OBJECTIVE The risk of carrying methicillin-resistant Staphylococcus aureus (MRSA) is higher among nursing home (NH) residents than in the general population. However, control strategies are not clearly defined in this setting. In this study, we compared the impact of standard precautions either alone (control) or combined with screening of residents and decolonization of carriers (intervention) to control MRSA in NHs. DESIGN Cluster randomized controlled trial SETTING NHs of the state of Vaud, Switzerland PARTICIPANTS Of 157 total NHs in Vaud, 104 (67%) participated in the study. INTERVENTION Standard precautions were enforced in all participating NHs, and residents underwent MRSA screening at baseline and 12 months thereafter. All carriers identified in intervention NHs, either at study entry or among newly admitted residents, underwent topical decolonization combined with environmental disinfection, except in cases of MRSA infection, MRSA bacteriuria, or deep skin ulcers. RESULTS NHs were randomly allocated to a control group (51 NHs, 2,412 residents) or an intervention group (53 NHs, 2,338 residents). Characteristics of NHs and residents were similar in both groups. The mean screening rates were 86% (range, 27%-100%) in control NHs and 87% (20%-100%) in intervention NHs. Prevalence of MRSA carriage averaged 8.9% in both control NHs (range, 0%-43%) and intervention NHs (range, 0%-38%) at baseline, and this rate significantly declined to 6.6% in control NHs and to 5.8% in intervention NHs after 12 months. However, the decline did not differ between groups (P=.66). CONCLUSION Universal screening followed by decolonization of carriers did not significantly reduce the prevalence of the MRSA carriage rate at 1 year compared with standard precautions

Treball de Fi de Carrera sobre la tecnologia .NET de Microsoft. En aquest treball s'ha creat un servei WCF i una petita aplicació de consola per hostatjar-lo. Aquest servei utilitza unes classes de LINQ To SQL per accedir a la base de dades. L'aplicació HotelSpaWPF, creada amb WPF i XAML, consumeix el servei WCF per treballar amb la informació de que es disposa a la base de dades.L'aplicació d'escriptori HotelSpaWPF consta de dues parts, la part corresponent als clients de l'hotel, on podran consultar el serveis, activitats i recursos oferts així com fer peticions, inscripcions o reserves. La part d'administrador permet gestionar tota aquesta informació.

El proyecto consiste en desarrollar un software para la gestión de los servicios que ofrece un Hotel Spa.Por un lado, un software con el que los clientes podrán interactuar para la petición de servicios al hotel. Estas peticiones de servicios son: -Peticiones servicios de habitaciones (incidencias, peticiones de material, servicio despertador, servicio limpieza, etc.) -Reservar un recurso (salas de reunión, pistas de deportes, etc.) -Planificar actividades realizadas en el recinto del hotel (gimnasio, masajista, conferencias, audiciones, espectáculos musicales, etc.)Y por otro lado el software necesario para gestionar estas peticiones que estará a la disposición de los encargados del hotel.El nombre de la aplicación es: eHotelSPANet.Para el desarrollo del software se utilizarán las herramientas de Microsoft usando la tecnología .NET, concretamente: -WPF (Windows Presentation Foundation). -WCF (Windows Communications Foundation) SQL Server 2008

Official certificates of stillbirth and infant death are analysed in the birth cohort of 1979-81. Congenital malformations account for approx. 40% of infant mortality. Cantonal differences in malformation rates are not explained by different incidence of such malformations only, but also by differences in lethality. Incidence of Anencephaly is examined in detail.

The MRSA-Screen test (Denka Seiken Co., Ltd., Tokyo, Japan), consisting of a slide latex agglutination kit that detects PBP 2a with a monoclonal antibody, was blindly compared to the oxacillin disk diffusion test, the oxacillin-salt agar screen, and PCR of the mecA gene for the detection of methicillin resistance in Staphylococcus aureus. A total of 120 methicillin-susceptible S. aureus (MSSA) and 80 methicillin-resistant S. aureus (MRSA) isolates, defined by the absence or presence of the mecA gene, respectively, were tested. The MRSA-Screen test, the oxacillin disk diffusion test, and the oxacillin-salt agar screening test showed sensitivities of 100, 61.3, and 82.5% and specificities of 99.2, 96.7, and 98.3%, respectively. We conclude that the MRSA-Screen is a very accurate, reliable, and fast test (15 min) for differentiation of MRSA from MSSA colonies on agar plates.

En 2004, en Hollande, une souche particulière de Staphylococcus aureus résistant à la méthicilline (MRSA2 pour Methicillin-Resistant Staphylococcus aureus) a été découverte chez des personnes en contact avec des porcs. Après investigations, il s'est avéré que 39 % des porcs hollandais et 23 % des éleveurs de porcs étaient porteurs (dans leur conduit nasal) de cette souche dont le profil génétique obtenu par la technique MLST3 (ST398) est différent des clones MRSA habituellement responsables des infections nosocomiales (Maugat et al. 2009 ; Lucet et al. 2005). Depuis, un nombre croissant d'études concernant l'émergence de cette souche dans d'autres pays (Canada, France, Allemagne, Angleterre, Belgique, Italie, Espagne, Danemark et Singapour) et chez d'autres animaux (chevaux, chiens, vaches et poulets) ont été publiées. En janvier 2009, une étude rapporte que cette souche vient d'être isolée aux USA lors du contrôle de deux très grands élevages de porcs. Les souches MRSA ST398 ont été retrouvées chez 70 % des animaux et chez 9 des 14 des employés de l'un des deux élevages concernés. En Hollande, la proportion des éleveurs de porcs colonisés par ces MRSA est passée de 23 % en 2004 à 50 % en 2008 (contre 0,03 % dans la population générale, c'est-à-dire sans contact avec des animaux de ferme). Dans plusieurs pays, la possibilité d'une transmission de l'animal à l'homme, puis inter-humaine a été confirmée par plusieurs études et concernerait en premier lieu les éleveurs, leur famille et les vétérinaires (Khanna et al. 2008 ; Smith et al. 2008 ; Wulf et al. 2008). Dès lors, les élevages de porcs sont susceptibles de constituer un réservoir4 important de MRSA qui peuvent se propager à d'autres animaux et à l'homme. L'apparition de cette nouvelle zoonose peut avoir de graves impacts sur la santé publique et constitue un risque professionnel émergent pour les éleveurs de porcs. En effet, si dans la très grande majorité des cas, la colonisation nasale de l'homme par ces MRSA reste asymptomatique, l'implication de ces MRSA ST398 dans des complications infectieuses (endocardite, pneumonie, septicémie et infection de la peau) a déjà été observée. To evaluate the long-term impact of successive interventions on rates of methicillin-resistant Staphylococcus aureus (MRSA) colonization or infection and MRSA bacteremia in an endemic hospital-wide situation. DESIGN:Quasi-experimental, interrupted time-series analysis. The impact of the interventions was analyzed by use of segmented regression. Representative MRSA isolates were typed by use of pulsed-field gel electrophoresis. SETTING:A 950-bed teaching hospital in Seville, Spain. PATIENTS:All patients admitted to the hospital during the period from 1995 through 2008. METHODS:Three successive interventions were studied: (1) contact precautions, with no active surveillance for MRSA; (2) targeted active surveillance for MRSA in patients and healthcare workers in specific wards, prioritized according to clinical epidemiology data; and (3) targeted active surveillance for MRSA in patients admitted from other medical centers. RESULTS:Neither the preintervention rate of MRSA colonization or infection (0.56 cases per 1,000 patient-days [95% confidence interval {CI}, 0.49-0.62 cases per 1,000 patient-days]) nor the slope for the rate of MRSA colonization or infection changed significantly after the first intervention. The rate decreased significantly to 0.28 cases per 1,000 patient-days (95% CI, 0.17-0.40 cases per 1,000 patient-days) after the second intervention and to 0.07 cases per 1,000 patient-days (95% CI, 0.06-0.08 cases per 1,000 patient-days) after the third intervention, and the rate remained at a similar level for 8 years. The MRSA bacteremia rate decreased by 80%, whereas the rate of bacteremia due to methicillin-susceptible S. aureus did not change. Eighty-three percent of the MRSA isolates identified were clonally related. All MRSA isolates obtained from healthcare workers were clonally related to those recovered from patients who were in their care. CONCLUSION:Our data indicate that long-term control of endemic MRSA is feasible in tertiary care centers. The use of targeted active surveillance for MRSA in patients and healthcare workers in specific wards (identified by means of analysis of clinical epidemiology data) and the use of decolonization were key to the success of the program.

Patients and healthy individuals intermittently and inconsistently carry different methicillin-resistant Staphylococcus aureus (MRSA) subtypes. In the present study, we assessed the clonality of methicillin-susceptible S. aureus (MSSA) and MRSA strains in patients admitted to 1 of 6 intensive care units (ICUs), using spa typing and multilocus variable number of tandem repeats analysis (MLVA).

Objective. To study the acquisition and cross-transmission of Staphylococcus aureus in different intensive care units (ICUs). Methods. We performed a multicenter cohort study. Six ICUs in 6 countries participated. During a 3-month period at each ICU, all patients had nasal and perineal swab specimens obtained at ICU admission and during their stay. All S. aureus isolates that were collected were genotyped by spa typing and multilocus variable-number tandem-repeat analysis typing for cross-transmission analysis. A total of 629 patients were admitted to ICUs, and 224 of these patients were found to be colonized with S. aureus at least once during ICU stay (22% were found to be colonized with methicillin-resistant S. aureus [MRSA]). A total of 316 patients who had test results negative for S. aureus at ICU admission and had at least 1 follow-up swab sample obtained for culture were eligible for acquisition analysis. Results. A total of 45 patients acquired S. aureus during ICU stay (31 acquired methicillin-susceptible S. aureus [MSSA], and 14 acquired MRSA). Several factors that were believed to affect the rate of acquisition of S. aureus were analyzed in univariate and multivariate analyses, including the amount of hand disinfectant used, colonization pressure, number of beds per nurse, antibiotic use, length of stay, and ICU setting (private room versus open ICU treatment). Greater colonization pressure and a greater number of beds per nurse correlated with a higher rate of acquisition for both MSSA and MRSA. The type of ICU setting was related to MRSA acquisition only, and the amount of hand disinfectant used was related to MSSA acquisition only. In 18 (40%) of the cases of S. aureus acquisition, cross-transmission from another patient was possible. Conclusions. Colonization pressure, the number of beds per nurse, and the treatment of all patients in private rooms correlated with the number of S. aureus acquisitions on an ICU. The amount of hand disinfectant used was correlated with the number of cases of MSSA acquisition but not with the number of cases of MRSA acquisition. The number of cases of patient-to-patient cross-transmission was comparable for MSSA and MRSA.

Osteoporosis is characterized by low bone mass, micro architectural impairment of bone tissue, and a subsequent in crease in fracture risk. Fractures or the vertebrae and distal forearm, as well as the proximal femur, or hip fracture, are included. Hip fracture is associated with high mortality, morbidity and medical expenses. There is a dramatic increase in the incidence of hip fracture with age. Hip fracture incidence is 350 times higher in women aged 85 years and over comparatively to women between 35 and 44 years of age. In recent studies in Switzerland, it was observed that the annual age adjusted incidence rate of hip fracture was comparable with similar rates for white population in industrialized countries, although in men the rates were relatively high. Among the major risk factors for osteoporosis are age, female gender, white and Asian race, and menopause. Postmenopausal estrogen replacement therapy reduces bone resorption. Family history of osteoporosis, frail constitution, as well as excessive alcohol intake, cigarette smoking, chronic insufficient nutritional calcium intake and physical inactivity are other risk factors. A cardinal element is the peak bone mass reached in the third or fourth decade of life. Independently of osteoporosis, falls are a key agent in fractures; several medical conditions and drugs increase the risk of falling. There is an enormous social and financial cost of osteoporosis; the annual cost of medical treatment only for hip fracture is close to Fr. 200 million in Switzerland. The burden of osteoporosis is likely to increase in the future because of the demographic aging of the population unless large scale preventive interventions are undertaken.
