Objective: The objective of this paper was to verify if the final irrigation at 17% EDTA, separately or followed by chlorhexidine digluconate at 2% interferes on the apical marginal leakage in root canal overfilling, due to the use of two root canal filling materials (Sealer 26TM or SealapexTM). Methods: Forty lower incisors extracted, with a single root canal, were biomechanically prepared, at 2.0mm beyond the radicular apex, with ProTaper SystemTM, finishing it with the F3 instrument. Irrigation was accomplished with 1.0 mL NaOCl at 2.5% at each change of equipment caliper and, in the end, with 5.0 mL normal saline solution. After achieving this procedure, the foraminal channels was standardized with a file K 25 until its DO appears in the foraminal opening. From this moment, the teeth were subdivided into two groups of 20 specimens each, because of the final irrigation method used: I – irrigation with EDTA at 17% for 3min and II – identical protocol, thus, after EDTA aspiration at17%, it was again irrigated with chlorhexidine digluconate at 2%, also kept intra-canal for 3min. Each group was again sub-divided into two sub-groups (ten teeth each), according to the cement used (Sealer 26TM or SealapexTM), and filled by using a single gutta-percha F1 (ProTaperTM, Universal Filling), in such a manner that it goes 2.0 mm beyond the radicular apex. Soon after that, the teeth were immersed in Rhodamine BTM for 72h, vacuum and after the roots have been bucolingually grooved, the leakages marked were measured with program Image ToolTM. Results and Conclusions: The leakage magnitude was similar among themselves (p > 0.05), except for group Sealapex®, thus the final use with chlorhexidine reduced apical leakage (p < 0.05).

jObjective: The objective of this study was to evaluate the thermoplasticity of the gutta-percha cones used for root canal obturation: Dentsply convencional (Dentsply Ind. e Com., Petrópolis, RJ, Brazil), Dentsply 0.04, (Dentsply Ind. e Com., Petrópolis, RJ, Brazil), Antaeos 0.04 (VDW GmbH, Munich, Germany), ProTaper (Dentsply Ind. e Com., Petrópolis, RJ, Brazil) and Alfa 0.06 (VDW GmbH, Munich, Germany). Material and method: Specimens of each material (n = 10) were prepared after heating. After 24 hours were again heated to 70 °C and placed between two glass plates and subjected to a compression of 5 kg for 2 minutes. The scanned image of the groups before and after the compression was analyzed with the software Image Tool (UTHSCSA Image Tool for Windows version 3.0, San Antonio, TX, USA). The flow capacity of the different materials was determined by the difference between the initial and final area of each specimen. The results were submitted to ANOVA and Tukey test with significance level of 5%. Result: Among the material evaluated, the gutta-percha cones Alpha 0.06 and ProTaper presented more thermoplasticity than other groups (p < 0.05). The Dentsply gutta-percha presented thermoplasticity intermediate. The groups Dentsply 0.04 and 0.04 showed lower thermoplasticity after heating (p > 0.05). Conclusion: The gutta-percha present different property of thermoplasticity, which is important to selection of thermoplastic obturation techniques.

Objective: The aim of this study was to evaluate the action of methyl salicylate in dissolving Resilon and gutta-percha. Material and method: Ten discs of Resilon, Alpha gutta-percha and ProTaper gutta-percha were prepared (10 mm diameter × 1 mm thick) and kept at 37 °C for 48 hours. Each sample was weighed in a balance of accuracy determining the initial mass. The specimens were immersed in methyl salicylate or xylene (control) for 5 minutes, rinsed in water for 30 minutes, dried and subsequently weighed in 24 hours intervals until the stabilization of weight (final mass). The solvent capacity was expressed by the difference between the final and initial mass. The data were analyzed by using ANOVA and Tukey test with significance level of 5%. Result: The results showed that the methyl salicylate was more effective on Resilon than on gutta-percha (p < 0.05). Methyl salicylate and xylene had similar solvent action on Resilon (p > 0.05). The xylene showed better performance on gutta-percha than methyl salicylate (p < 0.05). Conclusion: It was concluded that methyl salicylate did not show solvent action on gutta-percha, however it showed effectiveness in dissolution of Resilon. Thus methyl salicylate can be a possible alternative to the use of xylene during endodontic retreatment of teeth obturated with Resilon.

The aim of the study was to evaluate the photodynamic therapy (PDT) effect on root canals contaminated with Enterococcus faecalis using a light emitting diode (LED) light and a curcumin solution (CUR) as photosensitizer (PS). Eighty root canals from uniradicular human teeth were prepared with Protaper Universal rotary system and contaminated with E. faecalis for 21 days. They were divided as: GIa-PDT (CUR, pre-irradiation for 5 + 5 min of irradiation); GIb-PDT (CUR, pre-irradiation for 5 + 10 min of irradiation); GIIa-(CUR, pre-irradiation for 5 + 5 min without irradiation); GIIb-(CUR pre-irradiation for 5 + 10 min of irradiation); GIIIa-(physiological solution and irradiation for 5 min); and GIIIb-(physiological solution and irradiation for 10 min); positive and negative control groups. Collections from root canals were made at time intervals of 21 days after contamination, immediately after treatment, and 7 days after treatment, and submitted to colony forming units per milliter (CFU mL-1) counts. The data were submitted to ANOVA and Tukey multiple comparison tests, at a level of significance of 5 %. In the immediate post-treatment collection, group GIa showed greater bacterial reduction in comparison with GIIa, GIIb, GIIIa, GIIIb, and positive control (P < 0.05). At 7 days post-treatment, GIa showed significant bacterial reduction only in comparison with GIIIa (P < 0.05). Curcumin as sensitizer was effective by 5 min LED irradiation but not by 10 min irradiation PDT using LED light, and curcumin as PS was not effective in eliminating E. faecalis. No difference was observed for periods of irradiation.

Root canal preparation may damage NiTi instruments resulting in wear and deformation. The aim of this study was to make a comparative evaluation of the surface topography of the cervical third of four different rotary systems, before and after being used twelve times, in 1.440 resin blocks with simulated root canals with standardized 45 degrees curvatures, and analyzed by atomic force microscopy AFM. The blocks were divided into four groups and prepared according to the manufacturers recommendations: Group 1 - K3 (R); Group 2 - Protaper Universal (R); Group 3 - Twisted Files (R) and Group 4 - Biorace (R). After each preparation, the instruments were washed and autoclaved. A total of 240 instruments were selected, being 30 new instruments and 30 after having been used for the 12th time, from each group. These instruments were analyzed by AFM and for quantitative evaluation, the mean RMS (Root mean square) values of the cervical third of the specimens from the four groups were used. The result showed that all the rotary files used for the 12th time suffered wear with change in the topography of the cervical region of the active portion of the file (ANOVA p < 0.01). Classifying the specimens in increasing order, from the least to the greatest wear suffered, Group 3 (2.8993 nm) presented the least wear, followed by Group 4 (12.2520 nm), Group 1 (36.0043 nm) and lastly, Group 2 (59.8750 nm) with the largest amount of cervical surface wear. International Endodontic Journal, 45, 821828, 2012. Abstract Aim To evaluate the influence of thermomechanical compaction (Taggers hybrid technique THT) on the push-out strength of several root filling materials to root dentine. Methodology Root canals of eighty roots in human canines were prepared with the ProTaper system and filled with one of the following materials, using either lateral compaction (LC) (n = 40) or THT (n = 40): AH Plus/gutta-percha (GP) (n = 10), Sealer 26/GP (n = 10), Epiphany SE/Resilon (n = 10) and Epiphany SE/GP (n = 10). Three 2-mm-thick dentine slices were obtained from each third of each root. The root filling in the first slice was subjected to a push-out test to evaluate the bond strength of the materials to intraradicular dentine. Data (in MPa) were analysed using anova and post hoc Tukeys test (P < 0.05). Failure mode was determined at x25 magnification. The other two slices were prepared for scanning electron microscopy (SEM) to examine the surface of the filling materials. Results Lateral compaction (1.34 +/- 1.14 MPa) was associated with a significantly higher bond strength (P < 0.05) than the THT (0.97 +/- 0.88 MPa). AH Plus/GP (2.23 +/- 0.83 MPa) and Sealer 26/GP (1.86 +/- 0.50 MPa) had significantly higher bond strengths than the other materials and differed significantly from each other (P < 0.05). There was a significant difference (P < 0.05) between the coronal (1.36 +/- 1.15 MPa), middle (1.14 +/- 1.05 MPa) and apical thirds (0.95 +/- 0.83 MPa). Considering the technique and root filling material interaction, AH Plus/GP-LC was associated with the highest mean values (2.65 +/- 0.66 MPa) (P < 0.05). Sealer 26/GP-LC (2.10 +/- 0.46 MPa), AH Plus/GP-THT (1.81 +/- 0.78 MPa) and Sealer 26/GP-TH (1.63 +/- 0.44 MPa) had intermediate values that were not significantly different from each other (P > 0.05). Epiphany SE was associated with the lowest mean values (3.70 +/- 0.86 MPa) (P < 0.05), regardless of the root filling technique and type of solid material (cone). Adhesive failures predominated in the specimens filled with Epiphany SE, whilst mixed and cohesive failures were more frequent in those filled with AH Plus and Sealer 26, regardless of the root filling technique. SEM analysis revealed that LC produced a dense and well-compacted filling whilst the use of a hybrid thermomechanical technique resulted in the solid material (GP or Resilon) intermingled within sealer to form a nonhomogenous mass. Conclusion Lateral compaction was associated with higher bond strengths of the materials to intraradicular dentine than a hybrid technique using thermomechanical compaction. The greatest push-out strengths were obtained when the canals were filled with LC of AH Plus and GP cones.

The aim of this study was to evaluate the penetration of endodontic sealer into the dentin tubules, the integrity of the sealer layer perimeter, and the sealer area at the apical third after different filling techniques by confocal laser scanning microscopy (CLSM). Forty-five mandibular premolars were mechanically prepared with ProTaper files, until F5 file. Thereafter, they were filled with an epoxy-resin sealer (AH Plus) mixed with Rhodamine B dye (0.1% proportion) and allocated in three groups: Group 1, single master cone; Group 2, cold lateral compaction; and Group 3, Thermafil. For confocal laser scanning microscopy analysis, the specimens were transversely sectioned at 4 mm from the apex. The images at x10 and x40 were analyzed by Imagetool 3.0 software. Significant differences were not found among the three experimental groups according the dentin-impregnate area by the sealer (P = 0.68) and between the sealer and root canal perimeter (P = 0.18). However, root canal filling techniques were significantly different when apical sealer areas were compared (P = 0.001). Thermafil group showed smaller sealer areas (8.09%) while cold lateral compaction and gutta-percha master cone showed similar areas (17.37 and 21.18%, respectively). The dentin-impregnated area was not dependent on the root canal filling technique. Single master cone, cold lateral condensation and Thermafil techniques presented integrity of the sealer perimeter close to 100% and Thermafil resulted in a significantly thinner sealer layer. Introduction: A growing interest to preserve teeth into the mouth by patients resulted in the increasing number of endodontic retreatments, and when these happen, many different types of irritants are extruded through the foramen. Objective: This study analyzed in vitro the amount of debris extruded through the foramen using four instrumentation techniques during endodontic retreatment. Material and methods: Forty mesial-buccal roots of first molars were selected, instrumented with anatomical diameter up to size #30 ISO file and then obturated with gutta-percha and grossman sealer by lateral condensation. After, they were separated and randomly allocated into four groups with 10 teeth each for the endodontic retreatment procedure: G1 – conventional technique + solvent, G2 – conventional technique without solvent, G3 – ProTaper retreatment + solvent, G4 – ProTaper retreatment without solvent. In all groups, gutta-percha in the coronal portion was removed by using size 1-3 Gates Glidden drills. All teeth were irrigated with distilled water. The debris extruded through the foramen were collected and weighed by an analytical balance. Results: Group 4 had the lowest average for material extrusion through the foramen followed by groups 2, 3 and 1. When Tukey test for statistical analysis was applied, no significant difference among groups were found (p = 0.5664). Conclusion: We conclude that all instrumentation techniques used in this study produced debris which goes beyond the foramen.

Introdução: O trabalho elaborado desenvolve o tema seguinte: Novas tecnologias associadas ao retratamento endodôntico não cirúrgico. Desde o início da medicina dentária que somos deparados com o insucesso nos tratamentos realizados, na endodôntia sendo um acontecimento regular na prática diária. Quando o tratamento Endodontico não cirúrgico não é eficaz na resolução de patologias pulpares e ocorre recidiva da patologia, é necessário a realização do retratamento endodôntico não cirúrgico. Uma vez que o retratamento endodôntico, respeita os mesmos princípios do tratamento Endodontico, sendo estes a desinfeção do sistema de canais radiculares, a sua instrumentação e obturação. Esta prática está indicada por vários motivos, anatómicos, microbiológicos, erros de instrumentação, erros de obturação e as próprias limitações dos materiais. Objetivos: Esta dissertação tem como objetivo analisar, e verificar as razões que levam a necessidade da realização de retratamentos endodônticos não cirúrgicos, aos métodos utilizados na realização de retratamentos comparando-os entre si. Tendo sido realizada uma revisão bibliográfica de modo a verificar: as causas de insucesso, limitações dos materiais, técnicas de obturação, agentes químicos e sistemas de instrumentação. Materiais e Métodos: Para a obtenção da informação necessária para a elaboração da presente dissertação, foi realizada uma pesquiza bibliográfica nas bases de dados da Pubmed, B-on, Scielo, Science Direct e no Google Académico. Através das seguintes palavras-chave: “Root canal treatment”, “Endodontic sucess”, “Endodontic retreatment”, “Endodontic Failure causes”, “Root canal retreatment materials”, “Endodontic retreatment metods”, “Chloroform”, “ProTaper, Reciproc”, “Haloten”, “Orange oil”, “Eucaliptol”, “Ultrassonic instrumentation”, “obturation material”, “root filling”. Conclusão: No trabalho realizado é possível concluir que o insucesso tem múltiplas causas, que hoje em dia existem novos métodos e técnicas que nos permitem a resolução das falhas nos tratamentos primários, sendo que estes novos métodos e técnicas se revelaram mais eficazes que os tradicionais, demonstrando uma maior probabilidade de eliminação dos fatores causais das reinfeções.

Introdução: A extrusão apical detritos (EAD) consequência indesejável da instrumentação canalar pode ser associada a dor/edema, podendo atrasar a cicatrização periapical. O nosso trabalho teve como objectivo avaliar e quantificar a EAD em canais instrumentados por sistemas de instrumentação rotatória contínua e reciprocante. Materiais e Métodos: 80 dentes monocanalares sem tratamento endodôntico prévio foram aleatoriamente divididos em 4 grupos (n=20): One Shape® Protaper® NEXT, Hyflex® EDM e WaveOne® Gold. Um tubo de Eppendorf (TdE) foi pesado antecipadamente numa balança analítica de precisão e com um dente inserido foi montado num dispositivo modificado, similar ao método descrito por Myers & Montgomery. Os canais foram instrumentados e irrigados com água destilada. Os dentes instrumentados foram removidos dos TdE e estes preenchidos com água destilada até perfazer 1,5ml, incubados a 70ºC durante cinco dias sendo pesados novamente, calculando a diferença entre o peso inicial e final determinando o peso dos detritos. Os dados foram analisados estatisticamente utilizando o IBM SPSS Statistics 22, considerando α=0,05. Efetuaram-se testes Kruskal-Wallis e post-hoc com ajustamento do ρ-value pelo método Dunn-Bonferroni. Resultados: Houve EAD em todas as técnicas de instrumentação. A análise estatística mostrou haver diferenças significativas na EAD entre as técnicas utilizadas (α=0,002). Entre as técnicas WaveOne® Gold e One Shape® (α=0,003), WaveOne® Gold e Protaper® NEXT (α=0,023) e WaveOne® Gold e Hyflex® EDM (α=0,028). Conclusões: A técnica One Shape® apresentou menor EAD e a técnica WaveOne® Gold com movimento reciprocante constitui maior fator de risco tendo apresentado maior EAD. Os resultados deste estudo indicam que os profissionais devem estar cientes para a EAD que pode ocorrer com cada instrumento, o que poderá servir de base para a selecção de um instrumento particular. Implicações clínicas: A escolha do sistema de instrumentação canalar influencia a extrusão de detritos.
