Este trabalho trata do ensino da habilidade para diagnosticar estudantes de enfermagem. Tem como objetivo geral verificar a adequação da elaboração do processo de análise e síntese e sua relação com o estabelecimento do diagnóstico de enfermagem. Após serem desenvolvidos os conteúdos programáticos previstos na disciplina de Enfermagem Médica, em face a uma situação hipotética, os alunos elaboraram o processo diagnóstico (RISNER, 1986) individualmente, chegando a um total de 106 formulações de Diagnóstico (Taxonomia I da North American Nursing Diagnosis Association - NANDA). As maiores dificuldades se deram na área do estabelecimento de "relações" e de "agrupamento de dados". Sugere-se que o desenvolvimento do processo de pensamento inerente à análise e síntese seja estimulado desde o início do curso de graduação.

Hydrogen isotope and Ar-40/Ar-39 geochronological data are presented from muscovite within a crustal-scale extensional detachment of the Shuswap Metamorphic Complex, North American Cordillera. The hydrogen isotope compositions (deltaD(ms)) of precisely dated muscovite attain values as low as -156parts per thousand in the detachment mylonite, whereas footwall quartzite has a deltaD(ms) value of -81parts per thousand. The very low deltaD(ms) values in the detachment are best explained by infiltration of meteoric water, with maximum deltaD values of -135parts per thousand +/- 3parts per thousand, during extensional unroofing of the orogen at 49.0-47.9 Ma. On the basis of the empirically determined relationship between elevation and isotopic composition of precipitation, the reconstructed early Eocene paleoelevations of the orogen are 4060 +/- 250 m to 4320 +/- 250 m, at least 1000 m higher than the highest present-day peaks. We propose that the isotopic composition of surface-derived waters in extensional detachments represents a newly recognized method to estimate maximum paleoelevations attained immediately preceding extensional orogenic collapse.

O objetivo deste estudo foi identificar os diagnósticos de enfermagem mais freqüentes de pacientes em pós-operatório imediato. Foi elaborada uma ficha de avaliação pós-anestésica do período pós-operatório que foi utilizada em 28 pacientes nas primeiras duas horas em que permaneceram em centro de recuperação pós-anestésica. Os diagnósticos de enfermagem foram estabelecidos com base na Taxionomia I dos Diagnósticos de Enfermagem da North American Nursing Diagnosis Association -NANDA. Foram identificados os seguintes diagnósticos: risco para injúria (100,0%), risco para infecção (92,8%), senso-percepção alterada (89,2%), risco para aspiração (82,1%), mobilidade física prejudicada (71,4%), integridade tissular prejudicada (82,1%) e hipotermia (67,8%).

O Ministério da Saúde preconiza o atendimento integral e humanizado às mulheres vítimas de violência sexual. Teve-se por objetivo descrever o Protocolo de Enfermagem na Assistência às Mulheres Vítimas de Violência Sexual do Centro de Atenção Integral à Saúde da Mulher da Universidade Estadual de Campinas, recentemente revisado. Para tanto, seguiram-se as etapas do processo de enfermagem e após a identificação dos principais diagnósticos da North American Nursing Diagnosis Association (NANDA) foram determinadas as intervenções, com base em normas técnicas nacionais e internacionais. O protocolo atual engloba o atendimento imediato e tardio, o acompanhamento ambulatorial e as ações relacionadas à interrupção legal da gravidez decorrente do estupro. O protocolo de enfermagem tem proporcionado à cliente um atendimento integral e humanizado e à enfermeira, maior autonomia na sua área de atuação, favorecendo o trabalho colaborativo e interativo com a equipe multidisciplinar.

O objetivo foi implementar a Sistematização da Assistência de Enfermagem (SAE), tendo como referencial a Teoria das Necessidades Humanas Básicas de Wanda de Aguiar Horta e o Diagnóstico de Enfermagem da North American Nursing Diagnosis Association (NANDA), partindo da avaliação do conhecimento da equipe de enfermagem sobre a SAE e incluindo a sua participação nesse processo. É uma pesquisa-ação de cunho qualitativo, realizada na Unidade de Terapia Intensiva de um Hospital da cidade de Brusque, Santa Catarina, de outubro de 2006 a março de 2007. Pode-se perceber que os profissionais da enfermagem sabem pouco sobre a SAE, porém possuem grande interesse em aprender e desenvolvê-la em sua prática diária. Conclui-se que foi possível realizar uma sistematização de assistência de fácil aplicação, através da utilização de impressos simples que forneceram todas as informações necessárias para o desenvolvimento qualificado do cuidado de enfermagem.

The literature shows that obesity is a public health problem concerning especially the general paediatrician. While prevention has probably more chances of success than treatment, drugs or, in case of failure, the surgical approach are reserved for extreme cases. In the domain of infectiology the different laboratory tests allow only partially to diagnose severe infections. But in the context of a potential influenza pandemic rapid virologic tests become more and more important. They allow a more precise diagnosis and a reduction of hospitalisations and of antibiotic prescriptions. A review of the north American experience with the pneumococal vaccine shows that the heptavalent vaccine will change our approach in infants with fever in whom we suspect a severe infection.

Mesoamerica, defined as the broad linguistic and cultural area from middle southern Mexico to Costa Rica, might have played a pivotal role during the colonization of theAmerican continent. It has been suggested that the Mesoamerican isthmus could have played an important role in severely restricting prehistorically gene flow between North and SouthAmerica. Although the Native American component has been already described in admixedMexican populations, few studies have been carried out in native Mexican populations. In thisstudy we present mitochondrial DNA (mtDNA) sequence data for the first hypervariable region (HVR-I) in 477 unrelated individuals belonging to eleven different native populations from Mexico. Almost all the Native Mexican mtDNAs could be classified into the four pan-Amerindian haplogroups (A2, B2, C1 and D1); only three of them could be allocated to the rare Native American lineage D4h3. Their haplogroup phylogenies are clearly star-like, as expected from relatively young populations that have experienced diverse episodes of genetic drift (e.g. extensive isolation, genetic drift and founder effects) and posterior population expansions. In agreement with this observation is the fact that Native Mexican populations show a high degree of heterogeneity in their patterns of haplogroup frequencies. HaplogroupX2a was absent in our samples, supporting previous observations where this clade was only detected in the American northernmost areas. The search for identical sequences in the American continent shows that, although Native Mexican populations seem to show a closer relationship to North American populations, they cannot be related to a single geographical region within the continent. Finally, we did not find significant population structure on the maternal lineages when considering the four main and distinct linguistic groups represented in our Mexican samples (Oto-Manguean, Uto-Aztecan, Tarascan, and Mayan), suggesting that genetic divergence predates linguistic diversification in Mexico.

Background In a previous study, the European Organisation for Research and Treatment of Cancer (EORTC) reported a scoring system to predict survival of patients with low-grade gliomas (LGGs). A major issue in the diagnosis of brain tumors is the lack of agreement among pathologists. New models in patients with LGGs diagnosed by central pathology review are needed. Methods Data from 339 EORTC patients with LGGs diagnosed by central pathology review were used to develop new prognostic models for progression-free survival (PFS) and overall survival (OS). Data from 450 patients with centrally diagnosed LGGs recruited into 2 large studies conducted by North American cooperative groups were used to validate the models. Results Both PFS and OS were negatively influenced by the presence of baseline neurological deficits, a shorter time since first symptoms (<30 wk), an astrocytic tumor type, and tumors larger than 5 cm in diameter. Early irradiation improved PFS but not OS. Three risk groups have been identified (low, intermediate, and high) and validated. Conclusions We have developed new prognostic models in a more homogeneous LGG population diagnosed by central pathology review. This population better fits with modern practice, where patients are enrolled in clinical trials based on central or panel pathology review. We could validate the models in a large, external, and independent dataset. The models can divide LGG patients into 3 risk groups and provide reliable individual survival predictions. Inclusion of other clinical and molecular factors might still improve models' predictions.

We report the draft genome sequence of the red harvester ant, Pogonomyrmex barbatus. The genome was sequenced using 454 pyrosequencing, and the current assembly and annotation were completed in less than 1 y. Analyses of conserved gene groups (more than 1,200 manually annotated genes to date) suggest a high-quality assembly and annotation comparable to recently sequenced insect genomes using Sanger sequencing. The red harvester ant is a model for studying reproductive division of labor, phenotypic plasticity, and sociogenomics. Although the genome of P. barbatus is similar to other sequenced hymenopterans (Apis mellifera and Nasonia vitripennis) in GC content and compositional organization, and possesses a complete CpG methylation toolkit, its predicted genomic CpG content differs markedly from the other hymenopterans. Gene networks involved in generating key differences between the queen and worker castes (e.g., wings and ovaries) show signatures of increased methylation and suggest that ants and bees may have independently co-opted the same gene regulatory mechanisms for reproductive division of labor. Gene family expansions (e.g., 344 functional odorant receptors) and pseudogene accumulation in chemoreception and P450 genes compared with A. mellifera and N. vitripennis are consistent with major life-history changes during the adaptive radiation of Pogonomyrmex spp., perhaps in parallel with the development of the North American deserts.

The present study aims to analyze attitudes and beliefs of the French-speaking general Swiss population (n = 2500; female n = 1280; mean age = 43 years) as regards gambling, which are to date almost exclusively studied in the North American and Australian contexts. Beliefs related to gambling include the perception of the effectiveness of preventive measures toward gambling, the comparative risk assessment of different addictive behaviors, the perceived risks of different types of gambling and attitudes are related to the gambler's personality. The general population perceived gambling rather negatively and was conscious of the potential risks of gambling; indeed, 59.0% of the sample identified gambling as an addictive practice. Slot machines were estimated to bear the highest risk. Compared with women and older people, men and young people indicated more positive beliefs about gambling; they perceived gambling as less addictive, supported structural preventive measures less often, and perceived gambling as a less serious problem for society. Gamblers were more likely to put their practices into perspective, perceiving gambling more positively than non-gamblers. General population surveys on such beliefs can deliver insights into preventive actions that should be targeted to young men who showed more favorable views of gambling, which have been shown to be associated with increased risk for problematic gambling.

OBJECTIVE: The sensitivity and tolerance regarding ADHD symptoms obviously differ from one culture to another and according to the informants (parents, teachers, or children). This stimulates the comparison of data across informants and countries. METHOD: Parents and teachers of more than 1,000 school-aged Swiss children (5 to 17 years old) fill in Conners's questionnaires on ADHD. Children who are older than 10 years old also fill in a self-report questionnaire. Results are compared to data from a North American sample. RESULTS: Swiss parents and teachers tend to report more ADHD symptoms than American parents and teachers as far as the oldest groups of children are concerned. Interactions are evidenced between school achievement, child gender, and informants. A relatively low rate of agreement between informants is found. CONCLUSION: These results strengthen the importance to take into account all informants in the pediatric and the child psychiatry clinic, as well as in the epidemiological studies.

Relato da experiência de enfermeiros na implementação da sistematização da assistência de enfermagem para acompanhamento ambulatorial em um centro interdisciplinar de atendimento a pacientes com esclerose múltipla de um hospital público de Fortaleza, Ceará. Essa implementação é baseada nas classificações da North American Nursing Diagnosis Association International, Classificação das Intervenções de Enfermagem e Classificação dos Resultados de Enfermagem. Um dos resultados diz respeito à sistematização do cuidado de enfermagem, partindo da identificação e da compreensão das respostas dos pacientes com esclerose múltipla aos problemas de saúde reais e potenciais. A sistematização enseja ampliar os conhecimentos por meio de uma prática pautada em evidências científicas, além de favorecer a atuação do enfermeiro em uma abordagem integral e fomentar outras investigações.
