With the advent of a global economy, Lean Six Sigma meets the demand of new production process that creates products within the customer’s specification without any wastage. The objectives of this work is to use the concepts Lean Manufacturing and Six Sigma parallel to minimize costs, increase the customer base, reduce the delivery time which implies an increase in the turnover of products on the shelves in a packaging company. We used the DMAIC method to continue working. The method DMAIC indicated the main failure modes that were fixed in the step improve. At this stage the tools of Lean Manufacturing gain tremendous value by optimizing processes with defects. The conclusion involves an increase in monthly billing, a corporate restructuring and, finally, a renewal in how to conduct the work of management

Solar heaters are an appropriate technology in tropical and sub-tropical climates to heat bath water by solar energy. Low-cost solar heaters meet the demand of low-income rural communities which currently do not have access to this technology. Current research analyzes the economic viability of solar heaters, built with recyclable materials, to reduce electric energy bill. A solar heating system was built consisting of recyclable materials in accordance with the manuals provided by the Secretariat of Environment of the state of Paraná (SEMA). Duration of use of electric showers by families of rural properties was determined to calculate expenses and billing of electricity. Simulation and material costs showed that the system was feasible. Commercial solar heaters could be replaced at a cost of R$ 22.61 per month during 13 months.

La tesi tratta di strumenti finalizzati alla valutazione dello stato conservativo e di supporto all'attività di manutenzione dei ponti, dai più generali Bridge Management Systems ai Sistemi di Valutazione Numerica della Condizione strutturale. Viene proposto uno strumento originale con cui classificare i ponti attraverso un Indice di Valutazione Complessiva e grazie ad esso stabilire le priorità d'intervento. Si tara lo strumento sul caso pratico di alcuni ponti della Provincia di Bologna. Su un ponte in particolare viene realizzato un approfondimento specifico sulla determinazione approssimata dei periodi propri delle strutture da ponte. Si effettua un confronto dei risultati di alcune modellazioni semplificate in riferimento a modellazioni dettagliate e risultati sperimentali.

L'imposta sul valore aggiunto è caratterizzata da un meccanismo applicativo complesso preordinato al raggiungimento della neutralità in cui rivestono fondamentale rilevanza particolari obblighi strumentali richiesti al contribuente (fatturazione, registrazioni contabili, dichiarazioni). Il presente progetto di ricerca è finalizzato a valutare la propensione e funzionalità del meccanismo applicativo rispetto al generale principio di neutralità dell’imposta e la valenza svolta dagli obblighi strumentali rispetto alla qualificazione della fattispecie impositiva.

For many services, consumers can choose among a range of optional tariffs that differ in their access and usage prices. Recent studies indicate that tariff-specific preferences may lead consumers to choose a tariff that does not minimize their expected billing rate. This study analyzes how tariff-specific preferences influence the responsiveness of consumers’ usage and tariff choice to changes in price. We show that consumer heterogeneity in tariff-specific preferences leads to heterogeneity in their sensitivity to price changes. Specifically, consumers with tariff-specific preferences are less sensitive to price increases of their preferred tariff than other consumers. Our results provide an additional reason why firms should offer multiple tariffs rather than a uniform nonlinear pricing plan to extract maximum consumer surplus.

The Mobile Cloud Networking project develops among others, several virtualized services and applications, in particular: (1) IP Multimedia Subsystem as a Service that gives the possibility to deploy a virtualized and on-demand instance of the IP Multimedia Subsystem platform, (2) Digital Signage Service as a Service that is based on a re-designed Digital Signage Service architecture, adopting the cloud computing principles, and (3) Information Centric Networking/Content Delivery Network as a Service that is used for distributing, caching and migrating content from other services. Possible designs for these virtualized services and applications have been identified and are being implemented. In particular, the architectures of the mentioned services were specified, adopting cloud computing principles, such as infrastructure sharing, elasticity, on-demand and pay-as-you-go. The benefits of Reactive Programming paradigm are presented in the context of Interactive Cloudified Digital Signage services in a Mobile Cloud Platform, as well as the benefit of interworking between different Mobile Cloud Networking Services as Digital Signage Service and Content Delivery Network Service for better performance of Video on Demand content deliver. Finally, the management of Service Level Agreements and the support of rating, charging and billing has also been considered and defined.

PURPOSE To quantify the coinciding improvement in the clinical diagnosis of sepsis, its documentation in the electronic health records, and subsequent medical coding of sepsis for billing purposes in recent years. METHODS We examined 98,267 hospitalizations in 66,208 patients who met systemic inflammatory response syndrome criteria at a tertiary care center from 2008 to 2012. We used g-computation to estimate the causal effect of the year of hospitalization on receiving an International Classification of Diseases, Ninth Revision, Clinical Modification discharge diagnosis code for sepsis by estimating changes in the probability of getting diagnosed and coded for sepsis during the study period. RESULTS When adjusted for demographics, Charlson-Deyo comorbidity index, blood culture frequency per hospitalization, and intensive care unit admission, the causal risk difference for receiving a discharge code for sepsis per 100 hospitalizations with systemic inflammatory response syndrome, had the hospitalization occurred in 2012, was estimated to be 3.9% (95% confidence interval [CI], 3.8%-4.0%), 3.4% (95% CI, 3.3%-3.5%), 2.2% (95% CI, 2.1%-2.3%), and 0.9% (95% CI, 0.8%-1.1%) from 2008 to 2011, respectively. CONCLUSIONS Patients with similar characteristics and risk factors had a higher of probability of getting diagnosed, documented, and coded for sepsis in 2012 than in previous years, which contributed to an apparent increase in sepsis incidence.

Unlike infections occurring during periods of chemotherapy-induced neutropenia, postoperative infections in patients with solid malignancy remain largely understudied. The purpose of this population-based study was to evaluate the clinical and economic burden, as well as the relationship of hospital surgical volume and outcomes associated with serious postoperative infection (SPI) – i.e., bacteremia/sepsis, pneumonia, and wound infection – following resection of common solid tumors.^ From the Texas Discharge Data Research File, we identified all Texas residents who underwent resection of cancer of the lung, esophagus, stomach, pancreas, colon, or rectum between 2002 and 2006. From their billing records, we identified ICD-9 codes indicating SPI and also subsequent SPI-related readmissions occurring within 30 days of surgery. Random-effects logistic regression was used to calculate the impact of SPI on mortality, as well as the association between surgical volume and SPI, adjusting for case-mix, hospital characteristics, and clustering of multiple surgical admissions within the same patient and patients within the same hospital. Excess bed days and costs were calculated by subtracting values for patients without infections from those with infections computed using multilevel mixed-effects generalized linear model by fitting a gamma distribution to the data using log link.^ Serious postoperative infection occurred following 9.4% of the 37,582 eligible tumor resections and was independently associated with an 11-fold increase in the odds of in-hospital mortality (95% Confidence Interval [95% CI], 6.7-18.5, P < 0.001). Patients with SPI required 6.3 additional hospital days (95% CI, 6.1 - 6.5) at an incremental cost of $16,396 (95% CI, $15,927–$16,875). There was a significant trend toward lower overall rates of SPI with higher surgical volume (P=0.037). ^ Due to the substantial morbidity, mortality, and excess costs associated with SPI following solid tumor resections and given that, under current reimbursement practices, most of this heavy burden is borne by acute care providers, it is imperative for hospitals to identify more effective prophylactic measures, so that these potentially preventable infections and their associated expenditures can be averted. Additional volume-outcomes research is also needed to identify infection prevention processes that can be transferred from higher- to lower-volume providers.^

Few studies have explored factors related to participation in cancer chemoprevention trials. The purpose of this dissertation was to conduct investigations in this emerging field by studying aspects of participation at three phases of cancer chemoprevention trials: at enrollment, during a placebo run-in period, and post-trial. In all three studies, subjects had a history of cancer and were at high risk of recurrence or second primary tumors.^ The first study explored correlates of enrollment in a head and neck cancer chemoprevention trial by comparing participants and eligible nonparticipants. Of 148 subjects who met the trial's preliminary eligibility criteria, 40% enrolled. In multivariate analysis, enrollment was positively associated with being male (OR 2.36) and being employed (OR 2.73). The most commonly cited reason for declining participation among nonparticipants was transportation.^ The second study examined outcomes of an eight-week placebo run-in period in a head and neck cancer chemoprevention trial. Of 391 subjects, 91.3% were randomized after the run-in. Adherence to drug capsules ranged from 0% to 120.3% (mean $\pm$ SD, 95.8% $\pm$ 15.1). In multivariate analysis, the main variable predicting run-in outcome was race; white subjects were 3.45 times more likely to be randomized than non-white subjects. Subjects with Karnofsky scores of 100 were 2.13 times more likely to be randomized than were subjects with lower scores.^ The third study used post-trial questionnaires to assess subjects' (n = 64) perceptions of participation in a cancer chemoprevention trial. The most highly rated trial benefit was the perception of potential colon cancer prevention, and the most troublesome barrier was erroneous billing for study visits. Perceived benefits were positively associated with interest in participating in future trials of the same (p = 0.05) and longer (p = 0.02) duration, and difficulty with trial pills and procedures was inversely related to interest in future placebo-controlled trials (p = 0.01).^ These are among the first behavioral studies to be completed in the rapidly growing field of cancer chemoprevention. Much work has yet to be done, however, to advance our understanding of the complex issues relating to chemoprevention trial participation. ^

Today P2P faces two important challenges: design of mechanisms to encourage users’ collaboration in multimedia live streaming services; design of reliable algorithms with QoS provision, to encourage multimedia providers employ the P2P topology in commercial streaming services. We believe that these two challenges are tightly-related and there is much to be done with respect. This paper proposes a novel monetary incentive for P2P multimedia streaming. The incentive model classifies the users in groups according to the perceived video quality. We apply the model to a streaming system’s billing model in order to evaluate its feasibility and visualize its quantitative effect on the users’ motivation and the provider’s profit. We conclude that monetary incentive can boost up users’ cooperation, loyalty and enhance the overall system integrity and performance. Moreover the model defines the constraints for the provider’s cost and profit when the system is leveraged on the cloud. Considering those constraints, a multimedia content provider can adapt the billing model of his streaming service and achieve desirable discount-profit trade-off. This will moreover contribute to better promotion of the service, across the users on the Internet.

In this paper, a model (called the elliptic model) is proposed to estimate the number of social ties between two locations using population data in a similar manner to how transportation research deals with trips. To overcome the asymmetry of transportation models, the new model considers that the number of relationships between two locations is inversely proportional to the population in the ellipse whose foci are in these two locations. The elliptic model is evaluated by considering the anonymous communications patterns of 25 million users from three different countries, where a location has been assigned to each user based on their most used phone tower or billing zip code. With this information, spatial social networks are built at three levels of resolution: tower, city and region for each of the three countries. The elliptic model achieves a similar performance when predicting communication fluxes as transportation models do when predicting trips. This shows that human relationships are influenced at least as much by geography as is human mobility.

Mediante la elaboración de esta tesis doctoral se pretende diseñar un sistema para la realización del plan de autoprotección en industrias con riesgo de incendio dentro de un polígono industrial. Para desarrollar esta línea de investigación se realizará una investigación cartográfica así como un reconocimiento visual de las distintas empresas industriales, de actividades heterogéneas, seleccionadas por su emplazamiento y condiciones. Este tipo de industria es esencial en el sistema económico nacional, ya que dota de recursos y servicios al tejido social, además de tener una relación directa con la generación de riqueza y por tanto incide de forma positiva en el empleo de cualquier región. Estas razones y otras son determinantes para fomentar el progreso, si bien nunca a expensas de la seguridad de los empleados que hacen posible la línea de producción así como de los posibles viandantes que transcurran por la zona de impacto en caso de incidentes como por ejemplo un incendio que afecte a cualquiera de los locales y/o empresas que se emplazen en los denominados polígonos industriales. La tesis incluye trabajos experimentales de los que se extraen recomendaciones y conclusiones encaminadas a la optimización de la instrumentalización utilizada, las técnicas de observación, diseño y cálculo necesarias que determinarán el acercamiento al método propuesto para nuestra valoración del riesgo por incendio. Las posibles emergencias que pueden darse a nivel de industrias localizadas en polígonos industriales son diversas y numerosas. De todas ellas se elige abordar el caso de “incendio” por su casuística más numerosa. No obstante, la planificación orientada desde el prisma de la investigación de esta tesis puede estenderse a cualquier otro riesgo, lo que se aportará como líneas futuras de investigación. Las aproximaciones y etapas de los trabajos que forman parte de esta investigación se han integrado en cada capítulo y son las siguientes: En primer lugar un capítulo de Introducción, en el que se realiza una reflexión justificada de la elección del tema tratado, se formula la hipótesis de partida y se enumeran los distintos objetivos que se pretenden alcanzar. En el segundo capítulo, titulado Aspectos de la Protección Civil en la Constitución Española de 1978, se ha realizado un estudio exhaustivo de la Constitución en todo lo relacionado con el ámbito de la protección, haciendo especial énfasis en las distintas competencias según el tipo de administración que corresponda en cada caso, así como las obligaciones y deberes que corresponden a los ciudadanos en caso de catástrofe. Así mismo se analizan especialmente los casos de Guerra y el estado de Alarma. En el tercer capítulo, titulado Repuesta de las administraciones públicas ante emergencias colectivas, se trata de formular el adecuado sistema de repuesta que sería preciso para tratar de mitigar desastres y catástrofes. Se analizan los distintos sistemas de gestión de emergencias constatando en cada caso los pros y los contras de cada uno. Se pretende con ello servir de ayuda en la toma de decisiones de manera coherente y racional. El capítulo cuarto, denominado Planes Locales de emergencia. Estudio en las distintas administraciones, se ha pormenorizado en la presentación así como puesta en funcionamiento de los planes, comparando los datos obtenidos entre ellos para concluir en un punto informativo que nos lleva a la realidad de la planificación. Para la realización y desarrollo de los capítulos 5 y 6, llamados Análisis de la Norma Básica de Autoprotección y Métodos de evaluación del riesgo de Incendio, se estudia la normativa actual en autoprotección prestando atención a los antecedentes históricos así como a todas las figuras que intervienen en la misma. Se analizan los distintos métodos actuales para la valoración del riesgo por incendio en industrias. Este aspecto es de carácter imprescindible en la protección civil de los ciudadanos pero también es de especial importancia en las compañías aseguradoras. En el capítulo 7, Propuesta metodológica, se propone y justifica la necesidad de establecer una metodología de estudio para estos casos de riesgo por incendio en industrias para así, acortar el tiempo de respuesta de los servicios de emergencia hasta la zona, así como dotar de información imprescindible sobre el riesgo a trabajadores y transeúntes. El último capítulo se refiere a las Conclusiones, donde se establecen y enuncian una serie de conclusiones y resultados como consecuencia de la investigación desarrollada, para finalizar esta tesis doctoral enunciando posibles desarrollos y líneas de investigación futuros. ABSTRACT The development of this thesis is to design a system for the implementation of the plan of self-protection in industries with risk of fire in an industrial park. To develop this line of research will be done cartographic research as well as visual recognition of the distinct and heterogeneous industrial companies selected by its location and conditions. This type of industry is an essential part in the economic national system providing economic resources to society as well as with a direct relationship in unemployment. For this reason it is crucial to promote their progress, but never at the expense of the security of the employees that make the line of production as well as possible walkers that pass by the area of impact in the event of a fire affecting the company. The thesis includes experimental works which are extracted recommendations and conclusions aimed at optimization of used exploit, techniques of observation, design and calculation needed to determine the approach to the method proposed for our assessment of the risk from fire. The approaches and stages of works that are part of this research have been integrated into each chapter and are as follows: In the first chapter, holder introduction, perform a supporting reflection of the choice of the subject matter, is formulated the hypothesis of departure and listed the different objectives that are intended to achieve. In the second chapter, holder aspects of Civil Protection in the Spanish Constitution of 1978, examines an exhaustive study of the Constitution in everything related to the scope of protection. With an emphasis on individual skills according to the type of management with corresponding in each case, as well as the obligations and duties which correspond to citizens in the event of a catastrophe. Also analyzes the particular cases of war and the State of alarm. In the third chapter, holder public administrations collective emergency response, discussed trafficking in defining the proper system response that would be precise to address disasters and catastrophes. We discusses the different systems of emergency management in each case, we pretend the pros and cons of each. We tried that this serve as decision-making aid coherent and rational way. The fourth chapter is holder Local Emergency Plans (LEP). Study on the different administrations, has detailed in the presentation as well as operation of the LEP, comparing the data between them to conclude in an information point that leads us to the reality of planning. For the realization and development of chapters 5 and 6, holder Analysis of the basic rule of self-protection and fire risk assessment methods, paying attention to the historical background as well as all the figures involved in the same studies with the current rules of self-protection. The current methods for the estimation of the risk are analyzed by fire in industries. This aspect is essential in the civil protection of the citizens, but it is also of special importance for insurance companies. The seventh chapter, holder Methodological proposal, we propose and justifies the need to establish a methodology for these cases of risk by fire in industries. That shorts the response time of emergency services to the area, and provides essential information about the risk to workers and walkers. The last chapter refers to the Conclusions, laying down a series of results as a consequence of the previous chapters to complete billing possible developments and research future.

Parchment hardcover bound volume containing quarter bill tallies for the Classes of 1757-1773 arranged first by seniority, and later alphabetically, and covering the bill period ending on March 11, 1759 through the period ending December 8, 1769. Billing categories are occasionally added or removed in the volume, including a Hebrew Grammar category in 1764, and one for the Hancock Professor in 1765. After each quarter's tallies, an additional section provides the totals for all students in each of the categories, and deductions for building repairs.

This one-and-a-half page handwritten draft of a report on a folio-sized leaf outlines a six-point process for drafting and authorizing student quarter bills. The procedures consolidate the billing areas, detail the Treasurer's responsibilities, and expand the oversight of the President and Tutors. The report, dated December 12, 1778, is signed by President Samuel Langdon. The draft includes two differently worded versions of the sixth proposal, one of which is struck through.
