Objective: The aim of this study was to compare the factors of adherence to physical activity in subjects attending a cardiac rehabilitation program, and subjects who have withdrawal this same program using the Transtheoretical Model of behavior change. Methods: We conducted an observational, cross sectional type study, with a sample of 33 individuals (15 currently participating in the Cardiac Rehabilitation Program and 18 who no more attended the same program), with the questionnaires being personally delivered or sent by mail. For data analysis, we used the computer program SPSS® version 16.0. The significance level was set at 0.05. Results: There were no significant differences in the states of Change, Self-efficacy, Decisional Balance and Change Processes in both groups. We obtained a high Spearman correlation between States of Change and Self-efficacy (r2 = 0.778) and the Pros (r2 = 0.764) and Againsts (r2 = -0.744) in Decisional Balance. However, there were no significant evidence to affirm that States of Change and experiential processes of change (p = 0.465) andbehavioral (p = 0.300) had a correlation. A relationship was found, in terms of proportions between physical activity incorporated or not in a Cardiac Rehabilitation Program and age (p = 0.003), occupation (p = 0.010) and the entity paying the costs of program (p = 0.027). Conclusion: It was concluded that perceived self-efficacy and Pros and Againsts of the Decisional Balance are related to adherence to physical activity. Results also point out that age, profession and the entity paying the costs of the program influences the dropout of Cardiac Rehabilitation Programs.

Postural control deficits are the most disabling aspects of Parkinson's disease (PD), resulting in decreased mobility and functional independence. The aim of this study was to assess the postural control stability, revealed by variables based on the centre of pressure (CoP), in individuals with PD while performing a sit-to-stand-to-sit sequence under single- and dual-task conditions. An observational, analytical and cross-sectional study was performed. The sample consisted of 9 individuals with PD and 9 healthy controls. A force platform was used to measure the CoP displacement and velocity during the sit-to-stand-to-sit sequence. The results were statistically analysed. Individuals with PD required greater durations for the sit-to-stand-to-sit sequence than the controls (p < 0.05). The anteroposterior and mediolateral CoP displacement were higher in the individuals with PD (p < 0.05). However, only the anteroposterior CoP velocity in the stand-to-sit phase (p = 0.006) was lower in the same individuals. Comparing the single- and dual-task conditions in both groups, the duration, the anteroposterior CoP displacement and velocity were higher in the dual-task condition (p < 0.05). The individuals with PD presented reduced postural control stability during the sit-to-stand-to-sit sequence, especially when under the dual-task condition. These individuals have deficits not only in motor performance, but also in cognitive performance when performing the sit-to-stand-to-sit sequence in their daily life tasks. Moreover, both deficits tend to be intensified when two tasks are performed simultaneously.

To improve surgical safety, and to reduce the mortality and surgical complications incidence, the World Health Organization (WHO) developed the Surgical Safety Checklist (SSC). The SSC is a support of information that aids health professionals to reduce the number of complications, induction of anaesthesia, period before skin incision and period before leaving the operating room (OR). The SSC was tested in several countries of the world and their results shown that after introduction of the SSC the incidence of patient complication lowered from 11.0% to 7.0% (P<0.001), the rate of death declined from 1.5% to 0.8% (P = 0.003) and the nurses recognized that patients identity was more often con rmed (81.6% to 94.2%, P<0.01) in many institutions. Recently the SSC was also implemented in Portuguese hospitals, which led us to its study in the real clinical environment. An observational study was performed: several health professionals were observed and interviewed, to understand the functioning of the SSC in an OR, during the clinical routine. The objective of this study was to understand the current use of the SSC, and how it may be improved in terms of usability, taking advantage of the technological advancements such as mobile applications. During two days were observed 14 surgeries, only 2 surgeries met the requirements for the three phases of the SSC, as de ned by the WHO. Of the remaining 12 observed surgeries, 9 surgeries completed the last phase at the correct time. It was also observed that only in 2 surgeries all the phases of the SSC were read aloud to the team and that, in 7 surgeries, several items were read aloud and answered but no one was checking the SSC, only after the end of the phase. The observational study results disclose that several health professionals do not meet with rules of the WHO manual. This study demonstrates that it is urgent to change the mindset of health professionals, and that di erent features in the SSC may be useful to make it more easy to use. With the results of the observational study, a SSC application proposal was developed with new functionalities to improve and aid the health professional in its use. In this application the user can chose between a SSC already created to a speci c surgery or to create a new SSC, adding and adapting some questions from the WHO standard. To create a new SSC, the application is connected to an online questionnaire builder (JotForm). The choice for this online questionnaire builder went through three essential characteristics: number of types of questions, mainly checkbox, radio button and text; the possibility of to create sections inside sections and the API. In addition, in this proposal the improvements are focused in forcing the user to focus in the work ow of the SSC and to save the input timestamps and any actions made by them. Therefore, the following features was implemented to achieve that goal: display one item of the SSC at a time; display the stage where the SSC is; do not allow going back to the previous step; do not allow going forward to the next item if the current is not lled; do not allow going forward to the next item if the time it took to ll the item was too short and log any action made by the user.

To determine the prevalence of intestinal microsporidiosis in HIV-infected patients, we performed a prospective study of HIV-infected patients with diarrheal illnesses in three US hospitals and examined an observational database of HIV-infected patients in 10 US cities. Among 737 specimens from the three hospitals, results were positive for 11 (prevalence 1.5%); seven (64%) acquired HIV through male-to-male sexual contact, two (18%) through male-to-male sexual contact and injection drug use, and one (9%) through heterosexual contact; one (9%) had an undetermined mode of transmission. Median CD4 count within six months of diagnosis of microsporidiosis was 33 cells/µL (range 3 to 319 cells/µL). For the national observational database (n = 24,098), the overall prevalence of microsporidiosis was 0.16%. Prevalence of microsporidiosis among HIV-infected patients with diarrheal disease is low, and microsporidiosis is most often diagnosed in patients with very low CD4+ cell counts. Testing for microsporidia appears to be indicated, especially for patients with very low CD4+ cell counts.

The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.

Objectives: Literature review of classification, epidemiology, pathophysiology, microbiology, clinical presentation, diagnosis, complications, treatment and prevention of urinary tract infections (UTI) in pregnancy. Data Sources and Review Methods: Bibliographic research in Medline, through PubMed and Medscape, of systematic reviews, observational studies, clinical guidelines, meta-analyses and randomized controlled trials published between January 1992 and December 2010. Results: Asymptomatic bacteriuria occurs in 2 to 10% of pregnant women, 30% of which will develop acute pyelonephritis if left untreated. Treatment of asymptomatic bacteriuria has been shown to reduce the risk of pyelonephritis in pregnancy. Therefore, screening and treatment of this form of UTI has become a standard of obstetrical care, although the ideal duration of the treatment is still controverse. Acute pyelonephritis is one of the most common medical complications of pregnancy and may be associated with maternal, obstetric and perinatal complications. There is no clear consensus in the literature regarding antibiotic choice or duration of therapy for symptomatic UTI. Increasing antibiotic resistance complicates the choice of empirical regimens and local resistance rates need to be taken into account when deciding the therapy. Considering the high rate of recurrence of UTI during pregnancy, prophylactic measures need to be taken in pregnant women who have already experienced UTI during their current pregnancy. Conclusions: Although UTI is a generally benign condition in non-pregnant women, it may be a potentially serious complication during pregnancy. Early diagnosis and treatment of UTI during pregnancy are mandatory and can prevent severe maternal and perinatal complications.

AIMS: An epidemiological survey of diabetic foot infections (DFIs) in Lisbon, stratifying the bacterial profile based on patient demographical data, diabetic foot characteristics (PEDIS classification), ulcer duration and antibiotic therapy. METHODS: A transversal observational multicenter study, with clinical data collection using a structured questionnaire and microbiological products (aspirates, biopsies or swabs collected using the Levine method) of clinically infected foot ulcers of patients with diabetes mellitus (DM). RESULTS: Forty-nine hospitalized and ambulatory patients were enrolled in this study, and 147 microbial isolates were cultured. Staphylococcus was the main genus identified, and methicillin-resistant Staphylococcus aureus (MRSA) was present in 24.5% of total cases. In the clinical samples collected from patients undergoing antibiotic therapy, 93% of the antibiotic regimens were considered inadequate based on the antibiotic susceptibility test results. The average duration of an ulcer with any isolated multi-drug resistant (MDR) organism was 29 days, and previous treatment with fluoroquinolones was statistically associated with multi-drug resistance. CONCLUSIONS: Staphylococcus aureus was the most common cause of DFIs in our area. Prevalence and precocity of MDR organisms, namely MRSA, were high and were probably related to previous indiscriminate antibiotic use. Clinicians should avoid fluoroquinolones and more frequently consider the use of empirical anti-MRSA therapy.

Introduction: Antibiotics are one of the most common prescribed drugs in the NICU; despite this, studies on its use are scarce. Aim: To assess antibiotics utilization ratio in a medical surgical NICU. Methods: Prospective, observational study. Daily registry of antibiotics given to newborn infants; two periods of two months, 2010; data collected every day after the second medical round. Variables: treated patients, days on antibiotics, treatment/patient days, number of courses, number of antibiotics. Antibiotics utilization ratio – ratio days on antibiotics/days at the NICU. Results: Patients enrolled - 113; admission days – 1722; length of stay - 15.2 days; 85 newborn infants were given antibiotics; days on antibiotics - 771; antibiotics utilization ratio – 44.8; 292 antibiotics were prescribed; 61.8% of patients were given more than two antibiotics and 15.3% had more than one course. The most frequents were gentamicin, cefotaxime, ampicillin, vancomycin and metronidazole. Conclusion: Antibiotics utilization ratio should be subject of audits and a quality criteria on NICUs evaluation.

This work attempts to establish dermatological identification patterns for Brazilian cnidarian species and a probable correlation with envenoming severity. In an observational prospective study, one hundred and twenty-eight patients from the North Coast region of São Paulo State, Brazil were seen between 2002 and 2008. About 80% of these showed only local effects (erythema, edema, and pain) with small, less than 20 cm, oval or round skin marks and impressions from small tentacles. Approximately 20% of the victims had long, more than 20 cm, linear and crossed marks with frequent systemic phenomena, such as malaise, vomiting, dyspnea, and tachycardia. The former is compatible with the common hydromedusa from Southeast and Southern Brazil (Olindias sambaquiensis). The long linear marks with intense pain and systemic phenomena are compatible with envenoming by the box jellyfish Tamoya haplonema and Chiropsalmus quadrumanus and the hydrozoan Portuguese man-of-war (Physalis physalis). There was an association between skin marks and probable accident etiology. This simple observation rule can be indicative of severity, as the Cubozoa Class (box jellyfish) and Portuguese man-of-war cause the most severe accidents. In such cases, medical attention, including intensive care, is important, as the systemic manifestations can be associated with death.

Introduction. IgA nephropathy is the dominant primary glomerular disease found throughout the majority of the world’s developed countries. Accurately identifying patients who are at risk of progressive disease is challenging. We aimed to characterise clinical and histological features that predict poor prognosis in adults. Patients and Methods. We performed a single-centre retrospective observational study of biopsy-proven IgA nephropathy. The primary outcome was renal survival and death from any cause, and the secondary outcome was proteinuria remission. Results. Data from 49 cases were available for analysis with a median follow-up of 4 years. There were no deaths. Univariable analyses identified acute renal failure, low estimated glomerular filtration rate for ≥3 months (low eGFR), arterial hypertension, baseline proteinuria, glomerular sclerosis >50% and interstitial fibrosis >50% as poor prognostic markers. Low eGFR persisted significant by multivariable model that used only clinical parameters. Multivariable models with histopathologic parameters observed that tubular atrophy/interstitial fibrosis >50% was independently associated with the primary outcome. Proteinuria remission throughout follow-up had no prognostic value in our revision. Conclusions. Two independent predictors of poor renal survival at time of biopsy were found: low eGFR and tubular atrophy/interstitial fibrosis >50%.

Objective: To investigate the results of vaginal obliterate surgery in elderly women with pelvic organ prolapse. Design: observational retrospective study. Material and Methods: a total of 69 women with the diagnosis of pelvic prolapse were submitted to obliterative surgery in the urogynecology unit of a tertiary care hospital centre over the course of 8 years (2001 to 2008). The following data were collected from their clinical records: age, number of vaginal births, body mass index (BMI), hormone therapy, other existing diseases, type of prolapse and stage, anaesthetic risk score, duration of surgery, length of hospital stay, and short-term complications. Results: Of the 69 women studied, 31 were submitted to colpocleisis and the remaining 38 were managed by the LeFort technique. Mean age was 74.8 years with a standard deviation (sd) of 7.14 years. Average BMI was 26.2 (sd =3.76). Vaginal births were recorded in all patients. Only three patients were taking hormone therapy at the time of surgery. Sixty-three women were classified as having and anesthetic risk of II or III and 55 underwent local-regional anesthesia. Complications were reported in five cases, four of which in the first days after surgery. Nearly all were mild and resolved within the first 6 weeks. Conclusion: Complication rates appear to be low after obliterative surgery for pelvic organ prolapse in elderly women.

Transplant glomerulopathy is a sign of chronic kidney allograft damage. It has a distinct morphology and is associated with poor allograft survival. We aimed to assess the prevalence and clinic-pathologic features of transplant glomerulopathy, as well as determine the functional and histological implications of its severity. We performed a single-centre retrospective observational study during an eight-year period. Kidney allograft biopsies were diagnosed and scored according to the Banff classification, coupled with immunofluorescence studies. The epidemiology, clinical presentation, outcomes (patient and graft survival) and anti-HLA alloantibodies were evaluated. Transplant glomerulopathy was diagnosed in 60 kidney transplant biopsies performed for clinical reasons in 49 patients with ABO compatible renal transplant and a negative T-cell complement dependent cytotoxicity crossmatch at transplantation. The estimated prevalence of transplant glomerulopathy was 7.4% and its cumulative prevalence increased over time. C4d staining in peritubular capillaries (27.6%) was lower than the frequency of anti-HLA antibodies (72.5%), the majority against both classes I and II. Transplant glomerulopathy was associated with both acute (mainly glomerulitis and peritubular capillaritis) and chronic histologic abnormalities. At diagnosis, 30% had mild, 23.3% moderate and 46.7% severe transplant glomerulopathy. The severity of transplant glomerulopathy was associated with the severity of interstitial fibrosis. Other histological features, as well as clinical manifestations and graft survival, were unrelated to transplant glomerulopathy severity.

Introduction: Skin diseases in paediatric age are often distressing conditions with significant impact in children’s psychosocial development. Additionally, systemic therapeutic options are often limited in childhood, due to its potential toxicity in this vulnerable group. Phototherapy is therefore an endorsed option for photo-responsive dermatological conditions. Objective and Methods:This observational retrospective study aims to access efficacy and safety of Phototherapy in our paediatric population. Relevant clinical data from 1996 to present concerning patients aged 18 years or less was collected. Results: 78 patients were included, of which64,1%was female. Mean age was 12,9 years (range 2-18). Distribution according to diagnosis was:47,4%psoriasis, 34,6% alopecia areata, 9,0% vitiligo, 9,0% other diagnosis. Mean number of cycles was 1,5 (range 1-7), with an average of 16,3 treatments per cycle and mean cumulative dose 134 J/cm2. 70,5% was treated with one single cycle. Topic and systemic PUVA were the first choice in 37,2% and 39,7%, respectively, while UVB TL01 and broadband UVB were used in 11,5% each. On the first cycle 67,5% improved, 14,3% showed no sustained clinical response and 19,5% were lost to follow-up. Psoriasis patients had the best response rates (81,8%), followed by alopecia areata (59,3%). Side effects occurred in 21%, being erythema the most common (12%). None led to therapeutic interruption. Discussion: Phototherapy is a safe and effective option in childhood, yet the withdraw rate might be an important limitation.

Introduction. Pulmonary arterial hypertension (PAH) is a rare disease that must be managed in specialized centers; therefore, the availability of epidemiological national data is critical. Methods. We conducted a prospective, observational, and multicenter registry with a joint collaboration from five centers from Portugal and included adult incident patients with PAH or chronic thromboembolic pulmonary hypertension (CTEPH). Results. Of the 79 patients enrolled in this study, 46 (58.2%) were classified as PAH and 33 patients (41.8%) as CTEPH. PAH patients had a mean age of 43.4 ± 16.4 years. Idiopathic PAH was the most common etiology (37%). At presentation, PAH patients had elevated right atrial pressure (RAP) (7.7 ± 5.9mmHg) and mean pulmonary vascular resistance (11.4 ± 6.5 Wood units), with a low cardiac index (2.7 ± 1.1 L⋅min−1 m−2); no patient was under selective pulmonary vasodilators; however, at follow-up, most patients were on single (50%), double (28%), or triple (9%) combination vasodilator therapy. One-year survival was 93.5%, similar to CTEPH patients (93.9%), that were older (60.0 ± 12.5 years) and had higher RAP (11.0 ± 5.2mmHg,

RESUMO:Introdução: Reviu-se o conhecimento epidemiológico, fisiopatológico e clínico atual sobre a doença coronária, da sua génese até ao evento agudo, o Enfarte Agudo do Miocárdio (EAM). Valorizou-se, em especial, a teoria inflamatória da aterosclerose, que foi objeto de grandes desenvolvimentos na última década. Marcadores de instabilidade da placa aterosclerótica coronária: Aprofundou-se o conhecimento da placa aterosclerótica coronária instável. Descreveram-se detalhadamente os biomarcadores clínicos e laboratoriais associados à instabilidade da placa, com particular ênfase nos mecanismos inflamatórios. Objetivos:Estão divididos em dois pontos fundamentais:(1) Estudar em doentes com EAM a relação existente entre as moléculas inflamatórias: Interleucina-6 (IL-6), Fator de Necrose Tumoral-α (TNF-α) e Metaloproteinase de Matriz-3 (MMP3), não usados em contexto clínico, com um marcador inflamatório já em uso clínico: a Proteína C-Reativa ultrassensível (hs-CRP). Avaliar a relação de todas as moléculas inflamatórias com um biomarcador de lesão miocárdica: a Troponina Cardíaca I (cTnI). (2) Avaliar, no mesmo contexto de EAM, a Resposta de Fase Aguda (RFA) . Pretende-se demonstrar o impacto deste fenómeno, com repercussão clínica generalizada, no perfil lipídico e nos biomarcadores inflamatórios dos doentes. Métodos:(1) Estudo observacional prospetivo de doentes admitidos consecutivamente por EAM (grupo EAM) numa única unidade coronária, após exclusão de trauma ou infeção. Doseamento no sangue periférico, na admissão, de IL-6, TNF-α, MMP3, hs-CRP e cTnI. Este último biomarcador foi valorizado também nos valores séricos obtidos 6-9 horas depois. Procedeu-se a correlação linear (coeficiente de Pearson, de Rho-Spearman e determinação do R2) entre os 3 marcadores estudados com os valores de hs-CRP e de cTnI (valores da admissão e 6 a 9 horas após). Efetuou-se o cálculo dos coeficientes de regressão linear múltipla entre cTnI da admissão e cTnI 6-9h após, com o conjunto dos fatores inflamatórios estudados. (2) Estudo caso-controlo entre o grupo EAM e uma população aleatória de doentes seguidos em consulta de cardiologia, após exclusão de eventos cardiovasculares de qualquer território (grupo controlo) e também sem infeção ou trauma. Foram doseados os mesmos marcadores inflamatórios no grupo controlo e no grupo EAM. Nos dois grupos dosearam-se, ainda, as lipoproteínas: Colesterol total (CT), Colesterol HDL (HDLc), com as suas subfrações 2 e 3 (HDL 2 e HDL3), Colesterol LDL oxidado (LDLox),Triglicéridos (TG), Lipoproteína (a) [Lp(a)], Apolipoproteína A1 (ApoA1), Apolipoproteína B (ApoB) e Apolipoproteína E (ApoE). Definiram-se, em cada grupo, os dados demográficos, fatores de risco clássicos, terapêutica cardiovascular e o uso de anti-inflamatórios. Procedeu-se a análise multivariada em relação aos dados demográficos, fatores de risco e à terapêutica basal. Compararam-se as distribuições destas mesmas caraterísticas entre os dois grupos, assim como os valores séricos respetivos para as lipoproteínas estudadas. Procedeu-se à correlação entre as moléculas inflamatórias e as lipoproteínas, para todos os doentes estudados. Encontraram-se os coeficientes de regressão linear múltipla entre cada marcador inflamatório e o conjunto das moléculas lipídicas, por grupo. Finalmente, efetuou-se a comparação estatística entre os marcadores inflamatórios do grupo controlo e os marcadores inflamatórios do grupo EAM. Resultados: (1) Correlações encontradas, respetivamente, Pearson, Rho-Spearman e regressão-R2: IL-6/hs-CRP 0,549, p<0,001; 0,429, p=0,001; 0,302, p<0,001; MMP 3/hsCRP 0,325, p=0,014; 0,171, p=0,202; 0,106, p=0,014; TNF-α/hs-CRP 0,261, p=0,050; 0,315, p=0,017; 0,068, p=0.050; IL-6/cTnI admissão 0,486, p<0,001; 0,483, p<0,001; 0,236, p<0,001; MMP3/cTnI admissão 0,218, p=0,103; 0,146, p=0,278; 0,048, p=0,103; TNF-α/cTnI admissão 0,444, p=0,001; 0,380, p=0,004; 0,197, p=0,001; IL-6/cTnI 6-9h 0,676, p<0,001; 0,623, p<0,001; 0,456, p<0,01; MMP3/cTnI 6-9h 0,524, p=0,001; 0,149, p=0,270; 0,275, p<0,001; TNF-α/cTnI 6-9h 0,428, p=0,001, 0,452, p<0,001, 0,183, p<0,001. A regressão linear múltipla cTnI admissão/marcadores inflamatórios produziu: (R=0,638, R2=0,407) p<0,001 e cTnI 6-9h/marcadores inflamatórios (R=0,780, R2=0,609) p<0,001. (2) Significância da análise multivariada para idade (p=0,029), IMC>30 (p=0.070), AAS (p=0,040) e grupo (p=0,002). Da comparação entre os marcadores inflamatórios dos dois grupos resultou (mediana/intervalo interquartil, grupo controlo vs EAM): hs-CRP (0,19/0,27 vs 0,42/2,53 mg/dl) p=0,001, IL-6 (4,90/5,48 vs 13,07/26,41 pg/ml) p<0,001, MMP3 (19,70/13,70 vs 10,10/10,40 ng/ml) p<0,001;TNF-α (8,67/6,71 vs 8,26/7,80 pg/dl) p=0,805. Conclusões: (1) Nos doentes com EAM, existe correlação entre as moléculas inflamatórias IL-6, MMP3 e TNF-α, quer com o marcador inflamatório hs-CRP, quer com o marcador de lesão miocárdica cTnI. Esta correlação reforça-se para os valores de cTnI 6-9 horas após admissão, especialmente na correlação múltipla com o grupo dos quatro marcadores inflamatórios. (2) IL-6 está inversamente ligada às lipoproteínas de colesterol; hs-CRP e IL-6 têm excelentes correlações com o perfil lipídico valorizado no seu conjunto. No grupo EAM encontram-se níveis séricos mais reduzidos para as lipoproteínas de colesterol. Para TNF-α não foram encontradas diferenças significativas entre os grupos, as quais foram observadas para a IL-6 e hs-CRP (mais elevadas no grupo EAM). Os valores de MMP3 no grupo controlo estão mais elevados. ------------- ABSTRACT: Introduction: We reviewed the epidemiology, pathophysiology and current clinical knowledge about coronary heart disease, from its genesis to the acute myocardial infarction (AMI). The inflammatory theory for atherosclerosis, which has undergone considerable development in the last decade, was especially detailed. Markers of coronary atherosclerotic vulnerable plaque: The clinical and laboratory biomarkers associated with the unstable coronary atherosclerotic plaque vulnerable plaque are detailed. An emphasis was placed on the inflammatory mechanisms. Objectives: They are divided into two fundamental points: (1) To study in AMI patients, the relationship between the inflammatory molecules: Interleukin-6 (IL-6), Tumor Necrosis Factor-α (TNF-α) and Matrix metalloproteinase-3 (MMP3), unused in the clinical setting, with an inflammatory marker in clinical use: ultrasensitive C-reactive protein (hs-CRP), as well as a biomarker of myocardial injury: cardiac troponin I (cTnI). (2) To study, in the context of AMI, the Acute Phase Response (APR). We intend to demonstrate the impact of that clinical relevant phenomenon in the lipid profile and inflammatory biomarkers of our patients. Methods: (1) Prospective observational study of patients consecutively admitted for AMI (AMI group) in a single coronary care unit, after exclusion of trauma or infection. A peripheral assay at admission for IL-6, TNF-α, MMP3, hs-CRP and cTnI was performed. The latter was also valued in assays obtained 6-9 hours after admission. Linear correlation (Pearson's correlation coefficient, Spearman Rho's correlation coefficient and R2 regression) was performed between the three markers studied and the values of hs-CRP and cTnI (on admission and 6-9 hours after admission). Multiple linear regression was also obtained between cTnI on admission and 6-9h after, with all the inflammatory markers studied. (2) Case-control study between the AMI group and a random population of patients from an outpatient cardiology setting (control group). Cardiovascular events of any kind and infection or trauma were excluded in this group. The same inflammatory molecules were assayed in control and AMI groups. The following lipoproteins were also assayed: total cholesterol (TC), HDL cholesterol (HDLc) and subfractions 2 and 3 (HDL2 and HDL 3), oxidized LDL cholesterol (oxLDL), Triglycerides (TG), Lipoprotein (a) [Lp(a)], Apolipoprotein A1 (apoA1), Apolipoprotein B (ApoB) and Apolipoprotein E (ApoE). Demographics, classical risk factors, cardiovascular therapy and the use of anti-inflammatory drugs were appreciated in each group. The authors conducted a multivariate analysis with respect to demographics, risk factors and baseline therapy. The distribution of the same baseline characteristics was compared between the two groups, as well as the lipoprotein serum values. A correlation was performed between each inflammatory molecule and each of the lipoproteins, for all the patients studied. Multiple linear regression was determined between each inflammatory marker and all the lipid molecules per group. Finally, the statistical comparison between the inflammatory markers in the two groups was performed. Conclusions: (1) In AMI patients there is a correlation between the inflammatory molecules IL-6, TNF-α and MMP3 with both the inflammatory marker hs-CRP and the ischemic marker cTnI. This correlation is strengthened for the cTnI at 6-9h post admission, particularly in the multiple linear regression to the four inflammatory markers studied. (2) IL-6 correlates negatively with the cholesterol lipoproteins. Hs-CRP and IL-6 are strongly correlated to the whole lipoprotein profile. AMI patients display reduced serum lipid levels. For the marker TNF-α no significant differences were found between groups, which were observed for IL-6 and hs-CRP (higher in the AMI group). MMP3 values are higher in the control group.
