema de sitges amb ventilació forçada.

L’ostéoarthrose (OA) est une pathologie qui touche les articulations principalement chez les personnes âgées. Il devient capital de mieux cerner cette pathologie à cause des coûts économiques qu’elle engendre mais surtout à cause du vieillissement de la population. Cette maladie se caractérise par une dégradation du cartilage articulaire, une sclérose osseuse, une inflammation de la membrane synoviale ainsi que la présence d’ostéophytes. L’étiologie de cette pathologie est restée nébuleuse car la recherche sur la maladie touchait principalement le cartilage articulaire. Toutefois, le rôle clé de l’os sous-chondral dans l’OA est maintenant reconnu. L’obésité étant un facteur de risque de l’OA, nous avons émis l’hypothèse que la leptine, une adipocytokine clé dans l’obésité, joue un rôle important dans l’OA. En effet, la leptine modifie le phénotype des ostéoblastes (Ob) normaux humain et puisque les Ob OA humains ont un phénotype altéré, notre objectif était de déterminer le rôle potentiel de la leptine dans ces cellules. Pour ce faire, nous avons préparé des cultures primaires d’Ob issus de la plaque sous-chondral du plateau tibial de patients OA et d’individus normaux (N). L’expression de la leptine et de son récepteur actif (OB-Rb) ont été mesurées par RT-PCR en temps réel, et leur production a été mesurée par ELISA et immunobuvardage (IB). La prolifération des Ob OA a été déterminée par incorporation de BrdU. La phosphorylation de p42/44 MAPK dans les Ob OA a été déterminée par IB. Le phénotype des Ob fut déterminé par la mesure de l’activité de la phosphatase alcaline (ALP) et la sécrétion d’ostéocalcine (OC), en présence ou non de leptine. De plus, les effets des ARNs d’interférences (SiRNA) anti-leptine et anti OB-Rb sur le phénotype des Ob OA furent déterminés via leur impact sur l’activité de l’ALP et sur la sécrétion d’OC. L’effet dose-réponse de la leptine sur les expressions d’OB-Rb, du facteur de croissance TGF-1 ou encore sur sa propre expression furent déterminées par RT-PCR en temps réel. Pour terminer, la signalisation de la leptine a été étudiée en évaluant l’effet dose réponse de celle-ci sur la production des protéines JAK2 et STAT3 phosphorylées par IB. Les résultats obtenus ont montrés que les Ob OA expriment et produisent plus de leptine que les Ob N. Au niveau phénotypique, ces Ob OA possèdent une activité de l’ALP ainsi qu’une sécrétion d’OC plus importante que celles observées chez les Ob N. L’ajout d’anticorps inactivant l’interaction leptine et OB-Rb ou d’inhibiteurs chimiques comme tyrphostin ou piceatannol diminuèrent l’activité de l’ALP ainsi que la sécrétion d’OC dans les Ob OA. Par contre, l’ajout de leptine exogène aux Ob OA augmenta l’activité de l’ALP sans pour autant faire varier la sécrétion d’OC. La leptine à des doses de 1ng/ml à 10mg/ml stimula la prolifération des Ob OA ainsi que la phosphorylation de p42/44 MAPK. La leptine exogène diminua l’expression de TFG-1 tandis qu’elle stimula la phosphorylation de JAK2 et STAT3 ou encore sa propre expression de manière dose-dépendante. Cependant, l’expression d’OB-Rb diminua de manière dose-dépendante. Enfin, le traitement des Ob OA avec des Si leptine ou Si OB-Rb diminua l’activité d’ALP, la sécrétion d’OC, l’expression de la leptine, l’expression d’OB-RB ainsi que l’expression du facteur TGF-1. L’ensemble de ces données démontre que la leptine endogène des Ob OA est sous contrôle des facteurs de croissance et qu’elle contribue à maintenir le phénotype anormal de l’os sous-chondral OA. De plus, ceci suggère que la leptine serait un acteur important dans la régulation du remodelage osseux.

Existing fuel taxes play a major role in determining the welfare effects of exempting the transportation sector from measures to control greenhouse gases. To study this phenomenon we modify the MIT Emissions Prediction and Policy Analysis (EPPA) model to disaggregate the household transportation sector. This improvement requires an extension of the GTAP data set that underlies the model. The revised and extended facility is then used to compare economic costs of cap-and-trade systems differentiated by sector, focusing on two regions: the USA where the fuel taxes are low, and Europe where the fuel taxes are high. We find that the interplay between carbon policies and pre-existing taxes leads to different results in these regions: in the USA exemption of transport from such a system would increase the welfare cost of achieving a national emissions target, while in Europe such exemptions will correct pre-existing distortions and reduce the cost.

La migraña es el síndrome de cefalea primaria mejor conocido. Es una cefalea incapacitante de curso fluctuante. Con importante impacto en la calidad de vida de los pacientes y de altos costos económicos para estos ya que se presenta generalmente en los años más productivos de la persona, así como altos costos para el sistema. La meta es la supresión de las crisis por lo cual una de las piedras angulares del tratamiento es la profilaxis de los ataques. Para esto se han estudiado múltiples medicamentos, en los últimos años la toxina botulínica ha despertado entusiasmo. El objetivo de este trabajo era, mediante una revisión sistemática de la literatura, establecer si la toxina botulínica es un medicamento eficaz en la prevención de las crisis migrañosas.

En la Enfermedad Coronaria (EC) existen factores genéticos, socioculturales, medioambientales y raciales adicionales a los factores de riesgo cardiovascular mayores que podrían influir en su presentación. Se desconoce el impacto de la raza en la severidad de la enfermedad coronaria en los pacientes extranjeros que son enviados a nuestro Servicio. Objetivos: Comparar la severidad de la EC multivaso en una población de pacientes de las Antillas y Nacionales, pareados por la escala Framingham. Metodología: Realizamos un estudio de corte transversal, comparando pacientes colombianos contra pacientes provenientes de las Antillas holandesas con similares factores de riesgo según escala de Framingham, catalogándolos por grupos de riesgo bajo, intermedio, alto y muy alto. Todos con EC severa multivaso documentada por angiografía coronaria desde enero del 2009 hasta Junio de 2011. Se excluyeron pacientes con antecedentes de intervención percutánea o quirúrgica previa. Resultados: Ingresaron 115 pacientes internacionales y 115 pacientes nacionales. La relación hombres/mujeres 3:1. La proporción de grupos de riesgo fue de bajo riesgo 2.5%, intermedio 15%, alto 19.3%, y muy alto 63.4%. El Syntax Score en pacientes nacionales fue 14.3+/-7.4 y en internacionales 22.2+/-10.5 p: 0.002. Conclusiones: En pacientes provenientes de las Antillas Holandesas, valorados en nuestra institución, se observó una mayor severidad de la enfermedad coronaria comparada con una población nacional con factores de riesgo similares. Estos hallazgos sugieren la influencia de la raza y factores genéticos en la severidad y extensión de la EC

Introducción: El aumento de la resistencia bacteriana, el uso inadecuado de antibióticos y las formulaciones empíricas, en infecciones urinarias, obligan a establecer las características epidemiológicas y de resistencia de nuestro medio. Método: Se revisaron todos los urocultivos positivos (RUFC mayor a 100000) solicitados en JAVESALUD entre junio/2011 y marzo/2012 y las historias clínicas correspondientes, con el objeto de realizar análisis descriptivo de variables demográficas, microorganismos aislados y resistencia bacteriana. Posteriormente se identificaron los factores de riesgo que favorecen aparición de multirresistencia en los pacientes mediante una regresión logística binaria. Resultados: Se obtuvieron 204 urocultivos, correspondientes a 120 pacientes. El 87% fueron mujeres (edad promedio 58,9 años). La bacteria más aislada: E. coli (64%). La resistencia antibiótica fue: ampicilina 57,39%, ciprofloxacina 28,9%, nitrofurantoína 9,71% y TMP/SMX 32,47%. La multirresistencia (24,3%) muestra asociación con el antecedente de múltiples tratamientos recibidos (p 0.015) y las infecciones urinarias a repetición (p 0.005). Discusión: La distribución por géneros y la resistencia son similares a lo reportados en la literatura, sin embargo, la frecuencia de infecciones por E. coli resulta menor a lo reportado. Los altos niveles de multirresistencia se encuentran relacionados con el tipo de pacientes manejados en la institución. Los manejos empíricos, con nitrofurantoína se deben limitar a los pacientes que cumplan a cabalidad con los criterios diagnósticos de infección urinaria simple. El urocultivo es fundamental en el manejo de pacientes con infecciones a repetición que hayan recibido múltiples tratamientos y que consulten de manera repetida al servicio de medicina general.
