Preeclampsia is among the leading causes of perinatal mortality and morbidity, affecting 2-7% of pregnancies. Its incidence increases to 10-25% in already hypertensive women. To date, no treatment, aside from delivery, is known. Interestingly, several studies have reported that exercise training (ExT) can reduce preeclampsia prevalence although the available studies are considered insufficient. Therefore, the aim of this study is to determine the impact of ExT when practiced before and during gestation on pregnancy outcome in a mouse model of preeclampsia superimposed on chronic hypertension (SPE). To do so, mice overexpressing both human angiotensinogen and renin (R+A+) were used because they are hypertensive at baseline and they develop many hallmark features of SPE. Mice were trained by placing them in a cage with access to a running wheel 4 weeks before and during gestation. ExT in this study prevented the rise in blood pressure at term observed in the sedentary transgenic mothers. This may be realized through an increased activity of the angiotensin-(1-7) axis in the aorta. In addition, ExT prevented the increase in albumin/creatinine ratio. Moreover, placental alterations were prevented with training in transgenic mice, leading to improvements in placental and fetal development. Placental mRNA and circulating levels of sFlt-1 were normalized with training. Additionally, the increase in angiotensin II type I receptor and the decrease in Mas receptor protein were reversed with training. ExT appears to prevent many SPE-like features that develop in this animal model and may be of use in the prevention of preeclampsia in women.

BACKGROUND: Sex differences are present in many neuropsychiatric conditions that affect emotion and approach-avoidance behavior. One potential mechanism underlying such observations is testosterone in early development. Although much is known about the effects of testosterone in adolescence and adulthood, little is known in humans about how testosterone in fetal development influences later neural sensitivity to valenced facial cues and approach-avoidance behavioral tendencies. METHODS: With functional magnetic resonance imaging we scanned 25 8-11-year-old children while viewing happy, fear, neutral, or scrambled faces. Fetal testosterone (FT) was measured via amniotic fluid sampled between 13 and 20 weeks gestation. Behavioral approach-avoidance tendencies were measured via parental report on the Sensitivity to Punishment and Sensitivity to Rewards questionnaire. RESULTS: Increasing FT predicted enhanced selectivity for positive compared with negatively valenced facial cues in reward-related regions such as caudate, putamen, and nucleus accumbens but not the amygdala. Statistical mediation analyses showed that increasing FT predicts increased behavioral approach tendencies by biasing caudate, putamen, and nucleus accumbens but not amygdala to be more responsive to positive compared with negatively valenced cues. In contrast, FT was not predictive of behavioral avoidance tendencies, either through direct or neurally mediated paths. CONCLUSIONS: This work suggests that testosterone in humans acts as a fetal programming mechanism on the reward system and influences behavioral approach tendencies later in life. As a mechanism influencing atypical development, FT might be important across a range of neuropsychiatric conditions that asymmetrically affect the sexes, the reward system, emotion processing, and approach behavior.

The temporomandibular joint (TMJ) is a highly specialized articulation that differs from all the other synovial articulations for many reasons. In children, different from what we observe in adults, these articulations have rarely been studied under the morphofunctional aspect, mainly in the embryonary and fetal stages. In this study 10 fetuses with ages varying from 16 to 39 weeks of intrauterine life were used, and it could be observed that the fibers and thickness of the articular disc, as well as the articular capsule and the condylar process, suffer modifications according to age. It was also observed that the superior head of the lateral pterygoid muscle inserts itself in the articular disc and capsule in all the ages studied. Also, the maturation of the articular tissues, especially of the articular disc, as well as, the associated muscles, suggests that the TMJ was able to carry out mandibular movements since the 24(th) week of intrauterine life.

Vitamin A and its derivatives, retinoic acid, tretinoin and isotretinoin, are currently used in dermatological treatments. The administration of high doses of this vitamin provokes congenital malformations in mice: cleft palate, maxillary and mandibular hypoplasia and total or partial fusion of the maxillary incisors. This study compares the tooth germs of the first maxillary and mandibular molars of fetal mice submitted to isotretinoin during organogenesis. Twelve 60-day-old female Mus musculus were divided into two groups on the 7th day of pregnancy: treated group--1 mg isotretinoin per kg body weight, dissolved in vegetable oil, was administered from the 7th to the 13th day of pregnancy; control group--vegetable oil in equivalent volume was administered orally for the same period. On the 16th day of pregnancy, the females were sacrificed, the fetuses were removed and their heads amputated. After standard laboratory procedures, 6-micron thick serial slices were stained with hematoxylin and eosin for optical microscopy examination. The results showed that both groups had closed palates with no reminiscence of epithelial cells; however, the first molar germs of the isotretinoin-treated animals showed delayed development compared to the control animals.

Objectives: To evaluate the effects of folic acid supplementation on isolated oral cleft recurrence and fetal growth. Patients and Methods: The study included 2,508 women who were at-risk for oral cleft recurrence and randomized into two folic acid supplementation groups: 0.4 and 4 mg per day before pregnancy and throughout the first trimester. The infant outcome data were based on 234 live births. In addition to oral cleft recurrence, several secondary outcomes were compared between the two folic acid groups. Cleft recurrence rates were also compared to historic recurrence rates. Results: The oral cleft recurrence rates were 2.9% and 2.5% in the 0.4 and 4 mg groups, respectively. The recurrence rates in the two folic acid groups both separately and combined were significantly different from the 6.3% historic recurrence rate post the folic acid fortification program for this population (p = 0.0009 when combining the two folic acid groups). The rate of cleft lip with palate recurrence was 2.9% in the 0.4 mg group and 0.8% in the 4 mg group. There were no elevated fetal growth complications in the 4 mg group compared to the 0.4 mg group. Conclusions: The study is the first double-blinded randomized clinical trial (RCT) to study the effect of high dosage folic acid supplementation on isolated oral cleft recurrence. The recurrence rates were similar between the two folic acid groups. However, the results are suggestive of a decrease in oral cleft recurrence compared to the historic recurrence rate. A RCT is still needed to identify the effect of folic acid on oral cleft recurrence given these suggestive results and the supportive results from previous interventional and observational studies, and the study offers suggestions for such future studies. The results also suggest that high dosage folic acid does not compromise fetal growth. © 2013 by the authors; licensee MDPI, Basel, Switzerland.
