The comet assay has been conducted with numerous cell lines to assess in vitro genotoxicity. In order to use the comet assay as part of an in vitro test for evaluating genotoxicity, however, there are cell-specific factors that need to be better understood. In this present study we have evaluated some factors that may impact upon the DNA damage detected in whole blood (WB) cells and lymphocytes (ILs). Experiments were conducted comparing responses of both cells, and investigating the effects of the female hormonal cycle, and oral contraceptive (OC) use on DNA damage detection in the in vitro comet assay, at three sampling time. No significant differences were detected in the basal levels of DNA damage detected in ILs and WB cells from women OC users and non-users and from men. Basal DNA damage in ILs was unaffected by gender and stage of the menstrual cycle or the stage of the treatment schedule. Our results also indicated that the H2O2 induces DNA damage in human lymphocytes independently of gender, low-dose OC use and hormonal fluctuation. However, data showed that in 3rd sampling of menstrual cycle, lymphocytes were more resistant to H2O2-induced DNA damage than those from OC users and men. © 2007 Elsevier Ltd. All rights reserved.

Background. Hydatidiform mole (HM) is characterized by abnormal proliferation of human trophoblast with producers functioning tissues of human chorionic gonadotropin. It can evolve with ovarian cysts tecaluteínicos, hypertension of pregnancy or hyperthyroidism. The incidence of HM is variable and its etiology poorly known, associated with nutritional factors, environmental, age, parity, history of HM, oral contraceptives, smoking, consanguinity or defects in germ cells. There is no reference in literature on HM resulting from sexual violence, objective of this report. Method. Description of two cases of HM among 1146 patients with pregnancy resulting from sexual violence treated at Hospital Pérola Byington, São Paulo, from July 1994 to August 2011. Results. The cases affected young, white, unmarried, low educated and low parity women. Sexual violence was perpetrated by known offenders unrelated to the victims, under death threat. Ultrasound and CT of the pelvis showed bulky uterus compatible with HM without myometrial invasion. One case was associated with theca lutein cysts. The two cases were diagnosed in the second trimester of pregnancy and evolved with hyperthyroidism. There was no hypertension, disease recurrence, metastasis or sexually transmitted infection. Conclusion. The incidence of HM was 1:573 pregnancies resulting from rape, within the range estimated for Latin American countries. Trophoblastic material can be preserved to identify the violence perpetrator, considering only the paternal HM chromosomes. History of sexual violence should be investigated in cases of HM in the first half of adolescence and women in a vulnerable condition. © 2012 Drezzet et al; licensee BioMed Central Ltd.

A infecção genital pelo Papilomavírus humano (HPV) é a principal causa para o desenvolvimento de lesões precursoras e processos neoplásicos na cérvice uterina. O câncer cervical representa a segunda maior causa de óbito por câncer em mulheres brasileiras, constituindo-se em uma das principais causas de morbimortalidade feminina na região Norte do Brasil. Este estudo teve o intuito de investigar os aspectos epidemiológicos da infecção genital pelo Papilomavírus humano (HPV) em mulheres de população urbana e rural oriundas de duas regiões distintas da Amazônia Oriental Brasileira. Para tanto foi conduzido um estudo Transversal analítico com 444 mulheres de 13 a 74 anos que se submeteram ao exame preventivo do câncer do colo uterino, sendo 233 urbanas oriundas de uma unidade básica de saúde da cidade de Belém do Pará e 211 rurais provenientes das margens direita e esquerda do lago da U.H.T de Tucuruí - PA, no período de janeiro de 2008 a março de 2010. Amostras da cérvice uterina foram coletadas para a realização da colpocitologia convencional e para a detecção do DNA do HPV através da reação em cadeia da polimerase (PCR) mediada pelos oligonucleotídeos iniciadores universais MY9/11. Todas as mulheres responderam a um formulário clínico e epidemiológico. Para análise das associações epidemiológicas entre os fatores de risco e a infecção pelo HPV dividiu-se a amostra em três faixas etárias, sendo obtidas a Razão de Chances de Prevalência (ORp) com IC95%, com sua significância verificada por meio do teste do qui-quadrado ou exato de Físher, além do emprego final do modelo de regressão logística multivariado. Entre as 444 mulheres analisadas, a prevalência geral de infecção genital pelo HPV foi de 14,6%, variando entre 15,0% para a amostra urbana e 14,2% para a rural. A faixa etária mais acometida foi a de 13 a 25 anos (17,9%), tanto na amostra urbana (19,0%) quanto rural (17,2%). O DNA do HPV foi detectado em 13,6% das mulheres com citologia normal e em 41,6% daquelas com citologia alterada, sendo este resultado mais significativo para a porção urbana do estudo com idades compreendidas entre 26 a 44 anos. Anormalidades colpocitológicas, início precoce da atividade sexual, situação conjugal, número de parceiros sexuais novos e antigos, o uso pregresso de anticoncepcionais orais e preservativos, história de DST e de sintomas genitais, além de tabagismo atual, foram fatores que se mostraram associados à infecção genital pelo HPV de maneira diferenciada nas três faixas etárias analisadas entre amostras urbana e rural da Amazônia Oriental Brasileira.

A infecção genital pelo Papilomavírus humano (HPV) é considerada uma das doenças sexualmente transmissíveis (DSTs) mais comum, representando um importante problema na Saúde Pública, além de estar diretamente relacionado à promoção do câncer de colo uterino. Este estudo teve o intuito de investigar os aspectos epidemiológicos da infecção genital pelo HPV em dois grupos distintos: mulheres de população geral e mulheres encarceradas. Para tanto foi conduzido um estudo transversal analítico com 423 mulheres a partir dos 18 anos que se submeteram ao exame preventivo do câncer do colo uterino, sendo 233 mulheres da população geral oriundas de uma unidade básica de saúde da cidade de Belém do Pará e 190 provenientes do Centro de Reeducação Feminino em Ananindeua no mesmo Estado, no período de janeiro de 2008 a março de 2010. Amostras da cérvice uterina foram coletadas para a realização da colpocitologia convencional e para a detecção do DNA do HPV através da reação em cadeia da polimerase (PCR) mediada pelos oligonucleotídeos iniciadores universais MY9/11. Todas as mulheres responderam a um formulário clínico e epidemiológico. Entre as 423 mulheres analisadas, a prevalência geral de infecção genital pelo HPV foi de 13,0% com variação entre 15,0% para a amostra geral e 10,5% para a carcerária. A faixa etária mais acometida foi a de 13 a 25 anos (19%) na amostra geral; e em mulheres com 45 anos ou mais (21,1%), nas carcerárias. Anormalidades Colpocitológicas, situação conjugal, número de parceiros sexuais novos, o uso de anticoncepcionais orais, história de DST e de sintomas genitais, além de tabagismo atual, foram fatores que se mostraram associados à infecção genital pelo HPV de maneira diferenciada entre amostras da população geral e carcerária.

A adolescência é um período de vida da mulher marcado por transformações biológicas e psicológicas que influenciam intensamente sua saúde futura. Essas mulheres são mais suscetíveis às DST. A infecção pelo HPV é uma das DST mais frequentes, sendo importante avaliar a sua prevalência, devido sua ligação com o câncer uterino. Este estudo avaliou a prevalência da infecção genital por HPV na população adolescente do sexo feminino em Belém. Estes dados foram correlacionados com fatores sócio demográficos, comportamentais e reprodutivos. Foi realizado um estudo transversal entre agosto de 2009 e agosto de 2011, com 134 mulheres entre 13 e 19 anos que procuraram a Unidade Materno-Infantil e Adolescente de Belém para exame de rastreamento do câncer do colo do útero. As pacientes selecionadas responderam a um questionário sobre os dados sócio demográficos, comportamentais e reprodutivos. Foi realizada coleta de material cervicovaginal para citologia convencional e escovado cervical para detecção de DNA-HPV por técnica da reação em cadeia de polimerase (PCR). A associação da infecção por HPV e fatores de risco selecionados foi avaliada por meio do teste do Qui-quadrado (χ2) e/ou exato de Fisher, todos com um nível alfa de significância de 0,05. A infecção genital pelo Papillomavirus humano apresentou a prevalência de 22%, sendo o HPV 58 o mais prevalente com 31% e o HPV 11 o menos comum com 3,4%. Entre as adolescentes 51,7% apresentaram infecção por outros tipos de HPV não incluídos na vacina quadrivalente (6, 11, 16 e 18), e 76,2% apresentavam infecção por pelo menos um tipo de HPV do grupo de alto risco. Foram encontradas alterações citológicas em 6,2% dos esfregaços cervicais. Os fatores de risco associados à infecção genital por HPV encontrados neste estudo foram escolaridade superior a oito anos, coitarca com idade maior que 14 anos, uso de anticoncepcional oral por mais de um ano, uso atual de anticoncepcional oral, gravidez com 14 anos ou menos e achado citológico anormal. Este estudo evidenciou o predomínio de HPV de alto risco não imunoprevenível nas amostras cervicais, demonstrando a necessidade de novas políticas de prevenção primária e secundária, que envolvam as adolescentes através de discussão e orientação motivando-as a participar ativamente na promoção da própria saúde.

OBJECTIVE:The purpose of this study was to evaluate the long term clinical and ultrasonographic outcomes of thrombophilic patients with deep venous thrombosis (DVT).METHOD:Cohort study, retrospective case-control with cross-sectional analysis. Thirty-nine thrombophilic patients and 25 non-thrombophilic patients were assessed 76.3 ± 45.8 months after diagnosis. Demographic and family data were collected, as well as data from clinical and therapeutic progress, and physical and ultrasound examinations of the limbs were performed. Groups were matched for age and gender and the variables studied were compared across groups.RESULTS:Deep venous thrombosis was more frequent in women. The most common thrombophilias were antiphospholipid syndrome and factor V Leiden mutation. There was no difference between groups in terms of the number of pregnancies or miscarriages and the majority of women did not become pregnant after DVT. Non-spontaneous DVT prevailed. Proximal DVT and DVT of the left lower limb were more frequent, and the main risk factor was use of oral contraceptives. All patients were treated with anticoagulation. There was a higher frequency of pulmonary embolism in non-thrombophilic patients. Most patients considered themselves to have a normal life after DVT and reported wearing elastic stockings over at least 2 years. Seventy-one percent of patients had CEAP > 3, with no difference between groups. Deep venous reflux was more frequent in thrombophilic patients.CONCLUSION:There were no significant differences between groups with respect to most of the variables studied, except for a higher frequency of pulmonary embolism in non-thrombophilic patients and greater frequency of deep venous reflux in thrombophilic patients.

Melasma is a chronic acquired hypermelanosis of the skin, characterized by irregular brown macules symmetrically distributed on sun-exposed areas of the body, particularly on the face. It is a common cause of demand for dermatological care that affects mainly women (especially during the menacme), and more pigmented phenotypes (Fitzpatrick skin types III-V). Due to its frequent facial involvement, the disease has an impact on the quality of life of patients. Its pathogeny is not yet completely understood, although there are some known triggering factors such as sun exposure, pregnancy, sexual hormones, inflammatory processes of the skin, use of cosmetics, steroids, and photosensitizing drugs. There is also a clear genetic predisposition, since over 40% of patients reported having relatives affected with the disease. In this manuscript, the authors discuss the main clinical and epidemiological aspects of melasma.

Introduction: sexually transmitted diseases (DST) can represent an important impact on teenager’s health due its consequences. Objective: to identify the perception and knowledge of teenagers about DST, its transmission forms and contraception. Methods: it was made a cross-sectional study type investigation, with 136 teenager students from a Technical Education Public Institution in a middle-sized city. It was used a tested instrument, self answered, with opened and closed questions about theme and sociodemographic data. The same data were stored and analyzed by Program Epi Info version 3.5.1. Results: 97,1% affirmed to know male condoms, 89,0% oral contraceptives, 87,5% female condoms, 74,3% postcoital contraceptives, 47,8% natural family planning methods, 41,9% hormonal injection, 33,1% intrauterine devices, 31,6% coitus interruptus and 1,5% other contraceptive methods. According with them, the information was gave principally on high school (46,1%), by friends or partners (20,3%), doctor (10,9%), television (10,2%). In relation to DST, 98,5% affirmed that diseases can be transmitted during coitus. The aids (91,2%) and herpes (72,8%) were the most cited diseases. Just 31,6% of teenagers affirmed that run the risk to acquire some DST, and 88,4% of them, due no use of condoms, 30,2% due their partner make relation with other people, 18% due have many sexual partners and 9,3% due other causes. Conclusion: even teenagers knowing many contraceptive methods and sexually transmitted diseases, the teenagers have not prevented theirselves yet. It’s necessary that the high school reinforce the use of contraceptive methods, because the school is an important source of educative information on health area.

Breast cancer (BC) is the most common malignancy in women worldwide, with one million new cases confirmed each year. This study aimed at identifying the knowledge of patients? relatives concerning the risk factors (RF) for BC. It is a descriptive quantitative study carried out in the Chemotherapy Technical Division of a University Hospital in São Paulo state, Brazil, from September to October 2006. Only female (30) relatives were included due to the fact that BC is more frequent in females. The major RF cited were family history for BC 33.3% (10), smoking 16.6% (5), alcoholic-drink consumption 10% (3), animal-fat-rich diet 3.3% (1). Other factors such as not breastfeeding, environmental factors, use of oral contraceptives and hormone-replacement therapy were mentioned by 10% (3) of the relatives. It was concluded that the relatives of patients undergoing chemotherapeutic treatment showed to have little knowledge concerning RF for BC. Hence, it is important to rethink the inclusion of the health education process in the family scenario. This issue reaches far beyond thesimple transmission of information. Healthcare professionals and nurses in particular, must focus on these individuals? social context, values, beliefs and needs.

Background: Polycystic ovary syndrome (PCOS) is an endocrine disorder associated with metabolic dysfunction and changes in cardiovascular risk markers, and using oral contraceptives (OCs) may exert a further negative effect on these alterations in patients with PCOS. Thus, the primary objective of this study was to assess the effects on arterial function and structure of an OC containing chlormadinone acetate (2 mg) and ethinylestradiol (30 mcg), alone or combined with spironolactone (OC+SPL), in patients with PCOS. Study Design: This was a randomized, controlled clinical trial. Fifty women with PCOS between 18 and 35 years of age were randomized by a computer program to use OC or OC+SPL. Brachial artery flow-mediated vasodilation, carotid intima-media thickness and the carotid artery stiffness index were evaluated at baseline and after 6 and 12 months. Serum markers for cardiovascular disease were also analyzed. The intragroup data were analyzed using analysis of variance with Tukey's post hoc test. A multivariate linear regression model was used to analyze the intergroup data. Results: At 12 months, the increase in mean total cholesterol levels was greater in the OC+SPL group than in the OC group (27% vs. 13%, respectively; p=.02). The increase in mean sex hormone-binding globulin levels was greater in the OC group than in the OC+SPL group (424% vs. 364%, respectively; p=.01). No statistically significant differences between the groups were found for any of the other variables. Conclusion: The addition of spironolactone to an OC containing chlormadinone acetate and ethinylestradiol conferred no cardiovascular risk-marker advantages in young women with PCOS. Background: The objective of this study was to evaluate the effect of three contraceptive pills containing ethinylestradiol (EE) (20 or 30 mcg) in combination with drospirenone (DRSP) and levonorgestrel (LNG) on plasma concentration of adhesion molecules vascular cell adhesion molecule -1 (VCAM-1), intercellular adhesion molecule-1 (ICAM-1) and E-selectin. Study Design: A cross-sectional study was conducted with 72 participants (18-30 years old) distributed into three groups that used oral contraceptives containing EE 20 or 30 mcg combined with DRSP 3 mg or EE 30 mcg/LNG 150 mcg for at least 6 months. The control group was comprised of nonusers of contraceptives. Soluble VCAM-1, soluble ICAM-1 and soluble E-selectin were evaluated by enzyme-linked immunosorbent assay. Results: Compared to the control group, a significant decrease was found in VCAM-1 and ICAM-1 concentrations with use of DRSP/20 EE and LNG/30 EE. Conclusions: DRSP/20 EE and LNG/30 EE induce favorable changes in endothelial function. (C) 2012 Elsevier Inc. All rights reserved.

Introduction: The puerperium is the period of highest risk for thrombosis during a woman's reproductive life and it is an important time for initiating an effective contraceptive method in order to increase intergestational interval. Thus, the objective of the present study was to evaluated the effects of the etonogestrel (ENG)-releasing contraceptive implant inserted immediately postpartum on maternal hemostasis markers during the first six weeks of delivery. Materials and Methods: Forty healthy women aged 18 to 35 years-old were randomized to receive either the ENG-releasing implant 24-48 h after delivery (implant group; n=20) or nothing (control group) until the sixth postpartum week. Blood samples were collected at 24-48 h and at 6 weeks after delivery, and hemostatic variables, including fibrinogen, coagulation factors, protein C, free protein S, antithrombin, alpha 2-antiplasmin, plasminogen activator inhibitor 1, thrombin-antithrombin complex (TAT), prothrombin fragment (PF)1+2, and D-dimers, as well as normalized activated protein C sensitivity ratio (nAPCsr), thrombin time, activated partial thromboplastin time, and prothrombin time were evaluated. Results: Insertion of the ENG-releasing contraceptive implant did not change the physiological reduction in overall coagulation (TAT and PF1+2) and fibrinolysis (D-dimer) markers, or nAPCsr. Reductions in factors II, VII, X and fibrinogen and increases in factor V were greater in the control than in the implant group. Clotting factors remained within normal limits throughout the study. Conclusion: The ENG-releasing contraceptive implant inserted immediately postpartum did not have negative effects on physiological variations of the hemostatic system during the first 6 weeks postpartum.
