Purpose: The purpose of this work was to study the bone tissue reaction after porous polyethylene (Polipore) implantation into surgical defects in the parietal bones of rats with streptozotocin-induced diabetes, treated with salmon calcitonin. Materials and Methods: Porous polyethylene implants were placed in bone defects created in 36 adult female rats. The rats were divided into 3 equal groups: diabetic treated with calcitonin (DCa), diabetic (D), and control (C). The animals of the DCa group received applications of salmon calcitonin on alternating days immediately after the surgery until sacrifice. The rats were sacrificed after 15, 30, 60, and 90 days, and the defects were examined histologically and statistically through histomorphometric analysis. Results: Histomorphometric analysis showed that there was no statistically significant difference in the mean quantity of inflammatory cells among all study groups after 15 and 90 days. At 30 days, a statistically significant difference was observed between the D and C groups and the D and DCa groups. At 60 days, there was no statistically significant difference between the D and DCa groups. Discussion: Porous polyethylene can be considered an option for implant material when there are investigations that prove its biocompatibility and stability in the host tissues. Salmon calcitonin positively aided the bone repair and attenuated the inflammatory response until 30 days after the surgery. Conclusion: Porous polyethylene was tolerated by the host tissues in all groups, and moderate chronic inflammatory reaction was observed up to the 90-day period. Salmon calcitonin attenuated the inflammatory response up until 30 days.

Imaging diagnosis is a medical specialty that uses imaging techniques to perform diagnosis. In diagnostic imaging various methods are used such as direct absorption of photons - SPA and DPA, radiographic photometry, the dual-energy radiographic absorptiometry - DEXA, ultrasound, magnetic resonance imaging, computed tomography and optical densitometry in radiographic image. The dog can be considered one of the most widely used animals in the study of bone diseases and searching for a reliable diagnosis, although not an ideal model for the study of osteoporosis, because these animals tend not to develop a decrease in bone mineral density. The objective of this study was to analyze bone density in mongrel dogs from the determination of the variation of density along the radio-ulna bone and also the mean value related to gender, weight and age of individuals. The density analysis carried out showed that for this data set, there is a significant difference in the case of gender and age of the animal and may generalize according to these variables. The only significant difference was found in the weight, which increases bone mass is related to weight gain through the growth of the animal

OBJETIVO: Desenvolver um modelo experimental que permita estudar a regeneração de grandes falhas ósseas em condições de infecção. MÉTODO: Falhas ósseas segmentares de 15mm foram criadas cirurgicamente na ulna de 12 coelhos e inoculadas com 5x10(8) unidades formadoras de colônia (UFC) de S. aureus. O desbridamento da infecção foi realizado duas semanas após, seguida da aplicação sistêmica de gentamicina por quatro semanas. Os animais foram acompanhados por um período de 12 semanas para avaliação do controle da infecção e da regeneração óssea. RESULTADOS: A regeneração espontânea foi inferior a 25% do defeito na avaliação radiográfica e histológica. CONCLUSÃO: A Falha óssea infectada de 15mm na ulna de coelhos é incapaz de alcançar a regeneração completa sem tratamentos adicionais. Nível de Evidência V, Estudo experimental.

L’obiettivo della tesi è studiare il virus HIV-1 in relazione alle alterazioni sistemiche, riscontrate nel paziente HIV-infetto, in particolare alterazioni a carico del sistema scheletrico, indotte dal virus o dall’azione dei farmaci utilizzati nella terapia antiretrovirale (HAART). L’incidenza dell’osteoporosi nei pazienti HIV-positivi è drammaticamente elevata rispetto alla popolazione sana. Studi clinici hanno evidenziato come alcuni farmaci, ad esempio inibitori della proteasi virale, portino alla compromissione dell’omeostasi ossea, con aumento del rischio fratturativo. Il nostro studio prevede un follow-up di 12 mesi dall’inizio della HAART in una coorte di pazienti naïve, monitorando diversi markers ossei. I risultati ottenuti mostrano un incremento dei markers metabolici del turnover osseo, confermando l’impatto della HAART sull’omeostasi ossea. Successivamente abbiamo focalizzato la nostra attenzione sugli osteoblasti, il citotipo che regola la sintesi di nuova matrice ossea. Gli esperimenti condotti sulla linea HOBIT mettono in evidenza come il trattamento, in particolare con inibitori della proteasi, porti ad apoptosi nel caso in cui vi sia una concentrazione di farmaco maggiore di quella fisiologica. Tuttavia, anche concentrazioni fisiologiche di farmaci possono regolare negativamente alcuni marker ossei, come ALP e osteocalcina. Infine esiste la problematica dell’eradicazione di HIV-1 dai reservoirs virali. La HAART riesce a controllare i livelli viremici, ciononostante diversi studi propongono alcuni citotipi come potenziali reservoir di infezione, vanificando l’effetto della terapia. Abbiamo, perciò, sviluppato un nuovo approccio molecolare all’eradicazione: sfruttare l’enzima virale integrasi per riconoscere in modo selettivo le sequenze LTR virali per colpire il virus integrato. Fondendo integrasi e l’endonucleasi FokI, abbiamo generato diversi cloni. Questi sono stati transfettati stabilmente in cellule Jurkat, suscettibili all’infezione. Una volta infettate, abbiamo ottenuto una significativa riduzione dei markers di infezione. Successivamente la transfezione nella linea linfoblastica 8E5/LAV, che porta integrata nel genoma una copia di HIV, ha dato risultati molto incoraggianti, come la forte riduzione del DNA virale integrato.

Isolated cerebral folate deficiency was detected in a 13-year-old girl with cognitive and motor difficulties and juvenile rheumatoid arthritis. Her serum contains autoantibodies that block membrane-bound folate receptors that are on the choroid plexus and diminish the uptake of folate into the spinal fluid. Whereas her serum folate exceeded 21 ng/mL, her spinal fluid contained 3.2 ng/mL of 5-methyltetrahydrofolate as a consequence of the autoantibodies diminishing the uptake of this folate.

Extracellular matrix (ECM) is a component of a variety of organisms that provides both structural support and influence upon the cells it surrounds. The importance of the ECM is becoming more apparent as matrix defects are linked to human disease. In this study, the large, extracellular matrix heparan sulfate proteoglycan, perlecan (Pln) is examined in two systems. First, the role of Pln in the interaction between a blastocyst and uterine epithelial cells is investigated. In mice, blastocyst attachment and implantation occurs at approximately d 4.5 post coitus. In addition, a delayed implantation model has been used to distinguish between the response of the blastocyst to that of hatching and of becoming attachment competent. ^ The second series of experiments described in this study focuses on the process of chondrogenesis in mice. Pln, commonly expressed with other basement membrane (BM) proteins, was found to be expressed in cartilaginous tissue without other BM proteins. This unusual expression pattern led to further study and the development of an in vitro chondrogenesis assay using the mouse embryonic fibroblast cell line, C3H/10T1/2. When cultured on Pln in vitro, these cells form aggregates and express the cartilage proteins, collagen type II and aggrecan. In examining the participation of the heparan sulfate (HS) chains in this process, the proteoglycan was enzymatically digested to remove the HS chains before the initiation of 10T1/2 cell culture. After digestion, the ability of Pln to stimulate aggregate formation was greatly diminished. Thus, the HS chains participate in the cell induction process. To determine which domain of Pln might be responsible for this activity, recombinant fragments of Pin were used in the cell culture assay. Of all recombinant protein fragments tested, only the domain including the HS chains, domain 1, was able to initiate the morphological change exhibited by the 10T1/2 cells. Similar to native Pln, when HS chains were removed from domain I, chondrogenic activity was abolished. A variant of domain I carrying both HS and chondroitin sulfate (CS) chains retained activity when only HS chains were removed. When both HS and CS chains were removed, then activity was lost. ^ The ability to rapidly stimulate differentiation of 10T1/2 cells in vitro may lead to better control of chondrogenesis in vitro and in vivo, providing better understanding and manipulation of the chondrogenic process. This greater understanding may have benefits for study of cartilage and bone diseases and subsequent treatment options. (Abstract shortened by UMI.)^

Neste trabalho é proposto um modelo mecanobiológico de remodelagem óssea para a estimativa de variações, provocadas por perturbações mecânicas ou biológicas, na matriz de rigidez estrutural da escala macroscópica e na densidade mineral em uma região do osso. Na cooperação entre as áreas da saúde e da engenharia, como nos estudos estruturais de biomecânica no sistema esquelético, as propriedades mecânicas dos materiais devem ser conhecidas, entretanto os ossos possuem uma constituição material altamente complexa, dinâmica e variante entre indivíduos. Sua dinâmica decorre dos ciclos de absorção e deposição de matriz óssea na remodelagem óssea, a qual ocorre para manter a integridade estrutural do esqueleto e adaptá-lo aos estímulos do ambiente, sejam eles biológicos, químicos ou mecânicos. Como a remodelagem óssea pode provocar alterações no material do osso, espera-se que suas propriedades mecânicas também sejam alteradas. Na literatura científica há modelos matemáticos que preveem a variação da matriz de rigidez estrutural a partir do estímulo mecânico, porém somente os modelos mais recentes incluíram explicitamente processos biológicos e químicos da remodelagem óssea. A densidade mineral óssea é um importante parâmetro utilizado no diagnóstico de doenças ósseas na área médica. Desse modo, para a obtenção da variação da rigidez estrutural e da densidade mineral óssea, propõe-se um modelo numérico mecanobiológico composto por cinco submodelos: da dinâmica da população de células ósseas, da resposta das células ao estímulo mecânico, da porosidade óssea, da densidade mineral óssea e, baseado na Lei de Voigt para materiais compósitos, da rigidez estrutural. Os valores das constantes das equações dos submodelos foram obtidos de literatura. Para a solução das equações do modelo, propõe-se uma implementação numérica e computacional escrita em linguagem C. O método de Runge-Kutta-Dorman-Prince, cuja vantagem consiste no uso de um passo de solução variável, é utilizado no modelo para controlar o erro numérico do resultado do sistema de equações diferenciais. Foi realizada uma avaliação comparativa entre os resultados obtidos com o modelo proposto e os da literatura dos modelos de remodelagem óssea recentes. Conclui-se que o modelo e a implementação propostos são capazes de obter variações da matriz de rigidez estrutural macroscópica e da densidade mineral óssea decorrentes da perturbação nos parâmetros mecânicos ou biológicos do processo de remodelagem óssea.

Objetivos: 1) Proponer una nueva clasificación de los oligoelementos fundamentada en el estudio detallado de las investigaciones más recientes sobre los mismos; 2) ofrecer información detallada y actualizada sobre todos los oligoelementos. Resultados: el análisis de todos los resultados de investigación consultados pone de manifiesto que los avances en las técnicas de análisis molecular permiten dilucidar la importancia que presentan ciertos oligoelementos para la salud humana. Se ofrece un análisis detallado de la función catalítica que podrían tener determinados elementos no considerados hasta ahora como esenciales o posiblemente esenciales, gracias al uso de plataformas informáticas que permiten el análisis integrado de datos sobre enzimas. Asimismo se presenta información integrada y actualizada del papel fisiológico, cinéticas y metabolismo, fuentes dietéticas y factores que propician la carencia o la toxicidad de cada uno de los oligoelementos. Conclusiones: La Oligoterapia plantea el uso de oligoelementos catalíticamente activos con fines terapéuticos. La nueva clasificación de oligoelementos planteada en este trabajo será de interés para diversos sectores profesionales: médicos y demás personal sanitario, nutricionistas, farmacéuticos, etc. Así podrán diseñarse nuevas estrategias terapéuticas que permitan paliar la sintomatología de diversas patologías, en particular las enfermedades carenciales y metabólicas.

Context: There is evidence suggesting that the prevalence of disability in late life has declined over time while the prevalence of disabling chronic diseases has increased. The dynamic equilibrium of morbidity hypothesis suggests that these seemingly contradictory trends are due to the attenuation of the morbidity-disability link over time. The aim of this study was to empirically test this assumption.Methods: Data were drawn from three repeated cross-sections of SWEOLD, a population-based survey among the Swedish men and women ages 77 and older. Logistic regression models were fitted to assess the trends in the prevalence of Activities of Daily Living (ADL) disability, Instrumental ADL (IADL) disability, and selected groups of chronic conditions. The changes in the associations between chronic conditions and disabilities were examined in both multiplicative and additive models.Results: Between 1992 and 2011, the odds of ADL disability significantly declined among women whereas the odds of IADL disability significantly declined among men. During the same period, the prevalence of most chronic morbidities including multimorbidity went up. Significant attenuations of the morbidity-disability associations were found for cardiovascular diseases, metabolic disorders, poor lung function, psychological distress, and multimorbidity.Conclusion: In agreement with the dynamic equilibrium hypothesis, this study concludes that the associations between chronic conditions and disability among the Swedish older adults have largely waned over time.

The regulation of osteoclast differentiation in the bone microenvironment is critical for normal bone remodeling, as well as for various human bone diseases. Over the last decade, our knowledge of how osteoclast differentiation occurs has progressed rapidly. We highlight some of the major advances in understanding how cell signaling and transcription are integrated to direct the differentiation of this cell type. These studies used genetic, molecular, and biochemical approaches. Additionally, we summarize data obtained from studies of osteoclast differentiation that used the functional genomic approach of global gene profiling applied to osteoclast differentiation. This genomic data confirms results from studies using the classical experimental approaches and also may suggest new modes by which osteoclast differentiation and function can be modulated. Two conclusions that emerge are that osteoclast differentiation depends on a combination of fairly ubiquitously expressed transcription factors rather than unique osteoclast factors, and that the overlay of cell signaling pathways on this set of transcription factors provides a powerful mechanism to fine tune the differentiation program in response to the local bone microenvironment.

Abstract: The first metatarsal sesamoid bones are not always taken into consideration when making a diagnosis, in pathologies that affect the region of the first metatarsal head. This is due to the insufficient knowledge of all the pathologies that can affect the sesamoids and the relative little incidence that they have. With the increment of sports activities, in particular the running, increasingly affects of the symptoms concerning this region are observed. Methods: A literature search was performed in 5 databases (Medline, PubMed, Scopus, Cochrane Library and BUCEA). The terms included in the search were: sesamoids, anatomy, biomechanics, sesamoids review and sesamoids pathology. In the initial search articles with no more than 10 years, only humans and revision texts are considered. Results: 24 articles were selected and include different pathologies with diagnosis using imaging tests and treatments, both conservative and surgical; as well as aspects from the biomechanics of the metatarsal-sesamoid joint. Conclusion: Sesamoids due of his anatomy, topography and function can be involved in a lot of pathologies; with similar signs and symptoms that can confuse the podiatry when he has to make a correct diagnosis or treatment.
