ANTECEDENTES.- Las infecciones urinarias representan un grave problema de salud que afecta a la población en general. En El Tambo representan una de las principales causas de consulta externa y hospitalización. OBJETIVO.- Identificar la prevalencia de infecciones de vías urinarias, agente etiológico, sensibilidad a los antimicrobianos y factores asociados en los habitantes de la comunidad de San Francisco de Cuchocorral, El Tambo 2015. METODOLOGÍA.- El estudio fue descriptivo de corte transversal. El universo fue de 208 habitantes y la muestra de 160 personas de la comunidad. Se realizó el examen elemental y microscópico de orina, el urocultivo y el antibiograma. Previo a la recolección de las muestras, los participantes firmaron el consentimiento o asentimiento informado y llenaron una encuesta. Se analizaron las muestras en el laboratorio de Microbiología de la Facultad de Ciencias Médicas. Para relacionar los resultados obtenidos y las variables de estudio se utilizó el programa SPSS V22 y Excel para la estadística descriptiva. RESULTADOS.- La prevalencia de infección urinaria fue del 10%, siendo el 100% de personas con infección del sexo femenino y de ellas el 44% mujeres de 19 – 45 años. Escherichia coli fue el microorganismo más frecuente con el 87,5% y mostró resistencia para Trimetoprim sulfametoxazol y Gentamicina en un 28,6%. Proteus y Klebsiella presentaron resistencia para Ampicilina sulbactam y Fosfomicina en un 100%. CONCLUSIÓN.- Contribuimos con datos epidemiológicos sobre prevalencia de infecciones de vías urinarias, agente etiológico y sensibilidad a los antimicrobianos en los habitantes de la comunidad de San Francisco de Cuchocorral

Background: Ureteropelvic junction obstruction (UPJO) is one of the most common causes of urinary tract obstruction in children. Several methods are used to diagnose upper urinary tract obstruction including renal ultrasonography (US), intravenous pyelogram (IVP), diuretic renography (DR), magnetic resonance urography (MRU) and antegrade or retrograde pyelography. Nowadays it is suggested to use diuretic renography as the best method for diagnosing of UPJO. There is no comparative study between IVP and DR scan for diagnosis of UPJO in children. Objectives: The aim of the present study was to compare IVP with furosemide injection and diuretic renography in diagnosis of clinically significant UPJO. Patients and Methods: This was a cross sectional study performed in 153 UPJO suspected children (121 boys, 32 girls) based on US findings in cases presented with urinary tract infection (UTI), prenatal hydronephrosis, abdominal/flank pain, abdominal mass and hematuria. Renal ultrasound was used as an initial screening tool for detection of urinary tract abnormality. Vesicoureteral reflux (VUR) was ruled out by voiding cystourethrography (VCUG). Serum creatinin, blood urea nitrogen, urinalysis and urine culture was screened in all cases. IVP with furosemide and DR were performed as soon as possible after the mentioned workup. Results: During a five year period, 46 out of 153 patients were diagnosed as UPJO based on diuretic renography: the age ranged from 4 months to 13 years (mean: 3.1 ± 0.78 years). There was a significant higher (76%) proportion of UPJO in the boys and in the left side (78%). The sensitivity of IVP with furosemide injection in diagnosis of UPJO was 91.3% whereas DR was accepted as standard for diagnostic procedure in diagnosis of UPJO. Conclusions: Although DR is accepted as the best method for diagnosis of UPJO, we found a small sensitivity difference between IVP and DR in kidneys with normal or near normal function. In many settings such as small cities lacking facilities for advanced isotope imaging technology, use of IVP with diuretic maybe an acceptable procedure for diagnosis of UPJO.

Introducción: La IVU es muy frecuenten en la (FCI - IC), Alrededor el 60% de los pacientes con diagnóstico de IVU nosocomial corresponden a gérmenes resistente, Desde el año 2010 el CLSI disminuyó los puntos de corte de sensibilidad en las enterobacteriaceae y removió la necesidad de tamizaje y confirmación de (BLEE), en el presente trabajo se pretende determinar el perfil epidemiológico de la formulación antibiótica en pacientes con IVU nosocomial. Diseño: Se realizó un estudio observacional analítico de corte transversal. Métodos: Se realizó un análisis univariado, bivariado y multivariado. El análisis bivariado y multivariado se realizó para determinar la medida de asociación teniendo en cuenta la formulación de Carbapenemico la variable dependiente, evaluándose mediante chi cuadrado. Resultados: Se revisaron 131 urocultivos, se incluyeron 116. Los aislamientos microbiológicos más frecuentemente encontrados fueron E. Coli y K. Pneumoniae, el 43.4% de los aislamientos, presentaron expresión de BLEE, 90% de los aislamientos fueron sensibles a Cefepime. La mayoría de los modelos obtenidos mostraron una fuerte asociación entre el reporte de BLEE en antibiograma con la formulación de carbapenémicos como terapia final OR 33,12 IC 95% (2,90 – 337,4). Conclusión: La epidemiologia de la IVU nosocomial en la FCI-IC no difiere de las referencias internacionales, no hay adherencia a las guías de manejo intrahospitalario y el reporte de la palabra BLEE en el antibiograma predice la formulación de antibiótico carbapenémico por el médico que lee el urocultivo

Background: Le early-onset sepsis (EOS) sono infezioni batteriche invasive definite dalla presenza di batteri nel sangue e/o nel liquor cefalorachidiano che esordiscono nelle prime 72 ore di vita e causano in epoca neonatale mortalità e morbilità importanti. Scopo: Determinare l’eccesso di trattamento antibiotico (Overtreatment index=OI) nei neonati di EG ≥34 settimane con sospetta sepsi ad esordio precoce. Metodi: Tutti i nati dal 1.01.2014 al 31.12.2018 di EG ≥34 settimane presso IRCCS Azienda Ospedaliero-Universitaria e l’Ospedale Maggiore di Bologna che hanno ricevuto terapia antibiotica endovenosa nelle prime 168 ore di vita nel sospetto di EOS. Sono stati identificati 2 gruppi: EOS provata (N=7) ed EOS sospetta (N=465). Risultati: L’incidenza di EOS è stata 0.22 su 1000 nati vivi, rispettivamente 0.12/1000 per Streptococcus agalactiae (GBS) e 0.06/1000 per Escherichia coli (E.coli). L’1.75% dei neonati ha ricevuto terapia antimicrobica empirica a largo spettro. L’OI è risultato 68. L’esposizione al trattamento antibiotico nella popolazione è stata di 85 giorni/1000 nati vivi. Tra i fattori di rischio materni, il tampone vagino-rettale (TVR) e l’urinocoltura positiva sono risultati associati al rischio di EOS provata (p=.017, p =.000). I valori di proteina C reattiva (PCR) al T0, T1 e T2 tra i due gruppi sono risultati significativi (p=.000). All’analisi multivariata è stata confermata la significatività delle variabili descritte. (TVR non noto OR=15.1, 95%CI 1.98-115.50, p =.009, urinocoltura positiva OR=30.1, 95%CI 3.6-252.1, p = .002, PCR T0 OR=1.6, 95% CI 1.29-2.07, p = .000.) Conclusioni: L’individuazione precoce di fattori di rischio e la valutazione degli indici di flogosi in neonati sintomatici può ridurre l’OI e la durata della terapia antibiotica in casi di sepsi non confermata. L’uso appropriato degli antibiotici in questa popolazione è particolarmente importante poichè riduce lo sviluppo di germi multiresistenti. Nelle Terapie Intensive Neonatali, i programmi di stewardship antimicrobica dovrebbero guidare la gestione delle sepsi.

The main objective of this thesis is to study the developing fields of aquaponics and its potential for aquaculture wastewater treatment and human urine treatment. Aquaponics is a food production system which combines fish farming (aquaculture) with soilless crop farming (hydroponics). In this thesis the concept of aquaponics and the underlying processes are explained. Research on aquaculture wastewater and human urine wastewater is reviewed and its potential application with aquaponic systems is studied. An overview of the different types of aquaponic systems and current research on the field is also presented. A case study was conducted in a farm in Askeröd, Sweden, which involved building two aquaponic systems (System 1 and System 2) and a human urine-based aquaponic system (System 3), with different degrees of component complexity and sizes. The design, building and monitoring of System 1, System 2 and System 3 was documented and described in detail. Four day experiments were conducted which tested the evolution in concentration of Total Ammonia Nitrogen (NH4+/NH3), Nitrite (NO2-), Nitrate (NO3-), Phosphate (PO43-), and Dissolved Oxygen (O2) after an initial nutrient input. The goal was to assess the concentrations of these parameters after four days and compare them with relevant literature examples in the aquaculture industry and in source-separated urine research. Neither of the two aquaponic systems (System 1 and System 2) displayed all of the parameter concentrations in the last day of testing below reference values found in literature. The best performing of the aquaponic systems was the more complex system (System 2) combining the hydroponic Nutrient Film Technique with a Deep Water Culture component, with a Total Ammonia Nitrogen concentration of 0,20 mg/L, a Nitrite concentration of 0,05 mg/L, a Nitrate concentration of 1,00-5,00 mg/L, a Phosphate concentration of <0,02 mg/L and a Dissolved Oxygen concentration of 8,00 mg/L. The human urine-based aquaponic system (System 3) underperformed in achieving the reference concentration values in literature for most parameters. The removal percentage between the higher recorded values after the input addition and the final day of testing was calculated for two literature examples of separated urine treatment and System 3. The system had a removal percentage of 75% for Total Ammonia Nitrogen, 98% for Nitrite, 25% for Nitrate and 50% for Phosphate. These percentages still underperformed literature examples in most of the tested parameters. The results gathered allowed to conclude that while aquaculture wastewater treatment and human urine treatment is possible with aquaponics systems, overall these did not perform as well as some examples found in recirculating aquaculture systems and source-separated urine treatment literature. However, better measuring techniques, longer testing periods and more research is recommended in this field in order to draw an improved representative conclusion.

The serum and urine proteins responsible for enhanced pigment production in Streptococcus agalactiae in culture media were purified by chromatography and were identified as amylases by comparison of their amino acid composition with that calculated for proteins with known sequences. Similar pigment-enhancing activity was displayed by other amylases of nonanimal origin and by maltooligosaccharides.

Toxicokinetics and the toxicological effects of culture material containing fumonisin B(1) (FB(1)) were studied in male weaned piglets by clinical, pathological, biochemical and sphingolipid analyses. The animals received a single oral dose of 5 mg FB(1)/kg of body weight. obtained from Fusarium verticillioides culture material. FB(1) was detected by H PLC in plasma collected at 1-h intervals up to 6 h and at 12-h intervals up to 96 h. FB(1) eliminated in feces and urine was quantified over a 96-h period and in liver samples collected 96 h post-intoxication. Blood samples were obtained at the beginning and end of the experiment to determine serum enzyme activity, total bilirubin, cholesterol, sphinganine (Sa), sphingosine (So) and the Sa/So ratio. FB(1) was detected in plasma between 30 min and 36 h after administration. The highest concentration of FB(1) was observed after 2 h, with a mean concentration of 282 mu g/ml. Only 0.93% of the total FB(1) was detected in urine between 75 min and 41 h after administration, the highest mean concentration (561 mu g/ml) was observed during the interval after 8 at 24 h. Approximately 76.5% of FB(1) was detected in feces eliminated between 8 and 84 h after administration, with the highest levels observed between 8 and 24 h. Considering the biochemical parameters, a significant increase only occurred in cholesterol, alkaline phosphatase and aspartate aminotransferase activities. In plasma and urine, the highest Sa and Sa/So ratios were obtained at 12 and 48 h, respectively. (C) 2010 Elsevier Ireland Ltd. All rights reserved.

O presente trabalho avaliou a PCR na detecção de leptospiras em sêmen e urina de dez touros sorologicamente reagentes, comparando seus resultados com aqueles obtidos por outras técnicas de diagnóstico. Foram realizadas duas colheitas de materiais em dias alternados. As amostras de sêmen e de urina foram separadas em alíquotas para visualização direta em microscopia de campo escuro, inoculação em hamsters (apenas para o sêmen), isolamento em meio de cultura e PCR. Nenhum hamster apresentou positividade na prova de soroaglutinação microscópica (SAM); fragmentos de rins e fígado desses animais foram utilizados para a tentativa de isolamento em meio de cultura, sendo positivo o cultivo a partir do rim de hamster inoculado com semen de um touro, e do fígado de hamsters inoculados com o semen de três touros. O isolamento em meio de cultura foi negativo para todas as amostras de sêmen, mas foi positivo para cinco amostras de urina. Na PCR não houve resultado positivo para as amostras de sêmen, e apenas uma amostra de urina apresentou resultado positivo, sendo coincidente com uma das culturas positivas. Não foi possível visualizar leptospiras em nenhuma das amostras por exame direto em microscopia de campo escuro.

O sistema Diramic foi avaliado para o diagnóstico das infecções do trato urinário (ITU). O sistema Diramic foi desenvolvido em Cuba e possibilita resultados de diagnóstico das infecções do trato urinário (ITU) em quatro horas e baseia-se na variação da turvação do crescimento microbiano no meio de cultura após incubação a 37ºC/4 horas. 396 amostras de urinas provenientes de ambulatórios e enfermarias do HC da FMB-UNESP-Botucatu/SP foram analisadas pelo sistema Diramic. O método da alça calibrada (AC) foi adotado como método de referência. A taxa de coincidência entre os dois métodos foi de 96,46% (382 amostras de urina), não havendo diferença significativa entre os resultados obtidos nos dois métodos. Os resultados para sensibilidade e especificidade foram 84,37 e 98,80% respectivamente e 10 resultados no Diramic foram falsos negativos (2,5%) e 4 falso positivos (1,01%). Os microrganismos identificados nas urinas positivas foram Escherichia coli (68,75%), Klebsiella pneumoniae (10,94%), leveduras (6,25%), Pseudomonas aeruginosa (4,69%), Enterobacter cloacae (3,12%) e Proteus mirabilis, Staphyloccocus coagulase negativo, Morganella morganii e Citrobacter freundii também foram identificadas (1,56% para cada espécie). O método Diramic foi eficiente na triagem das urinoculturas, porém verificou-se algumas restrições quanto ao diagnóstico das infecções do trato urinário quando causadas por leveduras e em pacientes submetidos a antibioticoterapia.

Shortages in supply of nutrients and freshwater for a growing human population are critical global issues. Traditional centralized sewage treatment can prevent eutrophication and provide sanitation, but is neither efficient nor sustainable in terms of water and resources. Source separation of household wastes, combined with decentralized resource recovery, presents a novel approach to solve these issues. Urine contains within 1 % of household waste water up to 80 % of the nitrogen (N) and 50 % of the phosphorus (P). Since microalgae are efficient at nutrient uptake, growing these organisms in urine might be a promising technology to concomitantly clean urine and produce valuable biomass containing the major plant nutrients. While state-of-the-art suspension systems for algal cultivation have mayor shortcomings in their application, immobilized cultivation on Porous Substrate Photobioreactors (PSBRs) might be a feasible alternative. The aim of this study was to develop a robust process for nutrient recovery from minimally diluted human urine using microalgae on PSBRs. The green alga Desmodesmus abundans strain CCAC 3496 was chosen for its good growth, after screening 96 algal strains derived from urine-specific isolations and culture collections. Treatment of urine, 1:1 diluted with tap water and without addition of nutrients, was performed at a light intensity of 600 μmol photons m-2 s-1 with 2.5 % CO2 and at pH 6.5. A growth rate of 7.2 g dry weight m-² day-1 and removal efficiencies for N and P of 13.1 % and 94.1 %, respectively, were determined. Pre-treatment of urine with activated carbon was found to eliminate possible detrimental effects of pharmaceuticals. These results provide a basis for further development of the technology at pilot-scale. If found to be safe in terms human and environmental health, the biomass produced from three persons could provide the P for annual production of 31 kg wheat grain and 16 kg soybean, covering the caloric demand in food for almost one month of the year for such a household. In combination with other technologies, PSBRs could thus be applied in a decentralized resource recovery system, contributing to locally close the link between sanitation and food production.

During kidney transplant procedure transplanted organs can undergo ischaemia reperfusion phenomena, often associated with the onset of acute kidney damage, loss of kidney function and rejection. These events promote cell turnover to replace damaged cells and preserve kidney function, thus cells deriving from nephrons structures are highly voided in urine. Urine derived cells represents a promising cell source since they can be easily isolated and cultured. The aim of this project was to characterise Urine-derived Renal Epithelial Cells (URECs) from transplanted kidney and to evaluate how these cells react to the co-culture with immune cells. URECs expressed typical markers of kidney tubule epithelial cells (Cytokeratin and CD13), and a subpopulation of these cells expressed CD24 and CD133, which are markers of kidney epithelial progenitor cells. The expression of immunosuppressive molecules as HLA-G and CD73 was also observed. As matter of fact, during the co-culture with PBMCs, UREC suppressed the proliferation of CD4 and CD8 Lymphocytes and reduce the T helper 1 subset, while increasing the T regulatory counterpart. Also, preliminary data observed in this study indicated that the exposition to kidney damage associated molecule, such as NGAL, could significantly affect UREC viability and immunomodulatory capacity. These results add new information about the phenotype of urine cells obtained after kidney transplant and reveal that these cells show promising immunomodulatory properties, suggesting their potential application in personalized cell therapy approaches.

This study aimed at evaluating the functional activation and activating receptors expression on resting, short- and long-term NK and NK-like T cells from blood of ovarian neoplasia patients. Blood from patients with adnexal benign alterations (n = 10) and ovarian cancer (grade I-IV n = 14) were collected after signed consent. Effector cells activation was evaluated by the expression of the CD107a molecule. Short-term culture was conducted overnight with IL-2 and long-term culture for 21 days, by a method designed to expand CD56(+) lymphocytes. Short-term culture significantly increased NK cells activation compared to resting NK cells (p<0.05), however, the long-term procedure supported an even higher increase (p<0.001). Resting NK-like T cells showed poor activation, which was not altered by the culture procedures. The long-term culture effectively increased the expression of the activating receptors on NK and NK-like T cells, either by increasing the number of cells expressing a given receptor and/or by up-regulating their expression intensity. As a conclusion, the long-term culture system employed, resulted in a high number of functional NK cells. The culture system was particularly efficient on the up-regulation of NKp30 and DNAM-1 receptors on NK cells.

Cecropia glaziovii is a tree with used in Brazilian popular medicine. Methods allowing the clonal propagation of this species are of great interest for superior genotype multiplication and perpetuation. For this reason, we examined the effect of different culture media and different types of explants on adventitious shoot regeneration from callus and buds of C. glaziovii. Leaves, petioles and stipules obtained from aseptically grown seedlings or from pre-sterilized plants were used to initiate cultures. Adventitious shoot regeneration was achieved when apical and axillary buds were inoculated on gelled Murashige & Skoog (MS) medium supplemented with 6-benzylaminopurine alone (BAP) (1.0, 5.0 or 10.0 mg L-1) or combined with -naphthalene acetic acid (NAA) (1.0 or 2.0 mg L-1), after 40 days of culture. Best callus production was obtained after 30 days of petioles' culture on gelled MS medium with 2,4 dichlorophenoxyacetic acid (2,4-D) (5.0 mg L-1) combined with BAP (1.0 mg L-1). Successful shoot regeneration from callus was achieved when MS medium supplemented with zeatin (ZEA) (0.1 mg L-1) alone or combined with 2,4-D (1.0 or 5.0 mg L-1) was inoculated with friable callus obtained from petioles. All shoots were rooted by inoculation on MS medium supplemented with indole-3-acetic acid (IAA) (1.0 mg L-1). Rooted plants transferred to potting soil were successfully established. All in vitro regenerated plantlets showed to be normal, without morphological variations, being also identical to the source plant. Our study has shown that C. glaziovii can be propagated by tissue culture methods, allowing large scale multiplication of superior plants for pharmacological purposes.

In vitro culture of the mutualistic fungus of leaf-cutting ants is troublesome due to its low growth rate, which leads to storage problems and contaminants accumulation. This paper aims at comparing the radial growth rate of the mutualistic fungus of Atta sexdens rubropilosa Forel in two different culture media (Pagnocca B and MEA LP). Although total MEA LP radial growth was greater all along the bioassay, no significant difference was detected between growth efficiencies of the two media. Previous evidences of low growth rate for this fungus were confirmed. Since these data cannot point greater efficiency of one culture medium over the other, MEA LP medium is indicated for in vitro studies with this mutualistic fungus due its simpler composition and translucent color, making the analysis easier.
