BACKGROUND: Mortality is increased after a hip fracture, and strategies that improve outcomes are needed. METHODS: In this randomized, double-blind, placebo-controlled trial, 1065 patients were assigned to receive yearly intravenous zoledronic acid (at a dose of 5 mg), and 1062 patients were assigned to receive placebo. The infusions were first administered within 90 days after surgical repair of a hip fracture. All patients received supplemental vitamin D and calcium. The median follow-up was 1.9 years. The primary end point was a new clinical fracture. RESULTS: The rates of any new clinical fracture were 8.6% in the zoledronic acid group and 13.9% in the placebo group, a 35% risk reduction (P = 0.001); the respective rates of a new clinical vertebral fracture were 1.7% and 3.8% (P = 0.02), and the respective rates of new nonvertebral fractures were 7.6% and 10.7% (P = 0.03). In the safety analysis, 101 of 1054 patients in the zoledronic acid group (9.6%) and 141 of 1057 patients in the placebo group (13.3%) died, a reduction of 28% in deaths from any cause in the zoledronic-acid group (P = 0.01). The most frequent adverse events in patients receiving zoledronic acid were pyrexia, myalgia, and bone and musculoskeletal pain. No cases of osteonecrosis of the jaw were reported, and no adverse effects on the healing of fractures were noted. The rates of renal and cardiovascular adverse events, including atrial fibrillation and stroke, were similar in the two groups. CONCLUSIONS: An annual infusion of zoledronic acid within 90 days after repair of a low-trauma hip fracture was associated with a reduction in the rate of new clinical fractures and improved survival. OBJECTIVES: The aim of this systematic review is to evaluate, analysing the dental literature, whether: * Patients on intravenous (IV) or oral bisphosphonates (BPs) can receive oral implant therapy and what could be the risk of developing bisphosphonate-related osteonecrosis of the jaw (BRONJ)? * Osseointegrated implants could be affected by BP therapy. MATERIAL AND METHODS: A Medline search was conducted and all publications fulfilling the inclusion and exclusion criteria from 1966 until December 2008 were included in the review. Moreover, the Cochrane Data Base of Systematic Reviews, and the Cochrane Central Register of Controlled Trials and EMBASE (from 1980 to December 2008) were searched for English-language articles published between 1966 and 2008. Literature search was completed by a hand research accessing the references cited in all identified publications. RESULTS: The literature search rendered only one prospective and three retrospective studies. The prospective controlled non-randomized clinical study followed patients with and without BP medication up to 36 months after implant therapy. The patients in the experimental group had been on oral BPs before implant therapy for periods ranging between 1 and 4 years. None of the patients developed BRONJ and implant outcome was not affected by the BP medication. The three selected retrospective studies (two case-controls and one case series) yielded very similar results. All have followed patients on oral BPs after implant therapy, with follow-up ranging between 2 and 4 years. BRONJ was never reported and implant survival rates ranged between 95% and 100%. The literature search on BRONJ including guidelines and recommendations found 59 papers, from which six were retrieved. Among the guidelines, there is a consensus on contraindicating implants in cancer patients under IV-BPs and not contraindicating dental implants in patients under oral-BPs for osteoporosis. CONCLUSIONS: From the analysis of the one prospective and the three retrospective series (217 patients), the placement of an implant may be considered a safe procedure in patients taking oral BPs for <5 years with regard to the occurrence of BRONJ since in these studies no BRONJ has been reported. Moreover, the intake of oral-BPs did not influence short-term (1-4 years) implant survival rates.

Introduction: Denosumab, a fully human anti-RANKL monoclonal antibody, reduces the incidence of skeletal-related events in patients with bone metastases from solid tumors. We present survival data for the subset of patients with lung cancer, participating in the phase 3 trial of denosumab versus zoledronic acid (ZA) in the treatment of bone metastases from solid tumors (except breast or prostate) or multiple myeloma. Methods: Patients were randomized 1:1 to receive monthly subcutaneous denosumab 120 mg or intravenous ZA 4 mg. An exploratory analysis, using Kaplan-Meier estimates and proportional hazards models, was performed for overall survival among patients with non-small-cell lung cancer (NSCLC) and SCLC. Results: Denosumab was associated with improved median overall survival versus ZA in 811 patients with any lung cancer (8.9 versus 7.7 months; hazard ratio [HR] 0.80) and in 702 patients with NSCLC (9.5 versus 8.0 months; HR 0.78) (p = 0.01, each comparison). Further analysis of NSCLC by histological type showed a median survival of 8.6 months for denosumab versus 6.4 months for ZA in patients with squamous cell carcinoma (HR 0.68; p = 0.035). Incidence of overall adverse events was balanced between treatment groups; serious adverse events occurred in 66.0% of denosumab-treated patients and 72.9% of ZA-treated patients. Cumulative incidence of osteonecrosis of the jaw was similar between groups (0.7% denosumab versus 0.8% ZA). Hypocalcemia rates were 8.6% with denosumab and 3.8% with ZA. Conclusion: In this exploratory analysis, denosumab was associated with improved overall survival compared with ZA, in patients with metastatic lung cancer.

Os bisfosfonatos (BFs) têm sido indicados para o tratamento de doenças do metabolismo ósseo. Atualmente, seu emprego terapêutico aumentou e, com ele, os efeitos adversos, dos quais um dos mais importantes é a indução da osteonecrose dos maxilares, uma complicação de difíceis tratamento e solução. Até o presente, não se sabe ao certo qual é o mecanismo de desenvolvimento da osteonecrose dos maxilares induzida por bisfosfonatos (ONMB), nem qual deve ser o tratamento estabelecido perante essa manifestação. Apesar de a literatura apresentar formas variadas de tratamento, não existe um protocolo definido. Apresentamos uma revisão sobre a ONMB, enfocando sua etiopatogenia e as formas reportadas de tratamento.

Die Bisphosphonat-assoziierte Osteonekrose der Kiefer (BP-ONJ) stellt eine ernstzunehmende Nebenwirkung der Therapie mit stickstoffhaltigen Bisphosphonaten (N-BP) dar, deren Ätiologie bisher noch nicht vollständig geklärt ist. Da entzündliche Prozesse eine wichtige Rolle zu spielen scheinen, wurde der Einfluss verschiedener Bisphosphonate auf die Mechanismen der granulozytären Erregerabwehr untersucht. Die N-BP Ibandronat, Pamidronat und Zoledronat steigerten die Phagozytose und den oxidativen Burst signifikant. Die fMLP-stimulierte Chemotaxis wurde durch Ibandronat und Zoledronat signifikant reduziert. Das stickstofffreie Clodronat zeigte keinen Effekt auf die getesteten Abwehrmechanismen. Auf der Suche nach therapeutischen Optionen gegen die BP-ONJ wurden die Isoprenoide Farnesol, Geranylgeraniol, Eugenol, Menthol, Limonene und Squalene auf deren Fähigkeit untersucht, die schädigenden Effekte Zoledronats auf verschiedene Zelllinien zu antagonisieren. Geranylgeraniol zeigte als einzige Verbindung eine protektive Wirkung auf gingivale Fibroblasten, Endothelzellen und Osteoblasten. Desweiteren kam es unter Zoledronat zum Anstieg der kleinen GTPasen RhoA und RhoB in gingivalen Fibroblasten. Auch der Gehalt an GTP-gebundenem RhoA stieg nach Zoledronat-Inkubation. Der Einfluss des N-BPs ließ sich auch auf Proteinebene durch Geranylgeraniol antagonisieren und nicht durch Farnesol. Die Tatsache, dass N-BP die granulozytäre Abwehr beeinflussen, unterstützt die Bedeutung keimreduzierender Maßnahmen im Rahmen der Nekroseprophylaxe und -therapie. Außerdem untermauern die Ergebnisse der Arbeit das Potential Geranylgeraniols als neue therapeutische Option.

Os fármacos anti-reabsorção óssea mais utilizados atualmente são os bifosfonatos que são medicamentos que inibem a actividade osteoclástica. O mecanismo de ação destes fármacos consiste na redução da reabsorção óssea pois inibem a apoptose dos osteoclastos e promovem a inibição da angiogénese. Estes fármacos são utilizados no tratamento de doenças como a osteoporose, hipercalcemia, doença de Paget e em pacientes oncológicos com metástases ósseas de tumores sólidos e mieloma múltiplo. Por outro lado, estes fármacos além de todos os benefícios que têm para os pacientes nestas condições, podem ter como complicação a osteonecrose dos maxilares (ONM). A ONM é uma complicação oral grave caracterizada, na maioria, por uma osteomielite inicial que normalmente evolui para uma exposição de osso necrosado acompanhada por dor na maxila ou na mandíbula. Nos doentes oncológicos têm sido diagnosticados vários casos de necrose óssea mais frequentemente na mandíbula. Esta patologia (ONM) apesar de ter baixa incidência e do seu aparecimento ser relativamente recente, levou à introdução de algumas recomendações internacionais para o uso de bifosfonatos, principalmente em pacientes oncológicos. Para a execução deste trabalho recorri a fontes de pesquisas como as bases de dados como a PubMed, B-On e, também ao Repositório Institucional da Universidade Fernando Pessoa das quais resultaram 198 artigos e duas monografias realizadas por exalunos da Universidade Fernando Pessoa. Com a realização deste trabalho foi possível esclarecer algumas dúvidas acerca dos efeitos que os fármacos anti-reabsorção óssea têm no organismo e mais especificamente na cavidade oral, de forma a estar consciente dos riscos que estes pacientes correm, caso lhes sejam realizados procedimentos como é o caso, das exodontias ou da colocação de implantes.

A previously undescribed Actinomyces-like bacterium was isolated from a lesion in the jaw of a cow. Based on its cellular morphology and the results of biochemical testing, the organism was tentatively identified as a member of the genus Actinomyces. Comparative 16S rRNA gene sequencing studies showed that the bacterium represents a hitherto unknown species within the genus Actinomyces, and is related to a group of species that includes Actinomyces turicensis and its close relatives. It is proposed that the unknown organism be classified as Actinomyces vaccimaxillae sp. nov. (the type strain is CCUG 46091(T)=CIP 107423(T)).

The Meiglyptini comprise eight species of woodpeckers grouped into three genera, two of which contain three species ( Meiglyptes and Mulleripicus), while one genus includes two species ( Hemicircus). The purpose of this study is to describe the mandibular apparatus found in six species of Meiglyptini and to compare them with each other and with other woodpecker species. The results reveal a number of structures that are worth mentioning: (i) the components of the external mandibular adductor system of Hemicircus concretus, particularly the M. adductor mandibulae externus caudalis medialis, are underdeveloped compared with the other investigated species; (ii) the muscles of the internal mandibular system are structurally different among Meiglyptes species and are less developed, both in size and in structure, in H. concretus; (iii) the M protractor quadrati is vestigial in Meiglyptes species; (iv) the muscles of the protractor system of the quadrate are relatively undeveloped in H. concretus; (v) most of the muscles of the pterygoideus system are structurally differentiated in Meiglyptes species. It appears that the complexity of the mandibular apparatus is associated with the type of food consumed, as the apparatus of the frugivorous species H. concretus is markedly different from that of the insectivorous species. © 2012 The Royal Swedish Academy of Sciences.

The Picini tribe comprises 25 Old World woodpecker species grouped into seven genera that are widely distributed in Asia and include several representatives from the Eurasian region. Given the absence of detailed anatomical studies of Picini in the literature, the purposes of this study were to describe the jaw musculature of 14 species of Picini in detail and to compare the musculature patterns of these species. The results of this analysis indicate the following: (1) there is a clear association between the ventralis lateralis and dorsalis lateralis muscles through fleshy fibers that are connected in all species, (2) the jaw musculature of the genus Picus differs from that of other Picini genera in terms of the poor development of the protractor muscle system of the quadrate (M. protractor quadrati and M. protractor pterygoidei), (3) generally, the M. pseudotemporalis superficialis originates in the ventrocaudal region of the laterosphenoid (the lower part of the orbit), with the only noteworthy exception being an origin in the upper part of the orbit in Dinopium javanense, and (4) the protractor pterygoidei muscle is more developed in Blythipicus rubiginosus, Dinopium rafflesii, and D. javanense than in the other species. © 2012 Reginaldo José Donatelli.

In the present study, pterosaur skull constructions were analysed using a combined approach of finite element analysis (FEA), static investigations as well as applying classical beam theory and lever mechanics. The study concentrates on the operating regime „bite“, where loads are distributed via the dentition or a keratinous rhamphotheca into the skull during jaw occlusion. As a first step, pterosaur tooth constructions were analysed. The different morphologies of the tooth construction determine specific operational ranges, in which the teeth perform best (= greatest resistance against failure). The incomplete enamel-covering of the pterosaur tooth constructions thereby leads to a reduction of strain and stress and to a greater lateral elasticity than for a complete enamel cover. This permits the development of high and lateral compressed tooth constructions. Further stress-absorption occurs in the periodontal membrane, although its mechanical properties can not be clarified unambiguously. A three-dimensionally preserved skull of Anhanguera was chosen as a case-study for the investigation of the skull constructions. CT-scans were made to get information about the internal architecture, supplemented by thin-sections of a rostrum of a second Anhanguera specimen. These showed that the rostrum can be approximated as a double-walled triangular tube with a large central vacuity and an average wall-thickness of the bony layers of about 1 mm. On base of the CT-scans, a stereolithography of the skull of Anhanguera was made on which the jaw adductor and abductor muscles were modelled, permitting to determine muscular forces. The values were used for the lever mechanics, cantilever and space frame analysis. These studies and the FEA show, that the jaw reaction forces are critical for the stability of the skull construction. The large jugal area ventral to the orbita and the inclined occipital region act as buttresses against these loads. In contrast to the orbitotemporal region which is subject to varying loading conditions, the pattern in the rostrum is less complex. Here, mainly bending in dorsal direction and torsion occur. The hollow rostrum leads to a reduction of weight of the skull and to a high bending and torsional resistance. Similar to the Anhanguera skull construction, the skulls of those pterosaur taxa were analysed, from which enough skull material is know to permit a reliable reconstruction. Furthermore, FEA were made from five selected taxa. The comparison of the biomechanical behaviour of the different skull constructions results in major transformational processes: elongation of rostra, inclination of the occipital region, variation of tooth morphology, reduction of the dentition and replacement of teeth by a keratinous hook or rhamphotheca, fusion of naris and antorbital fenestra, and the development of bony and soft-tissue crests. These processes are discussed for their biomechanical effects during bite. Certain optional operational ranges for feeding are assigned to the different skull constructions and previous hypotheses (e.g. skimming) are verified. Using the principle of economisation, these processes help to establish irreversible transformations and to define possible evolutionary pathways. The resulting constructional levels and the structural variations within these levels are interpreted in light of a greater feeding efficiency and reduction of bony mass combined with an increased stability against the various loads. The biomechanical conclusive pathways are used for comparison and verification of recent hypothesis of the phylogenetic systematics of pterosaurs.

The structure of the pharyngeal jaw apparatus (PJA) of Zenarchopterus dispar and Z. buffonis, carnivorous estuarine and freshwater West-Pacific halfbeaks, was investigated using dissection, light, and scanning electron microscopy as part of a comparison with estuarine and marine herbivorous confamilials. The Zenarchopterus PJA differs from published descriptions of hemiramphid PJAs in that the otic capsules are less pronounced; the pharyngocranial articulation facet is trough-like; the third pharyngobranchials are ankylosed; the second pharyngobranchial anterior processes are relatively hypotrophied; all pharyngeal teeth except the posterior teeth in the fifth ceratobranchial face posteriorly; the muscularis craniopharyngobranchialis 2 posterior is short; the muscularis craniopharyngobranchialis 2 anterior is lacking, as is its insertion site, the inferior parasphenoid apophysis; the protractor pectoralis is well developed; the pharyngocleithralis internus originates dorsal to the level of the fifth ceratobranchial bony process; the fifth ceratobranchial bony processes are directed ventrolaterally; the opposing upper and lower tooth fields appear not to occlude erosively; and the muscular portion of the pharyngohyoideus is well developed anteriorly. The extent of these differences and their implications for the function of the PJA support recent molecular studies that suggest that the Hemiramphidae is polyphyletic. (C) 2004 Wiley-Liss, Inc.
