BACKGROUND Multiple sclerosis (MS) is a multifactorial disease with a genetic basis. The strongest associations with the disease lie in the Human Leukocyte Antigen (HLA) region. However, except for the DRB1*15:01 allele, the main risk factor associated to MS so far, no consistent effect has been described for any other variant. One example is HLA-DRB1*03:01, with a heterogeneous effect across populations and studies. We postulate that those discrepancies could be due to differences in the diverse haplotypes bearing that allele. Thus, we aimed at studying the association of DRB1*03:01 with MS susceptibility considering this allele globally and stratified by haplotypes. We also evaluated the association with the presence of oligoclonal IgM bands against myelin lipids (OCMB) in cerebrospinal fluid. METHODS Genotyping of HLA-B, -DRB1 and -DQA1 was performed in 1068 MS patients and 624 ethnically matched healthy controls. One hundred and thirty-nine MS patients were classified according to the presence (M+, 58 patients)/absence (M-, 81 patients) of OCMB. Comparisons between groups (MS patients vs. controls and M+ vs. M-) were performed with the chi-square test or the Fisher exact test. RESULTS Association of DRB1*03:01 with MS susceptibility was observed but with different haplotypic contribution, being the ancestral haplotype (AH) 18.2 the one causing the highest risk. Comparisons between M+, M- and controls showed that the AH 18.2 was affecting only M+ individuals, conferring a risk similar to that caused by DRB1*15:01. CONCLUSIONS The diverse DRB1*03:01-containing haplotypes contribute with different risk to MS susceptibility. The AH 18.2 causes the highest risk and affects only to individuals showing OCMB.

A sequential treatment design was chosen in this trial to ensure complete resistance to single-agent non-steroidal aromatase inhibitor (AI) and trastuzumab both given as monotherapy before receiving the combination of a non-steroidal AI and trastuzumab. Key eligibility criteria included postmenopausal patients with advanced, measurable, human epidermal growth factor receptor-2 (HER-2)-positive disease (assessed by FISH, ratio (≥2)), hormone receptor (HR)-positive disease, and progression on prior treatment with a non-steroidal AI, e.g. letrozole or anastrozole, either in the adjuvant or in the advanced setting. Patients received standard dose trastuzumab monotherapy in step 1 and upon disease progression continued trastuzumab in combination with letrozole in step 2. The primary endpoint was clinical benefit rate (CBR) in step 2. Totally, 13 patients were enrolled. In step 1, six patients (46%) achieved CBR. Median time to progression (TTP) was 161 days (95% confidence interval (CI): 82-281). In step 2, CBR was observed in eight out of the 11 evaluable patients (73%), including one patient with partial response. Median TTP for all the 11 patients was 188 days (95% CI: 77-not reached). Results of this proof-of-concept trial suggest that complete resistance to both AI and trastuzumab can be overcome in a proportion of patients by combined treatment of AI and trastuzumab, as all patients served as their own control. Our results appear promising for a new treatment strategy that offers a chemotherapy-free option for at least a subset of patients with HR-positive, HER-2-positive breast cancer over a clinically relevant time period.

Marijuana use has been associated with increased appetite, high caloric diet, acute increase in blood pressure, and decreases in high-density lipoprotein cholesterol and triglycerides. Marijuana is the most commonly used illicit drug in the United States, but its long-term effects on body mass index (BMI) and cardiovascular risk factors are unknown. Using 15 years of longitudinal data from 3,617 black and white young adults participating in the Coronary Artery Risk Development in Young Adults (CARDIA) study, we assessed whether marijuana use was associated with caloric intake, BMI, and cardiovascular risk factors. Of the 3,617 participants, 1,365 (38%) reported ever using marijuana. Marijuana use was associated with male gender, tobacco smoking, and other illicit drug use. More extensive marijuana use was associated with a higher caloric intake (2,746 kcal/day in never users to 3,365 kcal/day in those who used marijuana for > or = 1,800 days over 15 years) and alcohol intake (3.6 to 10.8 drinks/week), systolic blood pressure (112.7 to 116.5 mm Hg), and triglyceride levels (84 to 100 mg/dl or 0.95 to 1.13 mmol/L, all p values for trend < 0.001), but not with higher BMI and lipid and glucose levels. In multivariate analysis, the associations between marijuana use and systolic blood pressure and triglycerides disappeared, having been mainly confounded by greater alcohol use in marijuana users. In conclusion, although marijuana use was not independently associated with cardiovascular risk factors, it was associated with other unhealthy behaviors, such as high caloric diet, tobacco smoking, and other illicit drug use, which all have long-term detrimental effects on health.

Estudio de la estructura de las comunidades bentónicas sublitorales de fondo blando mediante el análisis de 11 muestras de draga colectadas entre los 03°38 ' y 09°16 'S, entre el 12 y 19 de mayo de 1995. Se determinó la biodiversidad a nivel de especies de macrobentos para las subáreas A, B, C, D. Se evaluó el nivel de similaridad entre las 11 estaciones analizadas a nivel de grupos taxonómicos (Polychaeta, Crustacea, Mollusca, Nemertinea y otros), así como la relación entre el nivel de estrés de las comunidades de las subáreas latitudinales A, B, C, D y la agremiación en el grupo dominante (Polychaeta). Fueron diferenciadas un total de 73 especies macrobénticas de las cuales el 64,38% pertenece a la clase Polychaeta. El patrón de distribución de las biomasas totales del macrobentos es más simple que el de las abundancias, con una tendencia a la ubicación de los valores máximos al norte de los 04° S. Se comprueba una relación inversa entre el grado de agremiación y el nivel de estrés medido con el índice SEP.

El Crucero 9702-04 de Evaluación hidroacústica de Recursos Pelágicos se desarrolló en dos etapas. Al sur, a bordo del BIC SNP-1 (Cr. 9702-03), y al norte, a bordo del BIC Humboldt (Cr. 9704). La prospección se desarrolló en un momento de rápida transición entre las condiciones frías imperantes desde la primavera de 1995, a las actualmente cálida de 1997. Las anomalías térmicas, que en un principio eran sólo consideradas como un calentamiento, fueron identificadas como una fuerte intromisión de Aguas Ecuatoriales Superficiales, cuyo avance fue calificado luego como un fenómeno El Niño de intensidad moderada al momento de culminar el crucero. Los ejemplares juveniles de anchoveta estuvieron ausentes en la zona norte. Se estimó que la población de anchoveta fluctuó, a fines de abril de 1997, entre 9 y 10 millones de toneladas. Se considera que el fenómeno puede afectar los procesos biológicos de desove y reclutamiento de los recursos pelágicos. Durante el crucero, la sardina mostró una clara proyección migratoria hacia el sur, mientras que el jurel y la caballa se dispersaron en gran parte del área de estudio.

La temperatura y salinidad en la superficie del mar, fluctuaron entre 15,3°C a 26,5°C y 28,0‰ a 35,4‰ respectivamente; los bajos valores de salinidad (<34,8‰) se ubicaron próximos a la costa, relacionados con la descarga de los ríos. Aguas Subtropicales Subsuperficiales (S‰>35,1), en forma de cuña se pegaron a la costa frente a Matarani y hasta aproximadamente 30 mn frente a San Juan, originando las mayores anomalías positivas del área de estudio (3,6°C a 4,9°C); anomalías negativas sólo se detectaron entre Callao y Pisco, en un rango de 0,7°C y 39,9%.
