OBJECTIVE: To develop and validate a simple, integer-based score to predict functional outcome in acute ischemic stroke (AIS) using variables readily available after emergency room admission. METHODS: Logistic regression was performed in the derivation cohort of previously independent patients with AIS (Acute Stroke Registry and Analysis of Lausanne [ASTRAL]) to identify predictors of unfavorable outcome (3-month modified Rankin Scale score >2). An integer-based point-scoring system for each covariate of the fitted multivariate model was generated by their β-coefficients; the overall score was calculated as the sum of the weighted scores. The model was validated internally using a 2-fold cross-validation technique and externally in 2 independent cohorts (Athens and Vienna Stroke Registries). RESULTS: Age (A), severity of stroke (S) measured by admission NIH Stroke Scale score, stroke onset to admission time (T), range of visual fields (R), acute glucose (A), and level of consciousness (L) were identified as independent predictors of unfavorable outcome in 1,645 patients in ASTRAL. Their β-coefficients were multiplied by 4 and rounded to the closest integer to generate the score. The area under the receiver operating characteristic curve (AUC) of the score in the ASTRAL cohort was 0.850. The score was well calibrated in the derivation (p = 0.43) and validation cohorts (0.22 [Athens, n = 1,659] and 0.49 [Vienna, n = 653]). AUCs were 0.937 (Athens), 0.771 (Vienna), and 0.902 (when pooled). An ASTRAL score of 31 indicates a 50% likelihood of unfavorable outcome. CONCLUSIONS: The ASTRAL score is a simple integer-based score to predict functional outcome using 6 readily available items at hospital admission. It performed well in double external validation and may be a useful tool for clinical practice and stroke research.

L'argument d'autorité, schème argumentatif fréquent et occupant une place de choix dans toutes les typologies de types d'argument, est ici revu à l'aune des définitions qu'on lui donne. Partant d'une définition classique articulant une proposition P, un locuteur expert X et la conclusion Q «P est vrai», notre analyse suggère que la conclusion de l'argument d'autorité est moins une question de vérité que d'indubitabilité postulée. Cette force rhétorique de l'argument présenté, qui agit comme si toute mise en doute était exclue, est d'autant plus prégnante que la structure même de l'argument d'autorité ne permet pas de mettre en discussion le propos autorisé. En effet, l'immense majorité des exemples d'argument d'autorité dans les manuels d'argumentation montre que l'autorité est assurée au niveau d'une prémisse au sein d'une argumentation d'un autre type, ce qui sert la tentative d'imposer P à l'allocutaire comme allant de soi. Sans considérer le schème comme un sophisme, nous observerons s'il reste un schème identifiable en tant qu'argument d'autorité quand on lui retire les constituants classiques de ce type d'argumentation. Que se passe-t-il si l'expert cité n'est pas présenté comme tel ? Si le locuteur ne prend pas en charge le contenu référentiel de P ? Si P est un fait attesté par un expert et non une opinion ? Si, enfin, le locuteur compte sur sa propre autorité ou présuppose son autorité sans recourir à un tiers expert ou témoin ? Nous interrogeons ces différentes perspectives sur la base d'exemples tirés de la presse écrite, en insistant sur les effets rhétoriques de ce schème plutôt atypique dans son fonctionnement et en montrant l'importance de la modalité épistémique dans l'assertion autoritaire dont le schème de l'argument d'autorité serait une sous-catégorie.

This paper aims at investigating the socio-cultural factors that affect leisure-time sport participation in Switzerland. Data drawn from 8 waves of the Swiss Household Panel is used to evaluate a probit model with random effects, that takes into account the socioeconomic and demographic characteristics of the respondents. In line with existing literature, findings from the multivariate analysis show inequalities in sport involvement in Switzerland. These are significantly related to age, income, education, citizenship and cultural aspects. Appropriate and targeted policies promoting participation in sports among the community can be found on the basis of the critical modifiers in the model and their impact.

(Résumé de l'ouvrage) The idea that religion has to succeed in a «market», selling «salvation goods», has proved to be extremely attractive to scholars in sociology and the study of religion. Max Weber used the term «salvation good» to compare different religious traditions. Pierre Bourdieu employed the term in order to analyze «religious economy». And recently, an American group of researchers advocating «rational choice of religion» put the theme at the forefront of current debates. This book - the fruit of an International Congress in Lausanne in April 2005 - brings together leading specialists in the fields of sociology and the study of religion who discuss the terms «salvation goods» (or religious goods) and «religious market». The authors test the applicability of these concepts by using specific examples and they either deliberately advocate or criticize Weberian, Bourdieusian or rational-choice perspectives.
