The overall aim was to investigate the quality of palliative care from the patient perspective, to adapt and psychometrically evaluate the Quality from Patients’ Perspective instrument specific to palliative care (QPP-PC) and investigate the relationship between the combination of person- and organization-related conditions and patients’ perceptions of care quality. Methods: In the systematic literature review (I), 23 studies from 6 databases and reference lists in 2014 were synthesized by integrative thematic analysis. The quantitative studies (II–IV) had cross-sectional designs including 191 patients (73% RR) from hospice inpatient care, hospice day care, palliative units in nursing homes and home care in 2013–2014. A modified version of QPP was used. Additionally, person- and organization-related conditions were assessed. Psychometric evaluation, descriptive and inferential statistics were used. Main findings: Patients’ preferences for palliative care included living a meaningful life and responsive healthcare personnel, care environment and organization of care (I). The QPP-PC was developed, comprising 12 factors (49 items), 3 single items and 4 dimensions: medical–technical competence, physical–technical conditions, identity–oriented approach, and socio-cultural atmosphere (II). QPP-PC measured patients’ perceived reality (PR) and subjective importance (SI) of care quality. PR differed across settings, but SI did not (III). All settings exhibited areas of strength and for improvement (II, III). Person-related conditions seemed to be related to SI, and person- and organization-related conditions to PR, explaining 18–30 and 22-29% respectively of the variance (IV). Conclusions: The patient perspective of care quality (SI and PR) should be integrated into daily care and improvement initiatives in palliative care. The QPP-PC can measure patients’ perceptions of care quality. Registered nurses and other healthcare personnel need awareness of person- and organization-related conditions to provide high-quality person-centred care.

According to recent studies, informal learning accounts for more than 75% of our continuous learning through life. However, the awareness of this learning, its benefits and its potential is still not very clear. In engineering contexts, informal learning could play an invaluable role helping students or employees to engage with peers and also with more experience colleagues, exchanging ideas and discussing problems. This work presents an initial set of results of the piloting phase of a project (TRAILER) where an innovative service based on Information & Communication Technologies was developed in order to aid the collection and visibility of informal learning. This set of results concerns engineering contexts (academic and business), from the learners' perspective. The major idea that emerged from these piloting trials was that it represented a good way of collecting, recording and sharing informal learning that otherwise could easily be forgotten. Several benefits were reported between the two communities such as being helpful in managing competences and human resources within an institution.

This study provides an innovative perspective on empirically assessing HR by focusing on the duality of HR professionals’ experiences as both implementers and recipients or internal customers of HR practices given that they are also employees of the organization. We hypothesize that HR professionals experience HR practices more favorably from an implementer perspective as compared to an internal customer perspective. These differences in experiences are likely to be influenced by HR professionals’ hierarchical position in the HR department. Our analyses of 1,271 HR professionals employed by Indian Railways revealed a number of differences between the two types of experience. Some practices (recruitment and selection, training, and employee welfare) were viewed more negatively from the implementer perspective, whereas others (compensation, benefits, and employment relations) were rated more negatively from the internal customer perspective. Those holding more senior HR positions reported more positive experiences of training and employment relations from an internal customer perspective. Overall our contributions draw on the attribution theory and concepts of intraorganizational power and voice, and have implications concerning the effectiveness of HR practices. © 2015 Wiley Periodicals, Inc.

T cell recognition of autoantigens is critical to progressive immune-mediated destruction of islet cells, which leads to autoimmune diabetes. We identified a naturally presented autoantigen from the human islet antigen glutamic acid decarboxylase, 65-kDa isoform (GAD65), by using a combination of chromatography and mass spectrometry of peptides bound by the type I diabetes (insulin-dependent diabetes mellitus, IDDM)-associated HLA-DR4 molecule. Peptides encompassing this epitope-stimulated GAD65-specific T cells from diabetic patients and a DR4-positive individual at high risk for developing IDDM. T cell responses were antagonized by altered peptide ligands containing single amino acid modifications. This direct identification and manipulation of GAD65 epitope recognition provides an approach toward dissection of the complex CD4+ T cell response in IDDM.

We show indirect evidences for the possible involvement of NIT-2-like binding motifs in transcription modulation of the PbGP43 gene, which codes for an important antigen from the human fungal pathogen Paracoccidioides brasiliensis. This investigation was motivated by the finding of 23 NIT2-like sites within the proximal -2047 nucleotides of the PbGP43 5` intergenic region from the Pb339 isolate. They compose four clusters, two of them identical. We found four NIT2-containing probes that were positive in electrophoretic mobility shift assays and further analyzed them. PbGP43 could be modulated by nitrogen primary sources in Pb339, Pb3 and Pb18 isolates, as observed by reverse transcription (RT) real time-PCR. Gene reporter assays conducted in Aspergillus nidulans suggested that the minimal fragment responsible for nitrogen modulation lies within -480 bp of the PbGP43 gene. This is the first report on PbGP43 transcription modulation in response to nitrogen primary sources, which might help understand its regulation during infection. (C) 2008 Elsevier Inc. Nowadays it is known that the human body is continuous source of many types of energy and the devices used for collecting energy taken from the environment also have the required capabilities for the collection of the energy produced by the Human body (HB), but very limited and with very low efficiency. Low power and high yield converters are particularly needed in these cases of collecting energy from human activity and its movements due to the small amount of energy generated this way. But this situation can be improved. Enhancing or focusing the human movements by using mechanical amplifiers applied to the piezoelectric element. By doing so the input of energy in the element increases. As such increasing its output, therefore producing more energy.

HLA-DR antigens are polymorphic cell surface glycoproteins, expressed primarily in B lymphocytes and macrophages, which are thought to play an important role in the immune response. Two polypeptide chains, alpha and beta, are associated at the cell surface, and a third chain associates with alpha and beta intracellularly. RNA isolated from the human B-cell line Raji was injected in Xenopus laevis oocytes. Immunoprecipitates of translation products with several monoclonal antibodies revealed the presence of HLA-DR antigens similar to those synthesized in Raji cells. One monoclonal antibody was able to bind the beta chain after dissociation of the three polypeptide chains with detergent. The presence of all three chains was confirmed by two-dimensional gel electrophoresis. The glycosylation pattern of the three chains was identical to that observed in vivo, as evidenced in studies using tunicamycin, an inhibitor of N-linked glycosylation. The presence of alpha chains assembled with beta chains in equimolar ratio was further demonstrated by amino-terminal sequencing. An RNA fraction enriched for the three mRNAs, encoding alpha, beta, and intracellular chains, was isolated. This translation-assembly system and the availability of monoclonal antibodies make it possible to assay for mRNA encoding specific molecules among the multiple human Ia-like antigens.

Tietokantamarkkinointi voi olla vain apuväline markkinointitoimenpiteiden suorittamisessa, mutta se voidaan toisaalta nähdä myös olennaisena osana asiakassuhdehallintaa. Tietokantamarkkinointi asiakassuhdehallinnan näkökulmasta tähtää asiakastyytyväisyyteen ja asiakasuskollisuuteen, sekä asiakassuhteen tuottavuuteen ja kannattavuuteen, mikä voidaan saavuttaa tehokkaan tiedonhallinnan avulla. Tämä mahdollistaa räätälöityjen toimenpiteiden suorittamisen ja tehostaa kohdentamista ja segmentointia, myös asiakkaiden tuottavuuden perusteella. Normatiivinen case-tutkimus, joka tehtiin Alankomaissa Eurooppalaisessa tietotekniikan lisäarvoa tuottavassa jälleenmyyntikanavassa osoittaa, että tietokantamarkkinointi etenkin asiakas- suhdehallinnan näkökulmasta olisi sopiva keino lisätä asiakastyytyväisyyttä ja –tuottavuutta. Se myös tehostaisi sisäisiä tietovirtoja ja markkinointitoimenpiteitä, kuten esimerkiksi markkinointiviestintää, kampanjanhallintaa ja myyntiprosesseja yritysten välisessä kaupankäynnissä.

La innovación en Sistemas Intesivos en Software está alcanzando relevancia por múltiples razones: el software está presente en sectores como automóvil, teléfonos móviles o salud. Las empresas necesitan conocer aquellos factores que afectan a la innovación para incrementar las probabilidades de éxito en el desarrollo de sus productos y, la evaluación de productos sofware es un mecanismo potente para capturar este conocimiento. En consecuencia, las empresas necesitan evaluar sus productos desde la perpectiva de innovación para reducir la distancia entre los productos desarrollados y el mercado. Esto es incluso más relevante en el caso de los productos intensivos en software, donde el tiempo real, la oportunidad, complejidad, interoperabilidad, capacidad de respuesta y compartción de recursos son características críticas de los nuevos sistemas. La evaluación de la innovación de productos ya ha sido estudiada y se han definido algunos esquemas de evaluación pero no son específicos para Sistemas intensivos en Sofwtare; además, no se ha alcanzado consenso en los factores ni el procedimiento de evaluación. Por lo tanto, tiene sentido trabajar en la definición de un marco de evaluación de innovación enfocado a Sistemas intesivos en Software. Esta tesis identifica los elementos necesarios para construir in marco para la evaluación de de Sistemas intensivos en Software desde el punto de vista de la innovación. Se han identificado dos componentes como partes del marco de evaluación: un modelo de referencia y una herramienta adaptativa y personalizable para la realización de la evaluación y posicionamiento de la innovación. El modelo de referencia está compuesto por cuatro elementos principales que caracterizan la evaluación de innovación de productos: los conceptos, modelos de innovación, cuestionarios de evaluación y la evaluación de productos. El modelo de referencia aporta las bases para definir instancias de los modelos de evaluación de innovación de productos que pueden se evaluados y posicionados en la herramienta a través de cuestionarios y que de forma automatizada aporta los resultados de la evaluación y el posicionamiento respecto a la innovación de producto. El modelo de referencia ha sido rigurosamente construido aplicando modelado conceptual e integración de vistas junto con la aplicación de métodos cualitativos de investigación. La herramienta ha sido utilizada para evaluar productos como Skype a través de la instanciación del modelo de referencia. ABSTRACT Innovation in Software intensive Systems is becoming relevant for several reasons: software is present embedded in many sectors like automotive, robotics, mobile phones or heath care. Firms need to have knowledge about factors affecting the innovation to increase the probability of success in their product development and the assessment of innovation in software products is a powerful mechanism to capture this knowledge. Therefore, companies need to assess products from an innovation perspective to reduce the gap between their developed products and the market. This is even more relevant in the case of SiSs, where real time, timeliness, complexity, interoperability, reactivity, and resource sharing are critical features of a new system. Many authors have analysed product innovation assessment and some schemas have been developed but they are not specific to SiSs; in addition, there is no consensus about the factors or the procedures for performing an assessment. Therefore, it has sense to work in the definition of a customized software product innovation evaluation framework. This thesis identifies the elements needed to build a framework to assess software products from the innovation perspective. Two components have been identified as part of the framework to assess Software intensive Systems from the innovation perspective: a reference-model and an adaptive and customizable tool to perform the assessment and to position product innovation. The reference-model is composed by four main elements characterizing product innovation assessment: concepts, innovation models, assessment questionnaires and product assessment. The reference model provides the umbrella to define instances of product innovation assessment models that can be assessed and positioned through questionnaires in the proposed tool that also provides automation in the assessment and positioning of innovation. The reference-model has been rigorously built by applying conceptual modelling and view integration integrated with qualitative research methods. The tool has been used to assess products like Skype through models instantiated from the reference-model.

The human fundus is a complex structure that can be easily visualized and the world of ophthalmology is going through a golden era of new and exciting fundus imaging techniques; recent advances in technology have allowed a significant improvement in the imaging modalities clinicians have available to formulate a diagnostic and treatment plan for the patient, but there is constant on-going work to improve current technology and create new ideas in order to gather as much information as possible from the human fundus. In this article we shall summarize the imaging techniques available in the standard medical retina clinic (i.e. not limited to the research lab) and delineate the technologies that we believe will have a significant impact on the way clinicians will assess retinal and choroidal pathology in the not too distant future.

Training in information competencies or information literacy is one of the current challenges of university libraries at the possibilities of access to vast information resources that facilitate digital media, which require a better understand and apply the selection and assessment criteria to retrieval the highest quality and relevance of information as needed. In this situation, Ibero-American university libraries (Latin-America, Spain and Portugal) have been slowly incorporating this training either from direct training programs, offered from the library or through collaborative work with teachers and schools in curricula of various universities as a whole or in specific disciplines. In this text, it was identified that, at present, from the information displayed on Web sites of universities-HEI in Costa Rica, a very small percentage of university libraries would find taking actions in a level 1 or 2 of incorporating information literacy, since a large most developed is still very focused programs and processes to the traditional user training, while another large majority, unfortunately, has no action-information about actions from the forming perspective that should be any library.

The question of why most health policies do not achieve their intended results continues to receive a considerable attention in the literature. This is in the light of the recognized gap between policy as intent and policy as practice, which calls for substantial research work to understand the factors that improve policy implementation. Although there is substantial work that explains the reasons why policies achieve or fail to achieve their intended outcomes, there are limited case studies that illustrate how to analyze policies from the methodological perspective. In this article, we report and discuss how a mixed qualitative research method was applied for analyzing maternal and child health policies in Malawi. For the purposes of this article, we do not report research findings; instead we focus our dicussion on the methodology of the study and draw lessons for policy analysis research work. We base our disusssion on our experiences from a study in which we analyzed maternal and child health policies in Malawi over the period from 1964 to 2008. Noting the multifaceted nature of maternal and child health policies, we adopted a mixed qualitative research method, whereby a number of data collection methods were employed. This approach allowed for the capturing of different perspectives of maternal and child health policies in Malawi and for strengthening of the weaknesses of each method, especially in terms of data validity. This research suggested that the multidimensional nature of maternal and child health policies, like other health policies, calls for a combination of research designs as well as a variety of methods of data collection and analysis. In addition, we suggest that, as an emerging research field, health policy analysis will benefit more from case study designs because they provide rich experiences in the actual policy context.
