Objective: An inverse relationship between blood pressure and cognitive function has been found in adults, but limited data are available in adolescents and young adults. We prospectively examined the relation between blood pressure and cognitive function in adolescence. Methods: We examined the association between BP measured at the ages of 12-15 years in school surveys and cognitive endpoints measured in the Seychelles Child Development Study at ages 17 (n=407) and 19 (n=429) years respectively. We evaluated multiple domains of cognition based on subtests of the Cambridge Neurological Test Automated Battery (CANTAB), the Woodcock Johnson Test of Scholastic Achievement (WJTA), the Finger Tapping test (FT) and the Kaufman Brief Intelligence Test (K-BIT). We used age-, sex- and height-specific z-scores of systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean arterial pressure (MAP). Results: Six out of the 21 cognitive endpoints tested were associated with BP. However, none of these associations were found to hold for both males and females or for different subtests within the same neurodevelopmental domain or for both SBP and DBP. Most of these associations disappeared when analyses were adjusted for selected potential confounding factors such as socio-economic status, birth weight, gestational age, body mass index, alcohol consumption, blood glucose, and total n-3 and n-6 polyunsaturated fats. Conclusions: Our findings do not support a consistent association between BP and subsequent performance on tests assessing various cognitive domains in adolescents.

The aim of the study was to determine the prevalence and variables associated with the pattern of risky health behavior (PRHB) among adolescent students in Cartagena, Colombia. A cross-sectional study was designed to investigate PRHB in a random cluster sample of students from middle and high schools. The associations were adjusted by logistic regression. A total of 2,625 students participated in this research, with ages from 10 to 20 years, mean=13.8 years (SD=2.0), and 54.3% were women. A total of 332 students reported PRHB (12.7%, 95%CI 11.4–14.0). Age over 15 years (OR=2.19, 95%CI 1.72–2.79), not being heterosexual (OR=1.98, 95%CI 1.36-2.87), poor/mediocre academic performance (OR=1.87, 95%CI 1.47–2.38), family dysfunction (OR=1.78, 95%CI 1.40–2.28) and male gender (OR=1.58, 95%CI 1.24–2.01) were associated with PRHB. One in every eight students presented a PRHB. It is important to pay greater attention to students who are over 15 years of age, male, not heterosexual, with a poor/mediocre academic performance and a dysfunctional family.

The sample dimension, types of variables, format used for measurement, and construction of instruments to collect valid and reliable data must be considered during the research process. In the social and health sciences, and more specifically in nursing, data-collection instruments are usually composed of latent variables or variables that cannot be directly observed. Such facts emphasize the importance of deciding how to measure study variables (using an ordinal scale or a Likert or Likert-type scale). Psychometric scales are examples of instruments that are affected by the type of variables that comprise them, which could cause problems with measurement and statistical analysis (parametric tests versus non-parametric tests). Hence, investigators using these variables must rely on suppositions based on simulation studies or recommendations based on scientific evidence in order to make the best decisions.

A motivação é vista, desde meados do Século XX, como o principal factor de satisfação no trabalho, de aumento da produtividade e de melhoria da qualidade dos produtos e serviços. Na conclusão de um curso conducente ao Mestrado em Gestão Global, interessou-nos abordar o valor humano da organização, cujo componente importante é o nível de satisfação das pessoas no trabalho, obtido da avaliação a um leque de variáveis independentes, relacionadas com duas coisas – o conteúdo do trabalho e o contexto do trabalho. O objectivo da presente dissertação é identificar em que medida as variáveis do meio envolvente do trabalho contribui para a satisfação no trabalho. Inspirados na Teoria dos dois Factores (Herzberg, 1959 e 1982) e na Teoria da Avaliação Cognitiva (Deci, 1971), pretendemos justapor a satisfação das pessoas à diferentes variáveis particulares que formam dois subsistemas – o intrínseco ou motivador, que inclui a aprendizagem, as emoções, o trabalho em si, a realização e o reconhecimento; e o extrínseco ou higiénico, que inclui o salário, a supervisão e as condições físicas. Além de uma incursão à literatura especializada, realizamos dois inquéritos em empresas cabo-verdianas, um em Outubro de 2003 e outro em Outubro de 2007, que nos permitiram determinar índices específicos e gerais de satisfação no trabalho, fazer correlações e análises comparativas. Concluímos que as pessoas estão razoavelmente satisfeitas no trabalho, dão mais ênfase aos factores do conteúdo que aos factores do contexto do trabalho, mas a fonte de satisfação varia de categoria por categoria. Work motivation is viewed, since middle of the 20th Century, as a main factor of job satisfaction, also for increasing of productivity and quality of goods and services. By the ending of a Master’s Programme in Global Management, we was moved by a great interest to research and report about the organization human values, important component of which is the people’s satisfaction level in work conditions, obtained through an evaluation of a great number of independent variables, related to two items – work content and work context. The main objective of the present dissertation is to identify in which measure the job environmental variables contribute to work satisfaction. Inspired into the Two Factors Theory (Herzberg, 1959, 1982) and into The Cognitive Evaluation Theory (Deci, 1971), we intent to relate people satisfaction to different particular variables, which perform two subsystems – intrinsic or motivator, that includes learning, emotions, work itself, achievement and recognition and; extrinsic or hygienic, that includes salary, supervision and work physical conditions. Besides a specialized literature review, we took up two separated inquiries within capeverdian companies, the first conducted in October, 2003, and the second in October, 2007, which allowed specifics and general job satisfaction indexes, correlations and comparative analysis. We concluded that people are fair satisfied on job, and give more emphasis on work content factors, than on work context factors, but the source of satisfaction is different category by category.

BACKGROUND AND OBJECTIVES: It is well established by a large number of randomized controlled trials that lowering blood pressure (BP) and low-density lipoprotein cholesterol (LDL-C) by drugs are powerful means to reduce stroke incidence, but the optimal BP and LDL-C levels to be achieved are largely uncertain. Concerning BP targets, two hypotheses are being confronted: first, the lower the BP, the better the treatment outcome, and second, the hypothesis that too low BP values are accompanied by a lower benefit and even higher risk. It is also unknown whether BP lowering and LDL-C lowering have additive beneficial effects for the primary and secondary prevention of stroke, and whether these treatments can prevent cognitive decline after stroke. RESULTS: A review of existing data from randomized controlled trials confirms that solid evidence on optimal BP and LDL-C targets is missing, possible interactions between BP and LDL-C lowering treatments have never been directly investigated, and evidence in favour of a beneficial effect of BP or LDL-C lowering on cognitive decline is, at best, very weak. CONCLUSION: A new, large randomized controlled trial is needed to determine the optimal level of BP and LDL-C for the prevention of recurrent stroke and cognitive decline.

Abiotic factors such as climate and soil determine the species fundamental niche, which is further constrained by biotic interactions such as interspecific competition. To parameterize this realized niche, species distribution models (SDMs) most often relate species occurrence data to abiotic variables, but few SDM studies include biotic predictors to help explain species distributions. Therefore, most predictions of species distributions under future climates assume implicitly that biotic interactions remain constant or exert only minor influence on large-scale spatial distributions, which is also largely expected for species with high competitive ability. We examined the extent to which variance explained by SDMs can be attributed to abiotic or biotic predictors and how this depends on species traits. We fit generalized linear models for 11 common tree species in Switzerland using three different sets of predictor variables: biotic, abiotic, and the combination of both sets. We used variance partitioning to estimate the proportion of the variance explained by biotic and abiotic predictors, jointly and independently. Inclusion of biotic predictors improved the SDMs substantially. The joint contribution of biotic and abiotic predictors to explained deviance was relatively small (similar to 9%) compared to the contribution of each predictor set individually (similar to 20% each), indicating that the additional information on the realized niche brought by adding other species as predictors was largely independent of the abiotic (topo-climatic) predictors. The influence of biotic predictors was relatively high for species preferably growing under low disturbance and low abiotic stress, species with long seed dispersal distances, species with high shade tolerance as juveniles and adults, and species that occur frequently and are dominant across the landscape. The influence of biotic variables on SDM performance indicates that community composition and other local biotic factors or abiotic processes not included in the abiotic predictors strongly influence prediction of species distributions. Improved prediction of species' potential distributions in future climates and communities may assist strategies for sustainable forest management.

Purpose: To present the long-term outcome (LTO) of 10 adolescents and young adults with documented cognitive and behavioral regression as children due to non-lesional focal, mainly frontal epilepsy with continuous spike-waves during slow wave sleep (CSWS). Method: Past medical and EEG data of all patients were reviewed and neuropsychological tests exploring main cognitive functions were administered. Result: After a mean duration of follow-up of 15.6 years (range 8-23 years), none of the 10 patients had recovered fully, but four regained borderline to normal intelligence and were almost independent. Patients with prolonged global intellectual regression had the worst outcome, whereas those with more specific and short-lived deficits recovered best. The marked behavioral disorders that were so disturbing during the active period (AP) resolved in all but one patient. Executive functions were neither severely nor homogenously affected. Three patients with a frontal syndrome during the AP disclosed only mild residual executive and social cognition deficits. The main cognitive gains occurred shortly after the AP, but qualitative improvements continued to occur. LTO correlated best with duration of CSWS. Conclusion: Our findings emphasize that cognitive recovery after cessation of CSWS depends on the severity and duration of the initial regression. None of our patients had major executive and social cognition deficits with preserved intelligence as reported in adults with destructive lesions of the frontal lobes during childhood. Early recognition of epilepsy with CSWS and rapid introduction of effective therapy are crucial for a best possible outcome.

Background: To compare treatment outcomes of a cognitive-behavioral long-term (CBT-L) and short-term (CBT-S) treatment for binge eating disorder (BED) in a non-randomized comparison and to identify moderators of treatment outcome. Methods: 76 female patients with BED participated in the study: 40 in CBT-L and 36 in CBT-S. Outcome values were compared at the end of the active treatment phase (16 sessions for CBT-L, 8 sessions for CBT-S) and at 12-month follow-up. Results: Both treatments produced significant reductions in binge eating. At the end of active treatment, but not at the end of follow-up, effects of primary outcomes (e.g. remission from binge eating, EDE shape concern) were better for CBT-L than for CBT-S. Dropout rates were significantly higher in CBT-L (35%) than in CBT-S (14%). Moderator analyses revealed that treatment efficacy for rapid responders and individuals exhibiting high scores on the mixed dietary negative affect subtype differed between the CBT-L and CBT-S with respect to objective binges, restraint eating and eating concern. Conclusion: Findings suggest that CBT in general represents an effective treatment for BED, but that subgroups of patients might profit more from a prolonged treatment. Short, lessintensive CBT treatments could nevertheless be a viable option in the treatment of BED.

AIM: In normal aging, subjective cognitive decline (SCD) might reflect personality traits or affective states rather than objective cognitive decline. However, little is known on the correlates of SCD in mild cognitive impairment (MCI). The present study investigates SCD in MCI patients and healthy older adults, and explores the association of SCD with personality traits, affective states, behavioral and psychological symptoms (BPS), and episodic memory in patients with MCI as compared with healthy older adults. METHODS: A total of 55 patients with MCI and 84 healthy older adults were recruited. Standard instruments were used to evaluate SCD, episodic memory, BPS and affective states. Premorbid and current personality traits were assessed by proxies using the NEO Personality Inventory Revised. RESULTS: Patients with MCI generally reported SCD more often than healthy older adults. SCD was positively associated with depressive symptoms in both groups. With regard to personality, no significant relationship was found in the healthy older group, whereas agreeableness was significantly negatively related to SCD in the MCI group. No significant association was found between SCD and episodic memory. CONCLUSIONS: SCD is more prevalent in patients with MCI than in the healthy elderly, but it does not reflect an objective cognitive impairment. SCD rather echoes depressive symptoms in both patients with MCI and healthy subjects. The negative association of SCD with agreeableness observed in patients with MCI could indicate that MCI patients scoring high on the agreeableness trait would not report SCD in order to prevent their relatives worrying about their increasing cognitive difficulties. OBJECTIVETo determine if there is a relationship between adherence to nutritional recommendations and sociodemographic variables in Brazilian patients with type 2 diabetes mellitus.METHODSCross-sectional observational study using a stratified random sample of 423 individuals. The Food Frequency Questionnaire (FFQ) was used, and the Fisher's exact test was applied with 95% confidence interval (p<0.05).RESULTSOf the 423 subjects, 66.7% were women, mean age of 62.4 years (SD = 11.8), 4.3 years of schooling on average (SD = 3.6) and family income of less than two minimum wages. There was association between the female gender and adherence to diet with adequate cholesterol content (OR: 2.03; CI: 1.23; 3.34), between four and more years of education and adherence to fractionation of meals (OR: 1 92 CI: 1.19; 3.10), and income of less than two minimum wages and adherence to diet with adequate cholesterol content (OR: 1.74; CI: 1.03, 2.95).CONCLUSIONAdherence to nutritional recommendations was associated with the female gender, more than four years of education and family income of less than two minimum wages.

Résumé de la thèseCette étude repose sur un double constat initial : en premier lieu, les récits français d'explorationau Tibet de la période s'étendant de 1850 à 1912 relèvent d'une tradition particulière du voyagescientifique en partie irréductible aux traditions des autres nations impérialistes de la mêmeépoque, notamment la tradition britannique à laquelle on les a souvent assimilés. En second lieu,ces récits révèlent à la fois la place centrale dégagée à la description du paysage tibétain et uneévolution unique et originale des savoirs et des représentations qui lui sont liés.Ce travail montre la rapide évolution des images du Tibet que s'est façonnées l'Europe,l'interrelation de cette histoire des représentations avec une révision des connaissances sur laterre et l'homme dans les sciences du XIXe siècle, ainsi qu'une dimension différentielle entre lesobjectifs de connaissance émanant des explorateurs français et anglais, différence due à uncontexte géopolitique hétérogène. Sur le fond de cette démarche contextualisante, le recours àl'analyse textuelle met en évidence le rôle qu'ont joué les explorateurs français dans l'histoire dessavoirs sur le Tibet.Pour des raisons inhérentes à la qualité d'écriture de ses récits et à l'acuité remarquable de sonapproche du monde tibétain, une place centrale de ce travail a été ménagée à Jacques Bacot(voyages de 1906 à 1907 - Dans les Marches tibétaines - et de 1909 à 1910 - Le Tibet révolté). Il a dèslors été fructueux de replacer ses récits dans l'histoire des savoirs et des représentations du Tibetet de les comparer aux récits majeurs d'autres explorateurs français au Tibet comme GabrielBonvalot, Fernand Grenard et Henri d'Ollone.Le paysage est une notion-clé pour comprendre le regard que portent les explorateurs sur leTibet. Or le paysage revêt des acceptions variables selon les auteurs et affiche des facettesdifférentes. Il a ainsi été possible de distinguer plusieurs dimensions : une dimension cognitive, unedimension épistémique, une dimension intersubjective et, enfin, une dimension imaginaire et sacrée. Cettedimension sacrée du paysage peut également être actualisée en tant que dimension écosymbolique.C'est là l'aboutissement des voyages de Bacot. L'explorateur, attentif à la culture tibétaine, envient à réaliser la valeur sacrée qui traverse la relation des Tibétains eux-mêmes à leurenvironnement. Le paysage ainsi compris se place au fondement d'une expérience partagée avecdes acteurs issus d'une autre culture et apparaît ainsi comme un opérateur privilégié pour« comprendre les compréhensions » (Clifford Geertz) des explorateurs, mais aussi, de proche enproche, des Tibétains.D'un point de vue épistémologique, la mise au jour de ces dimensions du paysage permetd'éviter certains écueils de la critique historienne et littéraire sur les récits de voyage au Tibet, quin'y a vu tantôt que de simples documents inertes, tantôt que le reflet d'un imaginaire purementet indéfectiblement occidental. La présente étude montre au contraire la part irréductiblequ'occupe la rencontre in situ avec l'autre et l'ailleurs dans l'élaboration conjointe desreprésentations et des savoirs.

L'importance des déficits neurocognitifs des patients schizophrènes a conduit la section «E. Minkowski» du Département universitaire de psychiatrie adulte de Lausanne à développer un programme de remédiation cognitive pour jeunes patients schizophrènes. Les résultats préliminaires de ce programme indiquent une très nette amélioration des performances cognitives chez la plupart des patients. Les mécanismes cérébraux responsables de ces progrès ne sont cependant pas élucidés. De nouveaux éléments concernant les mécanismes de plasticité synaptique sont évoqués pour expliquer les modifications cérébrales associées aux fonctions cognitives exercées.

Aims: To describe personality traits and their changes in mild cognitive impairment (MCI) and control subjects. Methods: Sixty-three MCI and 90 control subjects were asked to describe their current personality traits by the Structured Interview for the Five-Factor Model (SIFFM). For each subject, a close relative retrospectively assessed these descriptions both as to the previous and current personality traits, using the Revised NEO Personality Inventory, Form R (NEO-PI-R). Results: Self-assessed MCI subjects reported significantly lower scores in the openness dimension than control subjects [F(1, 150) = 9.84, p = 0.002, ηp(2) = 0.06]. In current observer ratings, MCI subjects had higher scores on neuroticism [F(1, 137) = 7.55, p = 0.007, ηp(2) = 0.05] and lower ones on extraversion [F(1, 137) = 6.40, p = 0.013, ηp(2) = 0.04], openness [F(1, 137) = 9.93, p = 0.002, ηp(2) = 0.07], agreeableness [F(1, 137) = 10.18, p = 0.002, ηp(2) = 0.07] and conscientiousness [F(1, 137) = 25.96, p < 0.001, ηp(2) = 0.16]. Previous personality traits discriminated the groups as previous openness [odds ratio (OR) = 0.97, 95% confidence interval (CI) = 0.95-0.99, p = 0.014] and conscientiousness (OR = 0.96, 95% CI 0.94-0.98, p = 0.001) were negatively related to MCI group membership. In MCI subjects, conscientiousness [F(1, 137) = 19.20, p < 0.001, ηp(2) = 0.12] and extraversion [F(1, 137) = 22.27, p < 0.001, ηp(2) = 0.14] decreased between previous and current evaluations and neuroticism increased [F(1, 137) = 22.23, p < 0.001, ηp(2) = 0.14], whereas no significant change was found in control subjects. Conclusions: MCI subjects undergo significant personality changes. Thus, personality assessment may aid the early detection of dementia. © 2013 S. Karger AG, Basel.
