In order to develop applications for z;isual interpretation of medical images, the early detection and evaluation of microcalcifications in digital mammograms is verg important since their presence is often associated with a high incidence of breast cancers. Accurate classification into benign and malignant groups would help improve diagnostic sensitivity as well as reduce the number of unnecessa y biopsies. The challenge here is the selection of the useful features to distinguish benign from malignant micro calcifications. Our purpose in this work is to analyse a microcalcification evaluation method based on a set of shapebased features extracted from the digitised mammography. The segmentation of the microcalcifications is performed using a fixed-tolerance region growing method to extract boundaries of calcifications with manually selected seed pixels. Taking into account that shapes and sizes of clustered microcalcifications have been associated with a high risk of carcinoma based on digerent subjective measures, such as whether or not the calcifications are irregular, linear, vermiform, branched, rounded or ring like, our efforts were addressed to obtain a feature set related to the shape. The identification of the pammeters concerning the malignant character of the microcalcifications was performed on a set of 146 mammograms with their real diagnosis known in advance from biopsies. This allowed identifying the following shape-based parameters as the relevant ones: Number of clusters, Number of holes, Area, Feret elongation, Roughness, and Elongation. Further experiments on a set of 70 new mammogmms showed that the performance of the classification scheme is close to the mean performance of three expert radiologists, which allows to consider the proposed method for assisting the diagnosis and encourages to continue the investigation in the sense of adding new features not only related to the shape

A recent trend in digital mammography is computer-aided diagnosis systems, which are computerised tools designed to assist radiologists. Most of these systems are used for the automatic detection of abnormalities. However, recent studies have shown that their sensitivity is significantly decreased as the density of the breast increases. This dependence is method specific. In this paper we propose a new approach to the classification of mammographic images according to their breast parenchymal density. Our classification uses information extracted from segmentation results and is based on the underlying breast tissue texture. Classification performance was based on a large set of digitised mammograms. Evaluation involves different classifiers and uses a leave-one-out methodology. Results demonstrate the feasibility of estimating breast density using image processing and analysis techniques

Los niños que presentan dolor abdominal agudo representan una de las principales demandas de atención en los servicios de Urgencias Pediátricas, convirtiéndose en un reto para quien realiza la valoración inicial la decisión de que paciente amerita realizar estudios adicionales, para descartar una patología quirúrgica. Considerando que la apendicitis es la primera emergencia quirúrgica en niños, y que sus complicaciones son un problema frecuente, es imprescindible que el clínico conozca cual es la utilidad y beneficio que le brindan las herramientas diagnósticas disponibles para realizar un diagnóstico más certero. Objetivo: Describir el manejo de los pacientes menores de 18 años con sospecha de apendicitis en la FCI y determinar la sensibilidad y especificidad de la ecografía en esta población. Métodos: Estudio de Prueba Diagnóstica. Revisión expedita de los niños valorados en la FCI con sospecha clínica de apendicitis aguda durante un periodo de 4 meses. Se tomaron datos de hallazgos ecográficos para apendicitis (positivos, negativos o indeterminados) y del desenlace de tener o no apendicitis. Posteriormente se realizó una inferencia mediante una tabla de contingencia, teniendo en cuenta intervalos de confianza para determinar la sensibilidad, especificidad, VPP, VPN y exactitud de la prueba basados en el diagnóstico final. Resultados: En 52% de pacientes que consultaron por dolor abdominal se sospecho patología quirúrgica, grupo en el que se utilizó la ecografía abdominal como herramienta diagnóstica para descartar apendicitis encontrando: sensibilidad: 63%(IC 95%, 48.6-75.5), especificidad: 82,7%(IC 95%, 76- 87.8) y exactitud: 78,2%(IC 95%, 72-83,4). La prevalencia de la enfermedad fue 22,8%, con probabilidad postprueba positiva de 88.3%(IC 95%, 82,1-92.6). Conclusiones: Los resultados de este estudio evidencian un menor rendimiento de la ecografía como prueba diagnóstica para apendicitis en pediatría en nuestro medio comparado al descrito en la literatura. Dicha discordancia probablemente se encuentra determinada por los sesgos propios de un estudio histórico como este, en el que básicamente la falta de uniformidad en el registro de datos en la historia clínica por los observadores (clínicos y radiólogos), así como las limitaciones para determinar el seguimiento de todos los pacientes que no fueron operados, pueden alterar la capacidad operativa real de la prueba. De ahí la importancia de realizar a futuro un estudio concurrente.

En Colombia, la patología pulmonar es una de las causas mas frecuentes de consulta y morbilidad de servicios de medicina general y de medicina interna. Algunas de las patologías que se presentan a nivel neumológico requieren de un diagnostico exacto para su adecuado tratamiento, ya sean estas de índole infeccioso, tumoral u ocupacional. El Hospital Santa Clara (ESE) es una institución pública de referencia en enfermedades neumológicas en Bogotá y durante muchos años en el país. El servicio de anatomía patológica cuenta con un archivo sistematizado desde 1997 en el cual se identificó cada paciente con los diagnósticos emitidos por estudio de lavado broncoalveolar (BAL), biopsia transbronquial y biopsia abierta. Con los datos de este archivo y teniendo acceso a las historias clínicas de los pacientes fue revisado el diagnostico radiológico previo a las pruebas de patología para evaluar el grado de correlación entre los mismos. Se encontró registro de 268 pacientes en la base de datos ya mencionada, sobre los cuales se realizo el análisis por variables encontrando que la mayoría de procedimientos se realizo en hombres, con una edad promedio de 51 anos y diagnóstico más frecuente con el estudio anatomopatológico son las neoplasias primarias y el menos frecuente como es de esperarse por el fácil diagnostico de la misma con estudios no invasivos es la TBC. Para el estudio de concordancia diagnostica solo se tubo acceso a la información completa de 58 pacientes sobre los cuales se hizo el análisis encontrando un grado de concordancia débil (Kappa de 0.3236). El bajo grado de concordancia diagnostica encontrado podría estar influenciado por los problemas metodológicos propios de un estudio en el cual la información es tomada de historias clínicas, teniendo en cuenta que no se pudo hacer control de sesgos tan importantes como el grado de entrenamiento y experiencia de los diferentes radiólogos que realizaron la lectura, lo cual tiene también gran influencia en los resultados. Es necesario realizar estudios futuros donde se tenga en cuenta las dificultades técnicas que se tuvieron durante el desarrollo de este trabajo por la metodología utilizada y la influencia que pueden tener en los resultados para mejorarlos.

Comparar el resultado de la resonancia magnética en el diagnóstico de apendicitis aguda con el patrón de oro (estudio histopatológico o seguimiento clínico del paciente) con el fin de establecer la utilidad de ésta como prueba diagnóstica y poder incluirla dentro del algoritmo de estudio de esta patología cuando hay sospecha clínica y un resultado negativo o dudoso de la ecografía abdominal

: Los métodos imagenológicos para evaluar los nódulos tiroideos han sido motivo de estudio en las últimas décadas, especialmente la ecografía sobresale sobre las otras modalidades diagnósticas por su accesibilidad, portabilidad, y seguridad. A pesar de ello, las características ecográficas de cada nódulo han sido objeto de controversia en cuanto a su potencial detección de malignidad o benignidad. Se presenta un estudio de concordancia entre el estudio citopatológico y la ecografía para la caracterización nódulos tiroideos de naturaleza maligna y benigna, y su análisis de pruebas diagnósticas. Metodología: Se realizó un estudio descriptivo de concordancia con estudio de pruebas diagnósticas anidado. Se escogieron todos los pacientes con nódulos tiroideos a quienes se les realizó ecografía y estudio citopatológico de la lesión y se estudiaron los hallazgos ecográficos para evaluar su potencial diagnóstico para malignidad. Se incluyeron un total de 100 pacientes con nódulos tiroideos potencialmente malignos. La concordancia entre la ecografía en modo B y el estudio citopatológico fue moderada (índice kappa 0.55). La característica con mayor potencial para detectar malignidad fue la presencia de Microcalcificaciones (sensibilidad 75%, especificidad 92%).

With increasing age, there are greater numbers of older people who will be diagnosed with cancer. It must be remembered that such individuals have increased frailty and have a number of geriatric syndromes and conditions particularly pertinent to older age, including incontinence, poor cognition and impaired nutrition. It is often difficult to define the effects of cancer and its treatment or complications, and separate these from the effects of normal ageing and geriatric syndromes. The documentation of poor nutrition and its management must combine knowledge from both geriatric medicine and oncology. Nutrition serves to identify key healthcare professionals who are all essential in any patient at risk or suffering from malnutrition. Incontinence must be actively sought, its cause identified and efforts made to either 'cure' it or, in certain circumstances, 'manage' it. Older patients with cancer are cared for predominantly by older relations and informal care mechanisms and special consideration of their physical and practical needs are paramount. In this area, nurses, doctors, therapists and social workers should work to identify formal and informal mechanisms to support particularly the older carer.

Smart healthcare is a complex domain for systems integration due to human and technical factors and heterogeneous data sources involved. As a part of smart city, it is such a complex area where clinical functions require smartness of multi-systems collaborations for effective communications among departments, and radiology is one of the areas highly relies on intelligent information integration and communication. Therefore, it faces many challenges regarding integration and its interoperability such as information collision, heterogeneous data sources, policy obstacles, and procedure mismanagement. The purpose of this study is to conduct an analysis of data, semantic, and pragmatic interoperability of systems integration in radiology department, and to develop a pragmatic interoperability framework for guiding the integration. We select an on-going project at a local hospital for undertaking our case study. The project is to achieve data sharing and interoperability among Radiology Information Systems (RIS), Electronic Patient Record (EPR), and Picture Archiving and Communication Systems (PACS). Qualitative data collection and analysis methods are used. The data sources consisted of documentation including publications and internal working papers, one year of non-participant observations and 37 interviews with radiologists, clinicians, directors of IT services, referring clinicians, radiographers, receptionists and secretary. We identified four primary phases of data analysis process for the case study: requirements and barriers identification, integration approach, interoperability measurements, and knowledge foundations. Each phase is discussed and supported by qualitative data. Through the analysis we also develop a pragmatic interoperability framework that summaries the empirical findings and proposes recommendations for guiding the integration in the radiology context.

Objective. This study was designed to determine the precision and accuracy of angular measurements using three-dimensional computed tomography (3D-CT) volume rendering by computer systems. Study design. The study population consisted of 28 dried skulls that were scanned with a 64-row multislice CT, and 3D-CT images were generated. Angular measurements, (n = 6) based upon conventional craniometric anatomical landmarks (n = 9), were identified independently in 3D-CT images by 2 radiologists, twice each, and were then performed by 3D-CT imaging. Subsequently, physical measurements were made by a third examiner using a Beyond Crysta-C9168 series 900 device. Results. The results demonstrated no statistically significant difference between interexaminer and intraexaminer analysis. The mean difference between the physical and 3-D-based angular measurements was -1.18% and -0.89%, respectively, for both examiners, demonstrating high accuracy. Conclusion. Maxillofacial analysis of angular measurements using 3D-CT volume rendering by 64-row multislice CT is established and can be used for orthodontic and dentofacial orthopedic applications.

Introduction: Root resorption can cause damage in orthodontic patients. Digital subtraction radiography (DSR) is a useful resource for the detection of mineral losses. The purpose of this study was to compare the efficacy of digital radiography (DR) and DSR in detecting simulated external root resorption. Examiner agreement between the 2 techniques was also evaluated. Methods: Root resorptions of various sizes were simulated on the apical and lingual aspects of 49 teeth from 9 dry human mandibles. The teeth were radiographed in standardized conditions. The radiographs were registered with Regeemy Image Registration and Mosaicking (version 0.2.43-RCB, DPI-INPE, Sao Jose dos Campos, São Paulo, Brazil) and subtracted with Image Tool (University of Texas Health Science Center at San Antonio). The subtracted images and the digital radiographs were evaluated by 3 oral radiologists. Results: No statistically significant differences were found for the methods in the detection of apical root resorptions, independently from lesion size, and of lingual resorptions of 1.2 mm or greater. DSR was significantly better than DR for detection of lingual resorptions up to 1 mm. Resorptions less than 0.5 mm were not precisely detected by either method. DSR provided better intraexaminer and interexaminer agreement than did DR. Conclusions: Both methods are precise for detection of apical root resorptions as small as 0.5 mm and lingual resorptions of 1 mm or more. However, DSR frequently performed better than did DR. Objective: The aim of the present study was to describe the clinical and MRI findings of the temporomandibular joint (TMJ) in patients with major depressive disorders (MDDs) of the non-psychotic type.Methods: 40 patients (80 TMJs) who were diagnosed as having MDDs were selected for this study. The clinical examination of the TMJs was conducted according to the research diagnostic criteria and temporomandibular disorders (TMDs). The MRIs were obtained bilaterally in each patient with axial, parasagittal and paracoronal sections within a real-time dynamic sequence. Two trained oral radiologists assessed all images. For statistical analyses, Fisher's exact test and chi(2) test were applied (alpha = 0.05).Results: Migraine was reported in 52.5% of subjects. Considering disc position, statistically significant differences between opening patterns with and without alteration (p = 0.00) and between present and absent joint noises (p = 0.00) were found. Regarding muscular pain, patients with and without abnormalities in disc function and patients with and without abnormalities in disc position were not statistically significant (p = 0.42 and p = 0.40, respectively). Significant differences between mandibular pathway with and without abnormalities (p=0.00) and between present and absent joint noises (p=0.00) were observed.Conclusion: Based on the preliminary results observed by clinical and MRI examination of the TMJ, no direct relationship could be determined between MDDs and TMDs. OBJETIVO: Comparar os tempos de geração e digitação de laudos radiológicos entre um sistema eletrônico baseado na tecnologia de voz sobre o protocolo de internet (VoIP) e o sistema tradicional, em que o radiologista escreve o laudo à mão. MATERIAIS E MÉTODOS: Foi necessário modelar, construir e implantar o sistema eletrônico proposto, capaz de gravar o laudo em formato de áudio digital, e compará-lo com o tradicional já existente. Por meio de formulários, radiologistas e digitadores anotaram os tempos de geração e digitação dos laudos nos dois sistemas. RESULTADOS: Comparadas as médias dos tempos entre os sistemas, o eletrônico apresentou redução de 20% (p = 0,0410) do tempo médio de geração do laudo em comparação com o sistema tradicional. O tradicional foi mais eficiente em relação ao tempo de digitação, uma vez que a média de tempo do eletrônico foi três vezes maior (p < 0,0001). CONCLUSÃO: Os resultados mostraram diferença estatisticamente significante entre os sistemas comparados, sendo que o eletrônico foi mais eficiente do que o tradicional em relação ao tempo de geração dos laudos, porém, em relação ao tempo de digitação, o tradicional apresentou melhores resultados.

The reproducibility of measurements of alveolar bone loss on radiographs may be a problem on epidemiologic studies, as they are based on comparisons of the diagnosis of various examiners. The aim of the present research paper was to assess the inter- and intra-examiner reproducibility of measurements of the interproximal alveolar bone loss on non-manipulated digital radiographs and after the application of image filters. Five Oral Radiologists measured the distance between the cementoenamel junction (CEJ) to the alveolar crest or to the deepest point of the bony defect on 12 interproximal digital radiographs of molars and bicuspids of a dry human skull. The digital manipulation and the linear measurements were obtained with the Trophy Windows software (Throphy®). For each image, six different versions were created: 1) non-manipulated; 2) bright-contrast adjustment; 3) negative; 4) negative with brightness-contrast adjustment; 5) pseudo-colored; 6) pseudo-colored with brightness-contrast adjustment. In order to prevent interpretation bias because of the repetition of measurements, the examiners measured the radiographs in a random sequence. The two-way ANOVA test at 5% level of significance to compare the means of readings of the same operator with each filter indicated p<0.05 for the majority of operators, while the comparison between the mean values of operators using the same filter indicated p>0.05 for all filters. Based on the results, we concluded that linear measurements of interproximal alveolar bone loss on digital radiographs are highly reproducible among examiners. Nevertheless, the application of image filters significantly influenced the degree of intra-examiner reproducibility. Some filters even reduced the reproducibility of intra-examiner readings.
