Il tema del mio elaborato finale è la sottotitolazione del film “A passeggio per Mosca” di Georgij Danelija (1963), un noto regista russo. I suoi film più famosi sono “Мимино” (Mimino) e “Осенный марафон” (“Maratona d’autunno”). L’attore protagonista del film è Nikita Michalkov, famoso attore e regista, nonché personaggio controverso della vita culturale russa; il suo film più famoso è “Утомлённые солнцем” (“Sole ingannatore”). I personaggi più anziani furono interpretati da attori famosi quali Rolan Bykov, attore di teatro, e Vladimir Basov, regista e attore straordinariamente versatile. I ruoli principali, invece, furono affidati ad attori meno noti: Aleksej Loktev e Evgenij Steblov, che si dedicarono entrambi al teatro per gran parte della propria vita, e Galina Pol’skich, che divenne una grande attrice di film drammatici. Straordinari sono anche il lavoro dell’operatore cinematografico Vadim Jusov e quello del compositore Andrej Petrov. Il film suscitò diverse impressioni nel pubblico e nella critica e fu accolto positivamente da Chruščёv: presto sarebbe diventato un simbolo del “Disgelo”, un periodo in cui Chruščёv effettuò un’opera di destalinizzazione dell’URSS: il clima era quindi più disteso, sebbene vi fosse un malcontento diffuso a causa dell’economia fallimentare dello Stato. La canzone finale, che contribuì al successo del film, esprime perfettamente quest’atmosfera spensierata, che pervade anche Mosca, la vera protagonista del film. Della capitale vengono inquadrati i luoghi più importanti: tra questi il parco Gorkij, la metropolitana e il GUM. Tradurre i sottotitoli del film non è stato facile. In particolare, ho incontrato difficoltà nel tradurre alcune particelle russe che non hanno un equivalente in italiano e battute in cui erano presenti importanti riferimenti culturali. Il lavoro a cui mi sono dedicata con più impegno è stato la traduzione della canzone finale: desideravo che i sottotitoli italiani fossero in rima e conservassero il significato dei versi originari.

In der vorliegenden Arbeit wurde Neuroglobin (Ngb), ein evolutiv altes und in Metazoen konserviertes respiratorisches Protein, funktionell untersucht. Mittels des induzierbaren Tet on / Tet off Systems wurde Ngb ektopisch in der murinen Leber und im Gehirn überexprimiert. Die Transkriptome von Leber und Gehirnregionen Ngb-transgener Mäuse wurden mittels Microarrays und RNA-Seq im Vergleich zum Wildtyp analysiert, um Auswirkungen der Ngb-Überexpression zu ermitteln. Die Transkriptom-Analyse in Leber und Gehirn zeigte eine nur geringe Anzahl differenziell regulierter Gene und Stoffwechselwege nach Ngb-Überexpression. Ngb transgene Mäuse wurden CCl4-induziertem ROS-Stress ausgesetzt und die Leberfunktion untersucht. Zudem wurden primäre Hepatozyten-Kulturen etabliert und in diesen in vitro die extrinsische Apoptose induziert. Die Stressversuche zeigten: (i) Die Ngb-Überexpression hat keine protektive Wirkung in der Leber in vivo. (ii) In Leberzellen in vitro hingegen verminderte eine Ngb-Überexpression effizient die Aktivierung der apoptotischen Kaskade. Eine protektive Wirkung von Ngb ist vermutlich von betrachtetem Gewebe und dem verwendeten Stressor abhängig und keine generelle, selektierte Funktion des Proteins.rnWeiterhin wurde eine Ngb-KnockOut-Mauslinie mit einem LacZ-KnockIn-Genotyp etabliert. Hierbei zeigten die KO-Mäuse keinen offensichtlichen Phänotyp in ihrer Entwicklung, Fortpflanzung und Retina-Funktion. Unter Verwendung des LacZ-Knockin-Konstrukts konnten kontrovers diskutierte Ngb-Expressionsorte im adulten Mausgehirn (Hippocampus, Cortex und Cerebellum) sowie in Testes experimentell bestätigt werden. Parallel wurden öffentlich verfügbare RNA-Seq Datensätze ausgewertet, um die regionale Ngb-Expression systematisch ohne Antikörper-assoziierte Spezifitätsprobleme zu charakterisieren. Eine basale Ngb-Expression (RPKM: ~1-5) wurde im Hippocampus, Cortex und Cerebellum, sowie in Retina und Testes ermittelt. Eine 20-40fach höhere, starke Expression (RPKM: ~160) wurde im Hypothalamus bzw. im Hirnstamm nachgewiesen. Die „digitale“ Expressionsuntersuchung wurde mittels qRT-PCR und Western Blot bestätigt. Dieses Expressionsprofil von Ngb in der Maus weist auf eine besondere funktionelle Bedeutung von Ngb im Hypothalamus hin. Eine Funktion von Ngb in der Sauerstoffversorgung der Retina und eine generelle Funktion von Ngb in der Protektion von Neuronen sind mit dem beobachteten Expressionsspektrum weniger gut vereinbar.

Immediate breast reconstruction (IBR) has become an established procedure for women necessitating mastectomy. Traditionally, the nipple-areola complex (NAC) is resected during this procedure. The NAC, in turn, is a principal factor determining aesthetic outcome after breast reconstruction, and due to its particular texture and shape, a natural-looking NAC can barely be reconstructed with other tissues. The aim of this study was to assess the oncological safety as well as morbidity and aesthetic outcome after replantation of the NAC some days after IBR. Retrospective analysis of 85 patients receiving 88 mastectomies and IBR between 1998 and 2007 was conducted. NAC (n=29) or the nipple alone (n=23) were replanted 7 days (median, range 2-10 days) after IBR in 49 patients, provided the subareolar tissue was histologically negative for tumour infiltration. Local recurrence rate was assessed after 49 months (median, range 6-120 months). Aesthetic outcome was evaluated by clinical assessment during routine follow-up at least 12 months after the last intervention. Malignant involvement of the subareolar tissue was found in eight cases (9.1%). Patients qualifying for NAC replantation were in stage 0 in 29%, stage I in 15%, stage IIa in 31%, stage IIb in 17% and stage III in 8%. Total or partial necrosis occurred in 69% and 26% if the entire NAC or only the nipple were replanted, respectively (P<0.01). Depigmentation was seen in 52% and corrective surgery was done in 11 out of 52 NAC or nipple replantations. Local recurrence and isolated regional lymph node metastasis were observed in one single case each. Another 5.8% of the patients showed distant metastases. We conclude that the replantation of the NAC in IBR is oncologically safe, provided the subareolar tissue is free of tumour. However, the long-term aesthetic outcome of NAC replantation is not satisfying, which advocates replanting the nipple alone.

To date, obesity affects a substantial population in industrialised countries. Due to the increased awareness of obesity-related morbidity, efficient dietary regimens and the recent successes with bariatric surgery, there is now a high demand for body contouring surgery to correct skin abundancies after massive weight loss. The known risks for this type of surgery are mainly wound-healing complications, and, more rarely, thromboembolic or respiratory complications. We present two female patients (23 and 39 years of age) who, in spite of standard positioning and precautions, developed sciatic neuropathy after combined body contouring procedures, including abdominoplasty and inner thigh lift. Complete functional loss of the sciatic nerve was found by clinical and electroneurographic examination on the left side in patient one and bilaterally in patient two. Full nerve conductance recovery was obtained after 6 months in both patients. Although the occurrence of spontaneous neuropathies after heavy weight loss is well documented, this is the first report describing the appearance of such a phenomenon following body contouring surgery. One theoretical explanation may be the compression of the nerve during the semirecumbent positioning combined with hip flexion and abduction, which was required for abdominal closure and simultaneous access to the inner thighs. We advise to avoid this positioning and to include the risk of sciatic neuropathy in the routine preoperative information of patients scheduled for body contouring surgery after heavy weight loss.

In surgical animal studies anesthesia is used regularly. Several reports in the literature demonstrate respiratory and cardiovascular side effects of anesthesiologic agents. The aim of this study was to compare two frequently used anesthesia cocktails (ketamine/xylazine [KX] versus medetomidine/climazolam/fentanyl [MCF]) in skin flap mouse models. Systemic blood values, local metabolic parameters, and surgical outcome should be analyzed in critical ischemic skin flap models. Systemic hypoxia was found in the animals undergoing KX anesthesia compared with normoxia in the MCF group (sO(2): 89.2% +/- 2.4% versus 98.5% +/- 1.2%, P < 0.01). Analysis of tissue metabolism revealed impaired anaerobic oxygen metabolism and increased cellular damage in critical ischemic flap tissue under KX anesthesia (lactate/pyruvate ratio: KX 349.86 +/- 282.38 versus MCF 64.53 +/- 18.63; P < 0.01 and glycerol: KX 333.50 +/- 83.91 micromol/L versus MCF 195.83 +/- 29.49 micromol/L; P < 0.01). After 6 d, different rates of flap tissue necrosis could be detected (MCF 57% +/- 6% versus KX 68% +/- 6%, P < 0.01). In summary we want to point out that the type of anesthesia, the animal model and the goal of the study have to be well correlated. Comparing the effects of KX and MCF anesthesia in mice on surgical outcome was a novel aspect of our study.

OBJECTIVE: To determine the formation and dissolution of calcium fluoride on the enamel surface after application of two fluoride gel-saliva mixtures. METHOD AND MATERIALS: From each of 80 bovine incisors, two enamel specimens were prepared and subjected to two different treatment procedures. In group 1, 80 specimens were treated with a mixture of an amine fluoride gel (1.25% F-; pH 5.2; 5 minutes) and human saliva. In group 2, 80 enamel blocks were subjected to a mixture of sodium fluoride gel (1.25% F; pH 5.5; 5 minutes) and human saliva. Subsequent to fluoride treatment, 40 specimens from each group were stored in human saliva and sterile water, respectively. Ten specimens were removed after each of 1 hour, 24 hours, 2 days, and 5 days and analyzed according to potassium hydroxide-soluble fluoride. RESULTS: Application of amine fluoride gel resulted in a higher amount of potassium hydroxide-soluble fluoride than did sodium fluoride gel 1 hour after application. Saliva exerted an inhibitory effect according to the dissolution rate of calcium fluoride. However, after 5 days, more than 90% of the precipitated calcium fluoride was dissolved in the amine fluoride group, and almost all potassium hydroxide-soluble fluoride was lost in the sodium fluoride group. Calcium fluoride apparently dissolves rapidly, even at almost neutral pH. CONCLUSION: Considering the limitations of an in vitro study, it is concluded that highly concentrated fluoride gels should be applied at an adequate frequency to reestablish a calcium fluoride-like layer.

There is a demand for technologies able to assess the perfusion of surgical flaps quantitatively and reliably to avoid ischemic complications. The aim of this study is to test a new high-speed high-definition laser Doppler imaging (LDI) system (FluxEXPLORER, Microvascular Imaging, Lausanne, Switzerland) in terms of preoperative mapping of the vascular supply (perforator vessels) and postoperative flow monitoring. The FluxEXPLORER performs perfusion mapping of an area 9 x 9 cm with a resolution of 256 x 256 pixels within 6 s in high-definition imaging mode. The sensitivity and predictability to localize perforators is expressed by the coincidence of preoperatively assessed LDI high flow spots with intraoperatively verified perforators in nine patients. 18 free flaps are monitored before, during, and after total ischemia. 63% of all verified perforators correspond to a high flow spot, and 38% of all high flow spots correspond to a verified perforator (positive predictive value). All perfused flaps reveal a value of above 221 perfusion units (PUs), and all values obtained in the ischemic flaps are beneath 187 PU. In summary, we conclude that the present LDI system can serve as a reliable, fast, and easy-to-handle tool to detect ischemia in free flaps, whereas perforator vessels cannot be detected appropriately.

The diagnostic yield of prosthetic joint-associated infection is hampered by the phenotypic change of bacteria into a sessile and resistant form, also called biofilm. With sonication, adherent bacteria can be dislodged from the prosthesis. Species identification may be difficult because of their variations in phenotypic appearance and biochemical reaction. We have studied the phenotypic, genotypic, and biochemical properties of Escherichia coli variants isolated from a periprosthetic joint infection. The strains were collected from synovial fluid, periprosthetic tissue, and fluid from the explanted and sonicated prosthesis. Isolates from synovial fluid revealed a normal phenotype, whereas a few variants from periprosthetic tissue and all isolates from sonication fluid showed different morphological features (including small-colony variants). All isolates from sonication fluid were beta-galactosidase negative and nonmotile; most were indole negative. Because of further variations in biochemical properties, species identification was false or not possible in 50% of the isolates included in this study. In contrast to normal phenotypes, variants were resistant to aminoglycosides. Typing of the isolates using pulsed-field gel electrophoresis yielded nonidentical banding patterns, but all strains were assigned to the same clonal origin when compared with 207 unrelated E. coli isolates. The bacteria were repeatedly passaged on culture media and reanalyzed. Thereafter, most variants reverted to normal phenotype and regained their motility and certain biochemical properties. In addition, some variants displayed aminoglycoside susceptibility after reversion. Sonication of an explanted prosthesis allows insight into the lifestyle of bacteria in biofilms. Since sonication fluid also reveals dislodged sessile forms, species identification of such variants may be misleading.

Varicella zoster, limited to the mandibular nerve, is rare. Classical symptoms are pain, hypesthesia and vesicular eruption restricted to the third trigeminal segment (V3). Little is known on taste affection after mandibular nerve zoster. We report two cases of patients suffering from mandibular zoster associated with subjective taste disorder. In both cases, gustatory measures confirmed ipsilateral hemiageusia of the anterior two-thirds of the tongue. After 2 months, the symptoms regressed and psychophysical measures came back to normal values, whereas post-zoster neuralgia lasted for more than 1 year. Gustatory dysfunction is a possible symptom after mandibular nerve zoster. In contrast to post-zoster neuralgia, taste function seems to recover quickly.

Radiodermatitis is a known complication in patients having undergone radiotherapy. It usually appears 2 to 5 years after irradiation. We are reporting on a case of radiodermatitis that occurred within months after coronary dilatation and stenting. It started with painful swelling, followed by a typical appearance on the skin surface. Histological finding confirmed the diagnosis. However, magnetic resonance imaging showed changes in the subcutaneous tissue extending into the ribs. A radical debridement was performed including removal of a partially necrotic 4th rib. The defect was closed with a latissimus dorsi transposition flap. Our findings are compared with the literature reports.
