Objective: To understand the practices related to late-onset sepsis (LOS) in the centers of the Brazilian Neonatal Research Network, and to propose strategies to reduce the incidence of LOS.Methods: This was a cross-sectional descriptive multicenter study approved by the Ethics Committee. Three questionnaires regarding hand hygiene, vascular catheters, and diagnosis/treatment of LOS were sent to the coordinator of each center. The center with the lowest incidence of LOS was compared with the others.Results: All 16 centers answered the questionnaires. Regarding hand hygiene, 87% use chlorhexidine or 70% alcohol; alcohol gel is used in 100%; 80% use bedside dispensers (50% had one dispenser for every two beds); practical training occurs in 100% and theoretical training in 70% of the centers, and 37% train once a year. Catheters: 94% have a protocol, and 75% have a line insertion team. Diagnosis/treatment: complete blood count and blood culture are used in 100%, PCR in 87%, hematological scores in 75%; oxacillin and aminoglycosides is the empirical therapy in 50% of centers. Characteristics of the center with lowest incidence of LOS: stricter hand hygiene; catheter insertion and maintenance groups; use of blood culture, PCR, and hematological score for diagnosis; empirical therapy with oxacillin and aminoglycoside.Conclusion: The knowledge of the practices of each center allowed for the identification of aspects to be improved as a strategy to reduce LOS, including: alcohol gel use, hand hygiene training, implementation of catheter teams, and wise use of antibiotic therapy. Infections caused by the genus Staphylococcus are of great importance for human health. Staphylococcus species are divided into coagulase-positive staphylococci, represented by S. aureus, a pathogen that can cause infections of the skin and other organs in immunocompetent patients, and coagulase-negative staphylococci (CNS) which comprise different species normally involved in infectious processes in immunocompromised patients or patients using catheters. Oxacillin has been one of the main drugs used for the treatment of staphylococcal infections; however, a large number of S. aureus and CNS isolates of nosocomial origin are resistant to this drug. Methicillin resistance is encoded by the mecA gene which is inserted in the SCCmec cassette. This cassette is a mobile genetic element consisting of five different types and several subtypes. Oxacillin-resistant strains are detected by phenotypic and genotypic methods. Epidemiologically, methicillin-resistant S. aureus strains can be divided into five large pandemic clones, called Brazilian, Hungarian, Iberian, New York/Japan and Pediatric. The objective of the present review was to discuss aspects of resistance, epidemiology, genetics and detection of oxacillin resistance in Staphylococcus spp., since these microorganisms are increasingly more frequent in Brazil.

OBJECTIVE: Enterobacteriaceae bacteria harboring Klebsiella pneumoniae carbapenemase are a serious worldwide threat. The molecular identification of these pathogens is not routine in Brazilian hospitals, and a rapid phenotypic screening test is desirable. This study aims to evaluate the modified Hodge test as a phenotypic screening test for Klebsiella pneumoniae carbapenemase. METHOD: From April 2009 to July 2011, all Enterobacteriaceae bacteria that were not susceptible to ertapenem according to Vitek2 analysis were analyzed with the modified Hodge test. All positive isolates and a random subset of negative isolates were also assayed for the presence of blaKPC. Isolates that were positive in modified Hodge tests were sub-classified as true-positives (E. coli touched the ertapenem disk) or inconclusive (distortion of the inhibition zone of E. coli, but growth did not reach the ertapenem disk). Negative results were defined as samples with no distortion of the inhibition zone around the ertapenem disk. RESULTS: Among the 1521 isolates of Enterobacteriaceae bacteria that were not susceptible to ertapenem, 30% were positive for blaKPC, and 35% were positive according to the modified Hodge test (81% specificity). Under the proposed sub-classification, true positives showed a 98% agreement with the blaKPC results. The negative predictive value of the modified Hodge test for detection was 100%. KPC producers showed high antimicrobial resistance rates, but 90% and 77% of these isolates were susceptible to aminoglycoside and tigecycline, respectively. CONCLUSION: Standardizing the modified Hodge test interpretation may improve the specificity of KPC detection. In this study, negative test results ruled out 100% of the isolates harboring Klebsiella pneumoniae carbapenemase-2. The test may therefore be regarded as a good epidemiological tool.

Há comprovação de que o fenômeno de resistência ocorre quando a dose não lesiva da amicacina protege as células ciliadas contra a ototoxicidade da própria amicacina. OBJETIVO: O objetivo deste trabalho é verificar se o fenômeno de resistência é temporalmente persistente. MÉTODO: Estudo experimental com 14 cobaias albinas (Cavia porcellus) divididas em três grupos. Avaliação da função auditiva por emissões otoacústicas por produto de distorção (EOAPD): na pré-exposição à amicacina, no 15º dia de aplicação da dose não lesiva, no final da aplicação da dose lesiva e antes da decapitação. RESULTADOS: O Grupo A (controle) apresentou função auditiva e padrão histológico normais. No Grupo B (amicacina 20mg/kg/dia intramuscular por 30 dias e dose lesiva (400 mg/kg/dia) por 12 dias) e no Grupo C (mesmo esquema do grupo B, porém mantidos por 60 dias e sacrificados), as OEA-PD confirmaram função auditiva normal no período pré-exposição e manutenção do padrão após dose não lesiva, porém, houve perda importante da função auditiva após término do período de aplicação da dose lesiva. CONCLUSÃO: Não houve manutenção do fenômeno da autodefesa estendida por um período de 30 a 60 dias após a aplicação de doses lesivas de amicacina.

Staphylococcus aureus and Staphylococcus epidermidis are leading pathogens of implant-related infections. This study aimed at investigating the diverse distribution of different bacterial pathogen factors in most prevalent S. aureus and S. epidermidis strain types causing orthopaedic implant infections. In this study the presence both of the ica genes, encoding for biofilm exopolysaccharide production, and the insertion sequence IS256, a mobile element frequently associated to transposons, was investigated in relationship with the prevalence of antibiotic resistance among Staphylococcus epidermidis strains. The investigation was conducted on 70 clinical isolates derived from orthopaedic implant infections. Among the clinical isolates investigated a dramatic high level of association was found between the presence of ica genes as well as of IS256 and multiple resistance to all the antibiotics tested. Noteworthy, a striking full association between the presence of IS256 and resistance to gentamicin was found, being none of the IS256-negative strain resistant to this antibiotic. This association is probably because of the link of the corresponding aminoglycoside-resistance genes, and IS256, often co-existing within the same staphylococcal transposon. Moreover we investigated the prevalence of aac(6’)-Ie-aph(2’’), aph (3’) IIIa, and ant(4’) genes, encoding for the three forms of aminoglycoside-modifying enzymes (AME), responsible for resistance to aminoglycoside antibiotics. All isolates were characterized by automated ribotyping, so that the presence of antibiotic resistance determinants was investigated in strains exhibiting different ribopatterns. Interestingly, combinations of coexisting AME genes appeared to be typical of specific ribopatterns. 200 S. aureus isolates, categorized into ribogroups by automated ribotyping, i.e. rDNA restriction fragment length polymorphism analysis, were screened for the presence of a panel of adhesins genes, accessory gene regulatory (agr) polymorphisms and toxins. For many ribogroups, characteristic tandem genes arrangements could be identified. Surprisingly, the isolates of the most prevalent cluster, enlisting 27 isolates, were susceptible to almost all antibiotics and never possessed the lukD/lukE gene, thus suggesting the role of factors other than antibiotic resistance and the here investigated toxins in driving the major epidemic clone to the larger success. Afterwards, .in the predominant S. aureus cluster, the bbp gene encoding bone sialoprotein-binding protein appeared a typical virulence trait, found in 93% of the isolates. Conversely, the bbp gene was identified in just 10% of the remaining isolates of the collection. In this cluster, co-presence of bbp with the cna gene encoding collagen adhesin was a pattern consistently observed. These findings indicate a crucial role of both these adhesins, able to bind the most abundant bone proteins, in the pathogenesis of orthopaedic implant infections, there where biomaterials interface bone tissues. Moreover a PCR screening for the ebpS gene, conducted on over two hundred S. aureus clinical isolates from implant related infections revealed the detection of six strains exhibiting an altered amplicon size, shorter than expected. In order to elucidate the sequence changes present in these gene variants, the trait comprised between the primers was analyzed in all six isolates bearing the modification and in four isolates exhibiting the regular amplicon size. From nucleotide translation, the corresponding encoded protein was found to lack an entire peptide segment of 60 amino acids. These variants, missing an entire hydrophobic region, could actually facilitate current structural studies, helping to assess whether the absent domain is strictly necessary for a functional adhesin conformation and its contribution to the topology of the protein. This study suggests that epidemic clones appear to pursue different survival strategies, where adhesins, when present, exhibit diverse importance as virulence factors. A practical message arising from the present study is that strategies for the prevention and treatment of implant orthopaedic infections should target adhesins conjointly present in epidemic clones. Furthermore, the choice of reference strains for testing the anti-infective properties of biomaterials should focus on a selection of the most prevalent clones as they exhibit distinct profiles of adhesins.

Aminoglydosid-Antibiotika wie Neomycin B oder Cyclopeptid-Antibiotike wie Viaomycin sind dafür bekannt, daß sie selektiv an RNA binden können. Diese Interaktionen beruhen sowohl auf elektrostatischen Wechselwirkungen als auch auf H-Brücken-Bindungen. Des weiteren ist die definierte räumliche Anordnung von Donor- und Akzeptor-Resten in den Strukturen der RNA-Liganden wichtig für die Affinität. Eine Möglichkeit natürliche RNA-Liganden zu imitieren ist der Einsatz polyfunktioneller Template wie zum Beispiel das 2,6-Diamino-2,6-didesoxy-D-glucose-Scaffold. Mit Hilfe dieser Scaffolds können dann verschiedene positv geladene Reste und Donatoren sowie Akzeptoren für H-Brücken-Bindungen oder auch Interkalatoren räumlich definiert präsentiert werden. Für die unabhängige Funktionalisierung einer jeden Position ist ein Satz orthogonal stabiler Schutzgruppen nötig, wobei eine Hydroxylguppe durch einen Anker ersetzt wird, der eine Anbindung des Scaffolds an einen polymeren Träger ermöglicht. Das neu entwickelte 2,6-Diamino-2,6-didesoxy-D-glucose-Scaffold ist das erste Monosaccharid-Templat, das in allen fünf Positionen mit orthogonal stabilen Schutzgruppen blockiert ist. Alle Positionen könne in beliebiger Reihenfolge selektiv deblockiert und anschließend derivatisiert werden. Das Scaffold kann mit Aminosäuren, Guanidinen oder Interkalatoren umgesetzt werden, um so natürlich vorkommende RNA-bindende Aminoglycoside oder Peptide zu imitieren. Aufbauend auf diesem Monosaccharid-Templat wurde eine Bibliothek von über 100 potentiellen RNA-Liganden synthetisiert, die im Rahmen des Sonderforschungsbereichs 579 (RNA-Liganden-Wechselwirkungen) in Zellassays auf ihre Fähigkeit zur Hemmung der Tat/TAR-Wechselwirkung untersucht wurden, wobei bis jetzt 9 Verbindungen mit einer hemmenden Wirkung im micromolaren Bereich gefunden wurden.

Die Aufklärung der Schlüsselrolle der RNA in zahlreichen biologischen Prozessen, die sich aus ihren selektiven Wechselwirkungen mit anderen RNA-Molekülen, Proteinen, Peptiden bzw. Antibiotika ergibt, ist für die Wirkstoffforschung von großer Bedeutung. Die Aminoglycoside und Antibiotika, die durch eine Hemmung der Proteinbiosynthese schon seit längerem bekannt sind, dienen als Leitstrukuren für die Synthese von weiteren Wirkstoffen. Die meisten Aminoglycosid-Antibiotika beinhalten Aminozucker, die mit dem rn2-Desoxystreptamin-Gerüst verbunden sind. Die stereochemische Vielfalt der Substitutionsstellen für Amino- und Hydroxylgruppen in diesem Gerüst und deren beschränkte konformative Flexibilität bieten vielseitige Möglichkeiten, um potenzielle RNA-Liganden so zu gestalten, dass es zu einer spezifischen Erkennung von RNA-Strukturen kommen kann. Ein wichtiger Vertreter dieser Antibiotika, Neomycin B, von dessen Struktur die Entwicklung des Diaminogalactose-Templates abgeleitet wurde, wurde in dieser Arbeit als Leitstruktur gewählt. Die Synthese von Diaminogalactose-Scaffolds wurde zunächst in Lösung durchgeführt. Anschließend wurden die Bausteine 2 und 4 an einen polymeren Träger gebunden.rnNach Prüfung der orthogonalen Stabilität der Schutzgruppen wurde mit den Scaffolds 2 und 4 eine Bibliothek von 65 Verbindungen hergestellt. Mit 42 dieser Verbindungen wurden anschließend Zellassays im Rahmen des Sonderforschungsbereichs 579 (RNA-Liganden-Wechselwirkungen) durchgeführt, um ihre Cytotoxizität zu prüfen. Für einzelne Verbindungen konnten die optimalen Konzentrationen bestimmt werden, bei denen zukünftige Tests für die Tat/TAR Wechselwirkung ohne störende cytotoxische Effekte durchgeführt werden können.rn

Einleitung und Literaturdiskussion: Gentamicin ist ein aus Bakterien gewonnenes Aminoglykosid-Antibiotikum, das seit vielen Jahren im klinischen Alltag zur Therapie von bakteriellen Infektionen und zur Behandlung des Morbus Ménière eingesetzt wird. Ein bedeutender, jedoch noch nicht vollständig verstandener, Pathomechanismus ist dabei die Entstehung von 4-HNE durch Lipid Peroxidation und die konsekutive Schädigung durch das gebildete Aldehyd. Ziel dieser Arbeit war es, die Beeinflussung der Expression von 4-HNE in sieben verschiedenen Regionen der Kochlea (SV, SL, CO, NF, LF, IDZ und SGZ) durch Gentamicin zu beschreiben und quantitativ zu ermitteln.rnMaterial und Methoden: Die Meerschweinchen wurden in vier Gruppen unterteilt: eine unbehandelte Kontrollgruppe und je eine Gruppe 1, 2 und 7 Tage nach Gentamicin-Applikation. Nach Ablauf der Inkubationszeit wurden die Kochleae den Tieren entnommen, das Gewebe fixiert, geschnitten und auf Objektträgern aufgebracht. Die Schnitte wurden mit 4-HNE-Antikörpern behandelt und die Immunreaktion mikroskopisch lokalisiert und zelluläre quantitative Unterschiede am Computer berechnet.rnErgebnisse: Die Auswertung der Daten ergab signifikante Anstiege der Immunreaktion auf 4-HNE von der Kontrollgruppe zu allen drei Behandlungsgruppen in vier der sieben untersuchten Regionen (Stria vascularis, Spirales Ligament, Cortisches Organ und Nervenfasern). In zwei Bereichen (Fibrozyten im Limbus und Interdentalzellen) kam es zwischen Kontrollgruppe und nur einer Behandlungsgruppe D (7d) zu einer signifikanten Erhöhung. Lediglich die Spiralganglionzellen erbrachten keine signifikanten Differenzen. Der Vergleich der Einzelwindungen erbrachte für die Stria vascularis, das Spirale Ligament, das Cortische Organ und die Nervenfasern signifikante Anstiege innerhalb der drei Windungen von der Kontrollgruppe zu den drei Behandlungsgruppen. Bei der Stria vascularis zeigte sich als einzige Region eine signifikant erhöhte Immunfärbung in allen drei Einzelwindungen von der Kontrollgruppe zu allen Behandlungsgruppen. Beim Vergleich der Gesamtwindungen ließ sich ausschließlich für die Region der Stria vascularis von der ersten zur dritten Windung ein Anstieg der Braunfärbung feststellen. Zudem konnten Korrelationen der Färbeintensitäten einerseits zwischen den beiden Regionen der Lateralen Wand und andererseits zwischen zwei Zelltypen im Limbus aufgezeigt werden.rnDiskussion: Die durch Gentamicin-gesteigerte 4-HNE-Expression kann durch genomische und nicht-genomische Prozesse hervorgerufen werden.

The diagnostic yield of prosthetic joint-associated infection is hampered by the phenotypic change of bacteria into a sessile and resistant form, also called biofilm. With sonication, adherent bacteria can be dislodged from the prosthesis. Species identification may be difficult because of their variations in phenotypic appearance and biochemical reaction. We have studied the phenotypic, genotypic, and biochemical properties of Escherichia coli variants isolated from a periprosthetic joint infection. The strains were collected from synovial fluid, periprosthetic tissue, and fluid from the explanted and sonicated prosthesis. Isolates from synovial fluid revealed a normal phenotype, whereas a few variants from periprosthetic tissue and all isolates from sonication fluid showed different morphological features (including small-colony variants). All isolates from sonication fluid were beta-galactosidase negative and nonmotile; most were indole negative. Because of further variations in biochemical properties, species identification was false or not possible in 50% of the isolates included in this study. In contrast to normal phenotypes, variants were resistant to aminoglycosides. Typing of the isolates using pulsed-field gel electrophoresis yielded nonidentical banding patterns, but all strains were assigned to the same clonal origin when compared with 207 unrelated E. coli isolates. The bacteria were repeatedly passaged on culture media and reanalyzed. Thereafter, most variants reverted to normal phenotype and regained their motility and certain biochemical properties. In addition, some variants displayed aminoglycoside susceptibility after reversion. Sonication of an explanted prosthesis allows insight into the lifestyle of bacteria in biofilms. Since sonication fluid also reveals dislodged sessile forms, species identification of such variants may be misleading.

OBJECTIVES: We investigated whether Acinetobacter baumannii isolates of veterinary origin shared common molecular characteristics with those described in humans. METHODS: Nineteen A. baumannii isolates collected in pets and horses were analysed. Clonality was studied using repetitive extragenic palindromic PCR (rep-PCR) and multilocus sequence typing (MLST). PCR and DNA sequencing for various beta-lactamase, aminoglycoside-modifying enzyme, gyrA and parC, ISAba1 and IS1133, adeR and adeS of the AdeABC efflux pump, carO porin and class 1/2/3 integron genes were performed. RESULTS: Two main clones [A (n = 8) and B (n = 9)] were observed by rep-PCR. MLST indicated that clone A contained isolates of sequence type (ST) ST12 (international clone II) and clone B contained isolates of ST15 (international clone I). Two isolates of ST10 and ST20 were also noted. Seventeen isolates were resistant to gentamicin, 12 to ciprofloxacin and 3 to carbapenems. Isolates of ST12 carried bla(OXA-66), bla(ADC-25), bla(TEM-1), aacC2 and IS1133. Strains of ST15 possessed bla(OXA-69), bla(ADC-11), bla(TEM-1) and a class 1 integron carrying aacC1 and aadA1. ISAba1 was found upstream of bla(ADC) (one ST10 and one ST12) and/or bla(OXA-66) (seven ST12). Twelve isolates of different STs contained the substitutions Ser83Leu in GyrA and Ser80Leu or Glu84Lys in ParC. Significant disruptions of CarO porin and overexpressed efflux pumps were not observed. The majority of infections were hospital acquired and in animals with predisposing conditions for infection. CONCLUSIONS: STs and the molecular background of resistance observed in our collection have been frequently described in A. baumannii detected in human patients. Animals should be considered as a potential reservoir of multidrug-resistant A. baumannii.

New tetracycline and streptomycin resistance genes, tet(44) and ant(6)-Ib, were identified in Campylobacter fetus subsp. fetus within a transferable pathogenicity island that is typically unique to Campylobacter fetus subsp. venerealis. The 640-amino-acid tetracycline resistance determinant, Tet 44, belongs to a class of proteins that confers resistance to tetracycline and minocycline by ribosomal protection. The 286-amino-acid streptomycin resistance determinant, ANT(6)-Ib, belongs to a family of aminoglycoside nucleotidyltransferases. The resistance phenotypes were demonstrated by gene inactivation and expression.

Vancomycin and gentamicin act synergistically against penicillin-resistant pneumococci in vitro and in experimental rabbit meningitis. The aim of the present study was to investigate the underlying mechanism of this synergism. The intracellular concentration of gentamicin was measured by using the following experimental setting. Bacterial cultures were incubated with either gentamicin alone or gentamicin plus vancomycin for a short period (15 min). The gentamicin concentration was determined before and after grinding of the cultures by using the COBAS INTEGRA fluorescence polarization system (Roche). The grinding efficacies ranged between 44 and 54%, as determined by viable cell counts. In the combination regimen the intracellular concentration of gentamicin increased to 186% compared to that achieved with gentamicin monotherapy. These data suggest that the synergy observed in vivo and in vitro is based on an increased intracellular penetration of the aminoglycoside, probably due to the effect of vancomycin on the permeability of the cell wall.

PURPOSE: The primary purpose of the clinical trial was to assess the safety and efficacy of once-a-day compared with three-times-a-day gentamicin in patients with serious infections who had protocol-determined peak serum aminoglycoside concentrations. PATIENTS AND METHODS: A total of 249 hospitalized patients with suspected or proven serious infections were randomized in a 2:2:1 ratio to gentamicin given three times a day with ticarcillin-clavulanate (TC), gentamicin once a day with TC, or ticarcillin-clavulanate (TC) alone. The gentamicin once-a-day dosage for patients with estimated creatinine clearance values of > or =80 mL/min was 5.1 mg/kg. With lower creatinine clearance estimates, the mg/kg dosage of gentamicin was decreased, and the dosage intervals (once daily or three times a day) were maintained. Evaluability required documentation of achievement of protocol-defined peak serum gentamicin levels. RESULTS: Of the total 175 evaluable patients, there were no significant differences found between treatment regimens with respect to clinical or microbiologic efficacy. Bedside audiometry proved impractical due to the frequency of altered mental state in ill patients. Based on the traditional increase in serum creatinine values from baseline values, no differences in renal toxicity between the treatment groups was identified. When changes in renal function were reanalyzed based on maintaining, as opposed to worsening, of renal function, preservation of renal function was better in the gentamicin once-a-day patients as opposed to the gentamicin three-times-a-day patients, P <0.01. CONCLUSIONS: Gentamicin once a day plus TC, gentamicin three times a day plus TC, and TC alone had similar effects in seriously ill hospitalized patients. The incidence of nephrotoxicity was similar in the three treatment groups. Using a nonvalidated post-hoc analysis, renal function was better preserved in gentamicin once-a-day + TC and TC-only patients as opposed to gentamicin three-times-a-day + TC.

Using a convenient and fast HPLC procedure we determined serum concentrations of the fungistatic agent 5-fluorocytosine (5-FC) in 375 samples from 60 patients treated with this drug. The mean trough concentration (n = 127) was 64.3 mg/l (range: 11.8-208.0 mg/l), the mean peak concentration (n = 122) was 99.9 mg/l (range: 25.6-263.8 mg/l), the mean nonpeak/nontrough concentration (n = 126) was 80.1 mg/l (range: 10.5-268.0 mg/l). Totally 134 (35.7%) samples were outside the therapeutic range (25-100 mg/l), 108 (28.8%) being too high, 26 (6.9%) being too low. Forty-four (73%) patients showed 5-FC serum concentrations outside the therapeutic range at least once during the treatment course. In a prospective study we performed 65 dosage predictions on 30 patients by use of a 3-point method previously developed for aminoglycoside dosage adaptation. The mean absolute prediction error of the dosage adaptation was +0.7 mg/l (range: -26.0 to +28.0 mg/l). The root mean square prediction error was 10.7 mg/l. The mean predicted concentration (65.3 mg/l) agreed very well with the mean measured concentration (64.6 mg/l). The frequency distribution of 5-FC serum concentrations indicates that 5-FC monitoring is important. The applied pharmacokinetic method allows individual adaptations of 5-FC dosage with a clinically acceptable prediction error.
