Multiparametric Magnetic Resonance Imaging has been increasingly used for detection, localization and staging of prostate cancer over the last years. It combines high-resolution T2 Weighted-Imaging and at least two functional techniques, which include Dynamic Contrast–Enhanced Magnetic Resonance Imaging, Diffusion-Weighted Imaging, and Magnetic Resonance Imaging Spectroscopy. Although the combined use of a pelvic phased-array and an Endorectal Coil is considered the state-of-the-art for Magnetic Resonance Imaging evaluation of prostate cancer, Endorectal Coil is only absolute mandatory for Magnetic Resonance Imaging Spectroscopy at 1.5 T. Sensitivity and specificity levels in cancer detection and localization have been improving with functional technique implementation, compared to T2 Weighted-Imaging alone. It has been particularly useful to evaluate patients with abnormal PSA and negative biopsy. Moreover, the information added by the functional techniques may correlate to cancer aggressiveness and therefore be useful to select patients for focal radiotherapy, prostate sparing surgery, focal ablative therapy and active surveillance. However, more studies are needed to compare the functional techniques and understand the advantages and disadvantages of each one. This article reviews the basic principles of prostatic mp-Magnetic Resonance Imaging, emphasizing its role on detection, staging and active surveillance of prostate cancer.

En este trabajo se describe la miniaturización del diseño de un desfasador de cuadratura mediante líneas de transmisión metamaterial en tecnología microstrip. Para conseguir la miniaturización del diseño se utiliza la topología Spiral Resonator (SR). Los desfasadores de cuadratura han sido diseñados, fabricados y medidos. Mediante este proceso se ha consiguido un dispositivo metamaterial que presenta una reducción de un 77% del tamaño respecto a las líneas convencionales.

The large spatial inhomogeneity in transmit B, field (B-1(+)) observable in human MR images at hi h static magnetic fields (B-0) severely impairs image quality. To overcome this effect in brain T-1-weighted images the, MPRAGE sequence was modified to generate two different images at different inversion times MP2RAGE By combining the two images in a novel fashion, it was possible to create T-1-weigthed images where the result image was free of proton density contrast, T-2* contrast, reception bias field, and, to first order transmit field inhomogeneity. MP2RAGE sequence parameters were optimized using Bloch equations to maximize contrast-to-noise ratio per unit of time between brain tissues and minimize the effect of B-1(+) variations through space. Images of high anatomical quality and excellent brain tissue differentiation suitable for applications such as segmentation and voxel-based morphometry were obtained at 3 and 7 T. From such T-1-weighted images, acquired within 12 min, high-resolution 3D T-1 maps were routinely calculated at 7 T with sub-millimeter voxel resolution (0.65-0.85 mm isotropic). T-1 maps were validated in phantom experiments. In humans, the T, values obtained at 7 T were 1.15 +/- 0.06 s for white matter (WM) and 1.92 +/- 0.16 s for grey matter (GM), in good agreement with literature values obtained at lower spatial resolution. At 3 T, where whole-brain acquisitions with 1 mm isotropic voxels were acquired in 8 min the T-1 values obtained (0.81 +/- 0.03 S for WM and 1.35 +/- 0.05 for GM) were once again found to be in very good agreement with values in the literature. The Mental Health Act 2001 replaces the Mental Treatment Acts 1945-61 which provided the statutory framework for the detention of people with mental illness and the administration of psychiatric services for over 50 years.The 2001 Act was introduced on a phased basis to allow for the necessary preparatory work to be undertaken. In March 2002 sections 1 to 5, 7, and 31 to 55 were commenced with effect from the 5 April 2002 (Establishment Day). This allowed for the establishment of the Mental Health Commission and the Inspector of Mental Health Services to replace the Inspector of Mental Hospitals. The Programme for Government 2007-2012 states that '[a]ppropriate fiscal instruments, including a carbon levy, will be phased in on a revenue-neutral basis over the lifetime of this Government.' The terms of reference of the Commission on Taxation repeats the commitment to introduce measures to further lower carbon emissions and to phase in on a revenue neutral basis appropriate fiscal measures including a carbon levy over the lifetime of the Government and invites the Commission to [i]nvestigate fiscal measures to protect and enhance the environment including the introduction of a carbon tax. This paper presents thoughts and considerations about such a carbon tax. It discusses selected design issues, and presents a preliminary impact assessment for what the authors think is a reasonable design. More specifically, It addresses ten questions: 1. Why impose a carbon tax? 2. What level should the tax be? 3. Who should be taxed? 4. What is the expected revenue? 5. What to do with the revenue? 6. What are the macro-economic implications? 7. What are the effects on emissions? 8. What are the effects on income distribution? 9. How to tax internationally traded goods and services? 10. What about fuel tourism? On some of these questions, it presents arguments and evidence. Other questions call for further research. OBJECTIVE: To evaluate a transverse electromagnetic (TEM), a circularly polarized (CP) (birdcage), and a 12-channel phased array head coil at the clinical field strength of B0 = 3T in terms of signal-to-noise ratio (SNR), signal homogeneity, and maps of the effective flip angle alpha. MATERIALS AND METHODS: SNR measurements were performed on low flip angle gradient echo images. In addition, flip angle maps were generated for alpha(nominal) = 30 degrees using the double angle method. These evaluation steps were performed on phantom and human brain data acquired with each coil. Moreover, the signal intensity variation was computed for phantom data using five different regions of interest. RESULTS: In terms of SNR, the TEM coil performs slightly better than the CP coil, but is second to the smaller 12-channel coil for human data. As expected, both the TEM and the CP coils show superior image intensity homogeneity than the 12-channel coil, and achieve larger mean effective flip angles than the combination of body and 12-channel coil with reduced radio frequency power deposition. CONCLUSION: At 3T the benefits of TEM coil design over conventional lumped element(s) coil design start to emerge, though the phased array coil retains an advantage with respect to SNR performance.

Tämä insinöörityö tehtiin ABB Oy, Drivesin Product AC -tulosyksikön tuotekehitysosastolle Helsingissä. Työssä kehitettiin taajuusmuuttajien suorituskyvyn automaattinen testausympäristö. ABB:n taajuusmuuttajien suorituskykytestejä ei ole aikaisemmin automatisoitu. Testit on tehty käsin ja niiden suorittamiseen ja tulosten käsittelyyn on kulunut paljon aikaa. Automaattisella testauksella pyrittiin testien suorittamiseen ja tulosten käsittelyyn kuluvan ajan huomattavaan pienentymiseen. Työssä ei ollut tarkoituksena tehdä suorituskykytestejä vaan kehittää automaattinen testausympäristö eli suorituskykytestipenkki, jossa suorituskykytestit on mahdollista suorittaa. Työssä keskityttiin taajuusmuuttajan nopeus- ja momenttisäätäjien suorituskykyyn. Työ toteutettiin suunnittelu- ja ohjelmointityönä. Testausympäristön laitteisto perustuu ABB:n tuotekehityslaboratorioiden olemassaoleviin testipaikkoihin. Testausympäristössä käytetään taajuusmuuttajien lisäksi pääasiassa kolmivaiheisia oikosulkumoottoreita. Lisäksi laitteistoon kuuluu ACS800-sarjan taajuusmuuttaja kuormakäyttönä, momenttianturi ja takometri eli kierrosnopeusmittari. Ohjelmointi tehtiin National Instrumentsin LabVIEW-ohjelmointiympäristön versiolla 8.0. Testausympäristön käyttöliittymänä toimii saman yrityksen TestStand-testausohjelmiston versio 3.5. Testattavien taajuusmuuttajien ohjausta ja momenttianturin lukemista varten ohjelmoitiin virtuaali-instrumentteja. Virtuaali-instrumentteja kutsutaan TestStand-testisekvensseistä. Testisekvenssit luodaan TestStandin sekvenssieditorilla ja suoritetaan sekvenssieditorissa tai operaattorin käyttöliittymässä. Työn tuloksena syntyi taajuusmuuttajien suorituskyvyn automaattinen testausympäristö. Testausympäristöä voidaan hyödyntää sekä nykyisen että seuraavan sukupolven taajuusmuuttajien testauksessa. Sillä on mahdollista suorittaa yleisimmät taajuusmuuttajien suorituskykytestit, kuten nopeus- ja momenttisäätöjen staattinen ja dynaaminen tarkkuus, hyvin kattavasti. Testit voidaan automaattisesti suorittaa koko testikäytön sallimalla pyörimisnopeus- ja kuormitusalueella. Näytteenottotaajuus voi olla enintään 1 kHz luettaessa pyörimisnopeutta ACS800-sarjan taajuusmuuttajan kautta ja momenttianturia samanaikaisesti. Virtuaali-instrumenteista koostuvia testisekvenssejä voidaan vapaasti muokata ja kehittää testejä edelleen tai luoda kokonaan uusia testejä. Testausympäristö perustuu teollisuudessa yleisesti käytettyihin ohjelmistoihin ja tarjoaa hyvät mahdollisuudet jatkokehitykselle.

Astute control of brain activity states is critical for adaptive behaviours and survival. In mammals and birds, electroencephalographic recordings reveal alternating states of wakefulness, slow wave sleep and paradoxical sleep (or rapid eye movement sleep). This control is profoundly impaired in narcolepsy with cataplexy, a disease resulting from the loss of orexin/hypocretin neurotransmitter signalling in the brain. Narcolepsy with cataplexy is characterized by irresistible bouts of sleep during the day, sleep fragmentation during the night and episodes of cataplexy, a sudden loss of muscle tone while awake and experiencing emotions. The neural mechanisms underlying cataplexy are unknown, but commonly thought to involve those of rapid eye movement-sleep atonia, and cataplexy typically is considered as a rapid eye movement sleep disorder. Here we reassess cataplexy in hypocretin (Hcrt, also known as orexin) gene knockout mice. Using a novel video/electroencephalogram double-blind scoring method, we show that cataplexy is not a state per se, as believed previously, but a dynamic, multi-phased process involving a reproducible progression of states. A knockout-specific state and a stereotypical paroxysmal event were introduced to account for signals and electroencephalogram spectral characteristics not seen in wild-type littermates. Cataplexy almost invariably started with a brief phase of wake-like electroencephalogram, followed by a phase featuring high-amplitude irregular theta oscillations, defining an activity profile distinct from paradoxical sleep, referred to as cataplexy-associated state and in the course of which 1.5-2 s high-amplitude, highly regular, hypersynchronous paroxysmal theta bursts (∼7 Hz) occurred. In contrast to cataplexy onset, exit from cataplexy did not show a predictable sequence of activities. Altogether, these data contradict the hypothesis that cataplexy is a state similar to paradoxical sleep, even if long cataplexies may evolve into paradoxical sleep. Although not exclusive to overt cataplexy, cataplexy-associated state and hypersynchronous paroxysmal theta activities are highly enriched during cataplexy in hypocretin/orexin knockout mice. Their occurrence in an independent narcolepsy mouse model, the orexin/ataxin 3 transgenic mouse, undergoing loss of orexin neurons, was confirmed. Importantly, we document for the first time similar paroxysmal theta hypersynchronies (∼4 Hz) during cataplexy in narcoleptic children. Lastly, we show by deep recordings in mice that the cataplexy-associated state and hypersynchronous paroxysmal theta activities are independent of hippocampal theta and involve the frontal cortex. Cataplexy hypersynchronous paroxysmal theta bursts may represent medial prefrontal activity, associated in humans and rodents with reward-driven motor impulse, planning and conflict monitoring.
