BACKGROUND In contrast to objective structured clinical examinations (OSCEs), mini-clinical evaluation exercises (mini-CEXs) take place at the clinical workplace. As both mini-CEXs and OSCEs assess clinical skills, but within different contexts, this study aims at analyzing to which degree students' mini-CEX scores can be predicted by their recent OSCE scores and/or context characteristics. METHODS Medical students participated in an end of Year 3 OSCE and in 11 mini-CEXs during 5 different clerkships of Year 4. The students' mean scores of 9 clinical skills OSCE stations and mean 'overall' and 'domain' mini-CEX scores, averaged over all mini-CEXs of each student were computed. Linear regression analyses including random effects were used to predict mini-CEX scores by OSCE performance and characteristics of clinics, trainers, students and assessments. RESULTS A total of 512 trainers in 45 clinics provided 1783 mini-CEX ratings for 165 students; OSCE results were available for 144 students (87 %). Most influential for the prediction of 'overall' mini-CEX scores was the trainers' clinical position with a regression coefficient of 0.55 (95 %-CI: 0.26-0.84; p < .001) for residents compared to heads of department. Highly complex tasks and assessments taking place in large clinics significantly enhanced 'overall' mini-CEX scores, too. In contrast, high OSCE performance did not significantly increase 'overall' mini-CEX scores. CONCLUSION In our study, Mini-CEX scores depended rather on context characteristics than on students' clinical skills as demonstrated in an OSCE. Ways are discussed which focus on either to enhance the scores' validity or to use narrative comments only.

Systemic sclerosis (SSc) or Scleroderma is a complex disease and its etiopathogenesis remains unelucidated. Fibrosis in multiple organs is a key feature of SSc and studies have shown that transforming growth factor-β (TGF-β) pathway has a crucial role in fibrotic responses. For a complex disease such as SSc, expression quantitative trait loci (eQTL) analysis is a powerful tool for identifying genetic variations that affect expression of genes involved in this disease. In this study, a multilevel model is described to perform a multivariate eQTL for identifying genetic variation (SNPs) specifically associated with the expression of three members of TGF-β pathway, CTGF, SPARC and COL3A1. The uniqueness of this model is that all three genes were included in one model, rather than one gene being examined at a time. A protein might contribute to multiple pathways and this approach allows the identification of important genetic variations linked to multiple genes belonging to the same pathway. In this study, 29 SNPs were identified and 16 of them located in known genes. Exploring the roles of these genes in TGF-β regulation will help elucidate the etiology of SSc, which will in turn help to better manage this complex disease. ^

In this paper an analytical static approach to analyse buried tunnels under seismic surface waves (Rayleigh and Love waves), propagating parallel to the tunnels axis, is provided. In the proposed method, the tunnel is considered as a beam on elastic foundation by using a Winkler model to represent the subgrade reaction and the soil-structure interaction. The seismic load is imposed by giving at the base of the soil springs a determined configuration corresponding to the free-field motion. From the solution of the differential governing equations of the problem, results are obtained in form of relative displacements between points of tunnel, and therefore the seismic bending moments and shearing forces, acting on the tunnel cross section, can be computed.

Individuals from the same population share a number of contextual circumstances that may condition a common level of blood pressure over and above individual characteristics. Understanding this population effect is relevant for both etiologic research and prevention strategies. Using multilevel regression analyses, the authors quantified the extent to which individual differences in systolic blood pressure (SBP) could be attributed to the population level. They also investigated possible cross-level interactions between the population in which a person lived and pharmacological (antihypertensive medication) and nonpharmacological (body mass index) effects on individual SBP. They analyzed data on 23,796 men and 24,986 women aged 35-64 years from 39 worldwide Monitoring of Trends and Determinants in Cardiovascular Disease (MONICA) study populations participating in the final survey of this World Health Organization project (1989-1997). SBP was positively associated with low educational achievement, high body mass index, and use of antihypertensive medication and, for women, was negatively associated with smoking. About 7-8% of all SBP differences between subjects were attributed to the population level. However, this population effect was particularly strong (i.e., 20%) in antihypertensive medication users and overweight women. This empirical evidence of a population effect on individual SBP emphasizes the importance of developing population-wide strategies to reduce individual risk of hypertension.

The consensus from published studies is that plasma lipids are each influenced by genetic factors, and that this contributes to genetic variation in risk of cardiovascular disease. Heritability estimates for lipids and lipoproteins are in the range .48 to .87, when measured once per study participant. However, this ignores the confounding effects of biological variation measurement error and ageing, and a truer assessment of genetic effects on cardiovascular risk may be obtained from analysis of longitudinal twin or family data. We have analyzed information on plasma high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterol, and triglycerides, from 415 adult twins who provided blood on two to five occasions over 10 to 17 years. Multivariate modeling of genetic and environmental contributions to variation within and across occasions was used to assess the extent to which genetic and environmental factors have long-term effects on plasma lipids. Results indicated that more than one genetic factor influenced HDL and LDL components of cholesterol, and triglycerides over time in all studies. Nonshared environmental factors did not have significant long-term effects except for HDL. We conclude that when heritability of lipid risk factors is estimated on only one occasion, the existence of biological variation and measurement errors leads to underestimation of the importance of genetic factors as a cause of variation in long-term risk within the population. In addition our data suggest that different genes may affect the risk profile at different ages.

This Study examined the muldlevel reladonships among negadve affect Behavioural Inhibidon System (BIS) Sensidvit)' and performance. It also invesdgated whether the reladonship among these variables changed across pracdce. Pardcipants performed muldple trials of a simulated air traffic control task, A single measure of BIS was taken before pracdce, while negadve affect and performance were measured at repeated intervals. As expected, negadve affect was detrimental to performance at both a between-person and withinperson level, BIS was also found to be detrimental to performance. Contrary' to expectadons, the reladonship between BIS and performance was not mediated by overall levels of negadve affect. As predicted, the effects of overall levels of negadve affect and BIS strengthened across pracdce as pardcipants gained task knowledge and skill. The findings of this study are interpreted using resource allocadon theor}' and the implicadons for skiU acquisidon discussed.

Recently, goal orientation, a mental framework for understanding how individuals approach learning and achievement situadons, has emerged as an important predictor of performance. This study addressed the effects of domain-specific avoid and prove orientations on performance from the betweenand within-person levels of analysis. One hundred and three participants performed thirty trials of an airtraffic control task. Domain-specific avoid and prove orientations were measured before each trial to assess the effects of changes in goal orientadon on changes in performance (i.e. within-person relationships). Average levels of avoid and prove orientations were calculated to assess the effect of goal orientation on overall performance (i.e. between-person relationships). Findings from the between-person level of analysis revealed that high prove-orientated individuals performed better than low proveorientated individuals. Results also revealed that average goal orientation levels moderated the withinperson relationships. The effect of changes in avoid orientation on changes in performance was stronger for low versus high avoid-oriented individuals while the effect of changes in prove orientadon on changes in performances was stronger for low versus highprove oriented individuals. Implications of these findings are considered.

This study advances research on interpersonal exchange relationships by integrating social exchange, workplace friendship and climate research to develop a multilevel model. Data were collected from 215 manager-employee dyads working within 36 teams. At the individual level, LMX was positively associated with TMX and workplace friendship. Further, workplace friendship was positively related to TMX, and mediated the LMX-TMX relationship. At the team level, HLM results demonstrated that the relationship between LMX and workplace friendship was moderated by affective climate. Findings suggest that high-quality LMX relationships are associated with enhanced employees' perceptions of workplace friendship when affective group climate was strong.

Sponsorship fit is frequently mentioned and empirically examined as a success factor of sponsorship. While sponsorship fit has been considered as a determinant of sponsorship success, little knowledge exists about the antecedents of sponsorship fit. In the present paper, individual and firm-level antecedents of sponsorship fit are examined in a single hierarchical linear model. Results show that sponsorship fit is influenced by the perception of benefits, the firm’s regional identification, sincerity, relatedness to the sponsored activity, and its dominance. On a partnership level, results show that contract length contributes to sponsorship fit while contract value is found to be unrelated.

Background It is important to assess context to explain inequalities in oral health, particularly with regard to the type of service used; thus, this study aimed to identify the social determinants of public dental service use by adults and to assess whether, beyond the level individual, existing inequalities are also expressed in the context in which individuals are embedded. Methods A multilevel analysis with three levels of aggregation of variables was performed. The individual variables were derived from the database of the SB Minas Gerais project—a survey of oral health status of the population of Minas Gerais, a state of the Brazilian Southeast region. The variable at the neighborhood level came from the Census of 2010. The variables at the municipal level were obtained from available public databases relating to oral health services. At the municipal level, the Human Development Index (HDI) variable was chosen to represent quality of life in the municipalities. Results In the final model, the following individual variables were associated with greater use of public dental services: lower income (PR = 1.98, 95% CI = 1.53; 2.58), higher number of residents at home (PR = 1.37, 95% CI = 1.11; 1.68) and higher number of teeth requiring treatment (PR = 1.49, 95% CI = 1.20; 1.84). With regard to context variables, a poorer infrastructure (PR = 0.62, 95% CI = 0.40; 0.96) leads to a lower use of public services. Conclusion The use of public services is associated with family income, how this income is divided in households, the need for treatment presented by the individual and the organization of the existing oral health service infrastructure in the municipality.

La présente thèse de doctorat visait d’abord à valider les résultats des études antérieures démontrant un accroissement de la prévalence des problèmes de santé mentale et ensuite, à évaluer l’importance du contexte social dans l’explication des changements de prévalence, à partir des effets d’âge, de période et de cohorte. L’atteinte de ces objectifs s’est faite à partir de deux études empiriques, chacune ciblant sa propre problématique en santé mentale. La première étude, basée sur les données de l’Enquête longitudinale nationale sur les enfants et les jeunes (ELNEJ) de Statistique Canada, a permis de conclure à un accroissement réel de la prévalence du diagnostic de trouble déficitaire de l’attention/hyperactivité (TDA/H) et de la consommation de psychostimulants chez les enfants canadiens entre 1994 et 2007. Toutefois, cette tendance n’est ni constante, ni universelle, puisque des effets de période et d’âge apparaissent clairement : l’augmentation des prévalences est uniquement remarquée dans les années 2000, et survient seulement chez les enfants d’âge scolaire. L’identification d’inégalités de prévalence dues à la période historique et à l’âge des enfants souligne l’importance du contexte social dans la problématique du diagnostic de TDA/H et de la consommation de psychostimulants. La seconde étude a été réalisée à partir des données du Panel Study of Belgian Households (PSBH) et cherchait à expliquer l’accroissement des symptômes dépressifs observé ces vingt dernières années chez les adultes belges. L’utilisation de l’analyse multiniveaux longitudinale permettant la distinction des effets d’âge et des effets de cohortes a été privilégiée. Bien que l’intensité des symptômes dépressifs ait varié de manière relativement importante chez les individus au cours des années 1990, nos conclusions démontrent que les symptômes auto-rapportés de dépression sont davantage associés aux conditions de vie, qu’à la personnalité. L’augmentation résulte d’un effet de la succession des cohortes, où les individus des cohortes les plus récentes rapportent toujours une plus grande intensité de symptômes dépressifs que les individus des cohortes précédentes. Les membres d’une même cohorte de naissance partagent donc des expériences communes à un âge similaire, ce qui a un impact durable sur leurs comportements et sur leur santé mentale. De manière générale, les résultats des deux articles empiriques ont, chacun à leur manière, confirmé la réalité de l’accroissement des problèmes de santé mentale dans les sociétés occidentales contemporaines, et permis de constater que la prévalence diffère selon l’âge et la cohorte de naissance des individus, ainsi que selon la période historique, renforçant ainsi l’hypothèse de l’importance des facteurs sociaux dans l’étiologie des problèmes de santé mentale. Bien que la nature de ces facteurs n’ait pu être testée de manière directe, de nombreuses explications sociales furent tout de même proposées. À cet égard, des changements dans les normes comportementales associées à l’âge, dans les normes sociales, dans la conceptualisation des troubles mentaux, des modifications dans la sphère éducative, ainsi que des innovations pharmacologiques, médicales et technologiques constituent des explications sociales aux effets d’âge, de période et de cohorte qui ont été observés.
