Estudou-se a prevalência do papilomavírus humano (HPV) em 228 mulheres portadoras de lesões em cérvice uterina, atendidas no Instituto Ofir Loiola, em Belém, Pará, no período de março de 1992 a maio de 1996. As pacientes foram submetidas à biópsia de colo uterino, sendo o material encaminhado para histopatologia e pesquisa de HPV por PCR e hibridização por dot-blot. Distribuíram-se as participantes em três grupos, conforme diagnóstico histopatológico. O grupo A constituiu-se de 155 mulheres com carcinoma epidermóide invasor ou com adenocarcinoma, o grupo B de 54 portadoras de neoplasia intra-epitelial cervical grau II ou III, e o grupo C de 19 pacientes com cervicite crônica. Observaram-se prevalências de HPV em 70,3%, 63,0% e 36,8% das mulheres dos grupamentos A, B e C, respectivamente, sendo o HPV 16 registrado em 60,4% das amostras positivas do grupo A e 54,5% daquelas do grupo B. Os tipos 16, 18 e 33 representaram 71,4% dos detectados no grupo C.

A infecção genital pelo Papilomavírus humano (HPV) é considerada uma das doenças sexualmente transmissíveis (DSTs) mais comum, representando um importante problema na Saúde Pública, além de estar diretamente relacionado à promoção do câncer de colo uterino. Este estudo teve o intuito de investigar os aspectos epidemiológicos da infecção genital pelo HPV em dois grupos distintos: mulheres de população geral e mulheres encarceradas. Para tanto foi conduzido um estudo transversal analítico com 423 mulheres a partir dos 18 anos que se submeteram ao exame preventivo do câncer do colo uterino, sendo 233 mulheres da população geral oriundas de uma unidade básica de saúde da cidade de Belém do Pará e 190 provenientes do Centro de Reeducação Feminino em Ananindeua no mesmo Estado, no período de janeiro de 2008 a março de 2010. Amostras da cérvice uterina foram coletadas para a realização da colpocitologia convencional e para a detecção do DNA do HPV através da reação em cadeia da polimerase (PCR) mediada pelos oligonucleotídeos iniciadores universais MY9/11. Todas as mulheres responderam a um formulário clínico e epidemiológico. Entre as 423 mulheres analisadas, a prevalência geral de infecção genital pelo HPV foi de 13,0% com variação entre 15,0% para a amostra geral e 10,5% para a carcerária. A faixa etária mais acometida foi a de 13 a 25 anos (19%) na amostra geral; e em mulheres com 45 anos ou mais (21,1%), nas carcerárias. Anormalidades Colpocitológicas, situação conjugal, número de parceiros sexuais novos, o uso de anticoncepcionais orais, história de DST e de sintomas genitais, além de tabagismo atual, foram fatores que se mostraram associados à infecção genital pelo HPV de maneira diferenciada entre amostras da população geral e carcerária.

A etiologia do carcinoma epidermóide do pênis (CEP) está relacionada a múltiplos fatores de risco como a presença de prepúcio, pobre higiene, dermatites crônicas e tabagismo. Entretanto, o fator de risco mais extensivamente estudado, éa infecção pelo papiloma vírus humano (HPV). Está bem estabelecido que esteexerce um importante papel etiológico em neoplasias da cérvice uterina; entretanto, sua associação com CEP tem sido foco de debates, demonstrando-se presença viral variando entre 15% a 80% dos casos, sugerindo que apenas um subgrupo desses tumores seja causado pelo HPV. O prognóstico da doença é influenciado negativamente principalmente pela presença de metástases em linfonodos inguinais. Desta forma, a abordagem cirúrgica destes, assume importância decisiva na cura da doença. Poroutro lado, os métodos disponíveis para o estadiamento desses linfonodos são imprecisos e as linfadenectomias são acompanhadas de morbidade significativa. A descrição de outros marcadores histológicos é escassa, devido a raridade desses tumores.O objetivo do presente trabalho é avaliar a prevalência, distribuição e associação do HPV com parâmetros histológicos de pior prognóstico, no sentido de determinar seu possível valor preditivo para metástases inguinais, assim como avaliar os fatores prognósticos já descritos. Espécimes tumorais em parafina de 82 pacientes portadores de carcinoma epidermóide do pênis, foram testados para prevalência e distribuição do genótipo do HPV por PCR. O estado do HPV foi correlacionado com fatores histopatológicos e ocorrência de metástases inguinais. Foi também avaliada a influência de diversascaracterísticas histológicas tumorais, na sobrevida livre de doença inguinal em 5 anos. O seguimento variou entre 1 e 71 meses (média 20 meses). O DNA do HPV foi identificado em 60,9% da amostra, tendo maior prevalência dos tipos virais 11 e 6 (64% e 32%, respectivamente). Não houve correlação independente significativa das variáveis histológicas de pior prognóstico com o estado do HPV. A probalidade de sobrevida livre de doença inguinal em 5 anos, também não foi influenciada pelo HPV (log Rank teste=0,45). Os únicos fatores patológicos independentes para metástases inguinais foram estadiamento T≥T1b-T4 (p=0,02), invasão linfovascular (p=0,04) e fronte de invasão infiltrativa (p-0,03). O estado e a distribuição do HPV não mostrou correlação com fatores histológicos de pior prognóstico, nem mostrou importância na predição de metástases linfonodais em CEP.

INTRODUÇÃO: A infecção pelo Papiloma Vírus Humano (HPV) é uma das ISTs mais comuns no mundo e possui alto potencial carcinogênico para a cérvice uterina. OBJETIVOS: Identificar possíveis défices de competência para o autocuidado relativo ao comportamento de saúde sexual de mulheres atendidas nas Unidades Saúde da Família Paraíso do Murinin com resultados alterados para HPV e desenvolver estratégias de educação para a saúde que contribuam para comportamentos sexuais saudáveis na prevenção e controle do HPV e suas consequências. METODOLOGIA: Pesquisa desenvolvida no município de Benevides, Estado do Pará. Estudo configurado como pesquisa convergente-assistencial (PCA), que teve como referencial teórico o Autocuidado de Orem. A estratégia educacional foi aplicada em 11 mulheres na faixa etária entre 25 e 64 anos, que realizaram o exame de PCCU entre os anos 2011 e 2012 e que tiveram resultado com alterações relacionadas à contaminação pelo HPV. No desenvolvimento da estratégia educacional foi utilizada a técnica do grupo focal, o qual perdurou por dois meses (19/03/13-14/05/2013), com sete encontros grupais. A análise das informações colhidas durante as atividades grupais foi baseada na PCA e no referencial de autocuidado de Orem, com foco nos objetivos definidos, buscando avaliar como a mudança na percepção dos comportamentos de saúde na prevenção e controle do HPV se processava ante a estratégia educacional desenvolvida, norteada pelos preceitos do sistema de enfermagem de apoio-educação de OREM. RESULTADOS: Durante a pesquisa foi identificado défice de competência para o autocuidado na prevenção, tratamento/controle do HPV e nos cuidados apropriados; défice de conhecimentos acerca do HPV, suas consequências e seu enfrentamento; défice de competência para o autocuidado em práticas de vida sexual satisfatoriamente partilhadas; défice de competência quanto ao cuidado relacionado à redução de riscos socioeconômicos; défice de autocuidado em desvio de saúde relacionado ao tratamento e controle do HPV. Posteriormente foram desenvolvidas ações educativas contribuintes para comportamentos sexuais seguros em face do HPV e a outras ISTs. Ao longo do processo educativo as mulheres passaram a demonstrar competência cognitiva quanto à infecção por HPV e competência para o autocuidado em práticas de vida sexual satisfatoriamente partilhadas. CONCLUSÃO E IMPLICAÇÕES: concluímos que as estratégias educacionais utilizadas contribuem na aprendizagem das mulheres infectadas pelo HPV, na medida em que demonstraram sinais deaquisição de competências e habilidades para autocuidado e higiene com vista a práticas sexuais mais saudáveis, de modo compartilhado com seus parceiros. As implicações para a prática assistencial estão relacionadas à necessidade da enfermagem no desenvolvimento de mecanismos para melhor acolher o par sexual como usuários, de acordo com a política de prevenção de ISTs e de promoção de saúde da mulher. Para o ensino, salienta-se a importância de capacitação em serviço para atender a unidade mulher/homem como par conjugal/sexual, como também na formação de profissionais com uma visão abrangente de unidade implicada: o casal, a família. Para a pesquisa, é imprescindível a investigação de comportamentos humanos que mantêm elevada a incidência de papilomavirus humano, no intuito de encontrar estratégias que debelem a incidência e intensifique o controle, o tratamento e a prevenção de agravos pelo HPV.

Belém, Pára, Brasil, apresenta taxas de mortalidade por câncer de colo uterino bastante elevadas, justificando-se a análise da confiabilidade e validade da causa básica do óbito declarada. Selecionou-se declarações de óbito de residentes de Belém, de 1998-1999, com causa básica de morte neoplasia do colo, corpo e porção não especificada do útero e aquelas que mencionavam essas neoplasias em qualquer linha do atestado, totalizando 188 declarações de óbito. Efetuou-se nova codificação para análise da confiabilidade, aferida pela concordância simples e pela estatística kappa. A causa básica do óbito, após revisão de prontuários médicos e/ou laudos histopatológicos, foi considerada como padrão-ouro para análise da validade de critério, através do valor preditivo positivo. Observou-se concordância simples de 94,0% e kappa de 0,87, sugerindo alta confiabilidade na codificação da causa básica câncer de útero no sistema oficial. Na análise da validade, confirmou-se 120 das 127 originais como colo de útero, três das quatro codificadas como corpo de útero e 18 das 48 classificadas como porção não especificada. Registrou-se aumento de 11,2 % nas neoplasias de colo uterino e redução de 62,5% nos óbitos de porção não especificada do útero.

Objective. To assess the value of vaginal screening cytology after hysterectomy for benign disease.Methods. This cross-sectional study used cytology audit data from 2,512,039 screening tests in the metropolitan region of Campinas from 2000 to 2012; the object was to compare the prevalence of abnormal tests in women who had undergone a hysterectomy for benign diseases (n = 53,891) to that of women who had had no hysterectomy. Prevalence ratios (95% confidence intervals, 95% Cl) were determined, and chi-square analysis, modified by the Cochrane-Armitage test for trend, was used to investigate the effects of age.Results. The prevalence of atypical squamous cells (ASC), low-grade squamous intraepithelial lesion (LSIL), and high-grade squamous intraepithelial lesion or squamous-cell carcinoma (HSIL/SCC) was 0.13%, 0.04% and 0.03%, respectively, in women who had undergone hysterectomy, and 0.93%, 0.51% and 0.26% in women who had not undergone hysterectomy. The prevalence ratios for ASC, LSIL and HSIL/SCC were 0.14(0.11-0.17), 0.08 (0.06-0.13) and 0.13 (0.08-020), respectively, in women with a hysterectomy versus those without. For HSIL/SCC, the prevalence ratios were 0.09 and 029, respectively, for women <50 or >= 50 years. The prevalence rates in women with a previous hysterectomy showed no significant variation with age.Conclusion. The prevalence rates of ASC, LSIL and HSIL/SCC were significantly lower in women with a previous hysterectomy for benign disease compared with those observed in women with an intact uterine cervix. This study reinforces the view that there is no evidence that cytological screening is beneficial for women who have had a hysterectomy for benign disease. (C) 2015 Elsevier Inc. Objectives: To evaluate the effects of conjugated equine estrogens (CE) alone or in combination with medroxyprogesterone acetate (MPA) on glycosaminoglycans (GAGs) in the cervix and horns of the rat uterus. Study design: Thirty days after ovariectomy, adult rats were randomly divided into four groups: Cl, control (treated with drug vehicle); GII CE (50 mu g/kg per day); GIII, MPA (0.2 mg/kg per day), and GIV, CE + MPA (doses as in GII and Gill). Drugs and vehicle were given by gavage during 28 days. Afterwards the animals were anesthetized, the cervix and uterine horns were dissected out and the middle portion fixed in 10% formaldehyde solution; other portions were fixed in acetone for histological examination and glycosaminoglycan quantification, respectively. Agarose gel electrophoresis was used for sulfated GAG analyses, and hyaluronic acid was assayed with an ELISA-like method. Statistical analysis was done by the Student's t test and the Tukey-Kramer test (P < 0.05). Results: The cervix and uterine horn structures presented signs of atrophy in the control group (GI). The other groups, mainly groups III and IV, had histological aspects of proliferation. In all groups the concentration of sulfated GAGs (especially dermatan sulfate) was higher than that of non-sulfated GAGs, both in cervix and in uterine horns. Estrogens increased sulfated GAG concentration at the cervix and the horn, whereas in uterine horns the amounts of sulfated GAGs were decreased after estrogens plus MPA treatment. The concentration of hyaluronic acid in uterine horns was higher than in cervices. Conclusions: The profiling and amounts of glycosaminoglycans in the two portions of the rat uterus are uneven. Dermatan sulfate occurs in higher concentrations in both cervix and uterine horns. Sulfated GAGs in rat cervix were increased by estrogens plus MPA, but were decreased by MPA alone in uterine horns. Cervical cancer is the third most common cancer in women worldwide. Persistent infection with high-risk HPV types, principally HPV16 and 18 is the main risk factor for the development of this malignancy. However, the onset of invasive tumor occurs many years after initial exposure in a minority of infected women. This suggests that other factors beyond viral infection are necessary for tumor establishment and progression. Tumor progression is characterized by an increase in secretion and activation of matrix metalloproteinases (MMPs) produced by either the tumor cells themselves or tumor-associated fibroblasts or macrophages. Increased MMPs expression, including MMP-2, MMP-9 and MT1-MMP, has been observed during cervical carcinoma progression. These proteins have been associated with degradation of ECM components, tumor invasion, metastasis and recurrence. However, few studies have evaluated the interplay between HPV infection and the expression and activity of MMPs and their regulators in cervical cancer. We analyzed the effect of HPV16 oncoproteins on the expression and activity of MMP-2, MMP-9, MT1-MMP, and their inhibitors TIMP-2 and RECK in cultures of human keratinocytes. We observed that E7 expression is associated with increased pro-MMP-9 activity in the epithelial component of organotypic cultures, while E6 and E7 oncoproteins co-expression down-regulates RECK and TIMP-2 levels in organotypic and monolayers cultures. Finally, a study conducted in human cervical tissues showed a decrease in RECK expression levels in precancer and cancer lesions. Our results indicate that HPV oncoproteins promote MMPs/ RECK-TIMP-2 imbalance which may be involved in HPV-associated lesions outcome.

Background: Vulvo-cervico-vaginal involvement has rarely been reported in pemphigus vulgaris (PV) and has not been reported in pemphigus foliaceus (PF). Objectives: We sought to evaluate genital lesions and Papanicolaou (Pap) smears in female patients with PV and PF. Methods: This prospective study includes all consecutive cases of female patients with PV and PF seen from May 2009 to February 2010. Gynecologic examination was performed and Pap smears were collected for cytologic analysis from each patient. Results: A total of 56 patients were given a diagnosis of pemphigus (41 PV and 15 PF). Genital involvement was observed in 9 patients with PV (22%) and the vulva was the most common genital site of involvement. Of these 9 patients, 8 presented with active skin/mucous lesions. Four of 15 patients with PF had genital lesions and vulva was the exclusive site of involvement. Three of 4 patients with PF and genital involvement also showed active cutaneous lesions. Six of 56 patients (5 PV and 1 PF) presented with atypical squamous cells of undetermined significance in Pap smear analysis. Upon further pathologic review, acantholytic cells were seen, confirming the diagnosis of pemphigus. Limitations: A small number of PF cases were studied. Conclusions: Vulvar lesions were the second most frequent site of mucous membrane PV. Herein we report the first case to our knowledge of symptomatic genital lesions in a patient with PF. Moreover, acantholytic cells in Pap smears were found in a patient with PF who was in complete remission off therapy with no clinical genital lesions and no circulating anti-desmoglein-1 and anti-desmoglein-3 autoantibodies. Gynecologic evaluation in patients with pemphigus, including a careful evaluation of Pap smears, should be recommended. (J Am Acad Dermatol 2012;67:409-16.)

Objective: To assess the risk factors for delayed diagnosis of uterine cervical lesions. Materials and Methods: This is a case-control study that recruited 178 women at 2 Brazilian hospitals. The cases (n = 74) were composed of women with a late diagnosis of a lesion in the uterine cervix (invasive carcinoma in any stage). The controls (n = 104) were composed of women with cervical lesions diagnosed early on (low-or high-grade intraepithelial lesions). The analysis was performed by means of logistic regression model using a hierarchical model. The socioeconomic and demographic variables were included at level I (distal). Level II (intermediate) included the personal and family antecedents and knowledge about the Papanicolaou test and human papillomavirus. Level III (proximal) encompassed the variables relating to individuals' care for their own health, gynecologic symptoms, and variables relating to access to the health care system. Results: The risk factors for late diagnosis of uterine cervical lesions were age older than 40 years (odds ratio [OR] = 10.4; 95% confidence interval [CI], 2.3-48.4), not knowing the difference between the Papanicolaou test and gynecological pelvic examinations (OR, = 2.5; 95% CI, 1.3-4.9), not thinking that the Papanicolaou test was important (odds ratio [OR], 4.2; 95% CI, 1.3-13.4), and abnormal vaginal bleeding (OR, 15.0; 95% CI, 6.5-35.0). Previous treatment for sexually transmissible disease was a protective factor (OR, 0.3; 95% CI, 0.1-0.8) for delayed diagnosis. Conclusions: Deficiencies in cervical cancer prevention programs in developing countries are not simply a matter of better provision and coverage of Papanicolaou tests. The misconception about the Papanicolaou test is a serious educational problem, as demonstrated by the present study.

Cervical cancer remains persistently the second most common malignancies among women worldwide, responsible for 500,000 new cases annually. Only in Brazil, the estimate is for 18,430 new cases in 2011. Several types of molecular markers have been studied in carcinogenesis including proteins associated with apoptosis such as BAG-1 and PARP-1. This study aims to demonstrate the expression of BAG-1 and PARP-1 in patients with low-grade squamous intraepithelial lesions (LSILs), high-grade squamous intraepithelial lesions (HSILs) and invasive squamous cell carcinomas (SCCs) of the uterine cervix and to verify a possible association with HPV infection. Fifty samples of LSILs, 50 samples of HSILs and 50 samples of invasive SCCs of the uterine cervix were analyzed by immunohistochemistry for BAG-1 and PARP-1 expression. PCR was performed to detect and type HPV DNA. BAG-1 expression levels were significantly different between LSILs and HSILs (p = 0,014) and between LSILs and SCCs (p = 0,014). In regards to PARP-1 expression, we found significant differences between the expression levels in HSILs and SCCs (p = 0,022). No association was found between BAG-1 expression and the presence of HPV. However, a significant association was found between PARP-1 expression and HPV positivity in the HSILs group (p = 0,021). In conclusion our research suggests that BAG-1 expression could contribute to the differentiation between LSIL and HSIL/SCC whereas PARP-1 could be useful to the differentiation between HSIL HPV-related and SCC. Further studies are needed to clarify the molecular aspects of the relationship between PARP-1 expression and HPV infection, with potential applications for cervical cancer prediction.

BACKGROUND Cytology is an excellent method with which to diagnose preinvasive lesions of the uterine cervix, but it suffers from limited specificity for clinically significant lesions. Supplementary methods might predict the natural course of the detected lesions. The objective of the current study was to test whether a multicolor fluorescence in situ hybridization (FISH) assay might help to stratify abnormal results of Papanicolaou tests. METHODS A total of 219 liquid-based cytology specimens of low-grade squamous intraepithelial lesions (LSIL), 49 atypical squamous cells of undetermined significance (ASCUS) specimens, 52 high-grade squamous intraepithelial lesion (HSIL) specimens, and 50 normal samples were assessed by FISH with probes for the human papillomavirus (HPV), MYC, and telomerase RNA component (TERC). Subtyping of HPV by polymerase chain reaction (PCR) was performed in a subset of cases (n=206). RESULTS There was a significant correlation found between HPV detection by FISH and PCR (P<.0001). In patients with LSILs, the presence of HPV detected by FISH was significantly associated with disease progression (P<.0001). An increased MYC and/or TERC gene copy number (>2 signals in>10% of cells) prevailed in 43% of ASCUS specimens and was more frequent in HSIL (85%) than in LSIL (33%) (HSIL vs LSIL: P<.0001). Increased TERC gene copy number was significantly correlated with progression of LSIL (P<.01; odds ratio, 7.44; area under the receiver operating characteristic curve, 0.73; positive predictive value, 0.30; negative predictive value, 0.94) CONCLUSIONS: The detection of HPV by FISH analysis is feasible in liquid-based cytology and is significantly correlated with HPV analysis by PCR. The analysis of TERC gene copy number may be useful for risk stratification in patients with LSIL.

Human papilloma virus (HPV) infection of the uterine cervix is linked to the pathogenesis of cervical cancer. Preclinical in vitro and in vivo studies using HPV-containing human cervical carcinoma cell lines have shown that the mammalian target of rapamycin (mTOR) inhibitor, rapamycin, and epidermal growth factor receptor (EGFR)-tyrosine kinase inhibitor, erlotinib, can induce growth delay of xenografts. Activation of Akt and mTOR are also observed in cervical squamous cell carcinoma and, the expression of phosphorylated mTOR was reported to serve as a marker to predict response to chemotherapy and survival of cervical cancer patients. Therefore, we investigated: a) the expression level of EGFR in cervical squamous cell carcinoma (SCC) and high-grade squamous intraepithelial lesions (HSIL) versus non-neoplastic cervical squamous epithelium; b) the state of activation of the mTOR pathway in these same tissues; and c) any impact of these signal transduction molecules on cell cycle. Formalin-fixed paraffin-embedded tissue microarray blocks containing 20 samples each of normal cervix, HSIL and invasive SCC, derived from a total of 60 cases of cervical biopsies and cervical conizations were examined. Immunohistochemistry was utilized to detect the following antigens: EGFR; mTOR pathway markers, phosphorylated (p)-mTOR (Ser2448) and p-p70S6K (Thr389); and cell cycle associated proteins, Ki-67 and S phase kinase-associated protein (Skp)2. Protein compartmentalization and expression were quantified in regard to proportion (0-100%) and intensity (0-3+). Mitotic index (MI) was also assessed. An expression index (EI) for pmTOR, p-p70S6K and EGFR, respectively was calculated by taking the product of intensity score and proportion of positively staining cells. We found that plasmalemmal EGFR expression was limited to the basal/parabasal cells (2-3+, EI = 67) in normal cervical epithelium (NL), but was diffusely positive in all HSIL (EI = 237) and SCC (EI 226). The pattern of cytoplasmic p-mTOR and nuclear p-p70S6K expression was similar to that of EGFR; all showed a significantly increased EI in HSIL/SCC versus NL (p<0.02). Nuclear translocation of p-mTOR was observed in all SCC lesions (EI = 202) and was significantly increased versus both HSIL (EI = 89) and NL (EI = 54) with p<0.015 and p<0.0001, respectively. Concomitant increases in MI and proportion of nuclear Ki-67 and Skp2 expression were noted in HSIL and SCC. In conclusion, morphoproteomic analysis reveals constitutive activation and overexpression of the mTOR pathway in HSIL and SCC as evidenced by: increased nuclear translocation of pmTOR and p-p70S6K, phosphorylated at putative sites of activation, Ser2448 and Thr389, respectively; correlative overexpression of the upstream signal transducer, EGFR, and increases in cell cycle correlates, Skp2 and mitotic indices. These results suggest that the mTOR pathway plays a key role in cervical carcinogenesis and targeted therapies may be developed for SCC as well as its precursor lesion, HSIL.
