Objective: To evaluate the effectiveness of a programme of static positional stretches and positioning of the stroke-affected shoulder for maintaining shoulder external rotation and decreasing hemiplegic shoulder pain. Design: Randomized controlled trial with pretest and posttest design. Setting: Inpatient rehabilitation unit. Subjects: Thirty-two participants ( 17 treatment, 15 comparison) with a first time stroke who were admitted for rehabilitation. Interventions: Treatment participants completed a programme of static positional stretches of the stroke-affected shoulder twice daily and positioned the stroke-affected upper limb in an armrest support at all other times when seated. Main measures: The main outcome measures were pain-free range of motion into external rotation, pain in the stroke-affected shoulder at rest and with movement, motor recovery and functional independence. Results: All participants demonstrated a significant loss of external rotation ( P = 0.005) with no significant group differences. All participants demonstrated a significant improvement in motor recovery ( P < 0.01) and functional independence ( P < 0.01) with no significant group differences. There were no significant effects for pain. The comparison group recorded a decrease in mean pain reported with movement from admission to discharge, and the treatment group recorded an increase. Conclusions: Participation in the management programme did not result in improved outcomes. The results of this study do not support the application of the programme of static positional stretches to maintain range of motion in the shoulder. The effect of increasing pain for the treatment group requires further investigation.

Der Schulweg ist ein Intermedium zwischen dem Zuhause und der Schule. Hier findet die Transformation vom Kind bzw. Jugendlichen zum Schüler oder Schülerin statt. Auf diesem Weg bilden sich einzigartige Kinder- und Jugendkulturen. Gerade diese „space-time“ fand in der erziehungswissenschaftlichen Forschung bisher kaum Beachtung. Denn Untersuchungen zum Schulweg befassen sich oftmals mit dem Weg als physikalische Größe oder Fragen aus psychologischer Perspektive nach Problemen wie Angstzuständen. Hintergrund ist dabei immer, den Schulweg für die Kinder angenehmer und schöner zu gestalten. Nicht beachtet wird, dass er von den Erwachsenen in der Regel nicht kontrolliert wird. Die vorliegende Studie blickt aus historischer Perspektive auf Schulwege in den beiden deutschen Staaten der 1970er Jahren. Es wird herausgearbeitet, wie sich Kinder- und Jugendkulturen in der Bundesrepublik und in der DDR auf Schulwegen konstituiert haben. (DIPF/Orig.)

The concept of equal opportunity for all students is deeply embedded in the Serbian constitution and in education laws. On that level, there is no doubt that everyone is ensured an opportunity to receive quality education. Many measures in education policy have been created specifically to achieve this objective and make the system fair and inclusive. The Coleman Report was linked to a wave of optimism that certain educational measures would help in achieving these noble goals. This aim is a high priority in education in a democratic country, and due to its importance needs to be re-examined. Thus, the present research examines the equity of students in the Serbian education system, detecting areas on all educational levels that could be (or already are) systemic sources of inequity (e.g., criteria for preschool institution enrolment, the system of student awards, rationalisation of the school network, the concept of entrance exams to secondary school or university, etc.). A number of measures have already been taken in the system specifically to deal with inequity (e.g., the Preschool Preparatory Programme, dropout measures, inclusion, scholarships, etc.). The effects of these measures in particular are analysed in the present work. In addition to an analysis of the systemic sources of inequity in the Serbian education system, the article also makes recommendations for their overcoming. (DIPF/Orig.)

O presente estudo tem como principal objetivo a compreensão da forma como lésbicas, gays e bissexuais gerem a visibilidade da sua orientação sexual no contexto do ensino superior, bem como a relação entre esta com a perceção da discriminação nesse contexto. Neste sentido, foi utilizada uma amostra de 289 estudantes de instituições do ensino superior de todo o país. Para esta avaliação, foram utilizados dois instrumentos: a Escala de Gestão da Visibilidade da Orientação Sexual (Lasser, Ryser & Price, 2010) e o Questionário de Gestão da Visibilidade e Discriminação em Contextos do Ensino Superior, criado para esta investigação. Os principais resultados apontam diferenças na gestão da visibilidade quanto ao género, orientação sexual, visibilidade da orientação sexual, visibilidade da orientação sexual perante a família, nível de religiosidade e ciclo de estudos. No que diz respeito à perceção de discriminação, os resultados apresentam diferenças quanto à orientação sexual e à área de estudos. Por fim, os resultados conclusivos mostram que existe uma correlação entre a visibilidade da orientação sexual e a perceção de discriminação, no contexto do ensino superior; Abstract: This study aims to understand how lesbian, gay and bisexual (LGB) manage the visibility of their sexual orientation in the context of higher education, as well as the relationship between this management with their perception of discrimination. In this sense, we used a sample of 289 students from higher school institutions across the country. For this evaluation, two instruments were used: the Visibility Management Scale of Sexual Orientation (Lasser, Ryser & Price, 2010) and the Visibility Management and Discrimination in Contexts of Higher Education Questionnaire, created for this research. The main results show that participant’s gender, sexual orientation, visibility of sexual orientation, visibility of sexual orientation within the family, level of religiosity and course of study are related to the management visibility of sexual orientation. With regard to the perception of discrimination, the results showed differences in sexual orientation and study area. Finally, the conclusive results show that there is a correlation between the visibility of sexual orientation and the perception of discrimination in the context of higher education.

Developed, piloted, and examined the psychometric properties of the Child and Adolescent Social and Adaptive Functioning Scale (CASAFS), a self-report measure designed to examine the social functioning of young people in the areas of school performance, peer relationships, family relationships, and home duties/self-care. The findings of confirmatory and exploratory factor analysis support a 4-factor solution consistent with the hypothesized domains. Fit indexes suggested that the 4-correlated factor model represented a satisfactory solution for the data, with the covariation between factors being satisfactorily explained by a single, higher order factor reflecting social and adaptive functioning in general. The internal consistency and 12-month test-retest reliability of the total scale was acceptable. A significant, negative correlation was found between the CASAFS and a measure of depressive symptoms, showing that high levels of social functioning are associated with low levels of depression. Significant differences in CASAFS total and subscale scores were found between clinically depressed adolescents and a matched sample of nonclinical controls. Adolescents who reported elevated but subclinical levels of depression also reported lower levels of social functioning in comparison to nonclinical controls.

This project investigated the relationship between attachment style and postnatal depression. In a sample of mothers with infants, those identifying themselves as depressed reported a more preoccupied attachment style by comparison with their nondepressed counterparts. Maternal attachment style was not related to perceived infant characteristics or to the reported mother-child relationship. Postnatal depression, however, was related to both perceived infant characteristics and the reported mother-child relationship. Although postnatal depression was not significantly related to marital quality, a trend did emerge between attachment style and marital quality. These findings suggest that further research is warranted to clarify the relationship between attachment style and postnatal depression.

BACKGROUND: We aimed to study the incidence and outcome of severe traumatic brain injury (TBI) in Switzerland and to test the feasibility of a large cohort study with case identification in the first 24 hours and 6-month follow-up. METHODS: From January to June 2005, we consecutively enrolled and followed up all persons with severe TBI (Abbreviated Injury Score of the head region >3 and Glasgow Coma Scale <9) in the catchment areas of 3 Swiss medical centres with neurosurgical facilities. The primary outcome was the Extended Glasgow Outcome Scale (GOSE) after 6 months. Secondary outcomes included survival, Functional Independence Mea - sure (FIM), and health-related quality of life (SF-12) at defined time-points up to 6 months after injury. RESULTS: We recruited 101 participants from a source population of about 2.47 million (ie, about 33% of Swiss population). The incidence of severe TBI was 8.2 per 100,000 person-years. The overall case fatality was 70%: 41 of 101 persons (41%) died at the scene of the accident. 23 of 60 hospitalised participants (38%) died within 48 hours, and 31 (53%) within 6 months. In all hospitalised patients, the median GOSE was 1 (range 1-8) after 6 months, and was 6 (2-8) in 6-month survivors. The median total FIM score was 125 (range 18-126); median-SF-12 component mea - sures were 44 (25-55) for the physical scale and 52 (32-65) for the mental scale. CONCLUSIONS: Severe TBI was associated with high case fatality and considerable morbidity in survivors. We demonstrated the feasibility of a multicentre cohort study in Switzerland with the aim of identifying modifiable determinants of outcome and improving current trauma care.

An interdisciplinary team of a pediatric university teaching hospital in the Montreal area, who treats annually a considerable number of child burn victims, developed a behavioral observation scale on the comfort of children of 7 years of age and less during painful procedures «Échelle d’observation comportementale du confort d’enfants brûlés» (OCCEB- BECCO). The goal of this study was to initiate the validation of OCCEB-BECCO, a new tool for evaluation of comfort for child burn victims. With samples of 16 patients and 5 experts, we proceeded to the evaluation of content validity, internal consistency and criterion validity. Results have shown adequate content validity, internal consistency with (T1) r = 0,96 p < 0,0001, (T2) r = 0,95 p < 0,0001, (T3) r = 0,95 p < 0,0001 and criterion validity with Cronbach alpha at 0,82. A future study with a larger sample and on a longer period of time would be required to pursue validation of this new scale.

A investigação tem demonstrado que as crianças em desvantagem social enfrentam muitos desafios e estão mais vulneráveis a desenvolver dificuldades ao nível da regulação emocional, sendo extremamente relevante identificar fatores protetores que moderem esta vulnerabilidade, de modo a identificar formas de alterar as trajetórias desenvolvimentais. No presente estudo, foi analisado o papel moderador das interações do educador de infância no desenvolvimento da regulação emocional. Os participantes incluíram 345 crianças em idade pré-escolar de dois ambientes sociais distintos: em risco sociocultural (n=183) e em situação de não risco sociocultural. A regulação emocional foi avaliada através de um questionário ao educador, o Emotion Regulation Checklist (ERC; Shields & Cichetti, 1997). A qualidade das interações do educador de infância com as crianças foi observada através de uma escala de observação, o Classroom Assessment Scoring System (CLASS; Pianta, La Paro & Hamre, 2008). Os educadores avaliaram ainda a sua relação com cada criança participante através da Student-Teacher Rating Scale (STRS; Pianta, 2001). Análises multi-grupo multinível indicaram que no grupo de crianças de risco socicultural, em salas em que os educadores de infância estabeleciam interações com mais qualidade a nível emocional e organizacional, as crianças demonstraram competências de regulação emocional mais elevadas, o que sugere o importante papel das interações dos educadores de infância com as crianças no desenvolvimento da regulação emocional.

This controlled trial of a parent management intervention aimed to increase parental competence in management of problem behaviours associated with Asperger syndrome. The intervention compared two formats, a 1 day workshop and six individual sessions. Measures were taken on three occasions: pre-intervention, at 4 weeks, and at 3 month follow-up. Variables of interest were number and intensity of problem behaviours, and parent evaluation of social interaction skills. Results showed parents reporting fewer and lower intensity of problem behaviours and increased social interactions at 4 weeks and 3 months. Results held across formats and suggest that parent management training can provide an effective intervention for parents of a child with Asperger syndrome. Group differences on outcome measures and in the use of strategies are discussed along with limitations of the study.

Despite the frequency with which fevers occur in children ages 1–3 years, lack of knowledge and understanding about the implications of fever and methods of fever management often results in anxiety among caretakers, sometimes prompting them to seek help at nearby emergency departments. Caretakers often look to health care professionals for advice and guidance over the telephone. The purpose of this study was to investigate caretakers' knowledge of the implications of fever, methods of fever management, perceptions of pediatric telephone triage and advice services regarding fever, and the effectiveness of after hour telephone triage directed toward improving the caretakers' ability to manage their child's fever at home. Pre-triage questionnaires were completed by 72 caretakers over the telephone before the triage encounter. Twenty-two of those same caretakers whose children were triaged using the fever guideline completed and returned the mailed post-triage questionnaire. Descriptive statistics were used to analyze responses for the larger pre-intervention group and describe comparisons for the pre and post-triage responses in the smaller sample subset (n = 22). ^

Aim: Fever and febrile illness are some of the most common conditions managed by parents. The aim of this study was to examine the knowledge, attitudes and beliefs of parents around fever in children under five years of age. Methods: Between July and August 2014, a convenience sample of parents was invited to participate in this study in Copenhagen, Denmark. Results were analysed thematically using a constant comparison method. Results: Twenty-one parents participated in the study. Five themes emerged from the data: parental concern, help-seeking behaviour, parental knowledge, parent fever management practices and initiatives. Parents used a range of information sources to obtain their knowledge on management of fever; however, due to issues of trust with these sources, reassurance was often sought from healthcare practitioners. There was a desire amongst most parents for initiatives to be introduced which provide general information on how to manage fever in children. Conclusion: Parents were very concerned when their child was febrile and instigated practices obtained from accessible information sources. This study has identified a need for specific and reliable information initiatives to be introduced as a means of reducing parental concern and ensuring evidence-based strategies for managing a child with fever.
