Intermittent claudication (IC) is leg muscle pain, cramping and fatigue brought on by exercise and is the primary symptom of peripheral arterial disease. The goals of pharmacotherapy for IC are to increase the walking capacity/quality of life and to decrease rates of amputation. In 1988, pentoxifylline was the only drug that had reasonable supportive clinical trial evidence for being beneficial in IC. Since then a number of drugs have shown benefit or potential in IC. Cilostazol, a specific inhibitor of phosphodiesterase 3 and activator of lipoprotein lipase, clearly increases pain-free and absolute walking distances in claudicants. However, cilostazol does cause minor side effects including headache, diarrhoea, loose stools and flatulence. Naftidrofuryl, a serotonin (5-HT2) receptor antagonist and antiplatelet drug, is beneficial in claudicants. Inhibitors of platelet aggregation (including nitric oxide from L-arginine or glyceryl trinitrate) and anticoagulants (low molecular weight heparin, defibrotide) probably have both short and long-term benefits in IC. In addition, intravenous infusions of prostaglandins (PGs) PGE1 and PGI2 have an established role in severe peripheral arterial disease and the recent introduction of longer lasting and/or oral forms of the PGs makes them more likely to be useful in the IC associated with less severe forms of the disease. There are some exciting new approaches to the treatment of IC, including propionyl-L-carnitine and basic fibroblast growth factor (bFGF).

RESUMO: O trabalho docente exige dedicação em sala de aula e fora dela, pois o professor vê-se na missão de educador, formador de opiniões e de futuros profissionais no mercado de trabalho. Uma das patologias que pode ocorrer devido ao estresse gerado pela docência é o desenvolvimento de Desordens Temporomandibulares, que acarretará cefaleias, dores musculares na região cervical e facial e alterações posturais. Tivemos a pretensão de conduzir nossas reflexões sobre o estresse do docente para procurar responder às nossas inquietações. Para isso, formulamos o seguinte questionamento: qual a incidência e a relação entre o estresse e o desenvolvimento de Desordens Temporomandibulares no docente de nível superior? O objetivo geral desta investigação foi descrever e analisar a relação entre o estresse e a incidência de Desordem Temporomandibular (DTM) em docentes de nível superior da cidade de Palmas – Tocantins – Brasil. Para contemplar o objetivo geral, buscamos verificar a presença ou não de estresse e os principais sintomas de estresse nos docentes de Ensino Superior, identificando a fase do estresse em que os professores se encontravam e verificar a presença ou não de DTM e seus graus. Procuramos, ainda, avaliar a relação entre sexo e presença de estresse, estado civil e presença de estresse, o tempo de docência e a presença de estresse e a relação entre a carga horária em sala de aula e a carga horária total de trabalho e a presença de estresse, entre sexo e presença de DTM, estado civil e presença de DTM, o tempo de docência e a presença de DTM e a relação entre a carga horária em sala de aula e carga horária total de trabalho e a presença de DTM. A metodologia utilizada, para a realização deste trabalho, foi de caráter exploratório e descritivo com uma abordagem quantitativa dos dados obtidos. A pesquisa caracterizou-se num estudo de campo, fundamentada em um instrumento denominado “Inventário de Sintomas de Stress Lipp – ISSL” da autora Marilda Lipp (2005), versão para adulto, ao qual acrescentamos dados sociodemográficos (estado civil, tempo de docência, carga horária de sala de aula e de trabalho total). Recorremos, ainda, ao Questionário Índice de Helkimo para identificar a ocorrência de DTMs. A coleta dos dados foi realizada a partir de uma amostra de duzentos e trinta e três (233) docentes de Ensino Superior, com idade entre 23 e 74 anos. Com base na análise realizada, os resultados revelaram que a presença de sintomas de estresse se encontra em 106 (45,49%) docentes, com predomínio da fase de resistência. Verificamos que 127 (54,5%) docentes não apresentam estresse. A sintomatologia predominante são sintomas psicológicos. Vimos que o tempo de docência e a jornada de trabalho não contribuem como um valor preditivo da presença de estresse, bem como o estado civil, e verificamos que o sexo feminino apresentou mais sintomas de estresse. Ao analisarmos a presença ou não de DTM, vimos que os docentes apresentam sintomas de DTM em sua maioria, encontrada em 187 docentes (80,25%), com predomínio do grau de DTM leve. Vimos que o tempo de docência e a jornada de trabalho não contribuem como um valor significativo para a presença de DTM, mas que o sexo feminino apresentou mais sintomas de DTM, bem como os casados. Buscando responder ao problema norteador desta 6 investigação, vimos que, mesmo nos docentes sem presença de estresse, havia sintomas de DTM. Esses dados indicam que a profissão docente pode causar sobrecargas e gerar a DTM. Diante dessa amostra de docentes pesquisados, concluímos que os dados aqui apresentados sugerem uma ampliação do estudo. Considerando a relevância do papel desempenhado pelos docentes das Instituições de Ensino Superior na formação dos acadêmicos e em sua efetiva transformação em futuros profissionais, nossa contribuição para o conhecimento do processo de estresse e os graus de DTM na atividade docente proporcionará oportunidade aos profissionais das diversas áreas do conhecimento e, principalmente, aos docentes para despertarem para a elaboração de programas de combate, controle e prevenção do estresse e do desenvolvimento de DTMs, resultando, dessa forma, na conquista de uma vida mais saudável, tanto na área física quanto na área psicológica. ABSTRACT: Abstract The teacher’s job requires dedication in and out of the classroom for a teacher is in a education mission, he/she is an opinion maker and responsible for future professionals in the market. One of the pathologies that may take place due to the stress caused by this job is the Temporomandibular Joint Disorder (TMD) that will trigger cephaleas, muscle pain in the cervical and facial area and posture changes. The purpose of this paper is to conduct reflections about teachers’ stress trying to answer the following question: What is the incidence rate and correlation between stress and the occurrence of Temporomandibular Joint Disorder (DTM) in teachers of institutions of higher learning in the city of Palmas – Tocantis – Brazil. To fulfill the general purpose we tried to verify the presence or not of stress and the principal symptoms in teachers in universities and colleges, identifying the levels of stress that they were, and verifying the presence or not of TMD and its levels. We tried to evaluate the correlation between gender and the occurrence of stress, marital status and the presence of stress, career time and the presence of stress, and time working in classroom and the total time of work and the presence of stress. The methodology used to perform this was an exploratory and descriptive study with a quantitative approach of the data gathered. The research was characterized by a field study, fundamented by a tool called “Lipp Stress Symptom Inventory – ISSL” from the author Marilda Lipp (2005), adult version, which we also added social-demographic data (marital status, time teaching, hours working in the classroom, and total time of work). We also used the Helkimo Index Questionnaire to identify the occurrence of TMD’s. The data was collected from a sample of two hundred three (233) teachers of universities and colleges between 23 to 74 years of age. Based on the analysis performed, the results showed the presence of stress among 106 (45.49%) of the teachers, with the predominance of the resistance level. We observed that some 127 teachers (54.5%) did not show stress. The predominant symptomology were psychological ones. We observed that time of experience and the schedule do not influence as a predictive value for the stress presence, neither does the marital status and we also observed that the female gender showed more stress symptoms. When we analyzed the presence or not of TMD, we observed that most of the teachers showed symptoms of TMD, some 187 teachers (80.25%), with the predominance of first level TMD. We observed that the time as teacher and the schedule do not influence with an important value for the TMD presence, however the feminine gender as well as married teachers showed more symptoms of TMD. Trying to answer the most important issue of this study, we saw that even without the occurrence of stress the TMD was present showing that the profession can cause overwork and consequently TMD. With this sample of teachers studied we conclude that the found data suggest a wider research. Considering the role performed by teachers of universities and colleges in the development of students and future professionals, our contribution for the understanding of the stress development and TMD levels in the teaching activity will enable the opportunity for professionals of different areas of knowledge, and 8 principally for teachers, to awaken to the need to develop programs to fight, control, and prevent stress and the development of TMD, therefore resulting in a healthier life, both a physical as well as a psychological one.

Giant cell arteritis (GCA) is a systemic large vessel vasculitis, with extracranial arterial involvement described in 10-15% of cases, usually affecting the aorta and its branches. Patients with GCA are more likely to develop aortic aneurysms, but these are rarely present at the time of the diagnosis. We report the case of an 80-year-old Caucasian woman, who reported proximal muscle pain in the arms with morning stiffness of the shoulders for eight months. In the previous two months, she had developed worsening bilateral arm claudication, severe pain, cold extremities and digital necrosis. She had no palpable radial pulses and no measurable blood pressure. The patient had normochromic anemia, erythrocyte sedimentation rate of 120 mm/h, and a negative infectious and autoimmune workup. Computed tomography angiography revealed concentric wall thickening of the aorta extending to the aortic arch branches, particularly the subclavian and axillary arteries, which were severely stenotic, with areas of bilateral occlusion and an aneurysm of the ascending aorta (47 mm). Despite corticosteroid therapy there was progression to acute critical ischemia. She accordingly underwent surgical revascularization using a bilateral carotid-humeral bypass. After surgery, corticosteroid therapy was maintained and at six-month follow-up she was clinically stable with reduced inflammatory markers. GCA, usually a chronic benign vasculitis, presented exceptionally in this case as acute critical upper limb ischemia, resulting from a massive inflammatory process of the subclavian and axillary arteries, treated with salvage surgical revascularization.

Introduction The European Foundation for the improvement of living and working conditions conducts a survey every 5 years since 1990. The foundation also offers the possibility to non-EU countries to be included in the survey: in 2005, Switzerland took part for the first time in the fourth edition of this survey. The Institute for Work and Health (IST) has been associated to the Swiss project conducted under the leadership of the SECO and the Fachhochschule Nordwestschweiz. The survey covers different aspects of work like job characteristics and employment conditions, health and safety, work organization, learning and development opportunities, and the balance between working and non-working life (Parent-Thirion, Fernandez Macias, Hurley, & Vermeylen, 2007). More particularly, one question assesses the worker's self-perception of the effects of work on health. We identified (for the Swiss sample) several factors affecting the risk to report health problems caused by work. The Swiss sample includes 1040 respondents. Selection of participants was based on a random multi-stage sampling and was carried out by M.I.S Trend S.A. (Lausanne). Participation rate was 59%. The database was weighted by household size, gender, age, region of domicile, occupational group, and economic sector. Specially trained interviewers carried out the interviews at the respondents home. The survey was carriedout between the 19th of September 2005 and the 30th of November 2005. As detailed in (Graf et al., 2007), 31% of the Swiss respondents identify work as the cause of health problems they experience. Most frequently reported health problems include back pain (18%), stress (17%), muscle pain (13%), and overall fatigue (11%). Ergonomic aspects associated with higher risk of reporting health problems caused by work include frequent awkward postures (odds ratio [OR] 4.7, 95% confidence interval [CI] 3.1 to 5.4), tasks involving lifting heavy loads (OR 2.7, 95% CI 2.0 to 3.6) or lifting people (OR 2.2, 95% CI 1.4 to 3.5), standing or walking (OR 1.4, 95% CI 1.1 to 1.9), as well as repetitive movements (OR 1.7, 95% CI 1.3 to 2.3). These results highlight the need to continue and intensify the prevention of work related health problems in occupations characterized by risk factors related to ergonomics.

13-16-vuotiaiden purentaelimistön toimintahäiriöt ja päänsärky Tutkimuksen tarkoituksena oli selvittää purentaelimistön toimintahäiriöihin (TMD) liittyvien oireiden ja kliinisten löydösten esiintyvyyttä ja muutosta kolmen vuoden seurannan aikana päänsärystä kärsivillä nuorilla ja heidän verrokeillaan, TMD-löydösten yhteyttä eri tyyppisiin päänsärkyihin ja päänsärkyyn liittyviin tekijöihin, TMD-löydöksiä ennustavia tekijöitä sekä perhetaustan osuutta TMD-löydösten esiintyvyyteen. Tutkimus perustui kahteen laajaan koululaismateriaaliin Turun seudulla. Ensimmäinen 13-vuotiaiden nuorten ryhmä (n = 311) osallistui myös 16-vuotiaana seuranta-tutkimukseen. Toisen 13-vuotiaiden nuorten ryhmän (n = 154) äidit (n = 154) osallistuivat myös tutkimukseen, jossa selvitettiin tapaus-verrokkiasetelmassa TMD-löydösten ja päänsärkyjen esiintyvyyttä kahdessa sukupolvessa. Nuoret jaettiin päänsärkyryhmiin ja terveisiin kontrolleihin IHS (1988) päänsärky-kriteerien mukaisesti kyselykaavakkeen ja lääkärin suorittaman haastattelun ja kliinisen tutkimuksen antamien tietojen perusteella. Lääkäri määritteli äitien päänsärkydiagnoosin kyselykaavakkeen tietojen perusteella. Kaikki nuoret ja heidän äitinsä haastateltiin ja heille tehtiin kliininen purentafysiologinen tutkimus sokkoutetusti. Tutkimuksen tulokset osoittivat, että tytöillä esiintyi TMD-löydöksiä selvästi enemmän kuin pojilla sekä ennen puberteettia että sen jälkeen. Tutkimuksessa havaittiin selkeä yhteys TMD-löydösten ja molempien päänsärkytyyppien, migreenin ja episodisen tensiotyyppisen päänsäryn, välillä. Kolmen vuoden seurannan aikana TMD-löydöksissä havaittiin runsasta muutosta ja iän myötä vähenemistä, mutta TMD-oireiden kohdalla ei vastaavaa muutosta todettu. TMD-löydösten ja niska-hartiaseudun lihaskipujen välillä havaittiin yhteys 13-vuotiailla nuorilla. Mikään päänsärkyyn liittyvistä tekijöistä 13-vuotiailla ei osoittautunut ennustavaksi taustatekijäksi myöhemmille TMD-löydöksille. TMD-löydösten suhteen ei todettu perheyhteyttä. Päänsäryistä kärsivillä nuorilla on enemmän myös muita kiputiloja, kuten purentaelimistön toimintahäiriöitä ja niska-hartiaseudun kipuja, kuin heidän terveillä verrokeillaan. 1316 vuoden iässä nämä löydökset ovat valtaosaltaan lieviä ja vaihtelevia.

L’objectif premier de notre projet était d’établir un modèle animal de douleur chronique orofaciale, lequel pourrait imiter la sensibilité retrouvée chez les patients souffrant de douleur orofaciale myalgique. Nous avons procédé à des injections intramusculaires de saline acide (2 injections à 2 jours d’intervalle pH 4.0) pour induire une sensibilisation mécanique des mucles massétérins. La réponse nocifensive a été mesurée à l’aide de filaments de von Frey avant et après ces injections dans des rats Sprague-Dawley. Par la suite, le potentiel analgésique de différents antagonistes des récepteurs glutamatergiques fût évalué par l’injection intramusculaire de ces antagonistes à différents moments. Nos résultats suggèrent que deux injections de saline acide, produisent une hypersensibilité mécanique signalée par l’augmentation du nombre de réponses à l’application de filaments de von Frey. Cet effet dure plusieurs semaines et est bilatéral, même lorsque les injections sont unilatérales, indiquant qu’une composante centrale est forcément impliquée. Toutefois, une composante périphérique impliquant les récepteurs glutamatergiques semble présider le tout puisque les antagonistes glutamatergiques, appliqués de façon préventive empêchent le développement de l’hypersensibilité. Cependant, le maintien de cette hypersensibilité doit dépendre de mécanismes centraux puisque l’application d’antagonistes une fois la sensibilisation induite, ne diminue en rien le nombre de réponses obtenues. Ce modèle semble approprié pour reproduire une hypersensibilité musculaire durable de bas niveau. Nos données indiquent que les récepteurs glutamatergiques périphériques participent à l’induction de cette hypersensibilité de longue durée. Nous croyons que ce modèle pourra éventuellement contribuer à une meilleure compréhension des mécanismes à l’origine des myalgies faciales persistantes.

Objetivo Identificar la prevalencia de síntomas osteomusculares, por segmentos y los factores de riesgo asociados, en los trabajadores de una empresa de Geomática, en Colombia en el año 2014. Metodología Se llevó a cabo un estudio descriptivo de corte transversal con una población de 169 trabajadores, distribuidos en 2 grupos, el grupo de campo que desarrolla actividades de topografía y el grupo de oficina donde se realizan procesamiento de datos en Geomática y actividades administrativas. A cada trabajador se le aplicó el cuestionario ERGOPAR que interroga la exposición o factores de riesgo y la presencia de síntomas osteomusculares. Resultados: El personal de oficina presenta mayor frecuencia de síntomas osteomusculares en el cuello 72%, la región lumbar 55%, los codos 17,7%, las manos y muñecas 57.3%. Presentándose con mayor frecuencia en las mujeres los síntomas en cuello 80% y manos 64%, mientras que los mayores porcentajes en personal de campo se presentan en las piernas 21%, las rodillas 26% y pies 11,5%. Se encontró asociación significativa entre la sedestación durante más de cuatro horas, con dolor en cuello (p=0.02) y dolor en región lumbar (p=0.03); inclinar el cuello hacia delante durante más de cuatro horas, con dolor en el cuello (p=0.006); repetir cada pocos segundos la flexión de muñecas (p=001) y utilizar los dedos de manera intensiva por más de 4 horas (p=0.01) con dolor en manos y las variables jornada laboral y puesto de trabajo con dolor en pies. Conclusiones La prevalencia de síntomas osteomusculares en los trabajadores de la empresa estudiada es alta. Dado que se encontró asociación significativa con las variables sociodemográficas y laborales. La alta prevalencia de sintomatología puede ser explicada por la exposición a carga física laboral, por posturas de trabajo, por movimientos repetitivos y características propias de género.

Objectives - To describe the clinical and epidemiological aspects of post-polio syndrome (PPS) and identify predictors of its severity. Materials and methods - 132 patients with PPS were selected at the Neuromuscular Disease Outpatient Clinic of the Federal University of Sao Paulo. Descriptive analysis was carried out and predictors of PPS severe forms were investigated using an unconditional logistic regression. Results - The average age at onset was 39.4 years. The most common symptoms were fatigue (87.1%), muscle pain (82.4%) and joint pain (72.0%); 50.4% of the cases were severe. The following were associated with PPS severity: a < 4-year period of neurological recovery (OR 2.8), permanent damage in two limbs (OR 3.6) and residence at the time of acute polio in a city with more advanced medical assistance (OR 2.5). Conclusions - Health professionals should carefully evaluate polio survivors for PPS and be aware of the implications of muscle overuse in the neurological recovery period.

There is increasing evidence that spinal glial cells play an important role in chronic pain states. However, so far no data on the role of microglia in muscle pain are available. The aim of the present study was to investigate the involvement of spinal microglial cells in chronic muscle pain. In a rat model of chronic muscle inflammation (injection of complete Freunds adjuvant into the gastrocnemius-soleus muscle) alterations of microglia were visualized with quantitative OX-42 immunohistochemistry in the dorsal horn of the segments L4 and L5 12 days after induction of inflammation. In behavioural experiments the influence of chronic intrathecally applied minocycline - a specific microglia inhibitor - or an antibody against tumour necrosis factor-alpha (TNF-alpha: a cytokine released from microglia) on pain-related behaviour was investigated after 1, 3, 6, and 12 days. The immunhistochemical data show that in the deep laminae of the spinal dorsal horn microglial cells reacted with morphological changes to the muscle inflammation. Following inflammation, the mean boundary length surrounding the OX-42 immunostained area was significantly shorter. This indicates that microglial cells were activated by the myositis and withdrew their processes. Chronic intrathecal administration of minocycline or anti TNF-alpha with an osmotic mini-pump largely normalised the inflammation-induced changes in spontaneous exploratory behaviour and attenuated the hypersensitivity to mechanical stimulation. Both the immunohistochemical and behavioural data show that spinal microglial cells are involved in nociceptive processes in the cause of a chronic muscle inflammation. Fabrication of occlusal splints in centric relation for temporomandibular disorders (TMD) patients is arguable, since this position has been defined for asymptomatic stomatognathic system. Thus, maximum intercuspation might be employed in patients with occlusal stability, eliminating the need for interocclusal records. This study compared occlusal splints fabricated in centric relation and maximum intercuspation in muscle pain reduction of TMD patients. Twenty patients with TMD of myogenous origin and bruxism were divided into 2 groups treated with splints in maximum intercuspation (I) or centric relation (II). Clinical, electrognathographic and electromyographic examinations were performed before and 3 months after therapy. Data were analyzed by the Student's t test. Differences at 5% level of probability were considered statistically significant. There was a remarkable reduction in pain symptomatology, without statistically significant differences (p>0.05) between the groups. There was mandibular repositioning during therapy, as demonstrated by the change in occlusal contacts on the splints. Electrognathographic examination demonstrated a significant increase in maximum left lateral movement for group I and right lateral movement for group II (p<0.05). There were no significant differences (p>0.05) in the electromyographic activities at rest after utilization of both splints. In conclusion, both occlusal splints were effective for pain control and presented similar action. The results suggest that maximum intercuspation may be used for fabrication of occlusal splints in patients with occlusal stability without large discrepancies between centric relation and maximum intercuspation. Moreover, this technique is simpler and less expensive.

Aims: To estimate the prevalence of symptoms of temporomandibular disorders (TMD) as a function of age and gender, in a representative urban sample from the Brazilian population. Methods: A total of 1,230 inhabitants (51.5% women) aged 15 to 65 years were interviewed by a validated phone survey. Sample size had been previously calculated. TMD symptoms were assessed through five questions, as recommended by the American Academy of Orofacial Pain, in an attempt to identify possible TMD. Data were derived by age and gender. Prevalence of each TMD symptom, and of combination of symptoms, was calculated. Results: At least one TMD symptom was reported by 39.2% of the individuals. Pain related to TMD was noted by 25.6% of the population. Temporomandibular joint (TMJ) sound was the most common symptom of TMD, followed by TMJ pain and masticatory muscle pain. All symptoms were more prevalent in women than in men. With men used as the reference, a relative risk (RR) of at least one TMD symptom in women was 1.31 (95% confidence interval [CI] = 1.14 to 1.52). When at least two symptoms were present, the RR was 1.93 (95% CI = 1.49 to 2.51). For three or more TMD symptoms, the RR was 2.49 (95% CI = 1.67 to 3.71). Women were also more likely than men to have TMD pain (RR = 1.78; 9% CI = 1.45 to 2.18). Conclusion: Individual symptoms, as well as a combination of TMD symptoms, are prevalent in the Brazilian urban population and are more frequent in women than in men. O objetivo deste estudo foi investigar, em mulheres idosas, o efeito agudo do alongamento estático sobre a taxa de desenvolvimento de força pico (TDFP) e contração voluntária máxima (CVM). A amostra foi composta por 10 mulheres idosas (idade 68,5 ± 7,0 anos; peso 70,9 ± 8,1 kg; estatura 159,4 ± 6,0 cm; índice de massa corporal 28,0 ± 3,8 kg/m²). A TDFP e a CVM foram testadas no exercício Leg Press, antes e após as condições controle ou alongamento (três séries de 30 segundos de alongamento estático do quadríceps femoral), em dois dias diferentes (24 horas de intervalo). A TDFP foi determinada como a inclinação mais íngreme da curva, calculada dentro da janela regular de 20 ms (∆Força/∆Tempo), para os primeiros 200 ms após o início da força muscular. A CVM foi determinada como o maior ponto registrado na tentativa. em cada dia, apenas uma condição foi testada e a ordem de emprego para cada condição foi determinada aleatoriamente. A intensidade do alongamento foi determinada pelo limiar de dor muscular. Quatro avaliações pós-condições (pós-tratamento; 10; 20 e 30 minutos) foram realizadas para acompanhar o comportamento da força muscular. A ANCOVA 2x5, seguida do teste post-hoc de Scheffé, não mostrou interações, condição vs. momento, significativas (P > 0,05) para a TDFP e CVM. em conclusão, séries agudas de alongamento estático para o quadríceps femoral não afetam a capacidade de produzir força muscular rapidamente e máxima de mulheres idosas.

Background. The authors compared the efficacy of bilateral balanced and canine guidance (occlusal) splints in the treatment of temporomandibular joint (TMJ) pain in subjects who experienced joint clicking with a nonoccluding splint in a double-blind, controlled randomized clinical trial.Methods. The authors randomly assigned 57 people with signs of disk displacement and TMJ pain into three groups according to the type of splint: bilateral balanced, canine guidance and nonoccluding. The authors followed the groups for six months using analysis of a visual analog scale (VAS), palpation of the TMJ and masticatory muscles, mandibular movements and joint sounds. They used repeated analysis of variance and a XI test to test the hypothesis.Results. The type of guidance used did not influence the pain reduction; yet both occlusal splints were superior to the nonoccluding splint, on the basis of the VAS. Despite similar outcomes in relation to opening, left; lateral and protrusive movements, TMJ and muscle pain on palpation, subjects who used the occlusal splints had improved clinical outcomes. The frequency of joint noises decreased over time, with no significant differences among groups. Subjects in the groups using the occlusal splints reported more comfort.Conclusion. The type of lateral guidance did not influence the subjects'; improvement: All of the subjects had a general improvement on the VAS, though subjects in the occlusal splint groups had better results that did subjects in the nonoccluding splint group.

Association between severe hypothyroidism and acute kidney injury (AKI) is rare. A 40-year-old woman presented with 15 days history of generalised muscle pain, weakness, weight gain and oedema. Medical history: hypertension and hypothyroidism. Physical examination: dry skin, peripheral/periorbital oedema, slow thought and speaking, thyroid increased. Laboratory examinations: high levels of creatine kinase , creatinine, uric acid and lactate dehydrogenase. Free T4 was very low (<0.3 ng/dL) and thyroid-stimulating hormone was high (21.7 mIU/mL). Urinalysis showed haem pigment without haematuria. We performed the diagnosis of AKI secondary to hypothyroidism-induced rhabdomyolysis. Intravenous fluids were started, urinary alkalisation and increased l-thyroxine dose replacement. On the day after admission, forced diuresis with furosemide was introduced leading to a progressive improvement of symptoms. Although hypothyroidism and AKI is unusual, it should be suspected in patients presenting decrease of renal function and high creatine kinase in the absence of other causes of rhabdomyolysis.
