Abstract:INTRODUCTION:The therapeutic scheme of triclabendazole (TCBZ), the recommended anthelmintic against Fasciola hepatica , involves 10mg/kg of body weight administered in a single dose; however, clinical trials in children are scarce. We evaluated the efficacy and tolerability of 2 schemes of TCBZ.METHODS: Eighty-four Peruvian children with F. hepatica eggs in their stools were allocated into 2 groups: 44 received 2 dosages of 7.5mg/kg each with a 12-h interval (Group I), and 40 received a single 10-mg/kg dose (Group II). Evaluation of efficacy was based on the presence of eggs in stools, and tolerability was based on the presence of symptoms and signs post-treatment.RESULTS: A parasitological cure was obtained in 100% of individuals from Group I and 95% of individuals from Group II. The most common adverse event was biliary colic.CONCLUSIONS: The tested scheme was efficacious and tolerable, and it might be an optimal scheme in the region. To the best of our knowledge, this represents the largest series of children treated with TCBZ in a non-hospital setting.

On assistive technology targeted for people with activity limitations and participation, usability issues becomes an essential tool to ensure that the product has the appropriate ergonomics characteristics, in other words, ensure that it fits the specific user´s needs. The aim of this study was to analyze the usability of an adaptive seating device for children with neuromotor impairments, by using kinematic indicators of the reaching movement. The study sample consisted of 13 children with associated neurologic conditions. The tests were developed by using a wooden bench height adjustable, integrated with the adaptive seating device under study, and a system to capture three-dimensional image, called Qualisys Track Manager. The following reaching kinematics variables were measured: maximum reaching velocity, movement duration, index of curvature, and unit movements. It was found that the use of the adaptive seating device had a positive impact on upper limb function in children with neuromotor impairments. It was also noticed an improvement in the reaching movement kinematics, which was statistical significant for the index of curvature and unit movements. As main conclusions, it is possible to point out some positive effects that the product under study seems to have on users' movements, such as the improved movement quality of the upper limb, which could mean a better postural adjustments and higher trunk postural control. By identifying new measures of usability in terms of effectiveness and efficiency for the analyzeddevice, the results obtained may serve also as performance indicators, providing new data that may help to improve the product and eventually modifying it, in order to turn it more compatible with the needs of the considered target population.

The Andalusian Public Health System Virtual Library (Biblioteca Virtual del Sistema Sanitario Público de Andalucía, BV-SSPA), was created in June 2006. In this paper an evaluation is made to see whether the investment made by the Andalusian Government has been worthwhile. The cost-effectiveness and efficiency of the project together with the management criteria are explored.

Depuis quelques années, Internet est devenu un média incontournable pour la diffusion de ressources multilingues. Cependant, les différences linguistiques constituent souvent un obstacle majeur aux échanges de documents scientifiques, culturels, pédagogiques et commerciaux. En plus de cette diversité linguistique, on constate le développement croissant de bases de données et de collections composées de différents types de documents textuels ou multimédias, ce qui complexifie également le processus de repérage documentaire. En général, on considère l’image comme « libre » au point de vue linguistique. Toutefois, l’indexation en vocabulaire contrôlé ou libre (non contrôlé) confère à l’image un statut linguistique au même titre que tout document textuel, ce qui peut avoir une incidence sur le repérage. Le but de notre recherche est de vérifier l’existence de différences entre les caractéristiques de deux approches d’indexation pour les images ordinaires représentant des objets de la vie quotidienne, en vocabulaire contrôlé et en vocabulaire libre, et entre les résultats obtenus au moment de leur repérage. Cette étude suppose que les deux approches d’indexation présentent des caractéristiques communes, mais également des différences pouvant influencer le repérage de l’image. Cette recherche permet de vérifier si l’une ou l’autre de ces approches d’indexation surclasse l’autre, en termes d’efficacité, d’efficience et de satisfaction du chercheur d’images, en contexte de repérage multilingue. Afin d’atteindre le but fixé par cette recherche, deux objectifs spécifiques sont définis : identifier les caractéristiques de chacune des deux approches d’indexation de l’image ordinaire représentant des objets de la vie quotidienne pouvant influencer le repérage, en contexte multilingue et exposer les différences sur le plan de l’efficacité, de l’efficience et de la satisfaction du chercheur d’images à repérer des images ordinaires représentant des objets de la vie quotidienne indexées à l’aide d’approches offrant des caractéristiques variées, en contexte multilingue. Trois modes de collecte des données sont employés : l’analyse des termes utilisés pour l’indexation des images, la simulation du repérage d’un ensemble d’images indexées selon chacune des formes d’indexation à l’étude réalisée auprès de soixante répondants, et le questionnaire administré aux participants pendant et après la simulation du repérage. Quatre mesures sont définies pour cette recherche : l’efficacité du repérage d’images, mesurée par le taux de succès du repérage calculé à l’aide du nombre d’images repérées; l’efficience temporelle, mesurée par le temps, en secondes, utilisé par image repérée; l’efficience humaine, mesurée par l’effort humain, en nombre de requêtes formulées par image repérée et la satisfaction du chercheur d’images, mesurée par son autoévaluation suite à chaque tâche de repérage effectuée. Cette recherche montre que sur le plan de l’indexation de l’image ordinaire représentant des objets de la vie quotidienne, les approches d’indexation étudiées diffèrent fondamentalement l’une de l’autre, sur le plan terminologique, perceptuel et structurel. En outre, l’analyse des caractéristiques des deux approches d’indexation révèle que si la langue d’indexation est modifiée, les caractéristiques varient peu au sein d’une même approche d’indexation. Finalement, cette recherche souligne que les deux approches d’indexation à l’étude offrent une performance de repérage des images ordinaires représentant des objets de la vie quotidienne différente sur le plan de l’efficacité, de l’efficience et de la satisfaction du chercheur d’images, selon l’approche et la langue utilisées pour l’indexation.

Las cirugías reconstructivas múltiples de miembros inferiores son intervenciones quirúrgicas que implican un difícil manejo analgésico en el postoperatorio, actualmente la técnica analgésica usada es la analgesia peridural; sin embargo los efectos adversos asociados a esta no la hacen una técnica ideal para manejo de dolor. Los bloqueos de nervio periférico han aparecido como una alternativa para el manejo del dolor; pero no se ha difundido ampliamente su uso. Se pretende evaluar la disminución de efectos adversos con el uso de bloqueos de nervio periférico sobre la analgesia epidural. Este estudio piloto inicial se realizo para verificar efectividad técnicas a usar y problemas que se pudieran presentar. Se aplico el protocolo en 23 pacientes que fueron llevados a cirugía de miembros inferiores, se dividieron en 2 grupos 11 recibieron bloqueos de nervio periférico y 12 analgesia epidural. Se les realizo seguimiento por 48 horas y se evaluó el control del dolor, consumo de opioides y efectos adversos. El tiempo de colocación del bloqueo fue similar en ambos grupos, el grupo de bloqueos presento menos episodios de dolor y menos episodios de dolor severo. No se presento retención urinaria en ningún paciente pero en el grupo de epidural se presento mayor incidencia de nausea y vomito (60% vs 45%). Se encontró que los bloqueos de nervio periférico son una adecuada opción para el manejo del dolor en este tipo de cirugías; y al parecer disminuye la incidencia de eventos adversos asociados a la analgesia epidural.

El objetivo de esta tesis es mejorar la efectividad y eficiencia de los entornos de aprendizaje virtual. Para lograr este propósito se define un Modelo de Usuario que considera las características del usuario, el contexto y la Interacción. Estas tres dimensiones son integradas en un Modelo de Usuario Integral (MUI) para proveer adaptación de contenido, formato y actividades en entornos educativos con heterogeneidad de usuarios, tecnologías e interacciones. Esta heterogeneidad genera la entrega de contenidos, formatos y actividades inadecuadas para los estudiantes. La particularización del MUI en un entorno educativo es definida Modelo de Estudiante Integral (MEI). Las principales aportaciones de esta tesis son la definición y validación de un MUI, la utilización de un MEI abierto para propiciar la reflexión de los estudiantes sobre sus procesos de aprendizaje, la integración tecnológica con independencia de plataforma y la validación del MEI con estudiantes en escenarios reales.

This paper examines the potential of using Participatory Farm Management methods to examine the suitability of a technology with farmers prior to on-farm trials. A study examining the suitability of green manuring as a technology for use with wet season tomato producers in Ghana is described. Findings from this case-study demonstrate that Participatory Budgeting can be used by farmers and researchers to analyse current cultivation practices, identify the options for including green manures into the system and explore the direct and wider resource implications of the technology. Scored-Causal Diagrams can be used to identify farmers' perceptions of the relative importance of the problem that the technology seeks to address. The use of the methods in this examine evaluation process appears to have the potential to improve the effectiveness and efficiency of the adaptive research process. This ensures that technologies subsequently examined in trials ate relevant to farmers' interests, existing systems and resources, thereby increasing the chances of farmer adoption. It is concluded that this process has potential for use-with other technologies and in other farming systems. (C) 2002 Elsevier Science Ltd. All rights reserved.

The 1988 Federal Constitution of Brazil by presenting the catalog of fundamental rights and guarantees (Title II) provides expressly that such rights reach the social, economic and cultural rights (art. 6 of CF/88) as a means not only to ratify the civil and political rights, but also to make them effective and practical in the life of the Brazilian people, particularly in the prediction of immediate application of those rights and guarantees. In this sense, health goes through condition of universal right and duty of the State, which should be guaranteed by social and economic policies aimed at reducing the risk of disease and other hazards, in addition to ensuring universal and equal access to actions and services for its promotion, protection and recovery (Article 196 by CF/88). Achieving the purposes aimed by the constituent to the area of health is the great challenge that requires the Health System and its managers. To this end, several policies have been structured in an attempt to establish actions and services for the promotion, protection and rehabilitation of diseases and disorders to health. In the mid-90s, in order to meet the guidelines and principles established by the SUS, it was established the Política Nacional de Atenção Oncológica PNAO, in an attempt to sketch out a public policy that sought to achieve maximum efficiency and to be able to give answers integral to effective care for patients with cancer, with emphasis on prevention, early detection, diagnosis, treatment, rehabilitation and palliative care. However, many lawsuits have been proposed with applications for anticancer drugs. These actions have become very complex, both in the procedural aspects and in all material ones, especially due to the highcost drugs more requested these demands, as well as need to be buoyed by the scientific evidence of these drugs in relation to proposed treatments. The jurisprudence in this area, although the orientations as outlined by the Parliament of Supreme Court is still in the process of construction, this study is thus placed in the perspective of contributing to the effective and efficient adjudication in these actions, with focus on achieving the fundamental social rights. Given this scenario and using research explanatory literature and documents were examined 108 lawsuits pending in the Federal Court in Rio Grande do Norte, trying to identify the organs of the Judiciary behave in the face of lawsuits that seeking oncology drugs (or antineoplastic), seeking to reconcile the principles and constitutional laws and infra constitutional involving the theme in an attempt to contribute to a rationalization of this judicial practice. Finally, considering the Rational Use of health demands and the idea of belonging to the Brazilian people SUS, it is concluded that the judicial power requires ballast parameters of their decisions on evidence-based medicine, aligning these decisions housing constitutional principles that the right to health and the scientific conclusions of efficacy, effectiveness and efficiency in oncology drugs, when compared to the treatments offered by SUS

Processing efficiency theory predicts that anxiety reduces the processing capacity of working memory and has detrimental effects on performance. When tasks place little demand on working memory, the negative effects of anxiety can be avoided by increasing effort. Although performance efficiency decreases, there is no change in performance effectiveness. When tasks impose a heavy demand on working memory, however, anxiety leads to decrements in efficiency and effectiveness. These presumptions were tested using a modified table tennis task that placed low (LWM) and high (HWM) demands on working memory. Cognitive anxiety was manipulated through a competitive ranking structure and prize money. Participants' accuracy in hitting concentric circle targets in predetermined sequences was taken as a measure of performance effectiveness, while probe reaction time (PRT), perceived mental effort (RSME), visual search data, and arm kinematics were recorded as measures of efficiency. Anxiety had a negative effect on performance effectiveness in both LWM and HWM tasks. There was an increase in frequency of gaze and in PRT and RSME values in both tasks under high vs. low anxiety conditions, implying decrements in performance efficiency. However, participants spent more time tracking the ball in the HWM task and employed a shorter tau margin when anxious. Although anxiety impaired performance effectiveness and efficiency, decrements in efficiency were more pronounced in the HWM task than in the LWM task, providing support for processing efficiency theory.

As predicted in the first bulletin, produced jointly by the Economic Commission for Latin America and the Caribbean (ECLAC) and the International Labour Organization (ILO), the impact of the economic crisis continued to be felt in Latin America and the Caribbean during the second quarter of 2009. Regional exports of goods and services contracted in response to sluggish demand on international markets, while remittances and foreign direct investment flows continued to fall, credit lost its buoyancy and the total wage bill diminished, owing mainly to job losses. As a result, the growth forecasts of many countries had to be adjusted downwards. Since the end of 2008, the countries of the region had started to implement countercyclical policies —albeit with significant differences— in an effort to use public spending to counter flagging investment and consumer-spending levels and boost aggregate demand. In this second bulletin, ECLAC and ILO show how the impact of the crisis has deepened in labour markets in the region in the first half of the year and examine existing options and the outcome of public-infrastructure and emergency employment programmes designed to mitigate the impact of the crisis on the labour market. The unemployment rate has risen in practically all countries compared with the previous year and this situation worsened further in the second quarter, when urban unemployment exceeded the rate of the corresponding period in 2008 by 1 percentage point (to stand at 8.5%, up from 7.5%), while in the first quarter, the variation was 0.6 of a percentage point. Labour indicators also point to an increase in informality, a decline in employment with social protection and a decrease in full-time employment. Labour-market trends observed in the first half-year, together with the forecast for a 1.9% decline in regional GDP in 2009, suggest that the average annual rate of urban unemployment in the region will be close to 8.5%. This forecast is slightly less pessimistic than the estimate given in the first bulletin; this is attributable to the fall in the participation rate in the first half-year to levels that are expected to remain low for the rest of the year. Without this reduction in the labour supply, due largely to the “discouragement effect”, the annual average urban unemployment rate would stand at between 8.8% and 8.9%. Thus, the open urban unemployment figure would increase by 2.5 million and if the “discouraged job-seekers” are included, then the number of additional persons not finding a niche in the urban labour market would climb to 3.2 million. In the region, as in the rest of the world, there are signs that the crisis may have reached bottom in the middle of the year. In many countries, production levels have ceased their decline and there are indications of an incipient recovery leading to cautious optimism that there may be a moderate upturn in labour markets in the fourth quarter. The pace of recovery will vary from one country to the next and is expected to be gradual at best. Even with the return to a growth path, there should be no illusion that the labour problems will immediately disappear. First, the recovery in employment is expected to lag behind the upturn in economic activity. Second, since economic growth is likely to remain moderate in the short term and well below the rates recorded between late 2003 and mid-2008, demand for labour and consequently the generation of good-quality jobs will continue to be weak. Thus, countries should not relax their efforts to defend and create decent jobs, but rather should take steps to improve the effectiveness and efficiency of available instruments. In this way, the region will be in a better position not only to confront the challenges of economic recovery, but also to strengthen the foundations for social inclusion and for advancing under more favourable conditions towards fulfilment of the Millennium Development Goals.

A baciloscopia de escarro é o exame mais difundido para o diagnóstico e controle da Tuberculose (TB), por ser um método de simples execução e de baixo custo, porém possui baixa sensibilidade e especificidade. Para que a eficácia e eficiência deste método sejam atingidas, é necessário o estabelecimento e aplicações de normas padrão de execução e leitura de lâminas. Nesse sentido, é fundamental conhecer a qualidade do exame baciloscópico disponível em uma rede de laboratórios. O objetivo deste estudo foi avaliar a qualidade das baciloscopias para diagnóstico e controle da TB realizadas nas Unidades Laboratoriais (UL) pertencentes à rede pública de laboratórios do município de Belém, Pará, no ano 2007, 2008 e 2009. A avaliação foi dividida em três etapas, correspondentes ao ano de confecção das lâminas. Participaram 18 UL que enviaram, regularmente, ao Laboratório Central do Pará, todas as lâminas de baciloscopia confeccionadas por elas para a Avaliação Externa da Qualidade da Baciloscopia (AEQB), que utiliza o método de releitura cega dos esfregaços. Este método compreende uma avaliação técnica dos esfregaços, classificando-os como “Adequados” e “Inadequados”, além da análise de discordância de resultados Falso positivo (FP) e Falso negativo (FN). No período do estudo, 4.117 lâminas foram processadas pelas UL. Na 1ª etapa de avaliação, todas as 18 UL apresentaram lâminas que foram classificadas como inadequadas, sendo que este número diminuiu para 12 UL na 2ª etapa de avaliação e para seis UL na 3ª etapa. Em relação às discordâncias, 03 UL apresentaram discordâncias de FP e 03 UL de FN. Os resultados obtidos no estudo reforçam a necessidade de manter e expandir a AEQB para todas as UL que realizam baciloscopia na rede de laboratórios.
