OBJECTIVE: To assess the prevalence of and the factors related to overweight and obesity in a sample of children from the region of Sintra, Portugal. METHODS AND PROCEDURES: Cross-sectional study, stratified for freguesia with random selection of schools. Height, weight, triceps skinfold, upper arm and waist circumferences were measured, and overweight/obesity defined according to international criteria. Breast-feeding, number of daily meals and parents' height and weight data were also collected. RESULTS: One thousand two hundred and twenty-five children aged 6-10 years were assessed. Overall prevalence of overweight and obesity was 35.6% (23% overweight and 12.6% obesity). Overweight or obese children had higher triceps skinfold, upper arm circumference, arm muscle area, and waist circumference than their normal weight counterparts (P < 0.001). On multivariate analysis, relatively to a child without obese progenitors, a child with one obese progenitor had an obesity risk multiplied by 2.78 (95% confidence interval (CI): 1.76-4.38), while a child with two obese progenitors had a risk multiplied by 6.47 (95% CI: 5.59-16.19). Conversely, being picky was significantly related with a smaller risk of obesity: for boys, odds ratio (OR) = 0.15 (95% CI: 0.04-0.63); for girls, OR = 0.19 (95% CI: 0.06-0.64). Finally, no relationships were found between obesity, birth weight, birth height or breast-feeding. DISCUSSION: Prevalence of overweight and obesity are elevated among children of the Sintra region in Portugal compared to most other regions of Europe. The relationship with the parents' nutritional state stresses the need to target families for preventing obesity.

Nutritional factors, especially the protein and fat content of the diet, may change pancreatic morphology after ethanol induced injury. This study was performed to delineate the combined effects of a low fat diet and longterm ethanol ingestion on the rat pancreas. Male Sprague-Dawley rats were maintained with five different diets for 12 weeks and the pancreas removed on the day they were killed. Rats fed a very low fat diet without ethanol (5% of total calories as lipid) developed malnutrition, pancreatic steatosis, and reduction in zymogen granules content. Animals fed a 35% lipid diet with ethanol also developed pancreatic steatosis but changes in zymogen granules content were not detected. Both malnutrition and longterm ethanol consumption increased pancreatic cholesterol ester content, and their effects were additive. Pancreatic steatosis was accompanied with hypercholesterolaemia. Amylase, lipase, and cholesterol esterase content were reduced in malnourished rats; but longterm ethanol ingestion, regardless of the nutritional state, increased lipase content and decreased amylase. It is suggested that high serum cholesterol concentrations and increased pancreatic lipase activity could cause accumulation of cholesterol esters in acinar cells. Fat accumulation in the pancreas has been reported as the earliest histopathological feature in alcoholic patients and may be responsible for cytotoxic effects on the acinar cells at the level of the cell membrane. Although it is difficult to extrapolate results in this animal study to the human situation, the results presented in this work might explain the higher incidence of pancreatitis is malnourished populations as well as in alcoholic subjects that is reported in dietary surveys.

In some cases of esophageal reconstruction, it is not possible to use the gastric tube. In those cases, the second option is the reconstruction with a colonic segment. In the present paper, the authors present the use of microsurgical technique to improve vascular supply in esophageal reconstructions using the colon. Therefore, the transposed segment becomes perfused by two vascular pedicles: a proximal one and a distal one. The authors describe a case of 52 years-old patient, suffering of middle third esophagus carcinoma, who underwent a primarily esophageal resection with an unsuccessful reconstruction using gastric transposition. A new reconstruction was proposed using a bipedicle microcirurgical colonic tube, four months later. The post operative was uneventful with rehabilitation of swallowing and satisfactory recovery of nutritional state.

Our objective was to determine whether anthropometric measurements of the midarm (MA) could identify subjects with whole body fat-free mass (FFM) depletion. Fifty-five patients (31% females; age: 64.6 ± 9.3 years) with mild/very severe chronic obstructive pulmonary disease (COPD), 18 smokers without COPD (39% females; age: 49.0 ± 7.3 years) and 23 never smoked controls (57% females; age: 48.2 ± 9.6 years) were evaluated. Spirometry, muscle strength and MA circumference were measured. MA muscle area was estimated by anthropometry and MA cross-sectional area by computerized tomography (CT) scan. Bioelectrical impedance was used as the reference method for FFM. MA circumference and MA muscle area correlated with FFM and biceps and triceps strength. Receiver operating characteristic curve analysis showed that MA circumference and MA muscle area cut-off points presented sensitivity and specificity >82% to discriminate FFM-depleted subjects. CT scan measurements did not provide improved sensitivity or specificity. For all groups, there was no significant statistical difference between MA muscle area [35.2 (29.3-45.0) cm²] and MA cross-sectional area values [36.4 (28.5-43.3) cm²] and the linear correlation coefficient between tests was r = 0.77 (P < 0.001). However, Bland-Altman plots revealed wide 95% limits of agreement (-14.7 to 15.0 cm²) between anthropometric and CT scan measurements. Anthropometric MA measurements may provide useful information for identifying subjects with whole body FFM depletion. This is a low-cost technique and can be used in a wider patient population to identify those likely to benefit from a complete body composition evaluation.

Les principaux substrats oxydés à l’exercice, soit les glucides, les lipides et les pro- téines ne contribuent pas tous au même niveau à la fourniture d’énergie lors de l’effort prolongé. De plus, le glucose peut provenir de différentes sources endogènes (muscle, foie) et exogènes. Plusieurs facteurs peuvent influencer leur contribution respective incluant : la masse musculaire impliquée et l’entraînement préalable, le sexe, l’état nutritionnel et les conditions environnementales. L’utilisation d’isotopes stables, tels que le carbone 13 (13C), combinée à la calorimétrie indirecte respiratoire corrigée pour l’excrétion d’urée dans l’urine et la sueur, permet de différencier les substrats endogènes et exogènes et d’évaluer la contribution de leur oxydation à la fourniture d’énergie. Ces méthodes d’investigation permettant d’apprécier la sélection des substrats lors de l’exercice prolongé avec ingestion de glucose ont permis d’effectuer les comparaisons qui ont fait l’objet des trois études de cette thèse. Dans la première étude, la sélection des substrats au cours d’un effort prolongé effectué avec les membres inférieurs ou les membres supérieurs a été comparée avec et sans ingestion de glucose. Une différence modeste fut observée entre la sélection des substrats selon le mode d’exercice avec l’ingestion d’eau, celle-ci favorisant légèrement l’oxydation des glucides lors de l’effort avec les membres supérieurs. La quantité de glucose exogène oxydée était plus faible lors de l’exercice avec les membres supérieurs qu’avec les membres supérieurs, mais sa contribution plus importante, conséquence d’une dépense énergétique plus faible. Dans la deuxième étude, on a comparé la sélection des substrats chez des sujets mas- culins et féminins et les effets d’une alimentation enrichie en glucides ou de l’ingestion de glucose, au cours d’un exercice prolongé d’une durée de deux heures. On reconnaît généralement que, pour une même puissance relative, les femmes utilisent moins de glucides et davantage de lipides que les hommes. Les effets séparés d’une alimentation riche en glucides ou de l’ingestion de glucose pendant l’exercice sur la sélection des substrats furent pourtant similaires chez les deux sexes. L’effet combiné des deux procédures de supplémentation est toutefois plus important chez la femme que chez l’homme, soutenant l’hypothèse qu’un léger déficit en glucides soit présent chez les femmes. Dans la troisième étude, l’oxydation des substrats et particulièrement celle d’amidon exogène au cours d’une marche prolongée à une faible puissance de travail a été décrite. Les individus qui pratiquent des activités physiques prolongées à des intensités faibles (< 40 %VO2max) sont encouragés à ingérer des glucides et de l’eau pendant l’effort, mais la contribution de leur oxydation à la fourniture d’énergie est relativement peu connue. Nous avons montré que, contrairement aux observations précédemment effectuées à jeun sans ingestion de glucides pendant l’effort, les glucides (incluant de source exogène) peuvent fournir une très grande partie de l’énergie lorsqu’ils sont ingérés à des intervalles réguliers au cours de l’exercice prolongé. Dans l’ensemble, les résultats des études expérimentales présentées dans cette thèse montrent que les glucides ingérés peuvent fournir une grande proportion de l’énergie pendant l’exercice prolongé. Toutefois, le mode d’exercice, le sexe et la puissance de travail mènent à des variations qui sont en grande partie liées à une dépense énergétique variable selon les conditions et les groupes d’individus ayant des caractéristiques différentes.

Introduction : Depuis les années 2000, la distribution du traitement antirétroviral hautement actif (TARVHA) dans les pays à bas et moyens revenus (PBMR) s’est accélérée. Dans ces milieux, l’épidémie du VIH s’est superposée à des conditions préexistantes de malnutrition. Le VIH et la malnutrition forment un cycle vicieux qui contribue à la détérioration des fonctions immunitaires et de l’état nutritionnel. Malgré l’accès au traitement, la malnutrition persiste comme déterminant de la progression de la maladie et de la mortalité chez les séropositifs. Par ailleurs, peu de données existent concernant les trajectoires et déterminants des statuts nutritionnels et immunologiques chez les patients débutant le TARVHA. Objectifs : Documenter les défis méthodologiques associés à la recherche nutritionnelle chez les personnes vivants avec le VIH (PVVIH) dans les PBMR. Investiguer les déterminants et trajectoires du statut nutritionnel à l’initiation et en cours du TARVHA. Investiguer l’association entre les fonctions immunologiques à l’initiation et au cours de la première année de TARVHA selon le statut nutritionnel. Méthodologie: Dans le cadre de la cohorte ATARAO (Mali), 250 PVVIH adultes naïfs au TARVHA ont été recrutés et suivis pendant 12 mois suite à l’initiation du traitement. Les données obtenues ont été jumelées à celles d’une étude prospective similaire provenant du Sénégal (n=372). Des analyses transversales et longitudinales ont été entreprises pour évaluer les covariables de la malnutrition à l’initiation et en cours de traitement. Parallèlement, la fiabilité des mesures anthropométriques collectées dans le cadre de la cohorte ATARAO a été documentée lors de deux sous-études. Finalement, une revue systématique de la littérature a été complétée pour investiguer la réponse immunologique selon le statut nutritionnel à l’initiation et en cours de traitement chez les PVVIH traités. Résultats: Seules les mesures de la taille et du poids se sont révélées fiables et ont pu être utilisées dans nos analyses prospectives. Le traitement a permis une amélioration initiale de l’état nutritionnel chez une majorité de patients. Par contre, ses effets ont atteint un plateau après 6 à 9 mois. Un an plus tard, la malnutrition persistait chez plusieurs patients vivants et actifs (entre 7 et 29 % selon la population et l’indicateur nutritionnel). Comme observé dans nos cohortes et revue systématique, l’attrition était fortement associée à la malnutrition. Par ailleurs, il n’existe pas de relation claire entre le statut immunologique et la malnutrition pré- ou post-HAART. La qualité et la quantité des évidences recensées ne nous permettent pas de tirer de conclusions définitives. Conclusions : Les évidences collectées suggèrent l’existence d’une association transversale et longitudinale entre les statuts nutritionnels et immunologiques chez les patients traités. Cependant, cette association ne fait pas consensus. L’état de la littérature ne nous permet pas de nous prononcer quant à l’impact de la malnutrition sur les fonctions immunologiques à l’initiation et eu cours de la première année de TARVHA. Les difficultés méthodologiques associées à la mesure de l’état nutritionnel ainsi que la forte attrition des patients malnutris suite à la mise sous traitement compliquent l’investigation de cette relation.

Antral follicle growth in cattle occurs in two distinct phases; the first 'slow' growth phase spans the time from antrum acquisition to a size of approximately 3 mm detectable by transrectal ultrasound, and the second 'fast' phase is gondadotrophin-dependent and includes cohort growth, dominant follicle (DF) selection, and DF growth. This review summarises current concepts of the relative roles FSH and LH, ovarian and metabolic hormones play mainly in the second phase of antral follicle growth in animals of different reproductive and nutritional states. It is proposed that differential FSH response may enable one cohort follicle to become selected, and that follicular secretions, particularly inhibin, suppress FSH and thus are responsible for DF selection and dominance. Acute dependence of the DF on LH pulses will determine DF lifespan, and the LH pulse profile can be influenced by metabolic hormones such as leptin, providing one possible link for nutritional state and reproduction. Direct ovarian effects of acute and chronic changes in growth hormone, insulin and insulin-like growth factor (IGF)-I have been described on cohort follicles, DF oestrogen activity and on DF growth. Influences of metabolic hormones on early antral follicles undergoing their first 'slow' growth phase are less well described, yet metabolic hormones appear to enhance growth into the cohort available for FSH-induced emergence, and may influence subsequent developmental competence of oocytes. The potential nutritional and clinical benefits of sip-feed supplements were investigated by means of a controlled trial in elderly female patients admitted for orthopaedic surgery. A nutritional risk assessment procedure (Nutritional Risk Questionnaire, NRQ) was used to identify patients who might benefit from supplementation. Patients identified as high risk who did not receive supplements showed significant losses in triceps skinfold thickness (TSF) and mid-upper arm muscle circumference (MUAMC) measurements during hospitalization. Such changes were not observed in high-risk supplemented patients, but significant losses of MUAMC were also recorded in a group of patients who failed to comply with the supplement. No differences in biochemical parameters, muscle function, or clinical outcome were observed between supplemented and unsupplemented and non-compliant patients. The problems of poor compliance to sip-feed supplements and failure to observe clinical benefit in supplemented patients are discussed.

A comparison was done between the F. Paulino jejunal pouch (FP) and a jejunal pouch (JP) as esophagusduodenum interpositional graft, for replacing the stomach after total gastrectomy. It was investigated the effect of the two procedures on esophagus histology, nutritional state and serum gastrin in rats. Methods: Male Wistar rats weighing 282±17g were randomly submitted to sham operation (S), FP and JP after total gastrectomy. After eight weeks the rats were killed with overdose of anesthetic and tissue was taken from the distal esophagus for histology. Serum levels of total proteins, albumin, iron, transferring, folate, cobalamine, calcium, as well as serum gastrin were determined. Survival was considered. Results: Fourty six rats were operated and thirty survived for eight weeks. Five (33.3%) died after FP and 11 (52.3%) after JP (p<0.05). Postoperative esophagitis occurred in 6 JP rats. At 8th week, no difference was observed on body weight when compared FP and JP rats (p>0.05). The JP rats had a significant decrease in serum albumin, glucose, transferrin, iron, folate and calcium, compared to sham (p<0.05). Serum gastrin, iron and calcium were significantly higher in JP rats than in FP rats (p<0.05). In FP rats, transferrin and cobalamine showed significant decrease comparing the preoperative with 8th week levels (p<0.05). Conclusion: F. Paulino pouch in rats had lower mortality than JP, and esophagitis was not detected in it. JP rats had serum gastrin, iron and calcium unaffected, possibly because of preservation of duodenal passage

This study aimed builds reference values for copper and zinc, of healthy adults in Natal-RN, and to identify the influence of the gender, age, body mass index (BMI) and diet, on those values. They were assessed 123 healthy students of the Universidade Federal do Rio Grande do Norte (UFRN), both genders, with age between 19 and 41 years. The project was approved by the Ethics Committee in Research of UFRN. BMI was determined and the food consume was accomplished through a 24h recordatory. Dietary was evaluated as the energy, macronutrients, copper and zinc, according to the recommendations of National Academy of Sciences (2001; 2002). Analyses of the copper and zinc concentrations in the plasma and erythrocytes were accomplished by flame atomic absorption spectrometry. The casuistic came quite homogeneous as for the distribution for gender and age, being the largest number of individuals between the 19 and 24 years old. Most of the volunteers presented anthropometric nutritional state inside of the normality patterns. Chronic diseases family antecedents and sedentarysm were observed. Diet was characterized with low consumption of zinc, appropriate of copper and of lipids. Average concentrations of plasma copper (p=0,002), erythrocyte copper (μg/dL, p=0,036; μg/gHb, p=0,038), and plasma zinc (p=0,022) were different among the genders, what was demonstrated by the largest values of copper in the female gender and larger of zinc in the masculine. Plasma copper values still suffered interference of the variables: energy, carbohydrate and copper consumption, all classified in agreement with the median, besides the protein classified according to the percentage contribution for the dietary total energy. The study allowed to establish reference values for erythrocyte zinc (1.261,6-1.344,0 μg/dL e 51,0-54,3 μg/gHb) and to suggest "indicative" of reference values for plasma (108,4 130,2 μg/dL) and erythrocyte (female = 85,0 91,4 μg/dL; masculine = 80,2 86,5 μg/dL) copper and plasma zinc (female = 98,8 105,8 μg/dL; masculine = 104,6 111,6 μg/dL)

O estudo teve por objetivo avaliar componentes do crescimento e do estado nutricional de porta-enxertos de limoeiro cravo, em função de doses de nitrogênio, fósforo e potássio via fertirrigação. O delineamento experimental foi inteiramente casualizado, em esquema fatorial 3³ + 1, sendo 3 fatores (nitrogênio, fósforo e potássio - NPK), em 3 doses e uma testemunha (sem adubação), com 3 repetições. A unidade experimental foi constituída pela média de dois porta-enxerto (Hipobioto) de limoeiro cravo (Citrus limonia L. Osbeck), cada qual situado em um tubete de 2,8 cm de diâmetro e 12,3 cm de altura, perfurados na base e preenchido substrato composto de casca de Pinus e vermiculita. As doses utilizadas foram constituídas por níveis de N (460; 920 e 1840 mg dm-3), P (50; 100 e 200 mg dm-3) e de K (395; 790 e 1580 mg dm-3). As adubações com N e K foram realizadas através de fertirrigações e o P adicionado ao substrato de casca de Pinus e vermiculita antes da semeadura. Quando as plantas estavam com 133 dias após a germinação, foram subdivididas em sistema radicular e parte aérea para determinação da massa seca, altura, área foliar, diâmetro do caule e conteúdo de nutrientes. As doses de N, K e P de 920 mg dm-3, 790 mg dm-3 e 100 mg dm-3, respectivamente, foram suficientes para o desenvolvimento adequado de porta-enxertos de limoeiro cravo em tubetes.

O experimento foi conduzido em estufa telada na FCAV/Unesp Câmpus Jaboticabal-SP, durante o período de novembro de 2005 a janeiro de 2007. O estudo teve por objetivo avaliar componentes do desenvolvimento e do estado nutricional de mudas de laranjeira 'Valência', enxertada sobre citrumeleiro 'Swingle', cultivado em substrato, em função de doses de nitrogênio, fósforo e potássio. O delineamento experimental foi o inteiramente casualizado, em esquema fatorial 3³ + 1, sendo 3 fatores (nitrogênio, fósforo e potássio), 3 doses e uma testemunha (sem adubação), com 3 repetições. A unidade experimental foi constituída de uma muda de laranjeira por sacolas de 5 dm-3, contendo 2,5 kg de substrato casca de Pinus e vermiculita. Os tratamentos foram constituídos pela aplicação das seguintes doses de nutrientes em mg por dm³ de substrato: N1/2:459, N1:918 e N2:1836; P1/2:92, P1:184 e P2: 368; K1/2:438, K1:876 e K2:1752. Aos 424 dias após a semeadura, as plantas foram divididas em raízes e parte aérea para a determinação da massa da matéria seca, altura, área foliar, diâmetro do caule e conteúdo de nutrientes. As adubações com N, P e K proporcionaram maior crescimento e maior acúmulo de N, P e K na parte aérea e nas raízes das mudas de laranjeira, em substrato de casca de Pinus e vermiculita, em relação à testemunha. A dose de 459 mg dm-3 de N e as doses de P e K 184 e 876 mg dm-3, respectivamente, proporcionaram melhor crescimento da parte aérea das mudas; porém, na dose recomendada de N de 918 mg dm-3, ocorreu maior crescimento do sistema radicular.
