The presence of Vibrio spp. and Salmonella spp. in crabs marketed at the Bezerra de Menezes Ave., Fortaleza, State of Ceará, Brazil, was assessed between February and May, 2003. The number of individuals sampled in each one of the fifteen weekly samplings ranged between four and eight. Seven strains of Salmonella, from four different samplings, were identified, being five of them identified as serotype S. Senftenberg and two as S. Poona. All strains of Salmonella were sensitive to the tested anti-microbial drugs, with the exception of tetracycline and nalidixic acid, for which an intermediary sensibility was found. The MPN's for Vibrio ranged between 110/g and 110,000/g. Of the forty five Vibrio strains isolated from the crab samples, only 10 were identified up to the species level: two V. alginolyticus and eight V. parahaemolyticus. Bacteria belonging to the Enterobacteriaceae and Pseudomonaceae families were also identified, namely Escherichia coli, Klebsiella pneumoniae, Enterobacter cloacae, Pantoea agglomerans and Pseudomonas aeruginosa. The proper cooking of the animals is recommended in order to avoid problems for the consumers of this crustacean.

Extended-spectrum β-lactamases (ESBL) in enterobacteria are recognized worldwide as a great hospital problem. In this study, 127 ESBL-producing Enterobacteriaceae isolated in one year from inpatients and outpatients at a public teaching hospital at São Paulo, Brazil, were submitted to analysis by PCR with specific primers for blaSHV, blaTEM and blaCTX-M genes. From the 127 isolates, 96 (75.6%) Klebsiella pneumoniae, 12 (9.3%) Escherichia coli, 8 (6.2%) Morganella morganii, 3 (2.3%) Proteus mirabilis, 2 (1.6%) Klebsiella oxytoca, 2 (1.6%) Providencia rettgeri, 2 (1.6%) Providencia stuartti, 1 (0.8%) Enterobacter aerogenes and 1 (0.8%) Enterobacter cloacae were identified as ESBL producers. BlaSHV, blaTEM and blaCTX-M were detected in 63%, 17.3% and 33.9% strains, respectively. Pulsed field gel eletrophoresis genotyping of K. pneumoniae revealed four main molecular patterns and 29 unrelated profiles. PCR results showed a high variety of ESBL groups among strains, in nine different species. The results suggest the spread of resistance genes among genetically different strains of ESBL-producing K. pneumoniae in some hospital wards, and also that some strongly related strains were identified in different hospital wards, suggesting clonal spread in the institutional environment.

Introduction: The objective of this study was to determine the prevalence and etiological profile of enteropathogens in children from a daycare center. Methods: From October 2010 to February 2011 stool samples from 100 children enrolled in a government daycare center in the municipality of São José do Rio Preto, in the state of São Paulo, were collected and analyzed. Results: A total of 246 bacteria were isolated in 99% of the fecal samples; 129 were in the diarrheal group and 117 in the non-diarrheal group. Seventy-three strains of Escherichia coli were isolated, 19 of Enterobacter, one of Alcaligenes and one of Proteus. There were 14 cases of mixed colonization with Enterobacter and E. coli. Norovirus and Astrovirus were detected in children with clinical signs suggestive of diarrhea. These viruses were detected exclusively among children residing in urban areas. All fecal samples were negative for the presence of the rotavirus species A and C. The presence of Giardia lamblia, Entamoeba coli, Endolimax nana and hookworm was observed. A significant association was found between food consumption outside home and daycare center and the presence of intestinal parasites. Conclusions: For children of this daycare center, intestinal infection due to pathogens does not seem to have contributed to the occurrence of diarrhea or other intestinal symptoms. The observed differences may be due to the wide diversity of geographical, social and economic characteristics and the climate of Brazil, all of which have been reported as critical factors in the modulation of the frequency of different enteropathogens.

Baseados num caso de associação de abcessos renal, perirrenal e de quisto renal infectado, ocorrendo após biopsia prostática, os autores fazem revisão bibliográfica dos abcessos renais, classificados segundo características anatómicas perinéfrico, cortical e cortico- -medular (nefrite bacteriana aguda focal/multifocal, pielonefrite enfisematosa e pielonefrite xantogranulomatosa). Os abcessos renais são mais frequentes em diabéticos, menos vezes em alcoólicos, desnutridos e imunodeprimidos, estando associados geralmente a pielonefrites recorrentes, refluxo vesicoureteral, litíase urinária, ou resultam de êmbolos sépticos hematogéneos originados em focos extra-renais de infecção. A Escherichia coli é responsável pela maioria dos casos, e menos vezes a Klebsiella Proteus, Enterobacter, Pseudomonas, e Estafilococo aureus. Descrevem-se a expressão clínica, complicações evolutivas e diagnóstico radiológico, enfatizando-se as infecções acompanhadas de formação de gás. Referem-se opções terapêuticas médico-cirúrgicas, cujas indicações dependem do diagnóstico precoce, complicações e factores predisponentes.

The evaluation of workers as potential reservoirs and disseminators of pathogenic bacteria has been described as a strategy for the prevention and control of healthcare-associated infections (HAIs). The aim of this study was to evaluate the presence of Enterobacteriaceae in the oral cavity of workers at an oncology hospital in the Midwest region of Brazil, as well as to characterize the phenotypic profile of the isolates. Saliva samples of 294 workers from the hospital’s healthcare and support teams were collected. Microbiological procedures were performed according to standard techniques. Among the participants, 55 (18.7%) were colonized by Enterobacteriaceae in the oral cavity. A total of 64 bacteria were isolated, including potentially pathogenic species. The most prevalent species was Enterobacter gergoviae (17.2%). The highest rates of resistance were observed for β-lactams, and 48.4% of the isolates were considered multiresistant. Regarding the enterobacteria isolated, the production of ESBL and KPC was negative. Nevertheless, among the 43 isolates of the CESP group, 51.2% were considered AmpC β-lactamase producers by induction, and 48.8% were hyper-producing mutants. The significant prevalence of carriers of Enterobacteriaceae and the phenotypic profile of the isolates represents a concern, especially due to the multiresistance and production of AmpC β-lactamases.

Em face à predominante eliminação biliar da rifamicina S.V. atingindo concentrações muitas vêzes superiores aos níveis séricos obtidos com as doses terapêuticas, e pelo possível interêsse dessa verificação para o tratamento dos portadores biliares crônicos de Salmonella typhi determinou-se a concentração mínima inibitória de 165 estirpes de enterobactérias, incluindo 77 amostras de S. typhi. Foi verificado que a maioria das cepas de Escherichia coli, Shigella e Proteus mirabilis correspondiam a uma concentração inibitória mínima entre 33 a 65 μg/ml. Entre 65 e 128 μg/ml foram determinadas as concetrações inibitórias mínimas da maioria das outras espécies de Proteus, de Providencia e de Klebsiella. Para Salmonella e Enterobacter o limite mínino de sensibilidade foi, em regra, igual ou superior a 128 μg/ml. Diferenças mais acentuadas de comportamento entre as enterobactérias foram observadas quanto à ação bactericida da rifamicidas S.V. De uma maneira geral, para E. coli e Shigella, as concentrações inibitórias mínimas já referidas. Para as espécies de Proteus e Providencia houve variação maior de comportamento, mas tendência a que o efeito bactericidas fôsse encontrado em concentrações que correspondiam a 4 vêzes as bacteriostáticas para as mesmas espécies. Finalmente, de modo pouco feliz para os propósitos visados, em Salmonella, com a inclusão de S. typhi, não foi atingido um efeito bactericida, com as mais altas concentrações usadas as quais corresponderam em média a 6 vêzes as concentrações bacteriostáticas para esse gênero.

Foi feita uma avaliação dos mecanismos de seletividade e inibição do meio de Rappaport em relação a espécies representativas de enterobactérias. O meio hipertônico original, com 3,6% de cloreto de magnésio demonstrou-se inibidor para Salmonella typhimurium, a qual foi ainda inibida à concentração de 3,1%. Não se verificou crescimento de tôdas as espécies de Shigella, a partir da concentração de 1,8%, Escherichia coli a partir de 2,2%, Enterobacter a partir de 3,1% e Proteus mirabilis só com 3,6%. O verde malaquita na concentração original de 0,012% demonstra capacidade de inibição do crescimento de todas as espécies testadas, com a exceção de Enterobacter e S. typhimurium. A substituição do fosfato diácido de potássio pelo correspondente sal de sódio, atenua o poder inibidor do meio para tôdas as espécies bacterianas estudadas. As shigelas revelaram-se as enterobactérias mais sensíveis em relação a êsse meio, sendo igualmente inibidas pelos meios à base do tiosulfado de cálcio ou de magnésio, também inspirados no emprêgo de soluções hipertônicas.

No presente trabalho foram dissecados o trato digestivo de 245 fêmeas de Lutzomyia longipalpis originários da Gruta da Lapinha, Município de Lagoa Santa, MG, formando 7 grupos de 35 flebotomíneos. Das 8 espécies de bactérias isoladas houve uma predominância de bactérias Gram negativas (BGN) pertencentes ao grupo de não fermentadoras de açúcar das seguintes espécies: Acinetobacter lowffii, Stenotrophomonas maltophhilia, Pseudomonas putida e Flavimonas orizihabitans. No grupo das fermentadoras tivemos: Enterobacter cloacae e Klebsiella ozaenae. No grupo dos Gram positivos foram identificados Bacillus thuringiensis e Staphylococcus spp.

O estudo foi realizado com o objetivo de isolar e identificar bactérias do gênero Vibrio em feridas cutâneas apresentando processo infeccioso, em pescadores do município de Raposa-MA. O material clínico foi obtido com o emprego de "swab" e mantido em meio de transporte de Cary-Blair. Para o processamento laboratorial foram utilizadas técnicas clássicas de enriquecimento em água peptonada alcalina, isolamento em meio seletivo-indicador (TCBS) e identificação bioquímica das espécies. Participaram do estudo 50 pescadores, com idade variando de 12 a 65 anos, tendo sido reconhecidos 21 indivíduos portadores de Vibrio. Houve predomínio da espécie V. alginolyticus (66,6%), seguido de V. parahaemolyticus (42,8%) e de V. cholerae não O1 (9,5%). As lesões predominaram nos membros inferiores, apresentando hiperemia, edema, secreção, dor. Associados aos vibrios foram isoladas espécies de bacilos gram negativos dos gêneros Serratia, Proteus, Escherichia, Citrobacter, Enterobacter, assim como outras bactérias não-fermentadoras (30,9%) e bactérias gram positivas do gênero Staphylococcus.

A prospective study was conducted from June 2001 to May 2002 at the Burns Unit of Hospital Regional da Asa Norte, Brasília, Brazil. During the period of the study, 252 patients were treated at the Burns Unit, 49 (19.4%) developed clinically and microbiologically proven sepsis. Twenty-six (53.1%) were males and 23 (46.9%) females with a mean age of 22 years (range one to 89 years) and mean burned body surface area of 37.7 ± 18.4% (range 7 to 84%). Forty-three patients had flame burns, five a scald and one an electric burn. These 49 patients had a total of 62 septic episodes. Forty (81.6%) patients had only one and nine (18.4%) had up to three episodes of sepsis. Thirty (61.2%) patients had their first septicemic episode either earlier or by one week postburn. Out of 62 septic episodes, 58 were due to bacteria and four due to Candida sp. The most common bacteria isolated from blood culture were Staphylococcus aureus, coagulase-negative Staphylococcus, Acinetobacter baumannii, Enterobacter cloacae and Klebsiella pneumoniae. Eleven (18.9%) episodes were due to oxacillin resistant Staphylococcus aureus. Acinetobacter baumannii was sensitive to ampicillin/sulbactam in 71.4% and to imipenem in 85.7% of the cases. The primary foci of sepsis were the burn wound in 15 ( 24.2% ) episodes. The most common clinical findings of sepsis in these patients were fever, dyspnea, hypotension and oliguria. The most common laboratory findings of these patients were anemia, leukocytosis, hypoalbuminemia and thrombocytopenia. Twelve (24.5%) patients died. The appropriate knowledge of clinical, epidemiological, laboratorial and microbiological aspects of sepsis in burned patients permits an adequate diagnosis and treatment of this complication.

Um estudo prospectivo, sobre a sensibilidade antimicrobiana da flora bacteriana em úlceras cutâneas leishmanióticas, foi realizado em pacientes portadores de leishmaniose tegumentar, em Corte de Pedra, Bahia. Foram estudados 84 pacientes, principalmente adolescentes e adultos dedicados à lavoura, apresentando lesão cutânea única. Staphylococcus aureus predominou (83%) nas culturas, sendo sensível à maioria dos antibióticos testados. Flora bacteriana mista esteve presente na úlcera em 37 (44,1%) pacientes. Entre as bactérias Gram-negativas isoladas, foram mais freqüentes Enterobacter sp (13,1%), Proteus sp (8,3%), Pseudomonas aeruginosa (7,1%) e Klebsiella sp (7,1%), sendo sensíveis principalmente à ciprofloxacina, aminoglicosídeos, cefalosporinas de terceira geração e carbapenêmicos.

INTRODUÇÃO: O principal mecanismo de resistência emergente entre Enterobacteriaceae é a produção de β-lactamases de espectro estendido, enzimas capazes de hidrolisar cefalosporinas-de-amplo-espectro, que são bastante utilizadas na terapia antimicrobiana de infecções por enterobactérias. Embora a resistência a esses agentes apresente grande variabilidade geográfica, os índices de resistência são elevados em diversos países MÉTODOS: Um estudo observacional, transversal, descritivo e retrospectivo foi desenvolvido para avaliar a frequência de ESBL entre cepas de Enterobacteriaceae obtidas no Hospital São Vicente de Paulo, Brasil RESULTADOS: A produção de ESBL foi observada em 24,8% (nº=208/838) dos isolados avaliados. Isolados de Escherichia coli representaram 46,2% (nº=96/208) do percentual de produtores de ESBL, seguido de espécies de Enterobacter 30,3% (nº=63/208). A sensibilidade desses isolados ao meropenem foi de 91,4% e a piperacilina/tazobactam de 67,4% CONCLUSÕES: Os índices de ESBL encontrados confirmam a preocupação mundial com este mecanismo de resistência.

Lithiasic cholecystitis is classically associated with the presence of enterobacteria, such as Escherichia coli, Enterococcus, Klebsiella, and Enterobacter, in the gallbladder. Cholecystitis associated with fungal infections is a rare event related to underlying conditions such as diabetes mellitus, steroid use, and broad-spectrum antibiotic use for prolonged periods, as well as pancreatitis and surgery of the digestive tract. Here, we present the first reported case of a gallbladder infection caused by Candida famata.

IntroductionRecently, pathogen ecology has been recognized as an important epidemiological determinant of healthcare-associated infections (HAIs). Acinetobacter baumannii is one of the most important agents known to cause HAIs. It is widespread in healthcare settings and exhibits seasonal variations in incidence. Little is known about the impact of competition with other hospital pathogens on the incidence of A. baumannii infection.MethodsWe conducted an ecological study, enrolling patients who presented with healthcare-associated bloodstream infections (HA-BSIs) from 2005 to 2010 at a 450-bed teaching hospital in Brazil. HA-BSIs were said to be present when bacteria or fungi were recovered from blood cultures collected at least three days after admission. Monthly incidence rates were calculated for all HA-BSIs (overall or caused by specific pathogens or groups of pathogens). Multivariate Poisson regression models were used to identify the impacts of the incidence of several pathogens on the incidence of A. baumannii.ResultsThe overall incidence rate of HA-BSI caused by A. baumannii was 2.5 per 10,000 patient-days. In the multivariate analysis, the incidence of HA-BSI caused by A. baumanniiwas negatively associated with the incidence rates of HA-BSI due to Staphylococcus aureus (rate ratio [RR]=0.88; 95% confidence interval [CI]=0.80-0.97), Enterobacter spp. (RR=0.84; 95%CI=0.74-0.94) and a pool of less common gram-negative pathogens.ConclusionsOur results suggest that competition between pathogens influences the etiology of HA-BSIs. It would be beneficial to take these findings into account in infection control policies.
