Despite the numerous health benefits, population physical activity levels are low and declining with age. A continued increase of Internet access allows for website-delivered interventions to be implemented across age-groups, though older people have typically not been considered for this type of intervention. Therefore, the purpose of this study was to evaluate a website-delivered computer-tailored physical activity intervention, with a specific focus on differences in tailored advice acceptability, website usability, and physical activity change between three age-groups. To mimic "real-life" conditions, the intervention, which provided personalized physical activity feedback delivered via the Internet, was implemented and evaluated without any personal contact for the entire duration of the study. Data were collected online at baseline, 1-week, and 1-month follow-up and analyzed for three age-groups (≤44, 45-59, and ≥60 years) using linear mixed models. Overall, 803 adults received the intervention and 288 completed all measures. The oldest age-group increased physical activity more than the other two groups, spent the most time on the website, though had significantly lower perceived Internet self-confidence scores when compared with the youngest age-group. No differences were found in terms of website usability and tailored advice acceptability. These results suggest that website-delivered physical activity interventions can be suitable and effective for older aged adults.

Childhood obesity affects children across all ages and genders. However, Latino children and adolescents are at an increased risk, with one out of three Latino children (ages 2-19) being classified as overweight. Physical inactivity is deemed a major factor contributing to the energy imbalance that leads to excess adiposity. The aims of this study are twofold: 1) to present relevant research regarding Latino children’s physical patterns, influences on their physical activity, and interventions designed to promote physical activity and fitness in this population; and 2) to discuss implications derived from this research to help health educators, practitioners, and policy makers increase awareness, and to motivate and enable Latino children to adopt an active lifestyle. Research reveals that Latino children and adolescents are consistently less active than their white counterparts. Latino girls are, in particular, at an increased risk for inactivity. Few studies have investigated the factors that contribute to low levels of physical activity among Latino children. Moreover, few physical activity interventions have involved Latino children. Some of our recent research studies have filled some gaps, including providing information on what physical activities Latino children like, what they intend to do, what they are actually doing, and where and when they do physical activity. Based on our research and review of related literature, we made specific physical activity recommendations for researchers, practitioners, and policy makers. These individual points should be applied and integrated within a broad framework and used in combinations to develop multi-component, coordinated approaches to enhancing physical activity among Latino youth.

Research on lifestyle physical activity interventions suggests that they help individuals meet the new recommendations for physical activity made by the Centers for Disease Control and Prevention (CDC) and the American College of Sports Medicine (ACSM). The purpose of this research was to describe the rates of adherence to two lifestyle physical activity intervention arms and to examine the association between adherence and outcome variables, using data from Project PRIME, a lifestyle physical activity intervention based on the transtheoretical model and conducted by the Cooper Institute of Aerobics Research, Dallas, Texas. Participants were 250 sedentary healthy adults, aged 35 to 70 years, primarily non-Hispanic White, and in the contemplation and preparation stages of readiness to change. They were randomized to a group (PRIME G) or a mail- and telephone-delivered condition (PRIME C). Adherence measures included attending class (PRIME G), completing a monthly telephone call with a health educator (PRIME C), and completing homework assignments and self-monitoring minutes of moderate- to vigorous physical activity (both groups). In the first results paper, adherence over time and between conditions was examined: Attendance in group, completing the monthly telephone call, and homework completion decreased over time, and participants in PRIME G were more likely to complete homework than those in PRIME C. Paper 2 aimed to determine whether the adherence measures predicted achievement of the CDC/ACSM physical activity guideline. In separate models for the two conditions, a latent variable measuring adherence was found to predict achievement of the guideline. Paper 3 examined the association between adherence measures and the transtheoretical model's processes of change within each condition. For both, participants who completed at least two thirds of the homework assignments improved their use of the processes of change more than those who completed less than that amount. These results suggest that encouraging adherence to a lifestyle physical activity intervention, at least among already motivated volunteers, may increase the likelihood of beneficial changes in the outcomes. ^

Introduction. There is a need for physical activity interventions based in primary care clinics that take advantage of community resources. The purpose of this randomized controlled trial was to compare the effects of two physical activity interventions: (1) physical activity prescription by a primary care provider plus referral to community physical activity resources and (2) physical activity prescription only. ^ Methods. Sedentary adult patients recruited from a general medicine clinic were randomized to receive a physical activity prescription, delivered by the primary care provider, plus referral to community physical activity resources (n=38) or physical activity prescription only (n=32). Outcomes were use of community resources (exercise facility and personal trainers), physical activity levels (self-report questionnaire and pedometer), and attitudes regarding physical activity assessed at 8 weeks. ^ Results. Three of 38 (7.9%) subjects referred to the community resources and none of the 32 subjects in the prescription only group used the community resources during the 8 week trial. Sixteen of 32 subjects in the prescription plus referral group and 19 of 38 in the prescription group completed the self-report follow-up forms at 8 weeks. For minutes of moderate- or vigorous-intensity physical activity per week, there were no between-group differences at baseline, follow-up, or change from baseline to follow-up. However, for moderate- and vigorous-intensity physical activity, there were significant improvements from baseline to follow-up within each group. For attitudes related to physical activity, there were no between-group differences at baseline, follow-up, or change from baseline to follow-up; neither were there any within-group changes. ^ Discussion. Physical activity prescription delivered by a healthcare provider in the context of a routine primary care visit can improve physical activity levels, with no additional improvement gained by referring to community resources. The intervention was feasible for primary care providers to deliver, but only 50% of subjects returned the self-report physical activity questionnaire at the 8 week assessment. ^

Research provides evidence of the positive health effects associated with regular physical activity participation in all populations. Activity may prove to be especially beneficial in those with chronic conditions such as cancer. However, the majority of cancer patients and survivors do not participate in the recommended amount of physical activity. The purpose of this dissertation was to identify factors associated with physical activity participation, describe how these factors change as result of a diet and exercise intervention, and to evaluate correlates of long term physical activity maintenance. ^ For this dissertation, I analyzed data from the FRESH START trial, a randomized, single-blind, phase II clinical trial focused on improving diet and physical activity among recently diagnosed breast and prostate cancer survivors. Analyses included both parametric and non-parametric statistical tests. Three separate studies were conducted, with sample sizes ranging from 400 to 486. ^ Common barriers to exercise, such as “no willpower,” “too busy,” and “I have pain,” were reported among breast and prostate cancer survivors; however, these barriers were not significantly associated with minutes of physical activity. Breast cancer survivors reported a greater number of total barriers to exercise as well as higher proportions reporting individual barriers, compared to prostate cancer survivors. Just less than half of participants reduced their total number of barriers to exercise from baseline to 1-year follow-up, and those who did reduce barriers reported greater increases in minutes of physical activity compared to those who reported no change in barriers to exercise. Participants in both the tailored and standardized intervention groups reported greater minutes of physical activity at 2-year follow-up compared to baseline. Overall, twelve percent of participants reached recommended levels of physical activity at both 1- and 2-year follow-up. Self-efficacy was positively associated with physical activity maintenance, and the number of total barriers to exercise was inversely associated with physical activity maintenance. ^ Results from this dissertation are novel and informative, and will help to guide future physical activity interventions among cancer survivors. Thoughtfully designed interventions may encourage greater participation in physical activity and ultimately improve overall quality of life in this population. ^

Background: Aflifle a growing literature supports the effectiveness of physical activity interventions delivered in the primary care setting, few studies have evaluated efforts to increase physician counseling on physical activity during routine practice (i.e., outside the context of controlled research). This paper reports the results of a dissemination trial of a primary care-based physical activity counseling intervention conducted within the context of a larger, multi-strategy, Australian community-based, physical activity intervention, the 10,000 Steps Rockhampton Project. Methods: All 23 general practices and 66 general practitioners (GPs, the Australian equivalent of family physicians) were invited to participate. Practice visits were made to consenting practices during which instruction in brief physical activity counseling was offered, along with physical activity promotion resources (print materials and pedometers). The evaluation, guided by the RE-AIM framework, included collection of process data, as well as pre-and post-inteivention data from a mailed GP survey, and data from the larger project's random-digit-dialed, community-based, cross-sectional telephone survey that was conducted in Rockhampton and a comparison community, Results: Ninety-one percent of practices were visited by 10,000 Steps staff and agreed to participate, with 58% of GPs present during the visits. General practitioner survey response rates were 67% (n =44/66 at baseline) and 71% (n =37/52, at 14-month follow-up). At follow-up, 62% had displayed the poster, 81% were using the brochures, and 70% had loaned pedometers to patients, although the number loaned was relatively small. No change was seen in GP self-report of the percentage of patients counseled on physical activity. However, data from the telephone surveys showed a 31% increase in the likelihood of recalling GP advice on physical activity in Rockhampton (95% confidence interval [CI]=1.11-1.54) compared to a 16% decrease (95% CI=0.68-1.04) in the comparison community. Conclusions: This dissemination study achieved high rates of GP uptake, reasonable levels of implementation, and a significant increase in the number of community residents counseled on physical activity. These results suggest that evidence-based primary care physical activity counseling protocols can be translated into routine practice, although the initial and ongoing investment of time to develop partnerships with relevant healthcare organizations, and the interest generated by the overall 10,000 Steps program should not be underestimated. Background. Both self-help print materials and telephone-assisted counseling have generally proved useful strategies to increase physical activity. This study examined their effectiveness in an intervention aimed specifically at promoting walking for specific purposes. Methods. Participants (n = 399) were randomly allocated to one of two 3-week intervention programs. The Print program comprised multiple mailing of brochures that emphasized walking within the local community environments. The Print plus Telephone program received the same brochures plus three telephone calls. Data collected via mailed self-completed surveys were analyzed by exploring outcomes related to walking for specific purposes. Results. There were no significant differences between the two programs in any of the walking measures. Both groups significantly increased time reported walking for exercise per week [Print: t(1,277) = -3.50, P < 0.001; Print plus telephone: t(1,106) = -2.44, P < 0.016]. Significantly, more participants in the Print plus Telephone group reported receiving and reading the materials (chi(2) = 20.11, P < 0.0001). Conclusions. The intervention programs were more successful at increasing walking for exercise than for any other purpose. The addition of brief telephone support was successful in focusing participants' attention on the print materials, but did not result in any additional increase in walking. Background: The effective evaluation of physical activity interventions for older adults requires measurement instruments with acceptable psychometric properties that are sufficiently sensitive to detect changes in this population. Aim: To assess the measurement properties (reliability and validity) of the Community Healthy Activities Model Program for Seniors (CHAMPS) questionnaire in a sample of older Australians. Methods: CHAMPS data were collected from 167 older adults (mean age 79.1 S.D. 6.3 years) and validated with tests of physical ability and the SF-12 measures of physical and mental health. Responses from a sub-sample of 43 older adults were used to assess 1-week test-retest reliability. Results: Approximately 25% of participants needed assistance to complete the CHAMPS questionnaire. There were low but significant correlations between the CHAMPS scores and the physical performance measures (rho=0.14-0.32) and the physical health scale of the SF-12 (rho=0.12-0.24). Reliability coefficients were highest for moderate-intensity (ICC=0.81-0.88) and lowest for vigorous-intensity physical activity (ICC=0.34-0.45). Agreement between test-retest estimates of sufficient physical activity for health benefits (>= 150 min and >= 5 sessions per week) was high (percent agreement = 88% and Cohen's kappa = 0.68). Conclusion: These findings suggest that the CHAMPS questionnaire has acceptable measurement properties, and is therefore suitable for use among older Australian adults, as long as adequate assistance is provided during administration. An assumption of theory-based physical activity interventions is that active participation positively affects the theoretical constructs upon which the intervention is based. This assumption is rarely tested. This study assessed whether participation, defined as completion of homework, in a lifestyle physical activity intervention was associated with changes over 6 months in constructs the homework addressed: the behavioral and cognitive processes of change, self-efficacy, and decisional balance (the pros and cons). Participants were 244 sedentary adults aged 25 to 75 years. They completed an average of 12 of 20 homework assignments. Those completing at least two-thirds of the homework (n = 113) had greater changes in the theoretical constructs from pretest to posttest than those completing less (n = 90). Post-hoc analyses suggest that completing theory-based homework may impact the processes of change and self-efficacy in lifestyle physical activity interventions and, therefore, are warranted in future interventions.

Exercise may be described as a polypill to prevent and/or treat almost every chronic disease, with obvious benefits such as its low cost and practical lack of adverse effects. Implementing physical activity interventions in public health is therefore a goal at the medical, social, and economic levels. This chapter describes the importance of health promotion through physical activity and discusses the impacts of exercise on the most prevalent chronic diseases, namely metabolic syndrome-related disorders, cardiovascular diseases, cancer, and Alzheimer's disease. For each of these chronic conditions, we discuss the epidemiological evidence supporting a beneficial role of exercise, provide guidelines for exercise prescription, and describe the biological mechanisms whereby exercise exerts its modulatory effects.

La présente thèse, organisée en trois volets, poursuivait trois objectifs : i) Estimer les coûts médicaux directs du traitement du DT2 dans 4 pays d’Afrique subsaharienne et dans le cas du Mali, rapprocher ces coûts médicaux directs estimés aux dépenses effectives des patients diabétiques ; ii) Examiner le coût-efficacité des interventions de prévention basées sur la modification du mode de vie chez les sujets à haut risque du DT2; iii) Cerner la perception et les attitudes des acteurs de la santé sur les outils de plaidoyer développés dans le cadre du projet DFN et leur potentiel d’impact sur les décideurs. Dans le premier volet, il s’est agi d’estimer les coûts du DT2 et de ses complications au moyen d’un calculateur et de le mettre à l’épreuve au Bénin, au Burkina- Faso, en Guinée et au Mali. Les composantes de soins pour le DT2 et ses complications avaient été définies au préalable par une équipe de spécialistes, sur la base de leur expérience clinique et des lignes directrices existantes. Les prix ont été relevés dans deux structures hospitalières du secteur public et deux du privé. Les coûts ont été estimés sur une base annuelle pour le DT2 avec ou sans complications chroniques puis par épisode pour les complications aiguës. Dans le cas du Mali, ces coûts ont été rapprochés des dépenses de patients diabétiques d’après une précédente enquête transversale dans ce pays. Cette enquête portait sur 500 sujets diabétiques sélectionnés au hasard dans les registres. Les dépenses pour les soins des trois derniers mois avaient été relevées. Les déterminants des dépenses ont été explorés. Il ressort des différences de coûts dans le même secteur puis entre le secteur privé et le secteur public. Le coût minimum du traitement du DT2 sans complications dans le secteur public représentait entre 21% et 34% de PIB par habitant, puis entre 26% - 47% en présence de la rétinopathie et au-delà de 70% pour la néphropathie, la complication chronique la plus coûteuse. Les dépenses des sujets diabétiques enquêtés au Mali, étaient en deçà des coûts minima estimatifs des différentes complications excepté la rétinopathie et le DT2 sans complication. Les facteurs comme l’insulinothérapie, le nombre de complications et la résidence dans la capitale étaient significativement associés aux dépenses plus élevées des patients. Dans le second volet, la revue systématique a consisté à recenser les études d’évaluation économique des interventions de prévention du DT2 dans des groupes à haut risque par l’alimentation et/ou l’activité physique. Les interventions de contrôle de l’obésité comme facteur de risque majeur de DT2 ont également été considérées. Les études ont été sélectionnées dans les bases de données scientifiques en utilisant les mots clés et des critères prédéfinis. Les études originales publiées entre janvier 2009 et décembre 2014 et conduites en français, anglais ou espagnol étaient potentiellement éligibles. La liste de contrôle de « British Medical Journal » a servi à évaluer la qualité des études. Des 21 études retenues, 15 rapportaient que les interventions étaient coût-efficaces suivant les limites d’acceptabilité considérées. Six études étaient non concluantes, dont quatre destinées à la prévention du DT2 et deux, au contrôle de l’obésité. Dans le troisième volet, les perceptions d’utilisateurs potentiels de ce calculateur et d’un autre outil de plaidoyer, à savoir, l’argumentaire narratif expliquant la nécessité de se pencher sur la lutte contre le DT2 en Afrique, ont été évaluées dans une étude qualitative exploratoire. Les données ont été collectées au cours d’entretiens individuels avec 16 acteurs de la santé de quatre pays d’Afrique subsaharienne et un groupe de discussion avec 10 étudiants de master de nutrition à l’issue d’un atelier de formation sur le plaidoyer faisant appel à ces outils, au Bénin. Les entretiens ont été enregistrés, transcrits et codés à l’aide du logiciel QDA Miner. Les participants ont souligné la pertinence des outils pour le plaidoyer et la convivialité du calculateur de coûts. Il demeure cependant que le contexte politique marqué par la compétition des priorités, l’absence de cohésion entre les décideurs et un défaut de données notamment sur le coût-efficacité des interventions sont des freins à la priorisation du DT2 dans les politiques de santé en Afrique subsaharienne que les répondants ont relevés. L’étude confirme que le traitement du DT2 est financièrement inabordable pour un grand nombre de patients. Elle souligne que les dépenses des patients sont en deçà des coûts estimés pour un traitement approprié avec quelques exceptions. La prévention du DT2 basée le mode de vie est coût-efficace mais devrait être étudiée en Afrique. On peut espérer que la pertinence des outils de ce travail telle que relevée par les acteurs de santé se traduise par leur utilisation. Ceci pour susciter des interventions de prévention afin d’infléchir l’évolution du DT2 et son impact économique en Afrique subsaharienne.

Objetivo: El cuestionario Barriers to Being Active Quiz (BBAQ), indaga las barreras para ser físicamente activo. El cuestionario fue traducido al español por el mismo equipo que desarrolló la versión inglésa original, pero carece de estudios de validez en la versión española. El objetivo de esta investigación fue evaluar las propiedades psicométricas del BBAQ (en la versión completa de 21 ítems), centrándose en la fiabilidad y validez. Material y métodos: Un total de 2.634 (1.462 mujeres y 1.172 varones; 18-30 años de edad) estudiantes universitarios completaron el cuestionario BBAQ-21. El alfa de Crombach se estimó como indicador de consistencia interna. El coeficiente de correlación intra-clase (CCI) y el grado de acuerdo se calcularon para evaluar la estabilidad temporal con un periodo de 7 días entre ambas administraciones como estimadores de la reproducibilidad. Se aplicó un análisis factorial exploratorio (AFE) y confirmatorio (AFC) para analizar la validez del BBAQ-21 ítems. Resultados: El BBAQ-21 mostró valores de un alfa de Cronbach entre 0,812 y 0,844 y un CCI entre el 0,46 y 0,87. El porcentaje de acuerdo por todos los conceptos individuales varió de 45 a 80%. El AFE determinó cuatro factores que explicaron el 52,90% de la varianza y el AFC mostró moderadas cargas factoriales. Conclusiones: Los resultados obtenidos en este cuestionario avalan la utilización de este instrumento con este tipo de muestra, desde el punto de vista de la fiabilidad y validez. El BBAQ-21 está disponible para evaluar las barreras para la actividad física en América Latina.

Lifestyle is directly related to the incidence of type 2 diabetes mellitus (DM-2), a risk dramatically elevated by obesity and inactivity. Several studies have verified that educational interventions can delay the onset of DM-2. Some of the interventions strategies utilized medication and diet, diet and/or physical exercise or the combination of diet and exercise, generally referred to a change in lifestyle. Despite the evidence that DM-2 can be preventive, there is still limited availability of effective prevention programs. DM-2 is considered an emerging public health problem as it is estimated that by the year of 2030 there will be about 366 million people with diabetes worldwide. DM2 remains a leading cause of cardiovascular disorders and many other complications. Our intent with this paper is to present researches and strategies (diet and physical activity interventions) that successfully improved plasma glucose control as a result of an effective lifestyle intervention program.

Objective: To devise more-effective physical activity interventions, the mediating mechanisms yielding behavioral change need to be identified. The Baron-Kenny method is most commonly used. but has low statistical power and May not identify mechanisms of behavioral change in small-to-medium size Studies. More powerful statistical tests are available, Study Design and Setting: Inactive adults (N = 52) were randomized to either a print or a print-plus-telephone intervention. Walking and exercise-related social support Were assessed at baseline, after file intervention, and 4 weeks later. The Baron-Kenny and three alternative methods of mediational analysis (Freedman-Schatzkin; MacKinnon et al.: bootstrap method) were used to examine the effects of social support on initial behavior change and maintenance. Results: A significant mediational effect of social support on initial behavior change was indicated by the MacKinnon et al., bootstrap. and. marginally. Freedman-Schatzkin methods, but not by the Baron-Kenny method. No significant mediational effecl of social support on maintenance of walking was found. Conclusions: Methodologically rigorous intervention studies to identify mediators of change in physical activity are costly and labor intensive, and may not be feasible with large samples. The Use of statistically powerful tests of mediational effects in small-scale studies can inform the development of more effective interventions. (C) 2006 Elsevier Inc. All rights reserved.
