Following early results of recent studies of intra-operative radiotherapy (IORT) in the adjuvant treatment of patients with early breast cancer, the clinical utility of IORT is a subject of much recent debate within the breast oncology community. This review describes the intra-operative techniques available, the potential indications, and the evidence to date pertaining to local control and toxicity. We also discuss any implications for current practice and future research.

Context. The magnetic activity of planet-hosting stars is an importantfactor for estimating the atmospheric stability of close-in exoplanetsand the age of their host stars. It has long been speculated thatclose-in exoplanets can influence the stellar activity level. However,testing for tidal or magnetic interaction effects in samples ofplanet-hosting stars is difficult because stellar activity hindersexoplanet detection, so that stellar samples with detected exoplanetsshow a bias toward low activity for small exoplanets.

Aims: Weaim to test whether exoplanets in close orbits influence the stellarrotation and magnetic activity of their host stars.

Methods: Wedeveloped a novel approach to test for systematic activity-enhancementsin planet-hosting stars. We use wide (several 100 AU) binary systems inwhich one of the stellar components is known to have an exoplanet, whilethe second stellar component does not have a detected planet andtherefore acts as a negative control. We use the stellar coronal X-rayemission as an observational proxy for magnetic activity and analyzeobservations performed with Chandra and XMM-Newton.

Results: Wefind that in two systems for which strong tidal interaction can beexpected the planet-hosting primary displays a much higher magneticactivity level than the planet-free secondary. In three systems forwhich weaker tidal interaction can be expected the activity levels ofthe two stellar components agree with each other.

Conclusions:Our observations indicate that the presence of Hot Jupiters may inhibitthe spin-down of host stars with thick outer convective layers. Possiblecauses for this effect include a transfer of angular momentum from theplanetary orbit to the stellar rotation through tidal interaction, ordifferences during the early evolution of the system, where the hoststar may decouple from the protoplanetary disk early because of a gapopened by the forming Hot Jupiter.

This article presents a critical review of the literature about the potential benefit of cardiac pacing in patients suffering from vasovagal or neurocardiogenic syncope. The manifestation of vasovagal syncope comprises some reflex bradycardia and vasoplegia resulting in cerebral hypoperfusion that ultimately leads to a loss of consciousness. The literature reports conflicting results of the potential benefit of cardiac pacing on the prevention of recurrence of vasovagal events. A detailed analysis of the inclusion criteria of these studies permits to clarify the discrepancy. Only patients older than 50 years with prolonged sinus pause at time of syncope benefit of the implantation of a cardiac pacemaker.

Depuis 2007, le vaccin vivant atténué contre le zona dispose d'une autorisation de mise sur le marché au niveau européen. Fin 2015-début 2016, il devrait enfin être disponible en France. Cet article répond à vos interrogations concernant son intérêt, son efficacité, sa tolérance et ses bénéfices pour la société, dans la prévention du zona et d'une de ses complications les plus fréquentes et la plus invalidante : les douleurs post-zostériennes.

Reléguée au rang de moyen diagnostique occasionnel avec l'avènement de la streptomycine dès 1945, la thoracoscopie a repris depuis trois à cinq ans un rôle prépondérant dans l'armement du chirurgien thoracique, comme moyen diagnostique et surtout comme outil thérapeutique. Dans cet article, nous faisons le point quant à la place actuelle de la thoracoscopie face aux différentes pathologies thoraciques, pulmonaires, médiastinales et oesophagiennes. Dans certaines indications (pneumothorax, infiltrats pulmonaires diffus, nodule pulmonaire isolé), cette technique en pleine expansion a déjà clairement démontré sa supériorité. Dans d'autres cas, en particulier dans les affections néoplasiques, son application est encore le sujet de controverses.
