PURPOSE: It was to determine the prevalence of depressive symptoms in a sample of puerperal women from Brasília, Brazil, distinguishing cases with onset after delivery from those already present during pregnancy. METHODS: A prospective cohort study with convenience sampling of patients submitted to elective cesarean section at two private hospitals. As an instrument for assessing depressive symptoms, the Edinburgh Postnatal Depression Scale with cutoff >13 was applied shortly before delivery and four to eight weeks after childbirth. RESULTS: Among the 107 women who completed the study, 11 (10.3%) had significant depressive symptoms during pregnancy and 12 (11.2%) during the postpartum period. Among the 12 patients with postpartum symptoms, 6 had symptoms during pregnancy, so that 5.6% of the sample had postpartum onset of depression. The higher overall frequency of depression was significantly among single women than among married women (p=0.04), a fact mainly due to a higher frequency of single women experiencing persistent depressive symptoms both before and after delivery (p=0.002). The risk of depression was not influenced by age, parity or educational level. CONCLUSION: Women with depression identified during the postpartum period comprise a heterogeneous group, in which symptoms may have started before pregnancy, during pregnancy or after childbirth. In this sample, half of the postpartum depression cases already presented symptoms during late pregnancy. Since depression can arise before and after childbirth, it may have different etiologies and, therefore, a different response to treatment, a possibility that should be considered by clinicians and researchers.

Dans des contextes de post-urgence tels que le vit la partie occidentale de la République Démocratique du Congo (RDC), l’un des défis cruciaux auxquels font face les hôpitaux ruraux est de maintenir un niveau de médicaments essentiels dans la pharmacie. Sans ces médicaments pour traiter les maladies graves, l’impact sur la santé de la population est significatif. Les hôpitaux encourent également des pertes financières dues à la péremption lorsque trop de médicaments sont commandés. De plus, les coûts du transport des médicaments ainsi que du superviseur sont très élevés pour les hôpitaux isolés ; les coûts du transport peuvent à eux seuls dépasser ceux des médicaments. En utilisant la province du Bandundu, RDC pour une étude de cas, notre recherche tente de déterminer la faisabilité (en termes et de la complexité du problème et des économies potentielles) d’un problème de routage synchronisé pour la livraison de médicaments et pour les visites de supervision. Nous proposons une formulation du problème de tournées de véhicules avec capacité limitée qui gère plusieurs exigences nouvelles, soit la synchronisation des activités, la préséance et deux fréquences d’activités. Nous mettons en œuvre une heuristique « cluster first, route second » avec une base de données géospatiales qui permet de résoudre le problème. Nous présentons également un outil Internet qui permet de visualiser les solutions sur des cartes. Les résultats préliminaires de notre étude suggèrent qu’une solution synchronisée pourrait offrir la possibilité aux hôpitaux ruraux d’augmenter l’accessibilité des services médicaux aux populations rurales avec une augmentation modique du coût de transport actuel.

En aquest projecte es vol donar solució a la contaminació que es produeix a les llars ja sigui en forma de gasos d’efecte hivernacle o la contaminació d’aqüífers i rius per les aigües residuals d’aquestes. Els objectius a aconseguir són la disminució del consum d’energia de la llar, reduint així el consum de combustibles fòssils, amb la conseqüent disminució de gasos d’efecte hivernacle i l’estalvi econòmic que això suposa. I per altra banda la depuració de les aigües residuals, tot i que actualment totes les grans poblacions de Catalunya de més de 10.000 habitants ja disposen de EDARS i en pocs anys en tindran totes les de més de 2000 habitants. Però en aquest cas es tracta d’una població molt petita que no disposa de xarxa de clavegueram. En primer lloc s’ha plantejat un sistema de calefacció d’energia geotèrmica que ens permet aprofitar l’energia tèrmica que es va emmagatzemant a la terra degut a l’escalfor del sol i que ens permet extreure d’aquesta fins a ¾ parts de l’energia tèrmica necessària per el sistema de calefacció. En segon lloc es planteja un sistema de producció d’energia elèctrica per panells fotovoltaics que produeixi el consum elèctric anual familiar, tot i que no de forma directe, sinó venent l’energia a la xarxa elèctrica i comprant-la després de la xarxa. Per últim es projecta un sistema de depuració d’aigües per mitjà d’un aiguamoll artificial que permetrà a la casa, que no disposa de xarxa de clavegueram, retornar les aigües residuals en bones mediambientals a l’entorn rural que envolta l’edificació, sense perjudicar la fauna i els rius que l’envolten

Los hospitales son el pilar fundamental para la atención de las víctimas de situaciones de emergencia o desastre. Las instituciones de salud son consideradas indispensables para la población, por lo que deben estar preparadas para funcionar no solo en condiciones normales sino en situaciones de alerta, como suele suceder en desastres de origen natural como los sismos. La relevancia de la problemática, radica en que de acuerdo con la amenaza sísmica de Colombia, Bogotá se encuentra en una zona de amenaza sísmica intermedia, los actuales eventos naturales como el terremoto de Haití con un saldo de 300.000 muertos y más de 700.000 mil heridos, y el de Chile, nos hace pensar en la importancia de la preparación hospitalaria ante un evento con múltiples víctimas como lo es un sismo. El Objetivo general del estudio es identificar la capacidad de respuesta hospitalaria distrital en Bogotá ante un evento con múltiples víctimas (terremoto). Además se identificaran las oportunidades de mejora para optimizar la respuesta hospitalaria de acuerdo a su nivel de atención. La Investigación se realizó por medio de un estudio de corte transversal, en donde se tomó una muestra de la red hospitalaria Distrital por conveniencia, bajo la aplicación de una encuesta dirigida. Los resultados fueron recopilados en una base de datos de Excel 2013, y fueron analizados bajo un software estadístico, STATA 12.0, donde se evaluaron variables, categóricas, nominales y cuantitativas. Como resultados se encontraron un porcentaje de ocupación de más del 100% en el 25% de los hospitales. Los 16 hospitales encuestados cuentan con comité hospitalario de emergencias, así como también con la elaboración de planes de emergencia y la revisión e implementación de estos. El 50% de los hospitales contemplan dentro su estructura de plan de emergencias, el sistema comando de incidentes; Sólo el 18.8% de los hospitales cuentan con reforzamiento estructural, y el 81,2% de los hospitales refieren tener una cooperación con organizaciones locales o externas. Solo 4 de los 16 hospitales cuentan con protocolos de diagnóstico y tratamiento médico en desastres. El plan de contingencia para un Sismo, es el plan bandera de todos los hospitales dado el esfuerzo por parte de la secretaria de salud y de FOPAE en información y capacitación en todo el Distrito, es por eso que el 93,85 de todos los hospitales cuentan con este plan. Al realizar el análisis general, la red hospitalaria no está en capacidad de una adecuada respuesta en caso de un evento con múltiples víctimas, en el escenario de un sismo de gran magnitud, teniendo en cuenta el porcentaje de ocupación actual donde el 25% de la red hospitalaria distrital cuenta con sobrecupo y el 50% se encuentra a tope de su capacidad instalada. En cuanto a la capacidad de respuesta, no se cuenta con protocolos de atención; Haciendo una evaluación según los niveles de atención, solo los hospitales de III nivel estarían medianamente preparados y con capacidad de respuesta ante un evento con víctimas en masa.

El projecte Torre Iberdrola s’engloba en les actuacions urbanístiques de Bilbao per a regenerar espais industrials no utilitzats situats al Paseo de la Ribera, la Avenida Abandoibarra i la passarel•la Pedro Arrupe. La Torre Iberdrola, situada a la parcel•la 204 del PERI de Abandoibarra (veure plànol nº2), en el districte de Abando (Bilbao), és un gratacels d’oficines de 41 plantes amb una altura total de 165 metres i una superfície construïda total d’aproximadament 62.100 m2 sobre rasant. A més, l’edifici a construir consta de 5 plantes sota rasant amb una superfície total construïda d’aproximadament 32.100 m2. L’objecte del present projecte és presentar la solució acordada conjuntament entre l’arquitecte Pelli Clarke Pelli Architects, les consultories Buro Happold i IDOM, i BELLAPART per a l’execució del revestiment del vestíbul d’entrada de la Torre Iberdrola

L’edifici a rehabilitar està situat en una important avinguda de l’eixample oest de Granollers. En l’actualitat l’edifici es troba a ple rendiment, amb l’ús de magatzem i obrador a la planta baixa i planta primera, i com a habitatge a les plantes segona i tercera. En concret es tracta de la formació d’un total de 6 habitatges i 5 trasters, a més de la reforma general del nucli de comunicació vertical on s’incorpora l’ascensor i la construcció de la paret mitgera que delimitarà l’espai de l’edifici en la seva projecció vertical. La rehabilitació també té com a objectiu reforçar els forjats, en els quals s’ha detectat aluminosi, així com l’enderroc d’envans a la segona i tercera planta per construir una millor distribució que permeti augmentar el nombre d’habitatges per planta

DKA is a severe metabolic derangement characterized by dehydration, loss of electrolytes, hyperglycemia, hyperketonemia, acidosis and progressive loss of consciousness that results from severe insulin deficiency combined with the effects of increased levels of counterregulatory hormones (catecholamines, glucagon, cortisol, growth hormone). The biochemical criteria for diagnosis are: blood glucose > 200 mg/dl, venous pH <7.3 or bicarbonate <15 mEq/L, ketonemia >3 mmol/L and presence of ketonuria. A patient with DKA must be managed in an emergency ward by an experienced staff or in an intensive care unit (ICU), in order to provide an intensive monitoring of the vital and neurological signs, and of the patient's clinical and biochemical response to treatment. DKA treatment guidelines include: restoration of circulating volume and electrolyte replacement; correction of insulin deficiency aiming at the resolution of metabolic acidosis and ketosis; reduction of risk of cerebral edema; avoidance of other complications of therapy (hypoglycemia, hypokalemia, hyperkalemia, hyperchloremic acidosis); identification and treatment of precipitating events. In Brazil, there are few pediatric ICU beds in public hospitals, so an alternative protocol was designed to abbreviate the time on intravenous infusion lines in order to facilitate DKA management in general emergency wards. The main differences between this protocol and the international guidelines are: intravenous fluid will be stopped when oral fluids are well tolerated and total deficit will be replaced orally; if potassium analysis still indicate need for replacement, it will be given orally; subcutaneous rapid-acting insulin analog is administered at 0.15 U/kg dose every 2-3 hours until resolution of metabolic acidosis; approximately 12 hours after treatment initiation, intermediate-acting (NPH) insulin is initiated at the dose of 0.6-1 U/kg/day, and it will be lowered to 0.4-0.7 U/kg/day at discharge from hospital.

Stem cells reside within tissue, ensuring its natural ability to repair an injury. They are involved in the natural repair of damaged tissue, which encompasses a complex process requiring the modulation of cell survival, extracellular matrix turnover, angiogenesis, and reverse remodeling. To date, the real reparative potential of each tissue is underestimated and noncommittal. The assessment of the biophysical properties of the extracellular environment is an innovative approach to better understand mechanisms underlying stem cell function, and consequently to develop safe and effective therapeutic strategies replacing the loss of tissue. Recent studies have focused on the role played by biomechanical signals that drive stem cell death, differentiation, and paracrinicity in a genetic and/or an epigenetic manner. Mechanical stimuli acting on the shape can influence the biochemistry and gene expression of resident stem cells and, therefore, the magnitude of biological responses that promote the healing of injured tissue. Nanotechnologies have proven to be a revolutionary tool capable of dissecting the cellular mechanosensing apparatus, allowing the intercellular cross-talk to be decoded and enabling the reparative potential of tissue to be enhanced without manipulation of stem cells. This review highlights the most relevant findings of stem cell mechanobiology and presents a fascinating perspective in regenerative medicine.

Background Switzerland is confronted with the problem of interpersonal violence. Violence is in the increase and the potential for aggression seems to be rising. Observations by hospitals discern an appalling increase of the severity of the injuries. The aim of this study is to collect accurate information about the social environment, the motivation and possible reasons for violence. We also intend to investigate whether sociocultural, or ethnic differences among male victims exist. Materials and methods For the first time in Switzerland, this survey employed a validated questionnaire from the division of violence prevention, Atlanta, Georgia. The first part of the questionnaire addressed social and demographic factors which could influence the risk of violence: age, gender, duration of stay in Switzerland, nationality and educational level. Beside these social structural factors, the questionnaire included questions on experience of violent offences in the past, information about the most recent violent offence and intra and interpersonal facts. The questionnaire itself consists of 27 questions, translated into German and French. In a pilot study, the questionnaire was checked with adolescents for feasibility and comprehensibility. Results 69 male VIVs were interviewed at two hospitals in the Canton of Bern. Most of the adolescents emphasised that weapons were not used during their confrontations. It is astonishing that all of the young men considered themselves to be victims. Most of the brawls were incited after an exchange of verbal abuse and provocations with unfamiliar individuals. The rivals could neither be classified with the help of ethnic categories nor identifiable groups of the youth scenes. The incidents took place in scenes, where violence was more likely to happen. Interestingly and contrary to a general perception the offenders are well integrated into sport and leisure clubs. A further surprising result of our research is that the attitude towards religion differs between young men with experience of violence and non-violent men. Discussion Youth violence is a health issue, which concerns us globally. The human and economic toll of violence on victims and offenders, their families, and on society in general is high. The economic costs associated with violence-related illness and disability is estimated to be millions of Swiss francs each year. Physicians and psychologists are compelled to identify the factors, which cause young people to be violent, to find out which interventions prove to be successful, and to design effective prevention programs. The identification of effective programs depends on the availability of reliable and valid measures to assess changes in violence-related attitudes. In our efforts to create healthier communities, we need to investigate; document and do research on the causes and circumstances of youth violence.

BACKGROUND: Painful invasive procedures are frequently performed on preterm infants admitted to a neonatal intensive care unit (NICU). The aim of the present study was to investigate current pain management in Austrian, German and Swiss NICU and to identify factors associated with improved pain management in preterm infants. METHODS: A questionnaire was sent to all Austrian, German and Swiss pediatric hospitals with an NICU (n = 370). Pain assessment and documentation, use of analgesics for 13 painful procedures, presence of written guidelines for pain management and the use of 12 analgesics and sedatives were examined. RESULTS: A total of 225 units responded (61%). Pain assessment and documentation and frequent analgesic therapy for painful procedures were performed more often in units using written guidelines for pain management and in those treating >50 preterm infants at <32 weeks of gestation per year. This was also the case for the use of opioid analgesics and sucrose solution. Non-opioid analgesics were used more often in smaller units and in units with written guidelines. There was a broad variation in dosage of analgesics and sedatives within all groups. CONCLUSION: Pain assessment, documentation of pain and analgesic therapy are more frequently performed in NICU with written guidelines for pain management and in larger units with more than 50 preterm infants at <32 weeks of gestation per year.

Introduction: Intervertebral disc degeneration is associated with loss of nucleus pulposus (NP) tissue and reduced disc height[1]. A number of therapies, including synthetic and natural biomaterials, have been developed to restore full disc function and to minimize the pain and disability caused by this disease. Fibrin-based biomaterials are used as a replacement for NP or as a cell carrier for tissue engineering approaches[2]. While the behavior of such gels is well-characterized from a material point of view, little is known about their contribution to intervertebral disc (IVD) restoration under dynamic loads. The aim of the present study is the evaluation of a hyaluronic acid fibrin-based hydrogel (ProCore) used to repair an in vitro model of disc degeneration under dynamic loading. Methods: In vitro model of disc degeneration was induced in intact coccygeal bovine IVD by papain digestion of the NP as previously described[3]. In order to characterize fibrin hydrogels, four experimental groups were considered: 1) intact IVD (control), 2) IVD injected with PBS, 3) injection of hydrogels in degenerative IVD and 4) injection of hydrogels in combination with human bone marrow-derived mesenchymal stem cells (MSC) in degenerative IVD. All of the groups were subjected to dynamic loading protocols consisting of 0.2MPa static compression superimposed with ±2° torsion at 0.2Hz for 8h per day and maintained for 7 days. Additionally, one group consisted of degenerative IVD injected with hydrogel and subjected to static compression. Disc heights were monitored after the duration of the loading and compared to the initial disc height. The macrostructure of the formed tissue and the cellular distribution was evaluated by histological means. Results: After one week of loading, the degenerative IVD filled with hydrogel in combination with MSC (dynamic load), hydrogels (dynamic load) and hydrogels (static load) showed a reduction in height by 30%, 15% and 20%, respectively, as compared to their initial disc height. Histological sections showed that the HA-fibrin gel fully occupied the nucleotomized region of the disc and that fibrin was effective in filling the discontinuities of the cavity region. Furthermore, the cells were homogenously distributed along the fibrin hydrogels after 7 days of loading. Discussion: In this study, we showed that fibrin hydrogels showed a good integration within the papain-induced model of disc degeneration and can withstand the applied loads. Fibrin hydrogels can contribute to disc restoration by possibly maintaining adequate stiffness of the tissue and thus preventing disorganization of the surrounding IVD. References: 1.
