As obstruções do sistema venoso superior podem ocorrer por trombose espontânea ou na presença de cateteres de hemodiálise ou quimioterapia, e mesmo na presença de compressão extrínseca por tumores intratorácicos. O uso de endopróteses para o tratamento dessas obstruções vem se tornando prática comum e sua indicação inclui situações de risco de embolia pulmonar, síndrome da veia cava superior e perda do acesso vascular. Nesse contexto, o uso do ultra-som intracardíaco pode ser útil na avaliação da extensão e da gravidade da obstrução, além de fornecer informações morfológicas murais e intramurais do sistema venoso. Relatamos o caso de uma paciente com síndrome da veia cava superior por trombose da via de entrada do átrio direito, submetida a implante de endoprótese monitorado por ultra-som intracardíaco, além de discutir o tema por meio de revisão da literatura.

No presente caso, relatamos a realização de angioplastia coronariana com implante de stent na artéria coronariana direita de paciente com quadro de angina instável de alto risco, portadora de artéria coronariana única com origem no seio coronariano direito. As artérias descendente anterior e circunflexa originavam-se isoladamente no terço proximal da artéria coronariana direita. Trata-se de rara anomalia coronariana com poucos relatos de intervenção coronariana percutânea na literatura. Este caso ilustra a necessidade da avaliação anatômica pormenorizada do trajeto das artérias coronarianas, precedendo a realização da angioplastia transluminal percutânea, objetivando-se a prevenção de complicações.

Cor triatriatum (CT) é uma cardiopatia congênita rara que geralmente cursa com sintomas nos primeiros anos de vida. Na ausência de outras malformações cardíacas associadas e na dependência do grau de comunicação entre a câmara superior e o átrio esquerdo (AE), os pacientes podem atingir a idade adulta. Relatamos um caso de uma paciente adulta assintomática portadora de CT diagnosticado pelo ecocardiograma transtorácico (ETT) e acompanhada clinicamente durante a gestação.

Relatamos um caso de um neonato portador de transposição completa das grandes artérias que foi submetido a atrioseptostomia de Rashkind por acesso trans-hepático por interrupção congênita da veia cava inferior. Os aspectos técnicos do procedimento são discutidos.

A estenose da válvula mitral é a lesão de válvula mais comum durante a gravidez. Apesar de um tratamento clínico eficaz e de uma anatomia valvular favorável, de acordo com o critério de Wilkins e Block [score de Wilkins-Block], a intervenção percutânea em pacientes sintomáticas mostra-se muito importante. Nessas pacientes, recomenda-se evitar ao máximo a exposição aos raios X para proteger o feto dos efeitos deletérios da radiação ionizante. Neste relato de caso, uma paciente de 24 anos, grávida, com grave estenose mitral (área valvular de 0,9 cm²), foi submetida com sucesso a um tratamento percutâneo com ETE-guiado, sem o uso de raios X.

In rats, neonatal treatment with monosodium L-glutamate (MSG) induces several metabolic and neuroendocrine abnormalities, which result in hyperadiposity. No data exist, however, regarding neuroendocrine, immune and metabolic responses to acute endotoxemia in the MSG-damaged rat. We studied the consequences of MSG treatment during the acute phase response of inflammatory stress. Neonatal male rats were treated with MSG or vehicle (controls, CTR) and studied at age 90 days. Pituitary, adrenal, adipo-insular axis, immune, metabolic and gonadal functions were explored before and up to 5 h after single sub-lethal i.p. injection of bacterial lipopolysaccharide (LPS; 150 microg/kg). Our results showed that, during the acute phase response of inflammatory stress in MSG rats: (1) the corticotrope-adrenal, leptin, insulin and triglyceride responses were higher than in CTR rats, (2) pro-inflammatory (TNFalpha) cytokine response was impaired and anti-inflammatory (IL-10) cytokine response was normal, and (3) changes in peripheral estradiol and testosterone levels after LPS varied as in CTR rats. These data indicate that metabolic and neroendocrine-immune functions are altered in MSG-damaged rats. Our study also suggests that the enhanced corticotrope-corticoadrenal activity in MSG animals could be responsible, at least in part, for the immune and metabolic derangements characterizing hypothalamic obesity.

The eggs of Anopheles cruzii and An. bellator are described and illustrated using scanning electron micrographs. Hatching was observed, and larval exit documented. No morphological differences were found between the eggs of the two species.

The spatial and temporal distribution of anopheline larvae was studied in two coastal malarious areas of Sucre, State, Venezuela. Seven habitat types were sampled in the village of Guayana and eight species of Anopheles were collected. Anopheles aquasalis was the predominant species collected and was most abundant in the brackish marsh habitat (71 larvae per 100 samples). It was most abundant during the rainy season. At the second location, Santa F e, six habitat types were sampled and four anopheline species were collected. Habitats where An. aquasalis was most abundant were temporary freshwater ponds (34 larvae per 100 samples) and mangroves (10.5 larvae per 100 samples). At this location it was also most abundant in the rainy season. During the dry season it was collected in small numbers in river pools (1.3 larvae per 100 samples) along with large numbers of An. pseudopunctipennis (479 larvae per 100 samples). Larval control could be an important component of the malaria control program because major habitats could be defined and presence and abundance of larvae was limited to specific times of year.

The principal vector of malaria in eastern Venezuela, Anopheles aquasalis, is exophagic and exophilic. Control using indoor insecticide house sprays has failed to lower the number of malaria cases. Therefore, studies were initiated in two villages of the eastern coastal state of Sucre to better understand this vector's biology and develop a more integrated control program. An. aquasalis was found to have a crepuscular biting behavior with a major peak at dusk and a minor peak at dawn. Mosquitos were collected more outdoors than indoors. Forty-seven percent of the biting took place before people went to bed (22:30 hr) and 69% of the mosquitos biting during this time period bite outdoors. Outdoor biting could be the reason why indoor spraying alone did not lower malaria cases. Seasonal abundance was greater in the rainy season compared to the dry season. Seasonal parous rates were high (78.3%-100%) and similar indoors and outdoors and between dry and wet season in Santa F e. In Guayana, the seasonal parity was lower (34.6%-42.2%) than Santa F e with indoor parity slightly higher than outdoors. Malaria cases were higher in Santa F e, but adult mosquito density was much lower than in Guayana. This difference could have been due to higher parity in Santa F e compared to Guayana. The greater distance to the nearest breeding site and presence of alternative hosts in Guayana can not be discounted as factors which contributed to the difference in malaria transmission between locations. We concluded that knowledge on seasonal occurrence, biting activity, resting behavior and breeding site location can be used to design a new control strategy for this vector.

Continuing to review the subgenus Helcocyrtomyia (Diptera, Psychodidae, Phlebotomiane) some specimens of sandflies from the vexator series were examined. Taxonomic remarks, geographic distribution and drawings of those species are presented.

Because of the relative epidemiological significance of Triatoma sordida, T. guasayana and T. patagonica, and the need to resolve doubts about their taxonomic validity, we report here a detailed taxonomic comparison of the three species using multivariate analysis of morphometric measures combined with comparisons of their genitalia and antennal structures. From the 17 metric variables studied, the length of the second segment of the rostrum and the anteocular length provided a discrimination function able to separate without error T. sordida from T. guasayana and T. patagonica. The multivariate discriminant functions classified T. guasayana and T. patagonica with an error of 2.44%. Comparison of the male genitalia of T. guasayana and T. sordida showed that there are minor differences in the articulatory apparatus, the median process of the pygophore, the phallosome support and the vesica, with bigger differences in the endosomal process and the phallosome. However, the already described male genitalia of T. patagonica is very similar to that of T. sordida. Analysis of antennal structure by scanning electron microscope showed that sensilla distribution around the pedicel is slightly different in the three species and sensilla density is highest in T. sordida and lowest in T. patagonica. The study showed that the three species form a closely related group. The results confirm the earlier classification of sordida and guasayana as separate species, but they raise some doubts about the taxonomic status of T. patagonica.
