Pertussis, or whopping cough, is an upper respiratory tract infection caused by Bordetella pertussis. It has long been a concern in pediatric populations, leading to aggressive vaccination strategies to help decrease pediatric disease. In recent years, recognition of pertussis infection in adult populations has increased, leading to more frequent diagnosis and recommendations for booster immunizations in the adult population. Early recognition and treatment as well as vaccination will help reduce the current increase in this disease. © 2013 Elsevier Inc.

We estimated the sensitivity, i.e., the proportion of all cases of adverse events following immunization (AEFIs) reported to the Brazilian passive surveillance for adverse events following immunization (PSAEFI) with the diphtheria-tetanus-whole-cell pertussis-Haemophilus influenzae type b (DTwP-Hib) vaccine, as well as investigating factors associated with AEFIs reporting. During 2003–2004, 8303 AEFIs associated with DTwP-Hib were reported; hypotonic-hyporesponsive episodes (HHEs), fever and convulsions being the most common. Cure without sequel was achieved in 98.4 per cent of the cases. The mean sensitivity of the PSAEFI was 22.3 per cent and 31.6 per cent, respectively, for HHE and convulsions, varying widely among states. Reporting rates correlated positively with the Human Development Index and coverage of adequate prenatal care, correlating negatively with infant mortality rates. Quality of life indicators and the degree of organization of health services are associated with greater PSAEFI sensitivity. In addition to consistently describing the principal AEFIs, PSAEFI showed the DTwP/Hib vaccine to be safe and allayed public fears related to its use

Short chain fatty acids (SCFAs) have recently attracted attention as potential mediators of the effects of gut microbiota on intestinal inflammation. Some of these effects have been suggested to occur through the direct actions of SCFAs on the GPR43 receptor in neutrophils, though the precise role of this receptor in neutrophil activation is still unclear. We show that mouse bone marrow derived neutrophils (BMNs) can chemotax effectively through polycarbonate filters towards a source of acetate, propionate or butyrate. Moreover, we show that BMNs move with good speed and directionality towards a source of propionate in an EZ-Taxiscan chamber coated with fibrinogen. These effects of SCFAs were mimicked by low concentrations of the synthetic GPR43 agonist phenylacetamide-1 and were abolished in GPR43(-/-) BMNs. SCFAs and phenylacetamide-1 also elicited GPR43-dependent activation of PKB, p38 and ERK and these responses were sensitive to pertussis toxin, indicating a role for Gi proteins. Phenylacetamide-1 also elicited rapid and transient activation of Rac1/2 GTPases and phosphorylation of ribosomal protein S6. Genetic and pharmacological intervention identified important roles for PI3K gamma, Rac2, p38 and ERK, but not mTOR, in GPR43-dependent chemotaxis. These results identify GPR43 as a bona fide chemotactic receptor for neutrophils in vitro and start to define important elements in its signal transduction pathways.

In human heart there is now evidence for the involvement of four beta-adrenoceptor populations, three identical to the recombinant beta(1)-, beta(2)- and beta(3)-adrenoceptors, and a fourth as yet uncloned putative beta-adrenoceptor population, which we designate provisionally as the cardiac putative beta(4)-adrenoceptor. This review described novel features of beta-adrenoceptors as modulators of cardiac systolic and diastolic function. We also discuss evidence for modulation by unoccupied beta(1)- and beta(2)-adrenoceptors. Human cardiac and recombinant beta(1)- and beta(2)-adrenoceptors are both mainly coupled to adenylyl cyclase through Gs protein, the latter more tightly than the former. Activation of both human beta(1)- and beta(2)-adrenoceptors not only increases cardiac force during systole but also hastens relaxation through cyclic AMP-dependent phosphorylation of phospholamban and troponin I, thereby facilitating diastolic function. Furthermore, both beta(1) and beta(2)-adrenoceptors can mediate experimental arrhythmias in human cardiac preparations elicited by noradrenaline and adrenaline. Human ventricular beta(3)-adrenoceptors appear to be coupled to a pertussis toxin-sensitive protein (Gi?). beta(3)-Adrenoceptor-selective agonists shorten the action potential and cause cardiodepression, suggesting direct coupling of a Gi protein to a K+ channel. In a variety of species, including man, cardiac putative beta(4)-adrenoceptors mediate cardiostimulant effects of non-conventional partial agonists, i.e. high affinity beta(1)- and beta(2)-adrenoceptor blockers that cause agonist effects at concentrations considerably higher than those that block these receptors. Putative beta(4)-adrenoceptors appear to be coupled positively to a cyclic AMP-dependent cascade and can undergo some desensitisation.

The macro phage-derived neutrophil chemotactic factor (MNCF) is an alpha-galactoside-binding lectin, known to induce dexamethasone-insensitive neutrophil recruitment. We further characterized MNCF effects on neutrophils and showed that it shares with TNF-alpha the ability to delay apoptosis and to trigger degranulation. MNCF and TNF-alpha effects show similar kinetics and involve Src kinases and MAPKinases dependent pathways. They were, however, clearly distinguished, since the soluble TNF-receptor etanercept prevented TNF but not MNCF effects, while melibiose disaccharide inhibited MNCF but not TNF effects. Absorption of MNCF on detoxi-gel did not alter its properties, precluding an LPS contamination effect. By contrast, galectin-3 required LPS to activate neutrophils. Specific antibodies allowed to further demonstrate that MNCF and galectin-3 are two distinct molecules. Finally, MNCF- and IL-8-induced neutrophil activation differed by their kinetic and sensitivity to pertussis toxin. In conclusion, MNCF is a distinct neutrophil agonist, with pro-inflammatory activities involving its carbohydrate recognition domain. (C) 2008 Elsevier Inc. All rights reserved.

1 The functional coupling of B-2-adrenoceptors (beta (2)-ARs) to murine L-type Ca2+ current (I-Ca(L)) was investigated with two different approaches. The beta (2)-AR signalling cascade was activated either with the beta (2)-AR selective agonist zinterol (myocytes from wild-type mice), or by spontaneously active, unoccupied beta (2)-ARs (myocytes from TG4 mice with 435 fold overexpression of human beta (2)-ARs). Ca2+ and Ba2+ currents were recorded in the whole-cell and cell-attached configuration of the patch- clamp technique, respectively. 2 Zinterol (10 muM) significantly increased I-Ca(L) amplitude of wild-type myocytes by 19+/-5%, and this effect was markedly enhanced after inactivation of Gi-proteins with pertussis-toxin (PTX; 76+/-13% increase). However, the effect of zinterol was entirely mediated by the beta (1)-AR subtype, since it was blocked by the beta (1)-AR selective antagonist CGP 20712A (300 nM). The beta (2)-AR selective antagonist ICI 118,551 (50 nM) did not affect the response of I-Ca(L) to zinterol. 3 In myocytes with beta (2)-AR overexpression I-Ca(L) was not stimulated by the activated signalling cascade. On the contrary, I-Ca(L) was lower in TG4 myocytes and a significant reduction of single-channel activity was identified as a reason for the lower whole-cell I-Ca(L). The beta (2)-AR inverse agonist ICI 118,551 did not further decrease I-Ca(L). PTX-treatment increased current amplitude to values found in control myocytes. 4 In conclusion, there is no evidence for beta (2)-AR mediated increases of I-Ca(L) in wild-type mouse ventricular myocytes. Inactivation of Gi-proteins does not unmask beta (2)-AR responses to zinterol, but augments beta (1)-AR mediated increases of I-Ca(L). In the mouse model of beta (2)-AR overexpression I-Ca(L) is reduced due to tonic activation of Gi-proteins.

Purinergic stimulation of airway epithelial cells induces Cl- secretion and modulates Na+ absorption by an unknown mechanism. To gain insight into this mechanism, we used a perfused micro-Ussing chamber to assess transepithelial voltage (V-te) and amiloride-sensitive short-circuit current (Isc-Amil) in mouse trachea. Exposure to apical ATP or UTP (each 100 mumol/l) caused a large initial increase in lumen negative V-te and I-sc corresponding to a transient Cl- secretion, while basolateral application of ATP/UTP induced only a small secretory response. Luminal, but not basolateral, application of nucleotides was followed by a sustained and reversible inhibition of Isc-Amil that was independent of extracellular Ca2+ or activation of protein kinase C and was not induced by carbachol (100 mumol/l) or the Ca2+ ionophore ionomycin (1 mumol/l). Removal of extracellular Cl- or exposure to 200 muM DIDS reduced UTP-mediated inhibition of Isc-Amil Substantially. The phospholipase inhibitor U73122 (10 mumol/l) and pertussis toxin (PTX 200 ng/ml) both attenuated UTP-induced Cl- secretion and inhibition of Isc-Amil. Taken together, these data imply a contribution of Cl- conductance and PTX-sensitive G proteins to nucleotide-dependent inhibition of the amiloride-sensitive Na+ current in the mouse trachea.

A partir de 255 crianças com sintomas de coqueluche atendidas no Hospital de Isolamento Emílio Ribas de São Paulo, procedeu-se a pesquisa de bactérias do gênero Bordetella. Em 46 delas, determinou-se, também, a elevação dos níveis de anticorpos circulantes aglutinantes e fixadores de complemento contra a B. pertussis, aglutinantes contra a B. parapertussis e fixadores de complemento contra adenovirus. Por outro lado, relacionaram-se os resultados do exame bacteriológico com o período da doença, em semanas, e com alguns sintomas principais apresentados. Foram também relacionados os resultados das provas bacteriológica e sorológica com a confirmação clínica do diagnóstico. Foram discutidos, por outro lado, o valor e as limitações das provas bacteriológica e sorológica no diagnóstico laboratorial da coqueluche.

Os soros antitetânico, antidiftérico e antibotrópico-crotálico e, ainda, a vacina diftérica-pertussis-tetânica foram submetidos a diferentes intensidades de radiação gama, com os seguintes intuitos: verificação da resistência das atividades específicas destes produtos à ação dos raios gama; avaliação das possibilidades de utilização deste tipo de energia radiante para a esterilização de alguns soros hiperimunes heterólogos e vacinas, mais comumente utilizados em saúde pública. Os resultados obtidos, segundo os parâmetros empregados, mostraram a possibilidade de esterilização sem alterações das atividades específicas, de ordem biológica ou química, dos produtos experimentados.

Em 1991 avaliou-se a cobertura vacinal em crianças de 12 a 23 meses de idade no território de responsabilidade de um Posto de Atenção Primária à Saúde, na periferia da Zona Norte de Porto Alegre, RS, Brasil, cinco anos após sua implantação, com a finalidade de melhorar a qualidade das ações de saúde desenvolvidas no serviço. Foram investigadas todas as crianças através de um inquérito domiciliar, observando-se a carteira de vacinas e as informações da mãe. Em 1986, um inquérito inicial havia identificado uma cobertura vacinal inferior a 60% para cada uma das vacinas. A atual cobertura vacinal (doses comprovadas) para três doses da vacina DPT (Difteria, Pertussis e Tétano), três doses da Sabin (antipoliomielite), uma dose da anti-sarampo (VAS) e uma dose de BCG são, respectivamente 87, 89, 88 e 79%. Apesar das altas coberturas observadas por tipos de vacinas, quando se verificou para cada criança se o esquema básico do primeiro ano de vida estava completo (3 doses de DPT + 3 doses de Sabin + 1 dose de VAS + 1 dose de BCG), encontrou-se apenas 75% das crianças na citada situação. A cobertura vacinal é heterogênea dentro do território, sendo maior naquelas áreas caracterizadas por piores condições socioeconômicas, onde a equipe de saúde havia intensificado esforços. A comparação com o método administrativo de avaliação de cobertura, realizado mensalmente, mostrou a não-adequação desse, que subestimava a cobertura vacinal. Avaliou-se a situação vacinal das mães, para vacina antitetânica, e apenas 49% das crianças estavam protegidas contra o tétano neonatal. Os dados obtidos subsidiaram a imediata reestruturação das ações do programa, com vistas a atingir uma cobertura vacinal de 100%, e melhorar a qualidade das ações de saúde prestadas pela equipe.

The humoral and cellular immune responses as well as the resistance to infection with bloodstream forms of T. cruzi were studied in mice immunized with acidic antigenic fractions from parasite cytosol, F III and F IV, plus Bordetella pertussis as adjuvant. The immunization with F III induced positive ITH and DTH responses to homologous antigens. In mice immunized with F IV, the ITH was negative and four out of six animals presented positive DTH reactions. In both groups of mice the analysis of IgG aginst T. cruzi showed that the major isotype elicited was IgG1. Specific IgE was also detected in sera from F III immunized mice, thus confirming the presence of homocytothropic antibodies. The parasitemias reached by F III and F IV immunized mice after challenge were lower than those of the controls showing in this way a partial protection against the acute infection. The histological studies of heart and skeletal muscle performed two months after the infection revealed variable mononuclear infiltration in all infected mice despite immunization.

Em controlo de infecção é importante definir os tipos de precauções. Elas podem ser universais ou baseadas nas vias de transmissão. As precauções universais devem ser aplicadas a todos os doentes em todas as unidades de saúde e baseiam-se no princípio de que todos os doentes podem ter eventualmente doenças potencialmente transmissíveis desconhecidas do prestador de cuidados. São exemplo deste tipo de precaução o uso de luvas para manipulação de fluidos orgânicos. As precauções baseadas na via de transmissão pressupõem o conhecimento da via de transmissão dos agentes microbiológicos que provocam determinada doença. – via aérea, por gotículas ou por contacto. Cada uma destas vias implica cuidados, condições físicas e arquitectónicas diferentes. A transmissão por contacto é a mais frequente e, digamos também a mais fácil de cumprir. Define-se como o uso de luvas e bata no contacto com o doente. Um doente colonizado com uma bactéria multirresistente deve ser colocado em isolamento de contacto. Se possível este deve ser colocado em quarto privado. Se não for possível, doentes colonizados ou com doença provocada pelo mesmo agente devem estar no mesmo quarto. A precaução de transmissão por via aérea ocorre por disseminação de pequenas partículas residuais ou de pó contendo agentes infecciosos ou esporos. São exemplo desta via de transmissão a tuberculose, o sarampo e a varicela. O doente deve ser colocado em quarto de pressão negativa. A transmissão por gotículas ocorre quando as gotículas que contém microorganismos são projectadas a curta distância e depositadas nas superfícies próximas ou nas mucosas ou conjuntivas de outro doente que esteja perto. Agentes exemplo deste tipo de transmissão são a Bordetella pertussis, o Adenovírus, vírus da rubéola, a escarlatina. As precauções máximas implicam o uso de todas as precauções: universais, da via aérea, de gotículas e de contacto.

Introdução: Nos últimos 30 anos, em vários países, tem sido relatado um aumento da incidência de tosse convulsa, sobretudo em adolescentes e adultos, apesar das altas taxas de cobertura de imunização primária. Objectivos: Rever aspectos da epidemiologia da tosse convulsa, descrever algumas estratégias de controlo, com ênfase nas que incluem a utilização de vacinas com menores quantidades do toxóide diftérico e de alguns componentes pertussis (dTpa), e a avaliação da eficácia e efectividade destas vacinas. Métodos: Selecção de artigos relevantes, através da base de dados PubMed e sítios de acesso livre da internet, publicados entre 1991 e 2011. Resultados: As alterações do padrão epidemiológico são atribuídas, principalmente, à diminuição da imunidade ao longo dos anos, após a vacinação ou infecção natural. Os adolescentes e adultos foram identificados como importantes fontes de transmissão da doença para lactentes muito jovens que, uma vez não imunizados ou parcialmente imunizados, são mais vulneráveis às complicações relacionadas com a doença e apresentam maior mortalidade. A vacina (dTpa), formulada para o uso em adolescentes e adultos é segura e eficaz. A sua utilização também reduz a transmissão da tosse convulsa para os grupos etários com alto risco de complicações. Conclusão: A disponibilidade da dTpa oferece novas oportunidades para reduzir o impacto da tosse convulsa. A modificação das estratégias preventivas pode levar a um melhor controlo global da doença.

During July and August of 1968, a Health survey was conducted on the Ilha da Conceição, an area of Niterói containing approximately one thousand households. The survey was conducted by students from the Universidade Federal Fluminense and the University of Maryland, and was under the supervision of faculty of the Department of Tropical Medicine at U.F.F. and from the Department of Preventive Medicine at the University of Maryland, Baltimore, Maryland, U.S.A. The survey was focused on a 25 percent random sample of the households on the island. Information was obtained from a responsible adult at each Household for completion of a Health questionnaire. Physical measurements, as well as laboratory study information were obtained from, all children in these households. A number of environmental sanitation problems were identified on the Ilha da Conceição. In addition, the survey indicated that approximately half the children had not been adequately immunized against diphteria, pertussis and typhoid. Preventable communicable diseases were the major cause of reported deaths which had occurred in infants ou Household members. The Health of the population on the Ilha da Conceição could well be enhanced by the development of an intelligence system indicating the immunization status of all children in the area. In addition a Health education program for the residents could well be beneficial for improvement of sanitary conditions on the island, as well as maternity and well baby care.
