Os objetivos deste trabalho foram determinar a absorção aparente, estimar as perdas endógenas fecais e a absorção real do Mg e determinar a ingestão ad libitum da água de beber e a concentração de Mg no soro sangüíneo de caprinos da raças Anglonubiana (AN) e Saanen (SN). Foram usados doze caprinos, seis de cada raça, com 19,8 kg PV médio. Dietas semipurificadas (baixo teor de Mg) à base de quirera de arroz, glúten de milho e celulose foram suplementadas com MgO, para se obterem os níveis de 0,05 (sem supplementação) 0,20 e 0,35% Mg (%MS). Os níveis de Mg influenciaram os coeficientes de absorção aparente de Mg e Ca, com valores médios de 57,8; 73,9; e 73,2% para Mg e 55,7; 39,6; e 49,5% para Ca, para dietas com níveis 0,05; 0,20; e 0,35% de Mg, respectivamente. Entretanto, para os coeficientes de absorção aparente de P, Na e K, não houve efeito de níveis de Mg na dieta. Os resultados de absorção real de Mg apresentaram interação de níveis de Mg e raças. A média para raça NA, no nível 0,05% Mg, foi de 61,0% e para os níveis 0,20 e 0,35% Mg, 77,2 e 73,2%, respectivamente. Entretanto, para a raça SN, as médias foram 73,3; 75,5; e 76,0%, para os mesmos níveis, sem diferenças. A digestibilidade de matéria seca, proteína bruta e extrato não-nitrogenado diminuiu com os níveis crescentes de Mg nas dietas. As excreções fecais (7,0; 20,8; e 34,4 mg/kg PV0,75.d) e urinárias (3,9; 30,8; e 44,6 mg/kg PV0,75.d) de Mg elevaram-se com o aumento dos níveis crescentes de Mg nas dietas. Houve, também, influência dos níveis de Mg dietético sobre as concentrações de Mg do soro sangüíneo (1,74; 2,23; e 2,80 mg/dL para níveis de 0,05; 0,20; e 0,35% de Mg, respectivamente).

As infecções de origem alimentar no homem, causadas por Campylobacter spp., resultam em grandes perdas econômicas e estão relacionadas à produção e o abate de frangos, etapas importantes na disseminação dessas bactérias. Baseando-se na importância do Campylobacter spp. na saúde pública e tendo em vista os dados constantes na literatura de que as aves comercializadas estão constantemente contaminadas com esse agente, sentiu-se a necessidade de realizar um estudo envolvendo a criação e o abate de frangos na região amazônica para que medidas profiláticas e de controle possam ser adotadas. O trabalho teve como objetivo estudar a ocorrência de Campylobacter spp. em granjas e abatedouro avícolas na mesorregião metropolitana de Belém – PA; isolar e identificar as espécies de Campylobacter spp. e identificar as fontes de contaminação nas granjas e os pontos críticos no abate. Foi coletado um total de 120 amostras em três granjas avícolas: 30 amostras de “swab” cloacal, 30 amostras de cama de frango, 30 amostras de ração e 30 amostras de água dos bebedouros. No abatedouro, foram colhidas 126 amostras: 36 amostras de água em 12 pontos diferentes da linha de abate e mais 30 amostras de pele do conjunto peito/ pescoço, 30 amostras de fígado e 30 amostras de moela. As amostras foram colhidas entre os meses de janeiro e maio de 2007 para o isolamento e identificação das espécies de Campylobacter spp. As amostras foram processadas na Seção de Bacteriologia e Micologia do Instituto Evandro Chagas – IEC da Secretaria de Vigilância em Saúde (SVS), Ministério da Saúde, Ananindeua – PA. Campylobacter spp. foi isolado em 33,3% (40/120) das amostras das granjas. Não houve diferença significativa (p>0,05) entre os percentuais de isolamentos positivos entre as três granjas pesquisadas. Ao analisar as freqüências dos isolados de Campylobacter spp. para cada tipo de amostra das granjas, observou-se que 96,6% (29/30) das amostras de “swab” cloacal, 33,3% (10/30) das amostras de cama e 3,3% (1/30) das amostras de água foram positivas para Campylobacter spp. Não foi isolada a bactéria nas amostras de ração. Campylobacter jejuni foi identificado bioquimicamente em 82,5% (33/40) das cepas isoladas nas granjas. No abatedouro, todas as cepas isoladas foram identificadas como C. jejuni., sendo isolado a bactéria em 8,73% (11/126) das amostras provenientes da linha de abate. Ao analisar as freqüências dos isolados de C. jejuni para cada tipo de amostra do abatedouro, observou-se que 27,8% (10/36) das amostras de água foram positivas para C. jejuni, seguido pela moela com 3,3% (1/30) das amostras positivas. Não foi isolado Campylobacter spp. nas amostras de fígado e de pele do conjunto peito/ pescoço. Houve diferença significativa (p<0,0001) entre os isolamentos positivos, negativos e os tipos de amostras processadas nas granjas e no abatedouro. As principais fontes de contaminação nas granjas foram o “swab” cloacal, a cama e, em menor escala, a água. Os principais pontos críticos observados no abatedouro foram a água, seguido pela moela. C. jejuni foi identificado em elevado percentual entre as cepas isoladas nas granjas e em todas as cepas do abatedouro.

OBJECTIVES: A number of complications exist with invasive mechanical ventilation and with the use of and withdrawal from prolonged ventilator support. The use of protocols that enable the systematic identification of patients eligible for an interruption in mechanical ventilation can significantly reduce the number of complications. This study describes the application of a weaning protocol and its results. METHODS: Patients who required invasive mechanical ventilation for more than 24 hours were included and assessed daily to identify individuals who were ready to begin the weaning process. RESULTS: We studied 252 patients with a median mechanical ventilation time of 3.7 days (interquartile range of 1 to 23 days), a rapid shallow breathing index value of 48 (median), a maximum inspiratory pressure of 40 cmH2O, and a maximum expiratory pressure of 40 cm H2O (median). Of these 252 patients, 32 (12.7%) had to be reintubated, which represented weaning failure. Noninvasive ventilation was used postextubation in 170 (73%) patients, and 15% of these patients were reintubated, which also represented weaning failure. The mortality rate of the 252 patients studied was 8.73% (22), and there was no significant difference in the age, gender, mechanical ventilation time, and maximum inspiratory pressure between the survivors and nonsurvivors. CONCLUSIONS: The use of a specific weaning protocol resulted in a lower mechanical ventilation time and an acceptable reintubation rate. This protocol can be used as a comparative index in hospitals to improve the weaning system, its monitoring and the informative reporting of patient outcomes and may represent a future tool and source of quality markers for patient care.

Seasonal snow cover is of great environmental and socio-economic importance for the European Alps. Therefore a high priority has been assigned to quantifying its temporal and spatial variability. Complementary to land-based monitoring networks, optical satellite observations can be used to derive spatially comprehensive information on snow cover extent. For understanding long-term changes in alpine snow cover extent, the data acquired by the Advanced Very High Resolution Radiometer (AVHRR) sensors mounted onboard the National Oceanic and Atmospheric Association (NOAA) and Meteorological Operational satellite (MetOp) platforms offer a unique source of information. In this paper, we present the first space-borne 1 km snow extent climatology for the Alpine region derived from AVHRR data over the period 1985–2011. The objective of this study is twofold: first, to generate a new set of cloud-free satellite snow products using a specific cloud gap-filling technique and second, to examine the spatiotemporal distribution of snow cover in the European Alps over the last 27 yr from the satellite perspective. For this purpose, snow parameters such as snow onset day, snow cover duration (SCD), melt-out date and the snow cover area percentage (SCA) were employed to analyze spatiotemporal variability of snow cover over the course of three decades. On the regional scale, significant trends were found toward a shorter SCD at lower elevations in the south-east and south-west. However, our results do not show any significant trends in the monthly mean SCA over the last 27 yr. This is in agreement with other research findings and may indicate a deceleration of the decreasing snow trend in the Alpine region. Furthermore, such data may provide spatially and temporally homogeneous snow information for comprehensive use in related research fields (i.e., hydrologic and economic applications) or can serve as a reference for climate models.

BACKGROUND Ribavirin (RBV) is an essential component of most current hepatitis C (HCV) treatment regimens and still standard of care in the combination with pegylated interferon (pegIFN) to treat chronic HCV in resource limited settings. Study results in HIV/HCV-coinfected patients are contradicting as to whether RBV concentration correlates with sustained virological response (SVR). METHODS We included 262 HCV treatment naïve HIV/HCV-coinfected Swiss HIV Cohort Study (SHCS) participants treated with RBV and pegIFN between 01.01.2001-01.01.2010, 134 with HCV genotype (GT) 1/4, and 128 with GT 2/3 infections. RBV levels were measured retrospectively in stored plasma samples obtained between HCV treatment week 4 and end of therapy. Uni- and multivariable logistic regression analyses were used to evaluate the association between RBV concentration and SVR in GT 1/4 and GT 2/3 infections. The analyses were repeated stratified by treatment phase (week 4-12, 13-24, >24) and IL28B genotype (CC versus CT/TT). RESULTS SVR rates were 35.1% in GT 1/4 and 70.3% in GT 2/3 infections. Overall, median RBV concentration was 2.0 mg/L in GT 1/4, and 1.9 mg/L in GT 2/3, and did not change significantly across treatment phases. Patients with SVR had similar RBV concentrations compared to patients without SVR in both HCV genotype groups. SVR was not associated with RBV levels ≥2.0 mg/L (GT 1/4, OR 1.19 [0.5-2.86]; GT 2/3, 1.94 [0.78-4.80]) and ≥2.5 mg/L (GT 1/4, 1.56 [0.64-3.84]; GT 2/3 2.72 [0.85-8.73]), regardless of treatment phase, and IL28B genotype. CONCLUSION In HIV/HCV-coinfected patients treated with pegIFN/RBV, therapeutic drug monitoring of RBV concentrations does not enhance the chance of HCV cure, regardless of HCV genotype, treatment phase and IL28B genotype.

Accumulation rates of Mg, Al, Si, Mn, Fe, Ni, Cu, Zn, opal, and calcium carbonate have been calculated from their concentrations in samples from equatorial Deep Sea Drilling Project sites. Maps of element accumulation rates and of Q-mode factors derived from raw data indicate that the flux of trace metals to equatorial Pacific sediments has varied markedly through time and space in response to changes in the relative and absolute influence of several depositional influences: biogenic, detrital, authigenic, and hydrothermal sedimentation. Biologically derived material dominates the sediment of the equatorial Pacific. The distributions of Cu and Zn are most influenced by surface-water biological activity, but Ni, Al, Fe, and Mn are also incorporated into biological material. All of these elements have equatorial accumulation maxima similar to those of opal and calcium carbonate at times during the past 50 m.y. Detritus distributed by trade winds and equatorial surface circulation contributes Al, non-biogenic Si, Fe, and Mg to the region. Detrital sediment is most important in areas with a small supply of biogenic debris and low bulk-accumulation rates. Al accumulation generally increases toward the north and east, indicating its continental source and distribution by the northeast trade winds. Maxima in biological productivity during middle Eocene and latest Miocene to early Pliocene time and concomitant well-developed surface circulation contributed toward temporal maxima in the accumulation rates of Cu, Zn, Ni, and Al in sediments of those ages. Authigenic material is also important only where bulk-sediment accumulation rates are low. Ni, Cu, Zn, and sometimes Mn are associated with this sediment. Fe is almost entirely of hydrothermal origin. Mn is primarily hydrothermal, but some is probably scavenged from sea water by amorphous iron hydroxide floes along with other elements concentrated in hydrothermal sediments, Ni, Cu, and Zn. During the past 50 m.y. all of these elements accumulated over the East Pacific Rise at rates nearly an order of magnitude higher than those at non-rise-crest sites. In addition, factor analysis indicates that some of this material is carried substantial distances to the west of the rise crest. Accumulation rates of Fe in basal metalliferous sediments indicate that the hydrothermal activity that supplied amorphous Fe oxides to the East Pacific Rise areas was most intense during middle Eocene and late Miocene to early Pliocene time.

Miocene paleoceanographic evolution exhibits major changes resulting from the opening and closing of passages, the subsequent changes in oceanic circulation, and development of major Antarctic glaciation. The consequences and timing of these events can be observed in variations in the distribution of deep-sea hiatuses, sedimentation patterns, and biogeographic distribution of planktic organisms. The opening of the Drake Passage in the latest Oligocene to early Miocene (25-20 Ma) resulted in the establishment of the deep circumpolar current, which led to thermal isolation of Antarctica and increased global cooling. This development was associated with a major turnover in planktic organisms, resulting in the evolution of Neogene assemblages and the eventual extinction of Paleogene assemblages. The erosive patterns of two widespread hiatuses (PH, 23.0-22.5 Ma; and NH 1, 20-18 Ma) indicate that a deep circumequatorial circulation existed at this time, characterized by a broad band of carbonate-ooze deposition. Siliceous sedimentation was restricted to the North Atlantic and a narrow band around Antarctica. A major reorganization in deep-sea sedimentation and hiatus distribution patterns occurred near the early/middle Miocene boundary, apparently resulting from changes in oceanic circulation. Beginning at this time, deep-sea erosion occurred throughout the Caribbean (hiatus NH 2, 16-15 Ma), suggesting disruption of the deep circumequatorial circulation and northward deflection of deep currents, and/or intensification of the Gulf Stream. Sediment distribution patterns changed dramatically with the sudden appearance of siliceous-ooze deposition in the marginal and east equatorial North Pacific by 16.0 to 15.5 Ma, coincident with the decline of siliceous sedimentation in the North Atlantic. This silica switch may have been caused by the introduction of Norwegian Overflow Water into the North Atlantic acting as a barrier to outcropping of silica-rich Antarctic Bottom Water. The main aspects of the present oceanic circulation system and sediment distribution pattern were established by 13.5 to 12.5 Ma (hiatus NH 3), coincident with the establishment of a major East Antarctic ice cap. Antarctic glaciation resulted in a broadening belt of siliceous-ooze deposition around Antarctica, increased siliceous sedimentation in the marginal and east equatorial North Pacific and Indian Oceans, and further northward restriction of siliceous sediments in the North Atlantic. Periodic cool climatic events were accompanied by lower eustatic sea levels and widespread deep-sea erosion at 12 to 11 Ma (NH 4), 10 to 9 Ma (NH 5), 7.5 to 6.2 Ma (NH 6), and 5.2 to 4.7 Ma (NH 7).
