Complete fetal bladder outlet obstruction was first diagnosed in a fetus at 13.5 weeks. After sequential vesico-centesis had shown good renal function, a vesico-amniotic shunt was inserted at 17 weeks with a Rodeck catheter. The procedure was successful and amniotic fluid volume re-accumulated to normal levels. A detailed scan at 20 weeks showed that the distal free end of the catheter was wound round the left fetal thigh. As the fetus grew, there was progressive constriction of the fetal thigh by the catheter. By 29 weeks, Doppler blood flow changes to the left leg were apparent. Fetoscopic surgery was performed at 30 weeks to release the constriction. The catheter was divided successfully, but the divided end of the shunt subsequently retracted into the fetal abdomen, producing urinary ascites, bilateral hydroureter and hydronephrosis. The baby was delivered at 31.5 weeks in good condition. Endoscopic resection of anterior and posterior urethral valves was performed at 6 months of age. At 2 years, the child has normal renal function, growth parameters and developmental milestones. Mild indentation of the left thigh was still apparent, although there was no functional impairment. In the periphery, physiological dopamine increases renal blood flow, decreases renal resistance and acts on the kidney tubule to enhance natriuresis and diuresis. The loss of dopamine function may be involoved in the deterioration in kidney function associated with ageing and may have a role in the pathogenesis of hypertension and diabetes. Intravenous dopamine is used as a positive inotrope in the treatment of acute heart failure and cardiogenic shock and as a diuretic in renal failure. The clinical uses of dopamine are limited, as it must be given intravenously, and also has widespread effects. The levels of peripheral dopamine can be increased by the administration of L-dopa to increase synthesis, prodrugs to release dopamine (docarpamine, glu-dopa) or by inhibiting the breakdown of dopamine (nitecapone). Preliminary clinical trials suggest that docarpamine may be useful in patients with low cardiac output syndrome after cardiac surgery and in refractory cirrhotic ascites. Ibopamine is an agonist at dopamine D1 and D2 receptors, which may retard the progression of chronic renal failure. Gludopa is selective for the kidney thus avoiding widespread side effects. The early clinical studies with ibopamine as a diuretic in heart failure were favourable but the subsequent large mortality study showed that ibopamine increased mortality. Fenoldopam is a selective dopamine D1 receptor agonist. Intravenous fenoldopam may be useful in the treatment of hypertension associated with coronary artery bypass surgery or in hypertensive emergencies. Although our understanding of physiological and pathological roles of peripheral dopamine has been increasing rapidly in recent times, we still need more information to allow the design of clinically useful drugs that modify these roles. One priority is an orally-active selective dopamine D1 receptor agonist.

Aims: This study was designed to investigate the influence of angiotensin II (Ang II) and nitric oxide (NO) on autoregulation of renal perfusion. Methods: Autoregulation was investigated in isolated perfused kidneys (IPRK) from Sprague-Dawley rats during stepped increases in perfusion pressure. Results: Ang II (75-200 pM) produced dose-dependent enhancement of autoregulation whereas phenylephrine produced no enhancement and impaired autoregulation of GFR. Enhancement by Ang II was inhibited by the AT(1) antagonist, Losartan, and the superoxide scavenger, Tempol. Under control conditions nitric oxide synthase (NOS) inhibition by 10 muM N-omega-nitro-L-arginine methyl ester (L-NAME) facilitated autoregulation in the presence of non-specific cyclooxygenase (COX) inhibition by 10 muM indomethacin. Both COX and combined NOS/COX inhibition reduced the autoregulatory threshold concentration of Ang II. Facilitation by 100 pM Ang II was inhibited by 100 muM frusemide. Methacholine (50 nM) antagonised Ang II-facilitated autoregulation in the presence and absence of NOS/COX inhibition. Infusion of the NO donor, 1 muM sodium nitroprusside, inhibited L-NAME enhancement of autoregulation under control conditions and during Ang II infusion. Conclusions: The results suggest than an excess of NO impairs autoregulation under control conditions in the IPRK and that endogenous and exogenous NO, vasodilatory prostaglandins and endothelium-derived hyperpolarizing factor (EDHF) activity antagonise Ang II-facilitated autoregulation. Ang II also produced a counterregulatory vasodilatory response that included prostaglandin and NO release. We suggest that Ang II facilitates autoregulation by a tubuloglomerular feedback-dependent mechanism through AT(1) receptor-mediated depletion of nitric oxide, probably by stimulating generation of superoxide.

Nitric oxide (NO) influences renal blood flow mainly as a result of neuronal nitric oxide synthase (nNOS). Nevertheless, it is unclear how nNOS expression is modulated by endogenous angiotensin II, an inhibitor of NO function. We tested the hypothesis that the angiotensin II AT1 receptor and oxidative stress mediated by NADPH oxidase contribute to the modulation of renal nNOS expression in two-kidney, one-clip (2K1C) hypertensive rats. Experiments were performed on male Wistar rats (150 to 170 g body weight) divided into 2K1C (N = 19) and sham-operated (N = 19) groups. nNOS expression in kidneys of 2K1C hypertensive rats (N = 9) was compared by Western blotting to that of 2K1C rats treated with low doses of the AT1 antagonist losartan (10 mg·kg-1·day-1; N = 5) or the superoxide scavenger tempol (0.2 mmol·kg-1·day-1; N = 5), which still remain hypertensive. After 28 days, nNOS expression was significantly increased by 1.7-fold in the clipped kidneys of 2K1C rats and by 3-fold in the non-clipped kidneys of 2K1C rats compared with sham rats, but was normalized by losartan. With tempol treatment, nNOS expression increased 2-fold in the clipped kidneys and 1.4-fold in the non-clipped kidneys compared with sham rats. The changes in nNOS expression were not followed by changes in the enzyme activity, as measured indirectly by the cGMP method. In conclusion, AT1 receptors and oxidative stress seem to be primary stimuli for increased nNOS expression, but this up-regulation does not result in higher enzyme activity.

Introdução: Muito embora os estudos apontem para um efeito positivo do exercício físico, em especial o treinamento com exercício aeróbio, sobre a pressão arterial e a distensibilidade arterial, pouco se sabe sobre os efeitos do treinamento com exercício de resistência aeróbia sobre a complacência vascular de indivíduos jovens saudáveis. Objetivos: Avaliar o efeito de 16 semanas de treinamento de resistência aeróbia sobre a função vascular e a pressão arterial de indivíduos jovens sedentários. Métodos: Foram avaliados 56 voluntários (de ambos os sexos, na faixa etária de 18 à 29 anos) antes e após 16 semanas de treinamento com corrida 3 vezes por semana. As medidas de pressão arterial foram realizadas de acordo com a VI Diretrizes Brasileiras de Hipertensão e a velocidade de onda de pulso (VOP) foi realizada com a utilização de um gravador automático computadorizado e os resultados foram analisados pelo programa Complior®. Resultados: Dos 56 indivíduos que participaram do presente estudo, 44 eram do sexo masculino (78,5%) e 12 do sexo feminino (21,5 %). Eles apresentaram idade de 22 ± 3 anos, estatura de 1,75 ± 0,07 metros, circunferência de cintura de 79,6 ± 7,8 cm e PAM de 79 ± 6,4 mmHg. O treinamento promoveu redução da FC repouso (69 ± 7,0 vs. 61 ± 7,1; p<0,05) e aumento do VO2pico (43,3 ± 7,3 vs. 50,1 ± 7,2; p<0,05). Entretanto, pressão arterial sistólica (107 ± 9,4 vs. 110 ± 10), pressão arterial diastólica (63 ± 5,7 vs. 62 ± 5,5), pressão de pulso (44 ± 7,0 vs. 48 ± 7,0) e VOP (6,5 ± 1,1 vs. 6,5 ± 1,1) não apresentaram alteração após o treinamento físico (p>0,05). Conclusões: Podemos concluir que 16 semanas de treinamento de resistência aeróbia foram capazes de aumentar a aptidão cardiorrespiratória, porém não provocaram alterações sobre a velocidade de onda de pulso e pressão arterial em voluntários saudáveis e sedentários. Sugere-se que a ausência de adaptações vasculares após o treinamento seja devido às características da amostra – indivíduos jovens e saudáveis.

Uma das mais graves complicações da endocardite infecciosa é o acidente vascular cerebral isquémico, responsável por uma alta taxa de mortalidade e morbilidade nos países desenvolvidos. Embora não seja o mecanismo dominante, o cardioembolismo é responsável por cerca de 20% dos enfartes cerebrais isquémicos. Sabe-se que o embolismo cerebral afeta mais de 40% dos pacientes com endocardite infecciosa, uma vez que a embolização da vegetação resultante da infeção das estruturas intracardíacas para a circulação arterial pode levar à oclusão das artérias cerebrais, dando origem, assim, a enfarte por ausência de aporte sanguíneo. O desenvolvimento de técnicas não invasivas baseadas na ultrassonografia tem potenciado um amplo estudo destas patologias, quer a nível cardíaco, quer vascular, promovendo uma melhor compreensão dos mecanismos fisiopatológicos que as envolvem. A ecocardiografia e o Doppler transcraniano detêm um papel central, respetivamente, no diagnóstico e predição do risco de fenómenos embólicos em pacientes com endocardite infecciosa e na avaliação inicial, prognóstico e follow-up de um episódio de acidente vascular cerebral.

OBJECTIVE To analyze the factors associated with stiffness of the great arteries in prepubertal children.METHODS This study with convenience sample of 231 schoolchildren aged 9-10 years enrolled in public and private schools in Vitória, ES, Southeastern Brazil, in 2010-2011. Anthropometric and hemodynamic data, blood pressure, and pulse wave velocity in the carotid-femoral segment were obtained. Data on current and previous health conditions were obtained by questionnaire and notes on the child’s health card. Multiple linear regression was applied to identify the partial and total contribution of the factors in determining the pulse wave velocity values.RESULTS Among the students, 50.2% were female and 55.4% were 10 years old. Among those classified in the last tertile of pulse wave velocity, 60.0% were overweight, with higher mean blood pressure, waist circumference, and waist-to-height ratio. Birth weight was not associated with pulse wave velocity. After multiple linear regression analysis, body mass index (BMI) and diastolic blood pressure remained in the model.CONCLUSIONS BMI was the most important factor in determining arterial stiffness in children aged 9-10 years.

Purpose: Pressure ulcers are a high cost, high volume issue for health and medical care providers, having a detrimental effect on patients and relatives. Pressure ulcer prevention is widely covered in the literature, but little has been published regarding the risk to patients in the radiographical setting. This review of the current literature is to identify findings relevant to radiographical context. Methods: Literature searching was performed using Science Direct and Medline databases. The search was limited to articles published in the last ten years to remain current and excluded studies containing participants less than 17 years of age. In total 14 studies were acquired; three were excluded as they were not relevant. The remaining 11 studies were compared and reviewed. Discussion: Eight of the studies used ‘healthy’ participants and three used symptomatic participants. Nine studies explored interface pressure with a range of pressure mat technologies, two studies measured shear (MRI finite element modelling, and a non-invasive instrument), and one looked at blood flow and haemoglobin oxygenation. A range of surfaces were considered from trauma, nursing and surgical backgrounds for their ability to reduce pressure including standard mattresses, high specification mattresses, rigid and soft layer spine boards, various overlays (gel, air filled, foam). Conclusion: The current literature is not appropriate for the radiographic patient and cannot be extrapolated to a radiologic context. Sufficient evidence is presented in this review to support the need for further work specific to radiography in order to minimise the development of PU in at risk patients.

Since the first in vivo studies of cerebral function with radionuclides by Ingvar and Lassen, nuclear medicine (NM) brain applications have evolved dramatically, with marked improvements in both methods and tracers. Consequently it is now possible to assess not only cerebral blood flow and energy metabolism but also neurotransmission. Planar functional imaging was soon substituted by single-photon emission computed tomography (SPECT) and positron emission tomography (PET); it now has limited application in brain imaging, being reserved for the assessment of brain death.

Myocardial perfusion imaging (MPI) is used on a daily basis to access coronary blood flow in patients that are suspected or have known Coronary Artery Disease (CAD). A Single Photon Emission Computed Tomography (SPECT) or and Positron Emission Tomography (PET) scan are used to access regional blood flow quantification either at rest or stress, the imaging acquisition is connected to an Electrocardiogram (ECG) and it is able to determine and quantify other myocardial parameters like myocardial wall thickness and wall motion. PET is not used so broadly due to its high procedure cost, the proximity with cyclotron, where are produced the majority of radiopharmaceuticals used in PET, due to their shor thalf-life. This work is intended to carry out a review of the tests relating to radiopharmaceuticals that are used in clinical practice in SPECT or PET for assessment of myocardial perfusion, also focusing very promising radiopharmaceuticals that are under investigation or in clinical trials with great potential for conventional nuclear medicine or PET, proceeding to a comparative analysis of both techniques and respective radiopharmaceuticals used.

O trabalho foi desenvolvido na empresa Masterprojetos – soluções integradas, através da elaboração e desenvolvimento do projeto de estabilidade de um edifício de cinco pisos, em que um deles se encontra enterrado. Este edifício incorpora um bloco de habitação multifamiliar e um bloco de habitação unifamiliar e comércio. Tanto o edifício como a empresa estão localizados no Peso da Régua. O trabalho pode-se dividir em duas partes: Numa primeira parte estão descritas as funções desempenhadas e contextualizada a metodologia de trabalho. Nesta parte está ainda descrita a ferramenta de trabalho Cypecad, a qual não houve oportunidade de aprender a usar durante o meu percurso académico e desenvolveu-se capacidades de maneira autodidata. São ainda referidos alguns softwares disponíveis no mercado, para o mesmo ramo de atividade. Numa segunda parte encontram-se descritos os pressupostos considerados no pré-dimensionamento do edifício assim como os elementos utilizados para a formulação do modelo de cálculo. Estão ainda feitas algumas verificações analíticas para a validação do modelo tido em conta para o dimensionamento. Estabeleceu-se também uma comparação entre as soluções obtidas através do software Cypecad e através do cálculo analítico para uma laje maciça. No final estão descritas as conclusões que foram obtidas, das quais se salientam a atenção que é necessário ter, quando se utiliza softwares de cálculo automático, quanto à introdução de dados e soluções que se obtêm , assim como a poupança de tempo que este tipo de ferramenta proporciona. É de referir também neste ponto as oportunidades que o estágio me proporcionou, como a visita de alguns locais de obras, as atividades de medição e orçamento de obra e acompanhamento de projetos de outras especialidades como redes prediais, térmica, acústica e ITED (infra-estruturas de Telecomunicações em Edifícios).

OBJECTIVE: To assess the frequency and severity of the anomalous origin of the left coronary artery (ALCA) from the pulmonary artery (PA). DESIGN OF THE STUDY: Prospective study of case series between March 1991 and December 1994. SETTING: Referral-based Paediatric Cardiology Department of a Tertiary Care Center. PATIENTS AND METHODS: Five consecutive patients (pts) with anomalous origin of the LCA from the PA; there were three infants aged 4 months and two children one 8 year and one 9 year old. There were three girls and two boys. All pts had clinical and 2D-echo and Doppler investigation prior to cardiac catheterization (CC). Indication for CC was based in the association of symptoms and signs of myocarditis or dilated cardiomyopathy of acute or subacute onset and electrocardiographic (ECG) signs of ischemia in infants. In older patients (pts) diagnosis was suspected mainly from ECG. During CC in all pts, aortograms and when necessary selective coronary angiograms were performed. Surgical correction was performed in all children. In two pts stress exercise ECG and stress Thallium studies before and after surgery were performed. RESULTS: two pts had "adult" an three had "infantile" type of ALCA from the PA. CC was performed and diagnosis was confirmed at surgery in all cases. In one child, correct diagnosis was made by ECO prior to CC and in one case LCA to PA fistula was suspected on Colour-Doppler study. No complications were attributed to CC. Several types of surgery were performed: reimplantation of the ALCA from the PA to the aorta (three pts); tunnel connection of the aorta to the ALCA via the PA (one pt) and left internal mammary to LCA anastomosis (one pt). Two infants died intraoperatively due to extensive myocardial infarction and poor left ventricular function. All the three survivors are asymptomatic after a mean follow up of 34 months. Two oldest pts are currently in New York Heart Association functional class I with normal ECG and improved myocardial perfusion on Thallium scan despite almost total occlusion of LCA at the site of implantation in the aorta as diagnosed on coronary angiogram. CONCLUSIONS: ALCA from PA is associated with major morbidity and mortality. Diagnosis should be suspected in pts with unexplained myocardial ischemia on ECG and even more if it is associated to clinical signs of dilated cardiomyopathy or myocarditis. Careful assessment on ECO and pulsed Doppler and colour flow mapping should make the diagnosis in most cases. Although surgery can be performed based only on ECO diagnosis, we strongly advise for angiography in all cases as in our experience there are false negative diagnosis by ECO. Preoperative Thallium studies can be useful for the selection of the type of surgery as pts with very little viable myocardium will not survive the establishment of a direct systemic to coronary blood flow and may be candidates for heart transplantation.

O presente relatório é um dos elementos que contribuirão para a concretização da avaliação individual da Unidade Curricular de Dissertação/Projeto/Estágio Profissional, inserida no plano de estudos do Mestrado em Engenharia de Instrumentação e Metrologia e traduz o trabalho realizado na empresa Caetano Aeronautic. O estágio, com a duração de sete meses, permitiu a concretização de finalidades e objetivos que deram suporte ao desenvolvimento das aprendizagens e mobilização dos saberes científicos relativos a domínios e conteúdos curriculares, nomeadamente inseridos nas áreas da Metrologia e Qualidade. A Metrologia é circunscrita como a ciência da medição. A medição é uma operação de extrema relevância em diversas atividades, porque é através da Metrologia que se garante um elevado nível de qualidade pretendido para a conceção, fabricação e utilização de serviços ou produtos. Face à indústria aeronáutica envolver um elevado conjunto de processos, de normas e documentação, foi necessário inicialmente um envolvimento integrado com o Sistema de Gestão da Qualidade da Caetano Aeronautic e com as operações inerentes, nomeadamente de melhoria contínua. Foram desenvolvidas, dentro do possível, propostas de melhoria no processo de inspeção dos componentes aeronáuticos. Dentro das mesmas destacam-se a alteração dos registos de medição, o estudo e posterior proposta de aquisição de ferramentas adicionais para equipamentos de medição. O fluxo de medição tornou-se mais fluído através da concretização da proposta de alteração do layout do laboratório. De salientar o trabalho desenvolvido na área da Engenharia da Qualidade, nomeadamente no tratamento de não conformidades, o que contribuiu para que o processo se encontre atualmente bastante simplificado na empresa.
