The objectives of this study were to determine if protein-energy malnutrition (PEM) could affect the hematologic response to lipopolysaccharide (LPS), the interleukin-1β (IL-1β) production, leukocyte migration, and blood leukocyte expression of CD11a/CD18. Two-month-old male Swiss mice were submitted to PEM (N = 30) with a low-protein diet (14 days) containing 4% protein, compared to 20% protein in the control group (N = 30). The total cellularity of blood, bone marrow, spleen, and bronchoalveolar lavage evaluated after the LPS stimulus indicated reduced number of total cells in all compartments studied and different kinetics of migration in malnourished animals. The in vitro migration assay showed reduced capacity of migration after the LPS stimulus in malnourished animals (45.7 ± 17.2 x 10(4) cells/mL) compared to control (69.6 ± 7.1 x 10(4) cells/mL, P ≤ 0.05), but there was no difference in CD11a/CD18 expression on the surface of blood leukocytes. In addition, the production of IL-1β in vivo after the LPS stimulus (180.7 pg·h-1·mL-1), and in vitro by bone marrow and spleen cells (41.6 ± 15.0 and 8.3 ± 4.0 pg/mL) was significantly lower in malnourished animals compared to control (591.1 pg·h-1·mL-1, 67.0 ± 23.0 and 17.5 ± 8.0 pg/mL, respectively, P ≤ 0.05). The reduced expression of IL-1β, together with the lower number of leukocytes in the central and peripheral compartments, different leukocyte kinetics, and reduced leukocyte migration capacity are factors that interfere with the capacity to mount an adequate immune response, being partly responsible for the immunodeficiency observed in PEM.

OBJECTIVE: This study aims to evaluate the production of interferon-gamma and interleukin-10 by stimulated peripheral blood mononuclear cells isolated from patients with supraglottic laryngeal cancer before and after surgical treatment. METHODS: Fourteen patients with advanced supraglottic laryngeal cancer were studied. Cultures of peripheral blood mononuclear cells isolated during the preoperative and late postoperative periods were stimulated with concanavalin A and Bacille Calmette-Guerin, and the supernatant concentrations of interferon-gamma and interleukin-10 were measured. RESULTS: For non-stimulated cultures, the interferon-gamma levels produced by the preoperative period and the late postoperative period cultures were lower than the levels produced by the control group cultures. The interferon-gamma levels after stimulation with concanavalin A were higher in the late postoperative period cultures than in the preoperative evaluation cultures. Stimulation with Bacille Calmette-Guerin led to the production of similar levels of interferon-gamma and interleukin-10 by all cultures; thus, stimulation increased the levels of interferon-gamma produced by both the preoperative and postoperative cultures relative to the levels produced by the corresponding unstimulated cultures. CONCLUSION: Patients with advanced supraglottic laryngeal cancer exhibit an in vitro deficiency in interferongamma secretion by mononuclear cells. Stimulated cells seem to recover this function during the postoperative period.

Periodontitis is an inflammatory disease that results from an interaction between dental biofilm agents and the host immune-inflammatory response. Periodontopathogenic organisms, such as Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola, as well as the host's susceptibility, represented by the host's genetic makeup, are the key factors that influence this complex disease. Recently, we identified haplotypes in the IL4 gene that were associated with chronic periodontitis (CP). This study aimed to evaluate whether subjects with different IL4 haplotypes (TCI/CCI and TTD/CTI) would be differentially colonized by periodontopathogens and whether they would respond differently to non-surgical periodontal therapy. Thirty-nine patients carrying the IL4 haplotype of genetic susceptibility to CP (IL4+) or protection against CP (IL4-) were evaluated. Those groups were further subdivided into individuals with CP (CP IL4+ or CP IL4-) and those that were periodontally healthy (H) (H IL4+ or H IL4-). CP patients were submitted to non-surgical periodontal therapy. Clinical and microbiological analyses were performed considering the data at baseline and 45 and 90 days after periodontal therapy. Periodontopathogens levels were evaluated by absolute quantitative polymerase chain reaction (qPCR). The baseline data revealed that the total levels of periodontopathogens were higher in the CP IL4+ than in the CP IL4- groups. Clinical analyses revealed that the periodontal therapy was equally effective, independent of the subject's IL4 genetic load. The TCI/CCI IL4 haplotype, previously associated with genetic susceptibility to CP, was also associated with increased levels of periodontopathogenic bacteria, but this genetic background did not influence the response to non-surgical periodontal treatment.

The aim of this study was to investigate the effect of non-surgical treatment of periodontitis on the levels of periodontopathogens and clinical parameters in patients with different genetic backgrounds produced by polymorphisms in the Interleukin (IL8) gene. Thirty patients grouped according to IL8 ATC/TTC or AGT/TTC haplotypes were submitted to non-surgical periodontal treatment. Levels of Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola were determined in 240 subgingival plaque samples by qPCR. The association between IL8 haplotypes and the levels of periodontopathogens and clinical parameters was investigated by multilevel analysis accounting for the clustering of diseased sites analyzed within patients. It was observed that neither levels of periodontopathogens nor non-surgical treatment was associated with the IL8 haplotype. The clinical parameters after periodontal treatment were similar in diseased and healthy sites, independently of the IL8 haplotype. Nonetheless, in the same period, diseased sites of AGT/TTC patients harbored higher levels of P. gingivalis, T. denticola, T. forsythia, and red complex than those of ATC/TTC patients. However, the non-surgical periodontal therapy decreased the levels of these periodontopathogens and of the tested clinical parameters of diseased sites in both groups. Non-surgical therapy is equally effective in improving clinical parameters and decreasing the levels of periodontopathogens, independent of the genotype groups produced by the IL8 haplotype.

BACKGROUND: Intestinal ischemia/reperfusion (IR) injury is a serious and triggering event in the development of remote organ dysfunction, from which the lung is the main target. This condition is characterized by intense neutrophil recruitment, increased microvascular permeability. Intestinal IR is also responsible for induction of adult respiratory distress syndrome, the most serious and life-threatening form of acute lung injury. The purpose of this study was to investigate the effect of annexin-A1 protein as an endogenous regulator of the organ remote injury induced by intestinal ischemia/reperfusion. Male C57bl/6 mice were subjected to intestinal ischemia, induced by 45 min occlusion of the superior mesenteric artery, followed by reperfusion. RESULTS: The intestinal ischemia/reperfusion evoked a high intensity lung inflammation as indicated by the number of neutrophils as compared to control group. Treatment with annexin-A1 peptidomimetic Ac2-26, reduced the number of neutrophils in the lung tissue and increased its number in the blood vessels, which suggests a regulatory effect of the peptide Ac2-26 in the neutrophil migration. Moreover, the peptide Ac2-26 treatment was associated with higher levels of plasma IL-10. CONCLUSION: Our data suggest that the annexin-A1 peptidomimetic Ac2-26 treatment has a regulatory and protective effect in the intestinal ischemia/reperfusion by attenuation of the leukocyte migration to the lung and induction of the anti-inflammatory cytokine IL-10 release into the plasma. The anti-inflammatory action of annexin-A1 and its peptidomimetic described here may serve as a basis for future therapeutic approach in mitigating inflammatory processes due to intestinal

Die Familie der IL-6-Typ-Zytokine (IL-6, IL-11, CT-1, CNTF, LIF, OSM, BSF-3) ist durch eine vierhelikale Faltung gekennzeichnet. Alle Zytokine dieser Familie agieren über einen Rezeptorkomplex, der als gemeinsame Komponente mindestens ein Molekül gp130 enthält. IL-6 und IL-11 signalisieren über ein gp130-Homodimer, während CT-1, CNTF, LIF und OSM ein Heterodimer aus gp130 und dem strukturell verwandten LIFR oder, im Falle des OSM, auch OSMR verwenden. Die Rezeptoren der vierhelikalen Zytokine sind in ihrem extrazellulären Bereich modulartig aus Ig- und Fibronektin-Typ-III-ähnlichen Domänen aufgebaut. Sie besitzen als gemeinsame Struktureinheit ein zytokinbindendes Modul (CBM) aus zwei Fibronektin-Typ-III-ähnlichen Domänen, die durch vier konservierte Cysteine in der N-terminalen und ein konserviertes WSXWS-Motiv in der C-terminalen Domäne charakterisiert sind. Auf Zielzellen bindet IL-6 an den spezifischen IL-6 Rezeptor, worauf der Komplex aus IL-6/IL-6R mit dem Signaltransduktor gp130 assoziiert. Der IL-6R besteht in seinem extrazellulären Bereich aus drei Domänen. Die N-terminale Ig-ähnliche Domäne ist für die biologische Aktivität nicht notwendig. Die Domänen 2 und 3 bilden das CBM, welches auch in löslicher Form agonistisch wirkt. In der vorliegenden Arbeit wurden die strukturellen und funktionellen Eigenschaften der dritten extrazellulären Domäne des IL-6R untersucht. Das Protein läßt sich effizient in Bakterien exprimieren und in vitro renaturieren. Es konnte gezeigt werden, daß Domäne 3 für die Bindung an IL-6 ausreichend ist, der Komplex aus D3 und IL-6 jedoch nicht mehr mit dem gp130-Molekül assoziieren kann. Da der lösliche IL-6R (bestehend aus D2 und D3) in der Lage ist, an gp130 zu binden und ein biologisches Signal auszulösen, weisen diese Daten der C-terminalen CBM-Domäne (D3) eine ligandenbindende Funktion und der N-terminalen CBM-Domäne eine wichtige Rolle bei der Komplexbildung mit gp130 und Signalinduktion zu. Die gezeigte Expressions- und Renaturierungsstrategie für D3 wurde zur Markierung des Proteins mit 15N und 13C für die mehrdimensionale, heteronukleare NMR-Spektroskopie angewandt. Die hierdurch ermöglichte Strukturaufklärung von D3 als einer eindeutig in die Ligandenbindung involvierten Teilstruktur wird umfassendere strukturelle Informationen über den IL-6R-Komplex liefern, als es die bisherigen Mutations- bzw. Modellbaustudien konnten.

Interleukin-12 ist ein Schlüsselregulator zellvermittelter Immunantworten. In der vorliegenden Dissertation wurde die Regulation des IL-12 p40 Promotors in Monozyten und Makrophagen untersucht. Mit Hilfe von 'In Vivo Footprinting'-, Bandshift- und Transfektionsexperimenten konnte eine wichtige Funktion der Transkriptionsfaktoren NF-kappaB, C/EBP-beta und PU.1 bei der Aktivierung des Promotors gezeigt werden. 'In Vivo Footprinting'-Experimente führten auch zur Identifikation eines bisher nicht beschriebenen regulatorischen Elementes im IL-12 p40 Promotor. Dieses GA-12 genannte Motiv war in unstimulierten primären Monozyten protektiert, nicht jedoch in Zellen, die mit LPS oder LPS/ Interferon-gamma stimuliert wurden. Eine genauere Charakterisierung ergab, daß dieses Motiv eine repressorische Funktion auf den Promotor ausübt und in unstimulierten, nicht jedoch in stimulierten Monozyten und Makrophagen von einem spezifischen GATA-ähnlichen Komplex (GAP-12) erkannt wird. Zudem führte die Behandlung von Monozyten mit Interleukin-4 und Prostaglandin-E2, Inhibitoren der IL-12 Produktion, zu einer Verstärkung der Komplexbildung am GA-12 Motiv. Zur Analyse der Promotorregulation im nukleosomalen Umfeld wurden außerdem IL-12 p40 Promotor/ Luziferase transgene Mäuse hergestellt. Die Analyse dieser Mäuse zeigte, daß der proximale Promotor ausreichend Sequenzinformation beinhaltet, um in vivo die stimulations- und gewebespezifische Regulation des Gens zu gewährleisten. Anhand der erhobenen Daten konnte ein Modell der Regulation des IL-12 p40 Promotors in Monozyten und Makrophagen entworfen werden.

Chronic liver inflammation during viral hepatitis is a major health problem worldwide. The role of proinflammatory cytokines, like IL-12, in breaking hepatic immune tolerance, and inducing acute liver inflammation and virus clearance is not clear. Nor is clear its role in uncontrolled severe inflammatory response, leading to fulminant hepatitis and hepatic failure. This work, focused in the study of the role of endogenous produced IL-12 in inducing hepatic inflammatory responses, demonstrates: In vitro, using adenovirus coding for IL-12, that hepatocytes stimulate CD4+ T cells in a tolerogenic manner, and that endogenous IL-12 is able to switch the immune response into Th1; and in vivo, that endogenous IL-12 induces hepatocyte damage and virus elimination in mice infected with adenovirus. In addition, and in order to study in vivo the relevance of IL-12 in acute inflammation, conditional IL-12 transgenic mice expressing IL-12 in the liver after cre-recombinase mediated induction were generated. For this purpose, an IL-12 fusion protein was created, which demonstrated high levels of bioactivity. Induction of IL-12 expression during embryonic development was achieved by crossbreeding with Act-Cre transgenic mice; induction of IL-12 expression in adult mice was achieved by a plasmid coding for the cre-recombinase. This study demonstrates that after induction, IL-12 is expressed in the liver of the transgenic mice. It also demonstrates that hepatic expression of IL-12 induces splenomegaly and liver inflammation, characterized by large infiltrations in portal tracts and veins, associated with hepatic damage, necrosis areas and lethality. Furthermore, constitutive hepatic IL-12 expression does not lead to abortion, but to total lethality, short after delivery. In conclusion, in this study, a transgenic mouse model has been generated, in which the expression of active IL-12 in the liver can be induced at any time; this model will be very helpful for studying hepatic pathologies. This study has also demonstrated that hepatic produced IL-12 is able of breaking liver tolerance inducing inflammation, virus elimination, severe hepatocyte damage, and lethality. These findings suggest IL-12 as a key cytokine in acute liver inflammation and fulminant hepatic failure. 5.1 Future studies Once the importance of IL-12 in inducing hepatic inflammation and virus elimination was demonstrated in this study, understanding the mechanisms of the IL-12 induced liver damage, and more important, how to avoid it will be the main focus in the future. It is very important to achieve hepatic inflammation for a more effective and faster viral elimination, but avoiding the toxicity of IL-12, which leads to massive liver injury and lethality is obviously necessary to allow IL-12 as therapy. For that purpose, future studies will be mainly base on three different points: 1. The determination of different cell populations present in the hepatic infiltration, which of them are responsible for liver injury, and as well their state of activation. 2. The measure of other pro- and anti-inflammatory cytokines and chemokines, which can play a role in IL-12-induced liver inflammation and hepatocyte damage. For these purposes, specific blocking antibodies (anti TNF-alpha, anti IL-12, anti IFN-g) will be used. The study with different transgenic mice: TNF-alpha Receptor knockout, TGF-b, will also help in determining the role of those cytokines during IL-12-induced liver damage and lethality. 3. The establishing of liver pathology models (viral infection, tumours, auto-antigens) in mice. Induction of IL-12 at any time of the pathology development will help in clarifying the role of IL-12 in those models. Finally, the transgenic mice expressing IL-23 in the liver will be generated.

In der vorliegenden Arbeit wurde die Rolle von Interleukin-18 (IL-18) in der Leber untersucht. IL-18 wurde eine Bedeutung bei verschiedenen Lebererkrankungen zugeschrieben, die genauen Mechanismen sind jedoch nicht geklärt. IL-18 kann ein wesentlicher Faktor bei der Regulierung des Th1/Th2-Gleichgewichts in der Leber sein. Es wurden transgene Mäuse mit konstitutiver Überexpression der inaktiven Form von IL-18 (proIL-18) sowie transgene Mäuse mit Expression der aktiven Form von IL-18 ohne Signalpeptid (FLAG-IL-18) und mit Signalpeptid (SP-IL-18) in Hepatozyten unter der transkriptionellen Kontrolle des murinen Albumin-Promoters etabliert. In eigenen Vorarbeiten wurde eine konstitutive Expression von proIL-18 in Wildtyp-Hepatozyten festgestellt. Bei der Überexpression von proIL-18 in Hepatozyten zeigte sich eine Akkumulierung des Proteins in der Zelle und eine spontane entzündliche und fibrotische Aktivität in der Leber. Die Expression der aktiven Form von IL-18 ohne Signalpeptid (FLAG-IL-18) in Hepatozyten führte zu einer Speicherkrankheit in der Leber. Das Fusionieren des aktiven Teils von IL-18 mit IgG-k-Leadersequenz (SP-IL-18) ermöglichte die Sezernierung von IL-18 aus der Zelle ohne Einfluß auf die Produktion von IFN-g, TNF-a, IL-4 sowie Transaminasen. Die Rolle von IL-18 bei einem LPS-abhängigen Leberschaden mit und ohne Präkonditionierung mit CpG-Oligonukleotiden in vivo wurde untersucht. Die Behandlung proIL-18-transgener Mäuse mit LPS führte zur Sezernierung von biologisch aktivem IL-18 aus Hepatozyten. IL-18 hatte bei proIL-18- sowie bei SP-IL-18-transgenen Mäusen nach der LPS-Gabe keinen Einfluß auf die Leberschädigung und Produktion von IFN-g, TNF-a, IL-4. Nach sequenzieller Behandlung mit CpG und LPS fand sich bei SP-IL-18-transgenen Tieren im Gegensatz zu proIL-18-transgenen Mäusen eine stärkere Produktion von GOT, IFN-g und TNF-a im Vergleich zu Wildtyptieren. In dieser Arbeit konnte gezeigt werden, daß IL-18 alleine keine hepatotoxische Wirkung und keinen Einfluß auf die Produktion von IFN-g, TNF-a, IL-4 hat, wofür ein zusätzlicher Stimulus notwendig ist. Die in der vorliegenden Arbeit etablierten proIL-18- und SP-IL-18-transgenen Mausmodelle liefern die Grundlage zu weiteren Untersuchungen der Rolle von IL-18 in der Leber bei der Immunmodulation.
