Numerical optimisation methods are being more commonly applied to agricultural systems models, to identify the most profitable management strategies. The available optimisation algorithms are reviewed and compared, with literature and our studies identifying evolutionary algorithms (including genetic algorithms) as superior in this regard to simulated annealing, tabu search, hill-climbing, and direct-search methods. Results of a complex beef property optimisation, using a real-value genetic algorithm, are presented. The relative contributions of the range of operational options and parameters of this method are discussed, and general recommendations listed to assist practitioners applying evolutionary algorithms to the solution of agricultural systems. Purpose: To determine whether constriction of proximal arterial vessels precedes involution of the distal hyaloid vasculature in the mouse, under normal conditions, and whether this vasoconstriction is less pronounced when the distal hyaloid network persists, as it does in oxygen-induced retinopathy (OIR). Methods: Photomicrographs of the vasa hyaloidea propria were analysed from pre-term pups (1-2 days prior to birth), and on Days 1-11 post-birth. The OIR model involved exposing pups to similar to 90% O-2 from D1-5, followed by return to ambient air. At sampling times pups were anaesthetised and perfused with india ink. Retinal flatmounts were also incubated with FITC-lectin (BS-1, G. simplicifolia,); this labels all vessels, allowing identification of vessels not patent to the perfusate. Results: Mean diameter of proximal hyaloid vessels in preterm pups was 25.44 +/- 1.98 mum; +/-1 SEM). Within 3-12 hrs of birth, significant vasoconstriction was evident (diameter:12.45 +/- 0.88 mum), and normal hyaloid regression subsequently occurred. Similar vasoconstriction occurred in the O-2-treated group, but this was reversed upon return to room air, with significant dilation of proximal vessels by D7 (diameter: 31.75 +/- 11.99 mum) and distal hyaloid vessels subsequently became enlarged and tortuous. Conclusions: Under normal conditions, vasoconstriction of proximal hyaloid vessels occurs at birth, preceding attenuation of distal hyaloid vessels. Vasoconstriction also occurs in O-2-treated pups during treatment, but upon return to room air, the remaining hyaloid vessels dilate proximally, and the distal vessels become dilated and tortuous. These observations support the contention that regression of the hyaloid network is dependent, in the first instance, on proximal arterial vasoconstriction.

Background: An orthopaedic management/patient-focused care unit (OMPFCU) involving a dedicated orthopaedic-geriatrics liaison team was established at the Royal Brisbane Hospital in 1994 in an effort to safely accelerate rehabilitation of patients with proximal femoral fractures. Methods: The surgical outcomes of the patients were monitored in order to determine whether accelerated rehabilitation had any significant adverse effects on the surgical outcomes, measured by mortality, readmission to hospital, deep wound infection, fracture union delay, mobility and the revision surgery rate. Results: No significant difference was recorded in mortality and morbidity, deep wound infection and revision surgery rates between patients in the Royal Brisbane Hospital OMPFCU and those in standard care in the orthopaedic surgery wards. Conclusion: Accelerated rehabilitation for patients with a proximal femoral fracture in a major teaching hospital can be accomplished safely.

Using benthic habitat data from the Florida Keys (USA), we demonstrate how siting algorithms can help identify potential networks of marine reserves that comprehensively represent target habitat types. We applied a flexible optimization tool-simulated annealing-to represent a fixed proportion of different marine habitat types within a geographic area. We investigated the relative influence of spatial information, planning-unit size, detail of habitat classification, and magnitude of the overall conservation goal on the resulting network scenarios. With this method, we were able to identify many adequate reserve systems that met the conservation goals, e.g., representing at least 20% of each conservation target (i.e., habitat type) while fulfilling the overall aim of minimizing the system area and perimeter. One of the most useful types of information provided by this siting algorithm comes from an irreplaceability analysis, which is a count of the number of, times unique planning units were included in reserve system scenarios. This analysis indicated that many different combinations of sites produced networks that met the conservation goals. While individual 1-km(2) areas were fairly interchangeable, the irreplaceability analysis highlighted larger areas within the planning region that were chosen consistently to meet the goals incorporated into the algorithm. Additionally, we found that reserve systems designed with a high degree of spatial clustering tended to have considerably less perimeter and larger overall areas in reserve-a configuration that may be preferable particularly for sociopolitical reasons. This exercise illustrates the value of using the simulated annealing algorithm to help site marine reserves: the approach makes efficient use of;available resources, can be used interactively by conservation decision makers, and offers biologically suitable alternative networks from which an effective system of marine reserves can be crafted.

In this paper we propose a second linearly scalable method for solving large master equations arising in the context of gas-phase reactive systems. The new method is based on the well-known shift-invert Lanczos iteration using the GMRES iteration preconditioned using the diffusion approximation to the master equation to provide the inverse of the master equation matrix. In this way we avoid the cubic scaling of traditional master equation solution methods while maintaining the speed of a partial spectral decomposition. The method is tested using a master equation modeling the formation of propargyl from the reaction of singlet methylene with acetylene, proceeding through long-lived isomerizing intermediates. In this paper we propose a novel fast and linearly scalable method for solving master equations arising in the context of gas-phase reactive systems, based on an existent stiff ordinary differential equation integrator. The required solution of a linear system involving the Jacobian matrix is achieved using the GMRES iteration preconditioned using the diffusion approximation to the master equation. In this way we avoid the cubic scaling of traditional master equation solution methods and maintain the low temperature robustness of numerical integration. The method is tested using a master equation modelling the formation of propargyl from the reaction of singlet methylene with acetylene, proceeding through long lived isomerizing intermediates. This paper delineates the development of a prototype hybrid knowledge-based system for the optimum design of liquid retaining structures by coupling the blackboard architecture, an expert system shell VISUAL RULE STUDIO and genetic algorithm (GA). Through custom-built interactive graphical user interfaces under a user-friendly environment, the user is directed throughout the design process, which includes preliminary design, load specification, model generation, finite element analysis, code compliance checking, and member sizing optimization. For structural optimization, GA is applied to the minimum cost design of structural systems with discrete reinforced concrete sections. The design of a typical example of the liquid retaining structure is illustrated. The results demonstrate extraordinarily converging speed as near-optimal solutions are acquired after merely exploration of a small portion of the search space. This system can act as a consultant to assist novice designers in the design of liquid retaining structures.

As fracturas da extremidade proximal do fémur são frequentes em pessoas de idade avançada, com índices de morbilidade e de mortalidade elevados. “Vimos ao mundo pela bacia e dele vamos pelo colo do fémur”, reflecte a atitude derrotista com que se encaravam, em 1955, as fracturas do colo do fémur. O desenvolvimento de novas técnicas cirúrgicas e de novos implantes veio melhorar substancialmente o prognóstico destas fracturas, que são hoje encaradas como uma patologia que permite uma recuperação nalguns casos total, restituindo o doente à sua vida social anterior à fractura. Esta publicação, dirigida a todos os profissionais directamente envolvidos no tratamento dos doentes com fractura da extremidade proximal do fémur, tem como objectivos estabelecer recomendações para intervenção terapêutica e alertar para princípios básicos e normas de procedimento que facilitem um tratamento ainda mais correcto e eficaz. O tratamento hospitalar, com particular relevância para a intervenção cirúrgica, é fundamental, mas, se não houver um grande empenhamento de uma equipa multi-interdisciplinar, o seu sucesso poderá estar em risco. A aplicação das recomendações agora compiladas, que certamente já são do conhecimento da grande maioria dos profissionais, necessitará do empenhamento de todas as instituições e de todos os profissionais, para que, com uma redução dos custos ou sem agravamento dos mesmos, se possa obter uma optimização dos resultados.

A marcha assegura uma progressão do corpo, compatível com o equilíbrio dinâmico e adaptada a potenciais factores destabilizadores, de um ponto de vista antecipatório, através de sinergias coordenadas entre os MSs, o tronco e os MIs. O tronco inferior tem um papel preponderante na marcha, sobretudo na estabilização necessária durante a fase de apoio. Esta actividade implica mobilidade pélvica e alongamento activo dos abdominais para conseguir a relação comprimento-tensão muscular óptima entre quadricípite e isquiotibiais, permitindo uma correcta sequência, timing e amplitude de activação. Nas crianças com alterações neuromotoras existem alterações no controlo do movimento e na estrutura do próprio movimento, alterando todo este processo. Como tal, este estudo tem como principal objectivo determinar a influência da actividade do tronco inferior na activação muscular proximal durante a fase de apoio da marcha, em crianças com quadro motor de diplegia, caracterizada por uma dificuldade na relação entre os membros e entre estes e o tronco. Para responder a este objectivo realizou-se um estudo de série de casos, com 2 crianças com quadro motor de diplegia. Efectuou-se EMG dos músculos abdominais, quadricípite e isquiotibiais e análise de imagem (para amplitude da CF) durante a marcha, em ambos os membros e em dois momentos de avaliação, separados por 2 meses, nos quais se realizou um protocolo de intervenção terapêutica adequado a cada caso. Os resultados indicam que a variação de amplitude da CF desde a fase de ataque ao solo à fase média de apoio é aproximadamente igual em M0e M1; concretamente, a amplitude inicial é inferior à de referência (pouca flexão) (melhor em M0) e a amplitude final é superior à de referência (pouca extensão) (melhor em M1). Estes resultados são idênticos em ambos os casos. Na EMG verificou-se uma actividade mais global e sincronizada de todos os músculos, mantendo-se aproximadamente a mesma percentagem de activação em M1, sobretudo no caso 1. No caso 2 verificou-se uma maior eficiência na variação da percentagem de activação dos abdominais, em M1, e dos isquiotibiais, à direita. Em conclusão, pode dizer-se que, em crianças com alterações neuromotoras (quadro motor de diplegia), uma actividade mais eficiente e sincronizada no tempo do tronco inferior, nomeadamente dos abdominais, contribui para uma maior capacidade de extensão da CF, durante a fase de apoio.

This work aims at investigating the impact of treating breast cancer using different radiation therapy (RT) techniques – forwardly-planned intensity-modulated, f-IMRT, inversely-planned IMRT and dynamic conformal arc (DCART) RT – and their effects on the whole-breast irradiation and in the undesirable irradiation of the surrounding healthy tissues. Two algorithms of iPlan BrainLAB treatment planning system were compared: Pencil Beam Convolution (PBC) and commercial Monte Carlo (iMC). Seven left-sided breast patients submitted to breast-conserving surgery were enrolled in the study. For each patient, four RT techniques – f-IMRT, IMRT using 2-fields and 5-fields (IMRT2 and IMRT5, respectively) and DCART – were applied. The dose distributions in the planned target volume (PTV) and the dose to the organs at risk (OAR) were compared analyzing dose–volume histograms; further statistical analysis was performed using IBM SPSS v20 software. For PBC, all techniques provided adequate coverage of the PTV. However, statistically significant dose differences were observed between the techniques, in the PTV, OAR and also in the pattern of dose distribution spreading into normal tissues. IMRT5 and DCART spread low doses into greater volumes of normal tissue, right breast, right lung and heart than tangential techniques. However, IMRT5 plans improved distributions for the PTV, exhibiting better conformity and homogeneity in target and reduced high dose percentages in ipsilateral OAR. DCART did not present advantages over any of the techniques investigated. Differences were also found comparing the calculation algorithms: PBC estimated higher doses for the PTV, ipsilateral lung and heart than the iMC algorithm predicted.

OBJETIVO: Analisar os fatores associados à mortalidade em pacientes para tratamento da fratura proximal do fêmur e as diferentes formas de ajuste de risco. MÉTODOS: Estudo de coorte não-concorrente com 390 pacientes com 50 anos ou mais. Os pacientes foram identificados no Sistema de Informações Hospitalares do Sistema Único de Saúde, internados para correção cirúrgica da fratura proximal do fêmur em um hospital universitário do município do Rio de Janeiro, entre 1995 e 2000. Foram levantados dados complementares nos prontuários desses pacientes e elaborados modelos de regressão logística para a chance de óbito em 90 dias após a data da admissão, segundo perfil dos pacientes e do tratamento. Foram calculados índices de classificação da gravidade do paciente. RESULTADOS: A taxa de mortalidade foi de 7,4% e os fatores que tiveram efeito sobre o óbito foram: idade (OR=1,06; IC 95%: 1,02;1,11), quantidade de comorbidades (OR=1,44; IC 95%: 1,12;1,69), índice de comorbidade de Charlson acima de 2 (OR=6,67; IC 95%: 2,98;22,16) e intervalo de tempo para a realização da cirurgia (OR=1,04; IC 95%:1,02;1,07). CONCLUSÕES: A quantidade de comorbidades e o índice de comorbidade de Charlson auxiliaram na predição da mortalidade.

Introduction: Image resizing is a normal feature incorporated into the Nuclear Medicine digital imaging. Upsampling is done by manufacturers to adequately fit more the acquired images on the display screen and it is applied when there is a need to increase - or decrease - the total number of pixels. This paper pretends to compare the “hqnx” and the “nxSaI” magnification algorithms with two interpolation algorithms – “nearest neighbor” and “bicubic interpolation” – in the image upsampling operations. Material and Methods: Three distinct Nuclear Medicine images were enlarged 2 and 4 times with the different digital image resizing algorithms (nearest neighbor, bicubic interpolation nxSaI and hqnx). To evaluate the pixel’s changes between the different output images, 3D whole image plot profiles and surface plots were used as an addition to the visual approach in the 4x upsampled images. Results: In the 2x enlarged images the visual differences were not so noteworthy. Although, it was clearly noticed that bicubic interpolation presented the best results. In the 4x enlarged images the differences were significant, with the bicubic interpolated images presenting the best results. Hqnx resized images presented better quality than 4xSaI and nearest neighbor interpolated images, however, its intense “halo effect” affects greatly the definition and boundaries of the image contents. Conclusion: The hqnx and the nxSaI algorithms were designed for images with clear edges and so its use in Nuclear Medicine images is obviously inadequate. Bicubic interpolation seems, from the algorithms studied, the most suitable and its each day wider applications seem to show it, being assumed as a multi-image type efficient algorithm.
