We examined the relations between selection for perception and selection for action in a patient FK, with bilateral damage to his temporal and medial frontal cortices. The task required a simple grasp response to a common object (a cup) in the presence of a distractor (another cup). The target was cued by colour or location, and FK made manual responses. We examined the effects on performance of cued and uncued dimensions of both the target and the distractor. FK was impaired at perceptually selecting the target when cued by colour, when the target colour but not its location changed on successive trials. The effect was sensitive to the relative orientations of targets and distractors, indicating an effect of action selection on perceptual selection, when perceptual selection was weakly instantiated. The dimension-specific carry-over effect on reaching was enhanced when there was a temporal delay between a cue and the response, and it disappeared when there was a between-trial delay. The results indicate that perceptual and action selection systems interact to determine the efficiency with which actions are selected to particular objects.

Many studies have assessed the neural underpinnings of creativity, failing to find a clear anatomical localization. We aimed to provide evidence for a multi-componential neural system for creativity. We applied a general activation likelihood estimation (ALE) meta-analysis to 45 fMRI studies. Three individual ALE analyses were performed to assess creativity in different cognitive domains (Musical, Verbal, and Visuo-spatial). The general ALE revealed that creativity relies on clusters of activations in the bilateral occipital, parietal, frontal, and temporal lobes. The individual ALE revealed different maximal activation in different domains. Musical creativity yields activations in the bilateral medial frontal gyrus, in the left cingulate gyrus, middle frontal gyrus, and inferior parietal lobule and in the right postcentral and fusiform gyri. Verbal creativity yields activations mainly located in the left hemisphere, in the prefrontal cortex, middle and superior temporal gyri, inferior parietal lobule, postcentral and supramarginal gyri, middle occipital gyrus, and insula. The right inferior frontal gyrus and the lingual gyrus were also activated. Visuo-spatial creativity activates the right middle and inferior frontal gyri, the bilateral thalamus and the left precentral gyrus. This evidence suggests that creativity relies on multi-componential neural networks and that different creativity domains depend on different brain regions.

Impulsivity is a personality trait exhibited by healthy individuals, but excessive impulsivity is associated with some mental disorders. Lesion and functional, neuroimaging Studies indicate that the ventromedial prefrontal region (VMPFC), including the orbitofrontal cortex (OFC), anterior cingulate cortex (ACC) and medial prefrontal cortex, and the amygdala may modulate impulsivity and aggression. However, no morphometric study has examined the association between VMPFC and impulsivity. We hypothesized that healthy subjects with high impulsivity would have smaller volumes in these brain regions compared with those with low impulsivity. Sixty-two healthy Subjects were Studied (age 35.4 +/- 12.1 years) using a 1.5-T MRI system. The Barratt impulsiveness scale (BIS) was used to assess impulsivity. Images were processed using an optimized voxel-based morphometry (VBM) protocol. We calculated the correlations between BIS scale scores and the gray matter (GM) and white matter (WM) volumes of VMPFC and amygdala. GM volumes of the left and right OFC were inversely correlated with the BIS total score (P = 0.04 and 0.02, respectively). Left ACC GM Volumes had a tendency to be inversely correlated with the BIS total score (P = 0.05. Right OFC GM Volumes were inversely correlated with BIS nonplanning impulsivity, and left OFC GM volumes were inversely correlated with motor impulsivity. There were no significant WM volume correlations with impulsivity. The results Of this morphometry Study indicate that small OFC volume relate to high impulsivity and extend the prior finding that the VMPFC is involved in the circuit modulating impulsivity. arginine-vasopressin in the parvocellular neurons of the hypothalamic paraventricular nucleus is known to play an important role in the control of the hypothalamo-pituitary-adrenal axis. In the present study, we verify plasma corticosterone levels, the distribution of glucocorticoid receptor- and arginine-vasopressin-positive neurons, and the co-localization of both glucocorticoid receptors and arginine-vasopressin in neurons in the anterior and medial parvocellular subdivisions of the paraventricular nucleus after manipulations of the hypothalamus-pituitary-adrenal axis. Normal, sham surgery, and adrenalectomized male rats were subjected to intraperitoneal injections of saline or dexamethasone to measure plasma corticosterone levels by a radioimmunoassay. We also examined arginine-vasopressin and glucocorticoid receptor immunofluorescence in sections from the paraventricular nucleus. Our results showed that the immunoreactivity of arginine-vasopressin neurons increased in the anterior parvocellular subdivision and decreased in the medial parvocellular subdivision from adrenalectomized rats treated with dexamethasone. On the other hand, we showed that the immunoreactivity of glucocorticoid receptors increased in the anterior and medial parvocellular subdivisions of these same animals. However, the immunoreactivity of glucocorticoid receptors is higher in the medial parvocellular than anterior parvocellular subdivision. The co-localization of arginine-vasopressin and glucocorticoid receptors was found only in the medial parvocellular subdivision. These findings indicate that glucocorticoids have direct actions on arginine-vasopressin-positive neurons in the medial parvocellular but not anterior parvocellular subdivision. There is a differentiated pattern of arginine-vasopressin-positive neuron expression between the anterior and medial parvocellular subdivisions. (C) 2010 Elsevier Inc. All rights reserved.

O conhecimento da localização da artéria etmoidal anterior (AEA) constitui etapa importante na cirurgia do recesso do seio frontal e do etmóide anterior. A tomografia computadorizada (TC), em especial no plano coronal pode fornecer reparos anatômicos que identificam o trajeto da AEA. OBJETIVO: Identificar os reparos anatômicos que caracterizam o trajeto da AEA na parede medial da órbita e na parede lateral da fossa olfatória. Verificar a correlação entre a presença de pneumatização supra-orbitária e a visualização do etmoidal anterior (canal da AEA). CASUÍSTICA E MÉTODOS: Estudo retrospectivo de 198 tomografias computadorizadas dos seios paranasais no plano coronal do período de agosto a dezembro de 2006. RESULTADOS: Pneumatização supra-orbitária foi identificada em 35% (70 exames). O canal da AEA foi caracterizado em 41% (81 exames). O sulco etmoidal anterior foi visualizado em 98% (194 dos exames) e o forame etmoidal anterior foi identificado em todos os exames (100%). CONCLUSÃO: O forame etmoidal anterior e o sulco etmoidal anterior foram referências anatômicas presentes em quase 100% dos exames avaliados. Houve correlação entre a presença de pneumatização supra-orbitária e a caracterização do canal da AEA.

O presente estudo tem por objectivo principal relatar o caso duma atleta de esqui na montanha com 18 anos de idade, com uma lesão do Ligamento Cruzado Anterior (LCA), lesão do menisco medial e fractura do prato tibial medial que foi submetida a tratamento cirúrgico, realizando tratamento de fisioterapia no pré e pós-operatório. O programa de reabilitação tinha como objectivo reduzir o edema e conquistar a amplitude de movimento livre de dor assim como reestabelecer os níveis de controlo neuromuscular prévios à lesão. A atleta passou por cinco reavaliações durante seu o período de reabilitação e como parâmetros utilizaram-se as escalas de funcionalidade de Cinccinatti e Lisholm, a perimetria da coxa, os graus de amplitude de movimento e a sua força muscular pelo método de 1RM. Os resultados foram os esperados para cada fase da sua reabilitação. No pré-operatório a atleta apresentava um déficit na força muscular do membro inferior lesionado, principalmente no quadricípite ( 3+ inicialmente e 4 na reavaliação antes da cirurgia), discrepância na perimetria muscular da coxa (-2,5 centímetros em relação ao membro contralateral medidos a 5 centímetros acima do bordo superior da patela, e -1,5 centímetros após o programa de reabilitação pré-operatória). No pós-operatório, a força muscular do quadricípite era de 3 inicialmente e de 5 no final do programa de reabilitação, a diferença de 2,5 centímetros de perimetria em relação ao membro contralateral inicialmente encontrada no pós-operatório foi reduzida a uma mínima diferença na sua última avaliação. Quanto à funcionalidade, a atleta inicialmente apresentava valores de 120 e 61 nas escalas de Cinccinatti e Lysholm e no final do programa de reabilitação de 320 e 95, respectivamente. Por fim, conclui-se que a atleta no final de quatro meses, apresentou melhorias sensíveis no seu quadro e muito provavelmente retornará às competições de forma integral.

Today, reconstructive arthroscopic knee surgery of the anterior cruciate ligament is a widely used technique. In addition to the usual complications of arthroscopic techniques, the extraction of the piece must be taken into consideration, with vascular complications being the rarest and most serious of them all. We present the case of a patient who suffered an arterial pseudoaneurysm during this procedure. The management and treatment are discussed and the current bibliography reviewed.

A memória pode ser definida como o armazenamento e a evocação de uma informação aprendida. Sendo um processo dinâmico, requer a ativação de diversos sistemas para que a informação adquirida seja consolidada. O ATP é um importante neurotransmissor que, após a sua liberação e conseqüente ativação de seus receptores específicos, precisa ser inativado. Esta remoção do ATP da fenda sináptica ocorre através de sua hidrólise promovida pelas ectonucleotidases. Como um dos produtos da hidrólise do ATP, a adenosina é considerada um potente neuromodulador e exerce suas ações através de receptores específicos com ações excitatórias (A2A e A2B ) ou inibitórias (A1 e A3). Vários trabalhos já demonstraram a participação do sistema purinérgico nos processos relacionados a memória da tarefa da esquiva inibitória. Estudos realizados em nosso laboratório mostraram a participação das ectonucleotidases sinaptossomais de hipocampo, córtex entorrinal e córtex parietal na consolidação da memória para esquiva inibitória. No entanto, existem poucos estudos a respeito do sistema purinérgico nos mecanismos de consolidação da memória em outras regiões cerebrais, tais como o córtex cingulado anterior (CA) e posterior (CP) e a área pré-central medial (FR2). Portanto, na primeira parte deste estudo, avaliamos as atividades ectonucleotidásicas em sinaptossomas de CA, CP e FR2 de ratos submetidos à tarefa de esquiva inibitória. Nossos resultados demonstraram um aumento na hidrólise de ATP em sinaptossomas de FR2 e CA; e na hidrólise de ADP em sinaptossomas de FR2 e CP. Este aumento na hidrólise de ATP e ADP ocorreu devido ao choque administrado na tarefa, já que o grupo de animais que não recebeu choque não apresentou alterações na hidrólise de ATP e ADP. Este efeito observado provavelmente não está associado à consolidação da memória, mas a mudanças neuroquímicas e neurohumorais induzidas pelo estresse após o choque. Além disso, foi observado um aumento relacionado ao aprendizado na atividade ATPásica e ADPásica em CP e CA, respectivamente. Estes resultados sugerem fortemente que estas enzimas participam da consolidação da memória em CP e CA. O aumento na hidrólise de ATP e ADP sugere um possível aumento na concentração de adenosina nestas regiões. Portanto, na segunda parte deste estudo, verificamos a influência de agonistas e antagonistas de receptores de adenosina nas memórias de curta (STM) e de longa (LTM) duração na tarefa de esquiva inibitória. Então, neste estudo administramos intrafusões de análogos de adenosina em CP, imediatamente após o treino em esquiva inibitória. Os resultados demonstraram que a intrafusão de DPCPX, um antagonista de receptores A1, em CP, na concentração de 50 nM, aumentou significativamente a retenção da tarefa em ambas STM e LTM. Além disso, a administração de CPA, um agonista de receptores A1 de adenosina, não alterou ambas STM e LTM nas concentrações testadas. Portanto, este estudo sugere que os receptores A1 exercem uma modulação inibitória em CP tanto na STM quanto na LTM para o aprendizado de esquiva inibitória. A regulação dos níveis de ATP e adenosina pelas ectonucleotidases em CP, CA e FR2 controlaria a ativação dos receptores A1 nestas estruturas, podendo exercer efeitos modulatórios na consolidação da memória nestas estruturas.

Based on the lack of electromyographic researches on sport and programmes of physical conditioning, we can say that it is necessary to reexamine some exercises routinely used in the programmes of physical conditioning. Thus, the trapezius and serratus anterior muscles were studied electromyographically so that we could evaluate the validity in some ways of execution of the frontal-lateral cross, dumbbells exercises for the development of these muscles. We analyzed 24 male volunteers, 18 to 25 years old, using a 2-channel TECA TE 4 electromyograph and Hewlett Packard surface electrodes. For the execution of the exercise it was used a supine bench, a straight board and two bars of 40 cm made of light wood. The results showed that TS acted preferentially in standing modality and in the inclined supine modality, however with activity levels that do not justify its inclusion in physical fitness programmes.

The trapezius and serratus anterior muscles were studied in four modalities of rowing exercises executed with two grips, middle and closed, in comparison with the four different modalities of frontal-lateral cross, dumbbells exercise. It was used 24 male volunteers, 18 to 25 years old using a 2-channel TECA TE 4 electromyograph and Hewlett Packard surface electrodes. The results showed that TS acted in a higher significant way in all the modalities of rowing than in the supine lateral raise, inclined supine lateral raise and reverse supine lateral raise, dumbbells exercises, and demonstrated no standing frontal-lateral cross, dumbbells. The SI acted more significantly difference among all the execution modalities of rowing and the inclined supine lateral raise, dumbbells exercises than in all the rowing exercises; even though the activity levels do not make us suggest them as an excellent group of exercises for the development of this muscle.

The electromyographic activity of the shoulder muscles deltoid - anterior portion (DA) and pectoralis major - clavicular portion (PMC) was tested on 24 male volunteers using a 2 channel TEC A TE4 electromyograph and Hewlett Packard surface electrodes during the execution of four different modalities of frontal-lateral cross, dumbbells exercises. The results showed that all of the tested exercises developed high levels of action potential for both muscles. So, we jusfity the indication of all of them for physical fitness programmes for DA and PMC. Some suggestions to the use of the tested exercises are presented.

The action potential level for shoulder muscles deltoid-anterior portion (DA) and pectoralis major-clavicular portion (PMC) determined by four different modalities of execution of rowing exercises, each one with two different grips, was recorded. These were compared with the action potential level determined for the same muscles by four different modalities of execution of the frontal-lateral cross, dumbbells exercises. Twenty-four male volunteers were examined using a 2 channel TECA TE4 electromyograph and Hewlett Packard surface electrodes. The statistic analysis showed significant (p<0,05) superiority for all the frontal-lateral cross, dumbbells exercises in comparison to all rowing exercises for the PMC, for the DA this generalized supremacy was not observed.

The muscles deltoid-anterior portion (DA) and pectoralis major-clavicular portion (PMC) were analysed to establish the muscular behavior and intensity patterns, as well to evaluate the connected participation of these muscles during supine and frontal elevation exercises. Twenty-four male volunteers were examined using a 2-channel TECA TE4 electromyograph and Hewlett Packard surface electrodes. Our results showed low levels of activity to PMC in frontal elevation exercises, whereas to DA the levels were very high. In the supine exercise, the action potential levels developed by the PMC were always lower than those presented by DA, however, with action simultaneity. Some suggestions to the use of the tested exercises are presented.

Inappropriate treatments of frontal sinus fractures may lead to serious complications, such as mucopyocele, meningitis, and brain abscess. Assessment of nasofrontal duct injury is crucial, and nasofrontal duct injury requires sinus obliteration, which is often accomplished by autogenous grafts such as fat, muscle, or bone. These avascular grafts have an increased risk of resorption and infection and donor site morbidity. For these reasons, pericranial flap, which is vascular, should be used for frontal sinus obliteration. The pericranial flap presented with less morbidity procedure and has decreased infection rates, which justifies its use in frontal sinus obliteration. This study aimed to report a case of a comminuted frontal sinus fracture with a brief literature review, regarding the use of pericranial flap. The authors report a case of a 23-year-old male subject with a severely comminuted fracture of the anterior and posterior walls of the frontal sinus. The patient was successfully treated by cranialization with frontal sinus duct obliteration, using anterior pericranial flap. The patient was followed up for 16 months with no postoperative complication, such as infection. Pericranial flap is a good resource for frontal sinus duct obliteration because it is a durable and well-vascularized flap, which determines low rates of postoperative complications. The orbit is an irregular conical cavity formed from 7 bones including the frontal, sphenoid, zygomatic, maxillary, ethmoid, lacrimal, and palatine bones. Fractures of the internal orbit can cause a number of problems, including diplopia, ocular muscle entrapment, and enophthalmos. Although muscle entrapment is relatively rare, diplopia and enophthalmos are relatively common sequelae of internal orbital fractures. Medial orbital wall fracture is relatively uncommon and represents a challenge for its anatomical reconstruction. In this context, autogenous bone graft has been the criterion standard to provide framework for facial skeleton and orbital walls. Therefore, it is possible to harvest grafts of varying size and contour, and the operation is performed through the bicoronal incision, which is the usual approach to major orbital reconstruction. Thus, this article aimed to describe a patient with a pure medial orbital wall fracture, and it was causing diplopia and enophthalmos. The orbital fracture was treated using autogenous bone graft from calvarial bone. The authors show a follow-up of 12 months, with facial symmetry and without diplopia and enophthalmos. In addition, a computed tomography scan shows excellent bone healing at the anterior and posterior parts of the medial orbital wall reconstruction. Copyright © 2013 by Mutaz B. Habal, MD.
