Background. There is considerable debate regarding the clinical issues surrounding the wish to hasten death (WTHD) in the terminally ill. The clinical factors contributing to the WTHD need further investigation among the terminally ill in order to enhance understanding of the clinical assessment and treatment needs that underlie this problem. A more detailed understanding may assist with the development of appropriate therapeutic interventions. Method. A sample of terminally ill cancer patients (N=256) recruited from an in-patient hospice unit, home palliative care service and a general hospital palliative care consulting service from Brisbane Australia between 1998–2001 completed a questionnaire assessing psychological (depression and anxiety), social (family relationship, social support, level of burden on others) and the impact of physical symptoms. The association between these factors and the WTHD was investigated. Results. A high WTHD was reported by 14% of patients. A discriminant function analysis revealed that the following variables were associated with a high WTHD (P<0·001): higher levels of depressive symptoms, being admitted to an in-patient hospice setting, a greater perception of being a burden on others, lower family cohesion, lower levels of social support, higher levels of anxiety and greater impact of physical symptoms. Conclusions. Psychological and social factors are related to a WTHD among terminally ill cancer patients. Greater attention needs to be paid to the assessment of psychological and social issues in order to provide appropriate therapeutic interventions for terminally ill patients.

Background: There are indications that pre-hospital emergency care and management of patients can help reduce the demand for hospital emergency departments (EDs). Ambulance services play a significant role at this stage of care. In 2003, the Queensland Government introduced a Community Ambulance Cover (CAC) levy in return for a free ambulance service at the point of access to all Queenslanders. This may have led to the impression in consumers of an entitlement to free ambulance services under any circumstances regardless of the urgency of the matter which may have in turn contributed to the crowding of EDs in Queensland. Objectives: This paper aims to answer the following questions: - How many patients arrive at hospital EDs by ambulance in Queensland, compared to other modes of arrival? - How has this changed over time, particularly after the CAC introduction in 2003? What percentage of ambulance arrivals are urgent ED patients? - Has the perceived free ambulance services created extra demand for EDs in Queensland, compared with other Australian jurisdictions that charge patients for ambulance services? Methods: We will secondary analyse the data from sources such as Queensland Ambulance Services, Department of Health and Australian Bureau of Statistics to answer the research questions. Findings and Conclusions Queensland has the highest utilization rate of ambulance services (about 18% in 2007-08) and the highest annual growth rate in demand for these services (7.7% on average since 2000-01), well above the population growth. On the other hand, the proportion of ED patients arriving by ambulance in Queensland has increased by about 4% annually. However, when compared with other states and territories with charge at the point of access, it seems that the growth in demand for EDs cannot be explained solely or mainly by CAC or ambulance utilisation in Queensland.

Background: Demand for pre-hospital emergency care is increasing in Australia as in many other countries. Using posthoc criteria such as triage, diagnosis and admission status, some authors view a considerable number of these as "inappropriate". Yet, calling an ambulance at the time of emergency is rarely studied from the patients’ or their carers’ perspective. This study interviewed patients about the decision, circumstances surrounding and reasons for calling an ambulance in Queensland, Australia. Methods: A cross-sectional survey of patients attending a sample of eight public hospital emergency departments in Queensland was undertaken between March and May 2011. In total, 911 questionnaires were collected (response rate: 67%), of whom 226 (24.8%) had arrived by ambulance. Results: In 35.6% of ambulance arrivals, the decision to request an ambulance was made by the patient; 25% by a doctor; 20% by a family member, friend or carer. Other callers included nurse, people at work or school, and passers-by. Reasons to request an ambulance included urgency (87%) and severity (84%) of the condition. Other reasons included requiring special care (76%), getting higher priority at the emergency department (34%), not having a car (34%), and financial concerns (17%). Decision to request an ambulance varied significantly according to the time of illness onset (e.g. on the day, week before), and location (e.g. home, outside). Conclusion: The decision to call an ambulance is made mostly by non-medical professionals in a perceived emergency situation. They call the ambulance for different reasons but mainly take into account the patient’s welfare and safety. Better understanding of these reasons will affect the direction and effectiveness of demand management strategies.

Background and context Since the economic reforms of 1978, China has been acclaimed as a remarkable economy, achieving 9% annual growth per head for more than 25 years. However, China's health sector has not fared well. The population health gains slowed down and health disparities increased. In the field of health and health care, significant progress in maternal care has been achieved. However, there still remain important disparities between the urban and rural areas and among the rural areas in terms of economic development. The excess female infant deaths and the rapidly increasing sex ratio at birth in the last decade aroused serious concerns among policy makers and scholars. Decentralization of the government administration and health sector reform impacts maternal care. Many studies using census data have been conducted to explore the determinants of a high sex ratio at birth, but no agreement has been so far reached on the possible contributing factors. No study using family planning system data has been conducted to explore perinatal mortality and sex ratio at birth and only few studies have examined the impact of the decentralization of government and health sector reforms on the provision and organization of maternal care in rural China. Objectives The general objective of this study was to investigate the state of perinatal health and maternal care and their determinants in rural China under the historic context of major socioeconomic reforms and the one child family planning policy. The specific objectives of the study included: 1) to study pregnancy outcomes and perinatal health and their correlates in a rural Chinese county; 2) to examine the issue of sex ratio at birth and its determinants in a rural Chinese county; 3) to explore the patterns of provision, utilization, and content of maternal care in a rural Chinese county; 4) to investigate the changes in the use of maternal care in China from 1991 to 2003. Materials and Methods This study is based on a project for evaluating the prenatal care programme in Dingyuan county in 1999-2003, Anhui province, China and a nationwide household health survey to describe the changes in maternal care utilization. The approaches used included a retrospective cohort study, cross sectional interview surveys, informant interviews, observations and the use of statistical data. The data sources included the following: 1) A cohort of pregnant women followed from pregnancy up to 7 days after birth in 20 townships in the study county, collecting information on pregnancy outcomes using family planning records; 2) A questionnaire interview survey given to women who gave birth between 2001 and 2003; 3) Various statistical and informant surveys data collected from the study county; 4) Three national household health interview survey data sets (1993-2003) were utilized, and reanalyzed to described the changes in maternity care utilization. Relative risks (RR) and their confidence intervals (CI) were calculated for comparison between parity, approval status, infant sex and township groups. The chi-square test was used to analyse the disparity of use of maternal care between and within urban and rural areas and its trend across the years in China. Logistic regression was used to analyse the factors associated with hospital delivery in rural areas. Results There were 3697 pregnancies in the study cohort, resulting in 3092 live births in a total population of 299463 in the 20 study townships during 1999-2000. The average age at pregnancy in the cohort was 25.9 years. Of the women, 61% were childless, 38% already had one child and 0.3% had two children before the current pregnancy. About 90% of approved pregnancies ended in a live birth while 73% of the unapproved ones were aborted. The perinatal mortality rate was 69 per thousand births. If the 30 induced abortions in which the gestational age was more than 28 weeks had been counted as perinatal deaths, the perinatal mortality rate would have been as high as 78 per thousand. The perinatal mortality rate was negatively associated with the wealth of the township. Approximately two thirds of the perinatal deaths occurred in the early neonatal period. Both the still birth rate and the early neonatal death rate increased with parity. The risk of a stillbirth in a second pregnancy was almost four times that for a first pregnancy, while the risk of early neonatal deaths doubled. The early neonatal mortality rate was twice as high for female as for male infants. The sex difference in the early neonatal mortality rate was mainly attributable to mortality in second births. The male early neonatal mortality rate was not affected by parity, while the female early neonatal mortality rate increased dramatically with parity: it was about six times higher for second births than for first births. About 82% early neonatal deaths happened within 24 hours after birth, and during that time, girls were almost three times more likely to die than boys. The death rate of females on the day of birth increased much more sharply with parity than that of males. The total sex ratio at birth of 3697 registered pregnancies was 152 males to 100 females, with 118 and 287 in first and second pregnancies, respectively. Among unapproved pregnancies, there were almost 5 live-born boys for each girl. Most prenatal and delivery care was to be taken care of in township hospitals. At the village level, there were small private clinics. There was no limitation period for the provision of prenatal and postnatal care by private practitioners. They were not permitted to provide delivery care by the county health bureau, but as some 12% of all births occurred either at home or at private clinics; some village health workers might have been involved. The county level hospitals served as the referral centers for the township hospitals in the county. However, there was no formal regulation or guideline on how the referral system should work. Whether or not a woman was referred to a higher level hospital depended on the individual midwife's professional judgment and on the clients' compliance. The county health bureau had little power over township hospitals, because township hospitals had in the decentralization process become directly accountable to the township government. In the township and county hospitals only 10-20% of the recurrent costs were funded by local government (the township hospital was funded by the township government and the county hospital was funded by the county government) and the hospitals collected user fees to balance their budgets. Also the staff salaries depended on fee incomes by the hospital. The hospitals could define the user charges themselves. Prenatal care consultations were however free in most township hospitals. None of the midwives made postnatal home visits, because of low profit of these services. The three national household health survey data showed that the proportion of women receiving their first prenatal visit within 12 weeks increased greatly from the early to middle 1990s in all areas except for large cities. The increase was much larger in the rural areas, reducing the urban-rural difference from more than 4 times to about 1.4 times. The proportion of women that received antenatal care visits meeting the Ministry of Health s standard (at least 5 times) in the rural areas increased sharply from 12% in 1991-1993 to 36% in 2001-2003. In rural areas, the proportion increase was much faster in less developed areas than in developed areas. The hospital delivery rate increased slightly from 90% to 94% in urban areas while the proportion increased from 27% to 69% in rural areas. The fastest change was found to be in type 4 rural areas, where the utilization even quadrupled. The overall difference between rural and urban areas was substantially narrowed over the period. Multiple logistic regression analysis shows that time periods, residency in rural or urban areas, income levels, age group, education levels, delivery history, occupation, health insurance and distance from the nearest health care facilities were significantly associated with hospital delivery rates. Conclusions 1. Perinatal mortality in this study was much higher than that for urban areas as well as any reported rate from specific studies in rural areas of China. Previous studies in which calculations of infant mortality were not based on epidemiological surveys have been shown to underestimate the rates by more than 50%. 2. Routine statistics collected by the Chinese family planning system proved to be a reliable data source for studying perinatal health, including still births, neonatal deaths, sex ratio at birth and among newborns. National Household Health Survey data proved to be a useful and reliable data source for studying population health and health services. Prior to this research there were few studies in these areas available to international audiences. 3.Though perinatal mortality rate was negatively associated with the level of township economic development, the excess female early neonatal mortality rate contributed much more to high perinatal mortality rate than economic factors. This was likely a result of the role of the family planning policy and the traditional preferences for sons, which leads to lethal neglect of female newborns and high perinatal mortality. 4. The selective abortions of female foetuses were likely to contribute most to the high sex ratio at birth. The underreporting of female births seemed to have played a secondary role. The higher early neonatal mortality rate in second-born as compared to first-born children, particularly in females, may indicate that neglect or poorer care of female newborn infants also contributes to the high sex ratio at birth or among newborns. Existing family planning policy proved not to effectively control the steadily increased birth sex ratio. 5. The rural-urban gap in service utilization was on average significantly narrowed in terms of maternal healthcare in China from 1991 to 2003. This demonstrates that significant achievements in reducing inequities can be made through a combination of socio-economic development and targeted investments in improving health services, including infrastructure, staff capacities, and subsidies to reduce the costs of service utilization for the poorest. However, the huge gap which persisted among cities of different size and within different types of rural areas indicated the need for further efforts to support the poorest areas. 6. Hospital delivery care in the study county was better accepted by women because most of women think delivery care was very important while prenatal and postnatal care were not. Hospital delivery care was more systematically provided and promoted than prenatal and postnatal care by township hospital in the study area. The reliance of hospital staff income on user fees gave the hospitals an incentive to put more emphasis on revenue generating activities such as delivery care instead of prenatal and postnatal care, since delivery care generated much profits than prenatal and postnatal care . Recommendations 1. It is essential for the central government to re-assess and modify existing family planning policies. In order to keep national sex balance, the existing practice of one couple one child in urban areas and at-least-one-son a couple in rural areas should be gradually changed to a two-children-a-couple policy throughout the country. The government should establish a favourable social security policy for couples, especially for rural couples who have only daughters, with particular emphasis on their pension and medical care insurance, combined with an educational campaign for equal rights for boys and girls in society. 2. There is currently no routine vital-statistics registration system in rural China. Using the findings of this study, the central government could set up a routine vital-statistics registration system using family planning routine work records, which could be used by policy makers and researchers. 3. It is possible for the central and provincial government to invest more in the less developed and poor rural areas to increase the access of pregnant women in these areas to maternal care services. Central government together with local government should gradually provide free maternal care including prenatal and postnatal as well as delivery care to the women in poor and less developed rural areas. 4. Future research could be done to explore if county and the township level health care sector and the family planning system could be merged to increase the effectiveness and efficiency of maternal and child care. 5. Future research could be done to explore the relative contribution of maternal care, economic development and family planning policy on perinatal and child health using prospective cohort studies and community based randomized trials. Trata-se de um estudo quantitativo, descritivo, visando uma análise das condições de trabalho em unidades intensivas de um hospital universitário no município do Rio de Janeiro. Foi definido como objeto de estudo a percepção dos trabalhadores de enfermagem sobre os riscos ocupacionais e os problemas de saúde inerentes às condições de trabalho em unidades intensivas e como problema de pesquisa: quais os riscos ocupacionais e problemas de saúde relacionados às condições de trabalho, percebidos pelos trabalhadores de enfermagem, em unidades intensivas de um hospital universitário? O objetivo geral foi estudar nas unidades intensivas os riscos ocupacionais e problemas de saúde da equipe de enfermagem e sua relação com condições de trabalho, a partir da percepção dos mesmos. Os objetivos específicos traçados foram: identificar as características pessoais e profissionais dos trabalhadores de enfermagem de unidades intensivas; descrever os fatores de risco do ambiente de trabalho percebidos pelos trabalhadores de enfermagem; levantar os problemas de saúde percebidos pelos trabalhadores e sua relação com o trabalho; analisar a associação entre os problemas de saúde percebidos pelos trabalhadores de enfermagem e as condições do trabalho em unidades intensivas. Participaram da pesquisa 125 profissionais de enfermagem de quatro unidades intensivas do Hospital Universitário (HU) entre Maio e Julho de 2009. A predominância foi de profissionais do sexo feminino, com idade acima dos 40 anos, com mais de um vínculo empregatício e trabalhando no HU há mais de 10 anos. Os riscos ocupacionais mais percebidos pelos trabalhadores foram os ergonômicos, seguido dos biológicos, de acidentes, físicos e químicos. Os problemas de saúde mais frequentes foram varizes, problemas oculares, lombalgias, estresse e depressão, transtornos do sono, lesões de coluna vertebral, dores de cabeça, mudanças no humor, dores musculares crônicas e hipertensão arterial. Pela associação entre riscos ocupacionais e problemas de saúde, conclui-se que os trabalhadores expostos a fatores de riscos ergonômicos e físicos têm maior probabilidade de adquirir problemas de saúde osteoarticulares e circulatórios (varizes). Diante dos dados desta pesquisa faz-se necessário aprofundamento da investigação sobre os fatores de riscos encontrados e possíveis medidas para minimizá-los, mediante novos estudos. Como recomendações destacam-se a criação de um espaço de discussão entre os gerentes e trabalhadores para a elaboração de um programa que vise a promoção e proteção da saúde do trabalhador de enfermagem de unidades intensivas; implementação de medidas de controle específicas para cada tipo de risco evidenciado e a criação de um Comitê de Ergonomia para operacionalizar a implementação das melhorias no HU, a fim de consolidar as transformações esperadas.

Apesar da definição da Estratégia de Saúde da Família (ESF) como porta de entrada preferencial do sistema de saúde e estratégia de reorganização da assistência, os usuários do SUS, vêm demonstrando historicamente preferência pelo serviço de urgência/emergência hospitalar. Neste contexto, o campo do presente estudo é a cidade de Piraí e seus habitantes, que desde 2002 contam com 100% de cobertura da ESF, modelo que dá ênfase: à lógica territorial na assistência, no cuidado continuado e transversal, no vínculo e no acesso facilitado pelo acolhimento humanizado e escuta qualificada; ocupando o centro da rede de serviços atuando como ordenador e coordenador do cuidado. Avaliando os dados de produtividade (com foco nas consultas médicas) hospitalar e da ESF notamos que a busca por assistência médica hospitalar, tem aumentado exponencialmente, e pode-se perceber que a grande maioria destes usuários se apresenta ao serviço com demandas de atenção básica, o que é considerado ilógico e contraditório na visão de gestores e profissionais. A prática profissional tem me levado a um processo de reflexão sobre as expectativas dos usuários ao procurarem o sistema de saúde (principalmente a ESF), sobre os caminhos que cada um deles constrói diante de uma questão de saúde e como se dá a tomada de decisão em busca da resolutividade da questão. Dessa forma, o objetivo deste trabalho é compreender como se constrói essa demanda; que critérios estão envolvidos na tomada de decisão desses usuários ao optarem pelo serviço de emergência como porta de entrada preferencial; mesmo em um município que oferece um serviço estruturado, pautado nas diretrizes da ESF e com uma cobertura que alcança toda a sua população. Acreditamos que o processo conhecido como medicalização da vida, que descreve o processo pelo qual problemas não médicos são definidos e tratados como problemas médicos, usualmente em termos de doenças e desordens (CONRAD, 2007); influencie na construção dessa demanda. Quanto a metodologia, foram realizadas entrevistas semiestruturadas, com usuários do SUS, residentes no município e que buscaram espontaneamente o serviço de urgência/emergência hospitalar. Verificou-se que a imagem que o usuário faz dos serviços de saúde se relaciona principalmente com o tempo de espera pelo atendimento, o acesso (interpretado principalmente como a certeza/incerteza do atendimento) e a acessibilidade. Os usuários frequentemente se referem à organização das unidades da ESF com o significado de barreiras ao acesso (principalmente pela necessidade de agendamento) e demonstram ter em relação às USF uma imagem de grande limitação de recursos humanos (quase exclusivamente em relação ao médico) e materiais. Por outro lado, prontos-socorros e hospitais se apresentam para eles, por várias razões, como espaços de acesso garantido. É importante ressaltar que o processo de medicalização da vida aparece como parte importante da engrenagem que move a construção dessa demanda.

A extensão universitária vem se institucionalizando nas universidades públicas brasileiras. Porém pouco se conhece sobre seu desenvolvimento nas unidades acadêmicas. Este trabalho teve como objeto analisar o desenvolvimento da extensão universitária no Instituto de Nutrição da UERJ (NUT) no período de 1990-2014. Trata-se de estudo historiográfico, baseado em pesquisa bibliográfica e na análise dos documentos coletados em arquivos e registros institucionais da UERJ, complementadas com entrevistas com informantes chave. A análise do material levantado abarcou: sistematização do histórico de institucionalização da extensão na UERJ, caracterização dos projetos desenvolvidos no NUT até 2014 e análise dos projetos ativos no período de 2005 a 2014 com pelo menos cinco anos de atividade. A caracterização dos projetos considerou, entre outros, as categorias: área temática, palavras-chave, abrangência geográfica, parcerias, situação curricular e equipe do projeto. A análise dos projetos se deu por meio da sistematização de seus relatórios e pelo seu cotejamento com as linhas de extensão propostas pelo FORPROEX e com as diretrizes da Política Nacional de Alimentação e Nutrição (PNAN) e da Política Nacional de Segurança Alimentar e Nutricional (PNSAN). Nas últimas décadas, houve grandes avanços na institucionalização da extensão na UERJ. De 1990 a 2014, foram identificados 73 projetos de extensão desenvolvidos no NUT. Foi observada ampliação do corpo docente envolvido e da carga horária alocada em atividades extensionistas. Foi notório o crescimento do número de projetos de extensão vigentes a cada ano e do público por eles atingido. O perfil dos projetos de extensão do NUT tem sido voltado, prioritariamente, para as áreas temáticas de saúde e de educação de forma articulada, convergindo com os campos clássicos de atuação da Nutrição e com as vertentes tradicionais da extensão universitária. É amplo o leque de temas abordados nos projetos, destacando-se aqueles ligados ao cuidado em saúde, com ênfase em agravos e grupos populacionais específicos, à promoção da saúde e à educação/ formação. A abrangência geográfica dos projetos se ampliou com o passar dos anos. A maioria deles estabeleceu parcerias, prioritariamente com unidades da área da saúde e com instituições públicas. Somente em uma minoria deles observamos articulação com o currículo de graduação e, em parte deles, relação com pesquisa. A análise dos projetos ativos no período de 2005-2014 indicou que a maioria deles está concentrada nas linhas Saúde Humana, Educação Profissional e Segurança Alimentar e Nutricional e que, no tocante à interface com a PNAN e a PNSAN, os enfoques predominantes se concentram no cuidado nutricional na rede de atenção à saúde, na educação alimentar e nutricional e a na formação continuada. A trajetória da extensão no NUT permite identificar os seguintes desafios para o seu desenvolvimento: avançar na inserção da extensão nos projetos pedagógicos do curso de graduação; traduzir, na prática, o princípio da indissociabilidade ensino-pesquisa-extensão; aprofundar a interface entre a extensão e as políticas públicas; ampliar o escopo de parcerias. Janelas de oportunidade devem ser buscadas e caminhos devem ser percorridos coletivamente no sentido da superação desses desafios na perspectiva de uma prática extensionista que transforme universidade e sociedade.

The aging population in many countries brings into focus rising healthcare costs and pressure on conventional healthcare services. Pervasive healthcare has emerged as a viable solution capable of providing a technology-driven approach to alleviate such problems by allowing healthcare to move from the hospital-centred care to self-care, mobile care, and at-home care. The state-of-the-art studies in this field, however, lack a systematic approach for providing comprehensive pervasive healthcare solutions from data collection to data interpretation and from data analysis to data delivery. In this thesis we introduce a Context-aware Real-time Assistant (CARA) architecture that integrates novel approaches with state-of-the-art technology solutions to provide a full-scale pervasive healthcare solution with the emphasis on context awareness to help maintaining the well-being of elderly people. CARA collects information about and around the individual in a home environment, and enables accurately recognition and continuously monitoring activities of daily living. It employs an innovative reasoning engine to provide accurate real-time interpretation of the context and current situation assessment. Being mindful of the use of the system for sensitive personal applications, CARA includes several mechanisms to make the sophisticated intelligent components as transparent and accountable as possible, it also includes a novel cloud-based component for more effective data analysis. To deliver the automated real-time services, CARA supports interactive video and medical sensor based remote consultation. Our proposal has been validated in three application domains that are rich in pervasive contexts and real-time scenarios: (i) Mobile-based Activity Recognition, (ii) Intelligent Healthcare Decision Support Systems and (iii) Home-based Remote Monitoring Systems.

In mental health services over recent decades, the positive move away from hospital-based care to community-based services has entailed that people with higher levels of need are being supported by community mental health services. This paper begins by reviewing the literature on coercion in the field of community-based mental health care and treatment. It is argued that the lack of a critical understanding of the concept and how it is used by practitioners and agencies can have serious repercussions for the rights of service users. Using a quasi-experimental, longitudinal design, the authors then seek to test some of the ideas about coercion by comparing the activities of assertive outreach and community mental health teams in Northern Ireland, particularly the key ideas of perceived coercion, workers’ strategies and engagement with services. Key findings were that assertive outreach appeared to be more successful at reducing perceived coercion, minimizing the need for coercive strategies, engaging high-risk clients and reducing inpatient bed use. These findings are compared with other studies in this area. The authors also argue that there is a need for greater transparency in the way that practitioners use coercive measures and more explicit guidance is required in this crucial area of mental health practice.

This paper details the implementation and operational performance of a minimum-power 2.45-GHz pulse receiver and a companion on-off keyed transmitter for use in a semi-active duplex RF biomedical transponder. A 50-Ohm microstrip stub-matched zero-bias diode detector forms the heart of a body-worn receiver that has a CMOS baseband amplifier consuming 20 microamps from +3 V and achieves a tangential sensitivity of -53 dBm. The base transmitter generates 0.5 W of peak RF output power into 50 Ohms. Both linear and right-hand circularly polarized Tx-Rx antenna sets were employed in system reliability trials carried out in a hospital Coronary Care Unit, For transmitting antenna heights between 0.3 and 2.2 m above floor level, transponder interrogations were 95% reliable within the 67-m-sq area of the ward, falling to an average of 46 % in the surrounding rooms and corridors. Overall, the circular antenna set gave the higher reliability and lower propagation power decay index.

Objectives: To determine the factors associated with a home death among older adults who received palliative care nursing home services in the home. Methods: The participants in this retrospective cohort study were 151 family caregivers of patients who had died approximately 9 months prior to the study telephone interview. The interview focused on the last year of life and covered two main areas, patient characteristics and informal caregiver characteristics. Results: Odds ratios [OR] and 95% confidence intervals [95% CI] were used to determine which of the 15 potential informal caregiver and seven patient predictor variables were associated with dying at home. Multivariate analysis revealed that the odds of dying at home were greater when the patient lived with a caregiver [OR = 7.85; 95% CI = (2.35, 26.27)], the patient stated a preference to die at home [OR= 6.51; 95% CI = (2.66,15.95)], and the family physician made home visits [OR = 4.79; 95% CI = (1.97,11.64)]. However the odds were lower for patients who had caregivers with fair to poor health status [OR = 0.22; 95% CI = (0.07, 0.65)] and for patients who used hospital palliative care beds [OR = 0.31; 95% CI = (0.12, 0.80)]. Discussion: The findings suggest that individuals who indicated a preference to die at home and resided with a healthy informal caregiver had better odds of dying at home. Home visits by a family physician were also associated with dying at home.

Aim: To compare the diagnostic performance of accredited glaucoma optometrists (AGO) for both the diagnosis of glaucoma and the decision to treat with that of routine hospital eye care, against a reference standard of expert opinion (a consultant ophthalmologist with a special interest in glaucoma). Methods: A directly comparative, masked, performance study was undertaken in Grampian, Scotland. Of 165 people invited to participate, 100 (61%) were examined. People suspected of having glaucoma underwent, within one month, a full ophthalmic assessment in both a newly established community optometry led glaucoma management scheme and a consultant led hospital eye service. Results: Agreement between the AGO and the consultant ophthalmologist in diagnosing glaucoma was substantial (89%; ? = 0.703, SE = 0.083). Agreement over the need for treatment was also substantial (88%; ? = 0.716, SE = 0.076). The agreement between the trainee ophthalmologists and the consultant ophthalmologist in the diagnosis of glaucoma and treatment recommendation was moderate (83%, ? = 0.541, SE = 0.098, SE = 0.98; and 81%, ? = 0.553, SE = 0.90, respectively). The diagnostic accuracy of the optometrists in detecting glaucoma in this population was high for specificity (0.93 (95% confidence interval, 0.85 to 0.97)) but lower for sensitivity (0.76 (0.57 to 0.89)). Performance was similar when accuracy was assessed for treatment recommendation (sensitivity 0.73 (0.57 to 0.85); specificity 0.96 (0.88 to 0.99)). The differences in sensitivity and specificity between AGO and junior ophthalmologist were not statistically significant. Conclusions: Community optometrists trained in glaucoma provided satisfactory decisions regarding diagnosis and initiation of treatment for glaucoma. With such additional training in glaucoma, optometrists are at least as accurate as junior ophthalmologists but some cases of glaucoma are missed.

Background: The incidence of delirium in ventilated patients is estimated at up to 82%, and it is associated with longer intensive care and hospital stays, and long-term cognitive impairment and mortality. The pathophysiology of delirium has been linked with inflammation and neuronal apoptosis. Simvastatin has pleiotropic properties; it penetrates the brain and, as well as reducing cholesterol, reduces inflammation when used at clinically relevant doses over the short term. This is a single centre randomised, controlled trial which aims to test the hypothesis that treatment with simvastatin will modify delirium incidence and outcomes.
Methods/Design: The ongoing study will include 142 adults admitted to the Watford General Hospital Intensive Care Unit who require mechanical ventilation in the first 72 hours of admission. The primary outcome is the number of delirium- and coma-free days in the first 14 days. Secondary outcomes include incidence of delirium, delirium- and coma-free days in the first 28 days, days in delirium and in coma at 14 and 28 days, number of ventilator-free days at 28 days, length of critical care and hospital stay, mortality, cognitive decline and healthcare resource use. Informed consent will be taken from patient's consultee before randomisation to receive either simvastatin (80 mg) or placebo once daily. Daily data will be recorded until day 28 after randomisation or until discharge from the ICU if sooner. Surviving patients will be followed up on at six months from discharge. Plasma and urine samples will be taken to investigate the biological effect of simvastatin on systemic markers of inflammation, as related to the number of delirium- and coma-free days, and the potential of cholinesterase activity and beta-amyloid as predictors of the risk of delirium and long-term cognitive impairment.
Discussion: This trial will test the efficacy of simvastatin on reducing delirium in the critically ill. If patients receiving the statin show a reduced number of days in delirium compared with the placebo group, the inflammatory theory implicated in the pathogenesis of delirium will be strengthened.

A investigação tem demonstrado que os enfermeiros tendem a interagir menos e a centrarem-se mais em cuidados físicos na prestação de cuidados a idosos dependentes, confusos e/ou desorientados. Contudo a literatura em enfermagem demonstra que a interacção social com pacientes dependentes é importante, pois permite melhoria da sua situação clínica e melhor reintegração. Este estudo exploratório procura contribuir para compreender melhor como se pode promover a qualidade dos cuidados de enfermagem a idosos dependentes em três contextos: hospital, cuidados continuados e em centro de saúde (domiciliares). Procura-se contribuir para a sensibilização e formação de enfermeiros na prestação de cuidados de qualidade a idosos dependentes e melhor definir cuidados relacionais a estas pessoas. Especificamente este estudo procura: i) identificar os cuidados de enfermagem a prestar aos pacientes idosos dependentes; ii) aprofundar o conhecimento sobre as dificuldades/obstáculos (emocionais, institucionais, relacionais, …) dos enfermeiros na promoção de uma relação de cuidados mais expressivos a idosos dependentes; iii) identificar factores que facilitam e dificultam a prestação de cuidados mais relacionais; iv) identificar modos de fomentar, sensibilizar e motivar os enfermeiros para a prestação de cuidados a pessoas idosas dependentes. A metodologia adoptada neste estudo é o PhotoVoice, aplicado a 3 grupos de enfermeiros que prestam cuidados a idosos dependentes em 3 contextos: hospital; comunidade (centro de saúde); cuidados continuados. Assim, abrangem-se os três principais contextos em que o idoso dependente pode estar inserido e o enfermeiro presta cuidados. A amostra envolve 12 enfermeiros (3 de contexto hospitalar, 3 de cuidados continuados e 6 de centro de saúde), com idades entre os 22 e 43 anos, 10 do sexo feminino. Os principais resultados sugerem que a promoção dos cuidados de enfermagem a pessoas idosas dependentes envolve: i) trabalho e decisão em equipa multidisciplinar; ii) mais recursos materiais e humanos; iii) melhores condições físicas e formação adequada. Paralelamente, o principal obstáculo à prestação de cuidados de enfermagem de qualidade nos três contextos centra-se na escassez de recursos e na desorganização, que se traduzem em falta de tempo, desordem e frustração. Além disso, os participantes salientam a importância do envolvimento da família, considerada parte da unidade de cuidado.

Introduction: bronchial asthma is a chronic disease that affects a high percentage of adolescents, with a significant restriction of daily activities, and is a cause of school absenteeism. The relationships between adolescents and asthma disease in school were assessed, with a view to improving knowledge about the asthmatic adolescent. Methods: a survey was conducted in the Lisbon metropolitan area, covering urban (Lisbon) and rural (Lourinh˜a ) zones and including 1879 students and 81 teachers from the 7th to 9th high school years. The study groups were asthmatic students, their peers, and teachers. A self-administered questionnaire was applied to collect information. The results were compared with a reference group of 91 asthmatic students attending our Department of Immunoallergy-Hospital Dona Estefânia. Cotinine urinary measurements were made in a sample of asthmatics and a control group. Results: the prevalence of current asthma among students was 10%. Estimates of asthma annual burden among 7th to 9th year students from Lisbon and Lourinh˜a high schools included 4,307 days missed from school, 4,148 medical consultations and a minimum of 351 hospital emergency care and 80 hospital admissions. Exposure to passive smoking was not significantly different between asthmatic students and theirs peers. Cotinine urinary measurements did not discriminate between exposed and non-exposed individuals. Cigarette smoking was almost as common among adolescent asthmatics (5.4%) as it was in non-asthmatic subjects (6.7%). However, 55% of asthmatics mentioned active and passive smoking as an asthma exacerbating factor. Asthmatic students, theirs peers and teachers showed a deficient knowledge about asthma (mean group scores: 17.6; 14.2 and 17.7 of a possible 30), particularly in the areas related to asthma recognition and its management. Asthmatics attending our Allergy Department had the highest scores. All groups showed tolerance in the sense of a positive and understanding attitude toward a person with asthma. However, traditional beliefs about asthma disease (dependence, inferiority...) were confirmed. A positive correlation between knowledge levels and tolerance attitudes was found. Conclusion: in view of the dimension of the asthma problem in adolescence and its social and economic impact, it is justifiable to assess the need for the implementation of asthma education programs in schools in order to improve asthma management by the adolescents and their schools.
