BACKGROUND: We investigated clinical predictors of appropriate prophylaxis prior to the onset of venous thromboembolism (VTE). METHODS: In 14 Swiss hospitals, 567 consecutive patients (306 medical, 261 surgical) with acute VTE and hospitalization < 30 days prior to the VTE event were enrolled. RESULTS: Prophylaxis was used in 329 (58%) patients within 30 days prior to the VTE event. Among the medical patients, 146 (48%) received prophylaxis, and among the surgical patients, 183 (70%) received prophylaxis (P < 0.001). The indication for prophylaxis was present in 262 (86%) medical patients and in 217 (83%) surgical patients. Among the patients with an indication for prophylaxis, 135 (52%) of the medical patients and 165 (76%) of the surgical patients received prophylaxis (P < 0.001). Admission to the intensive care unit [odds ratio (OR) 3.28, 95% confidence interval (CI) 1.94-5.57], recent surgery (OR 2.28, 95% CI 1.51-3.44), bed rest > 3 days (OR 2.12, 95% CI 1.45-3.09), obesity (OR 2.01, 95% CI 1.03-3.90), prior deep vein thrombosis (OR 1.71, 95% CI 1.31-2.24) and prior pulmonary embolism (OR 1.54, 95% CI 1.05-2.26) were independent predictors of prophylaxis. In contrast, cancer (OR 1.06, 95% CI 0.89-1.25), age (OR 0.99, 95% CI 0.98-1.01), acute heart failure (OR 1.13, 95% CI 0.79-1.63) and acute respiratory failure (OR 1.19, 95% CI 0.89-1.59) were not predictive of prophylaxis. CONCLUSIONS: Although an indication for prophylaxis was present in most patients who suffered acute VTE, almost half did not receive any form of prophylaxis. Future efforts should focus on the improvement of prophylaxis for hospitalized patients, particularly in patients with cancer, acute heart or respiratory failure, and in the elderly.

The indication for pulmonary artery banding is currently limited by several factors. Previous attempts have failed to produce adjustable pulmonary artery banding with reliable external regulation. An implantable, telemetrically controlled, battery-free device (FloWatch) developed by EndoArt SA, a medical company established in Lausanne, Switzerland, for externally adjustable pulmonary artery banding was evaluated on minipigs and proved to be effective for up to 6 months. The first human implant was performed on a girl with complete atrioventricular septal defect with unbalanced ventricles, large patent ductus arteriosus and pulmonary hypertension. At one month of age she underwent closure of the patent ductus arteriosus and FloWatch implantation around the pulmonary artery through conventional left thoracotomy. The surgical procedure was rapid and uneventful. During the entire postoperative period bedside adjustments (narrowing or release of pulmonary artery banding with echocardiographic assessment) were repeatedly required to maintain an adequate pressure gradient. The early clinical results demonstrated the clinical benefits of unlimited external telemetric adjustments. The next step will be a multi-centre clinical trial to confirm the early results and adapt therapeutic strategies to this promising technology.

Objective: Single port laparoscopy andNOTES aim at decreasing the number¦of trocars, at a price of increased technical difficulty and cost, without until now¦any proof of benefit for the patient.Morbidity related to 5 mm trocar sites (TS)¦is claimed to be low, but there are no good quality data on this topic. The aim¦of the present prospective study is to measure the morbidity and overall specific¦impact related to the 5 mm TS and compare them to larger TS.¦Methods:Wecollected prospectively data on 300 consecutive patients operated¦by laparoscopy in our institution between 2009 and 2010. Pain, morbidity,¦cosmetic, and overall patient discomfort were assessed specifically for each¦TS, using standardized questionnaires, at 3 time points: at discharge from¦the hospital, at 1 month and at 6 months after surgery. Results were compared¦between 5 mmand larger TS (10 mm, 12 mmand 15 mm).Trocar sites replaced¦by a minilaparotomy or a stoma were excluded from analysis. In this study we¦present the short-term results.¦Results: Three-hundred patients (mean age 47·5, women 55%) were operated¦with 1074 TS of which 477(44%) were 5 mm TS. Indication to laparoscopy was¦cholecystectomy (31·3%), appendectomy (26·6%), upper GI surgery (16·3%),¦colon resection (13·3%) or other (12·3%). Follow-up at 1 month was completed¦in 90%.¦The 5 mm TS had an infection rate of 0·2%, and a hematoma rate of 1·7%.¦VAS pain scores at the 5mm TS were ≤3 in 91·6% at rest and in 75·9% upon¦effort at discharge, and in 97% at 1 month. Median patient scar assessment¦score (PSAS) of the 5 mm TS at 1 month was 6 (IQR: 2-9) out of 60 (0 =¦best score). Overall discomfort of the 5 mm TS in a VAS scale was 0 in 77%¦and ≤3 in 95% of patients at 1 month. Morbidity, pain assessments, PSAS, and¦overall discomfort scores were all significantly better for 5 mm TS compared to¦larger TS.¦Conclusion: Morbidity, pain, cosmetic impact and overall patient's discomfort¦related to a 5 mm trocar site is extremely low. For this reason, any potential¦advantage related to omitting 5 mm trocars to perform the same type of surgery¦will be difficult to demonstrate.

The development and use of chemotherapy for patients with prostate cancer was slow and modest in comparison with other solid tumor sites. While many cytotoxic agents and multiple drug combinations were tested during the 1980, the first recognized drug for men with metastatic castration-resistant prostate cancer (mCRPC) was mitoxantrone combined with prednisone in 1996. Mitoxantrone showed an improvement in quality of life and pain compared to men treated with prednisone alone. In 2004, two randomized controlled trials found docetaxel superior to mitoxantrone in PSA response and survival. Despite the somewhat higher side effect rate, quality of life and pain were also improved with docetaxel. Unfortunately only about every second man benefits from this treatment and there are no approved criteria to select patients with better chances of response. It takes about 3 months of treatment to identify non-responders and all patients will ultimately progress and most of them will die of prostate cancer. Many men with mCRPC were treated with mitoxantrone after progression on docetaxel without a strong evidence for such a choice. More recently, the benefit of a second-line chemotherapy was addressed within a randomized controlled trial. A new taxane, cabazitaxel, was found superior to mitoxantrone in terms of overall survival despite the previous treatment with docetaxel and therefore recently approved in this indication. The most important challenges and opportunities for future studies will be the combination of chemotherapy with hormonal or non-hormonal targeted agents, the establishment of treatment algorithms to best sequence docetaxel and cabazitaxel and non-cytotoxic agents. Patient selection criteria as well as early biomarkers of response or resistance would also be important for patients and clinicians. Toxicity and quality of life are particular important aspects of drug development in current and future studies which aim to improve treatment options in this frequently elderly patient population.

Chronic hepatitis B predisposes to the development of cirrhosis and hepatocellular carcinoma. Treatment of chronic hepatitis B is aimed at halting viral replication and, thereby, hepatic inflammation. Treatment indication should be established carefully and with full knowledge of the advantages and limitations of currently available antiviral drugs. Patients on long-term nudcleos(t)ide analogue treatment should be followed regularly in order to avoid the appearance of antiviral resistance. The purpose of this review is to provide a concise overview of the diagnosis and management of chronic hepatitis B.

Résumé : La clozapine (Leponex®) est un neuroleptique atypique utilisé depuis une quarantaine d'années (synthétisée en 1959) dans le traitement de la schizophrénie résistante. Son utilisation a fait l'objet de controverses importantes en raison de son profil d'effets secondaires, potentiellement fatals (agranulocytose), et a entraîné son retrait de certains pays pendant de nombreuses années, malgré certaines caractéristiques qui en font une molécule très intéressante sur le plan clinique. On peut citer à cet égard une efficacité antipsychotique importante tant sur les symptômes positifs que sur les symptômes négatifs de la maladie, ainsi que l'absence d'effets secondaires extrapyramidaux, tels que ceux entraînés par les neuroleptiques classiques. Depuis la réadmission de la clozapine par la FDA aux Etats-Unis en 1990, suite notamment aux travaux montrant l'amélioration, grâce à cette molécule, des dyskinésies tardives induites par les neuroleptiques classiques, on assiste à un regain d'intérêt marqué pour ses possibilités thérapeutiques. L'indication reste toutefois limitée à l'heure actuelle au traitement de la schizophrénie résistante à d'autres neuroleptiques, donc un traitement en deuxième intention. Sachant que l'évolution de la schizophrénie est influencée, entre autres facteurs, par la durée des manifestations symptomatiques, il nous a semblé intéressant d'évaluer le devenir de jeunes patients schizophrènes mis d'emblée au bénéfice d'un traitement par la clozapine. C'est le sujet de la présente étude. Cette étude a été initialement conçue comme une étude de cohorte, randomisée, en double aveugle, multi-site à l'échelle romande, avec un suivi des patients sur une année. Les difficultés rencontrées, qui ont abouti à une interruption précoce ne permettant le relevé que des observations de six patients, sont détaillées. Après une brève introduction sur la schizophrénie et sur la clozapine, ce travail décrit l'historique de l'étude, ses buts et sa méthodologie. Suit une description des six patients et leur évolution au cours de l'étude. Dans le chapitre IV, les résultats sont analysés et discutés dans une perspective descriptive. Précédant la bibliographie et les annexes, la conclusion fournit l'opportunité de discuter les apports et les limites de ce travail, en le resituant dans le contexte international actuel de regain d'intérêt pour l'utilisation de la clozapine dans le traitement de la schizophrénie.

BACKGROUND: The quality of colon cleansing is a major determinant of quality of colonoscopy. To our knowledge, the impact of bowel preparation on the quality of colonoscopy has not been assessed prospectively in a large multicenter study. Therefore, this study assessed the factors that determine colon-cleansing quality and the impact of cleansing quality on the technical performance and diagnostic yield of colonoscopy. METHODS: Twenty-one centers from 11 countries participated in this prospective observational study. Colon-cleansing quality was assessed on a 5-point scale and was categorized on 3 levels. The clinical indication for colonoscopy, diagnoses, and technical parameters related to colonoscopy were recorded. RESULTS: A total of 5832 patients were included in the study (48.7% men, mean age 57.6 [15.9] years). Cleansing quality was lower in elderly patients and in patients in the hospital. Procedures in poorly prepared patients were longer, more difficult, and more often incomplete. The detection of polyps of any size depended on cleansing quality: odds ratio (OR) 1.73: 95% confidence interval (CI)[1.28, 2.36] for intermediate-quality compared with low-quality preparation; and OR 1.46: 95% CI[1.11, 1.93] for high-quality compared with low-quality preparation. For polyps >10 mm in size, corresponding ORs were 1.0 for low-quality cleansing, OR 1.83: 95% CI[1.11, 3.05] for intermediate-quality cleansing, and OR 1.72: 95% CI[1.11, 2.67] for high-quality cleansing. Cancers were not detected less frequently in the case of poor preparation. CONCLUSIONS: Cleansing quality critically determines quality, difficulty, speed, and completeness of colonoscopy, and is lower in hospitalized patients and patients with higher levels of comorbid conditions. The proportion of patients who undergo polypectomy increases with higher cleansing quality, whereas colon cancer detection does not seem to critically depend on the quality of bowel preparation.

(Résumé de l'ouvrage) C'est grâce aux portulans - ces cartes géographiques qui permettaient la navigation de port à port avec l'indication exacte de l'échelle des distances - que les navigateurs purent disposer d'une représentation beaucoup plus fidèle des côtes maritimes. Le portulan a permis au marin d'abandonner le cabotage craintif qui l'obligeait à naviguer le long des côtes, toujours tenues à distance de regard, pour la navigation en haute mer. Aujourd'hui la représentation de l'Europe est une inconnue. Le cabotage n'est plus de mise, mais le portulan manque. Nul ne peut dire où il faut commencer et où il faut jeter l'ancre. L'Europe est comme libérée de ses voeux, débordée en mille endroits. Europes intempestives présente neuf textes qui dressent une carte en filigrane, selon les termes d'une échelle inconnue qui doit faire passer pour saugrenue toute personne qui demande : « Qu'est-ce que l'Europe ? ». Il s'agit pour nous de sortir du cadastre de ce qu'il faut appeler une rhétorique Europe, avec ses métaphores fatiguées, frêles esquifs sur une mer de clichés : cap, parapet, occident, coucheries solaires et rapt.

PURPOSE OF REVIEW: Definition of T cell immune correlates in HIV infection remains a lofty goal towards our understanding of the HIV-specific immune response. This review will focus upon recent developments and controversies in our understanding of protective T cell responses against HIV. RECENT FINDINGS: It has become clear that multiple functions and phenotypic markers of T cells must be assessed to accurately characterize the complexity of CD4 and CD8 T cell responses. While evidence indicates that a hallmark of protective immune responses in HIV infection is the presence of 'polyfunctional' T cell responses, a disconnect remains between the function and phenotype of effective HIV-specific T cells. Moreover, there may be inherent differences in the ability of specific human leukocyte antigen class I families to promote CD8 T cell effector versus polyfunctional responses. It remains to be determined how polyfunctional responses arise in HIV infection, which functions are important for control, and whether surface phenotype markers provide an indication of protective capacity. SUMMARY: Polyfunctional and phenotypic assessment of T cell responses have clearly advanced our understanding of HIV specific immune responses. Critical questions remain, however, especially whether polyfunctional T cell responses control, or are controlled by, HIV replication.

Allele frequencies for 17 STR loci were analyzed in a sample of unrelated males from the Cabo Verde Archipelago. The samples were gathered in such a way that the origin of the subjects was perfectly identified, and they could be included in one of the leeward or windward groups of islands. This study reveals that there are significant differences between both groups of islands, and between Cabo Verdeans and other populations from sub-Sahara Africa including the Guineans, the most probable source population for Cabo Verdeans. This study confirms mtDNA data and, together with HLA and Y chromosome data already published, shows that the Cabo Verde population is substructured and atypical, diverging substantially from mainland sub-Saharan populations. Overall these differences are most probably due to admixture between sub-Saharan slaves brought into the islands and other settlers of European origin. In the absence of a clear indication of a different ethnic composition of the first sub-Saharan settlers of Cabo Verde, the differentiation exhibited in both groups of islands can be most probably be attributed to genetic drift.

Headcuts (known also as primary knickpoints) and knickpoints (known also as secondary knickpoints) have been found to contribute to the accelerated riverbed degradation problem in the midwestern United States. Step-changes that occur at the head of channel networks are referred to as headcuts, and those that occur within the confines of channel banks are referred to as knickpoints. The formation of headcuts and knickpoints and their upstream migration have been linked to the over-steepening of stream reaches when the flow plunges to the bed and creates a plunge pool. Secondary flow currents and seepage are believed to be some other parameters contributing to the formation and evolution of headcuts and knickpoints. Ongoing research suggests that headcuts and knickpoints, where they form and migrate, may account for 60% (or more) of the bed erosion in the streams. Based on preliminary observations, there is a strong indication that headcuts and knickpoints can also have a greater influence on flow thalweg alignment (line of deepest flow) for small rivers. A shift in thalweg toward a riverbank or embankment is usually a prime factor contributing to riverbank erosion and scour.
