Este trabalho tem o objetivo de ampliar conceitualmente a clínica, não para formular um modelo clínico, mas para promover uma forma de pensar sobre a intervenção clínica. Diz de um ponto de vista, que corresponde a substituição da noção de origem e de retorno à origem, que seria a valorização da representação, para dar relevância à expressão do corpo, como apreensão das forças existentes. Sustenta-se na linguagem corporal como constitutiva, que contrapõe o corpo como afecção ao corpo dos afetos e dos sentimentos, trazendo à tona a comunicação, que está associada ao originário, ao impessoal, onde o corpo é mudo, onde ele ainda não é; operando, portanto, uma fundação primeira. Nesta concepção, o corpo ganha um novo sentido. É o grande personagem conceitual, com função de ferramenta que opera como um heterônimo. Recoloca-se o corpo no plano da impessoalidade do ser; tratar-se-ia de esquecer a história do eu para dar passagem ao pré-individual e ao impessoal que já está e se potencializa no encontro dos afectos. Privilegia a região do pré-representativo, como um fundo indeterminado, que funciona como tendências, marcas, traços dos sujeitos; o tempo como duração que potencializa virtualidades no encontro. A produção de sentido desliza da origem para o entre os sujeitos. É na relação que se engendra o sujeito. No entre, onde há apenas o vazio, o não-ser, emergirá o sujeito na sua singularidade. A escuta clínica sustentar-se-ia nessa linguagem do corpo, um inconsciente que não conhece palavras, porque é corporal, indizível e invisível. Aquilo que não remete a nenhum significado, requer uma outra relação com a clínica, que se daria naquilo que não temos para pensar, mas antes, dando passagem ao que não tem nome, ao impensável, ao inominável, e que, ao fazê-lo produz o sentido.

This study aimed at assessing the interobserver reliability of the Brazilian-Portuguese version of the Berg Balance Scale (BBS). The assessment was made by physiotherapists with extensive or little clinical experience in non-institutionalized elderly individuals. Participants comprised 12 elderly subjects (10 women and 2 men) with mean ages of 75.8 ± 8.4 years (range = 63-87) and 18 physiotherapists with varying clinical experience. Inter-examiner reliability obtained for each scale item yielded weighted kappa value > 0.75 in 11 of the 14 items (varying from 0.37 to 1.0). The intra-class correlation coefficient (ICC) for the total sum of BBS scores between the two groups of physiotherapists was 0.996 (95% confidence interval, 0.987 0.999) with a Cronbach alpha coefficient of 0.996. We found no difference between the rater groups when we compared the sum score means obtained with the student s T-test (p = 0.86). Although some items had low reliability values, in general our results suggest that the Brazilian Version of the BBS showed good levels of interrater reliability and agreement when used by physiotherapists with different clinical practice levels and without previous training on non-institutionalized elderly patients. We concluded that the BBS can be useful as an important evaluation instrument on a protocol for Rehabilitation clinics. It may be used by various health professionals, as: Physicians, Physical therapists, Physical educators, Occupational therapists, Nurses and Phonoaudiologists, so confirming the interdisciplinary character of this study

Introduction: The chronic immunosuppression promotes the development of changes in the oral cavity of the kidney transplant recipients (KTR), however with the exception of gingival overgrowth, little is known regarding the prevalence of oral lesions in this population. Objective: To evaluate the prevalence of clinical and microbiological conditions of the oral cavity of the recipients of kidney transplantation and the associated factors. Methods: This was a cross-sectional study which examined 96 clinical KTR and experimental where collected saliva stimulated them to identify Candida sp. Data collection consisted of evaluation stomatologic, socio-demographic, clinical aspects of transplantation, condition of oral hygiene and dental caries, yonder to questions about knowledge of oral alteration after transplantation. Results: Of the total, 66.7% of KTR had some type of oral manifestation. The most common was saburral tongue, followed by gingival overgrowth, with both oral manifestations related to gender and concomitant use of cyclosporine and nifedipine (p <0.05). Tacrolimus showed a protective effect for gingival overgrowth (OR = 0.13). The oral hygiene was associated with saburral tongue(p = 0.03) and severity of gingival overgrowth (p = 0.0001). Oral candidiasis was diagnosed in 17.7% of patients and Candida albicans was isolated most frequently in the saliva of RTR with a colonization of 58.3%. The average DMF-T increased with age. The method of oral hygiene was most used brush and toothpaste to 61.5%. Changes in the oral cavity was seen in 54.2% of KTR, citing as the main growth and gingival ulcers. Instructions for oral hygiene after transplantation were neglected for 61.5% of RTR. Dry mouth and halitosis were reported in 30.2% and 36.5% respectively. Conclusions: More the half of the KTR had at least one injury of the mouth, the immunosuppressive drugs and oral hygiene are associated with these alterations. Prospective cohort studies are needed to elucidate the relationship between oral manifestations and levels of drug and risk of oral manifestations occur over time. The kidney transplant recipients showed to be aware of oral alterations occurred after transplantation and uninformed about the oral hygiene instructions. With regard to hygiene, the incidence of caries was considered high, conditions of risk were identified and improvements in primary attention should be encouraged and reflected in the monitoring of renal transplant
