With the aim of studying the contamination of soils with eggs of Toxocara spp. in an arid area in the central-western region of Argentina, 76 soil samples were collected from 18 towns belonging to six provinces of central-western Argentina. They were processed by the centrifugal flotation method. No eggs of Toxocara spp. were found. It can be concluded that the negative results are directly related to the characteristics of the environment and climate present in the studied area. The finding of eggs in soils depends on several factors: the presence of canine or feline feces, the hygienic behavior of pet owners, the presence of stray animals without veterinary supervision, the weather and environmental conditions, and laboratory techniques used; and all these circumstances must be considered when comparing the results found in different geographical regions. In order to accurately define the importance of public spaces in the transmission of infection to humans, it is important to consider the role of backyards or green spaces around housing in small towns, where the population is not used to walking pets in public spaces, and in such cases a significant fraction of the population may acquire the infection within households.

The dengue virus (DENV), which is frequently involved in large epidemics, and the yellow fever virus (YFV), which is responsible for sporadic sylvatic outbreaks, are considered the most important flaviviruses circulating in Brazil. Because of that, laboratorial diagnosis of acute undifferentiated febrile illness during epidemic periods is frequently directed towards these viruses, which may eventually hinder the detection of other circulating flaviviruses, including the Saint Louis encephalitis virus (SLEV), which is widely dispersed across the Americas. The aim of this study was to conduct a molecular investigation of 11 flaviviruses using 604 serum samples obtained from patients during a large dengue fever outbreak in the state of Mato Grosso (MT) between 2011 and 2012. Simultaneously, 3,433 female Culex spp. collected with Nasci aspirators in the city of Cuiabá, MT, in 2013, and allocated to 409 pools containing 1-10 mosquitoes, were also tested by multiplex semi-nested reverse transcription PCR for the same flaviviruses. SLEV was detected in three patients co-infected with DENV-4 from the cities of Cuiabá and Várzea Grande. One of them was a triple co-infection with DENV-1. None of them mentioned recent travel or access to sylvatic/rural regions, indicating that transmission might have occurred within the metropolitan area. Regarding mosquito samples, one pool containing one Culex quinquefasciatus female was positive for SLEV, with a minimum infection rate (MIR) of 0.29 per 1000 specimens of this species. Phylogenetic analysis indicates both human and mosquito SLEV cluster, with isolates from genotype V-A obtained from animals in the Amazon region, in the state of Pará. This is the first report of SLEV molecular identification in MT.

The aim of this study was to identify behavioral aspects of the sandfly fauna of a fishing tourism area in the municipality of Bonito (MS). Monthly captures were undertaken from December 2009 to November 2010, using automatic CDC type light traps, from 18h00 to 06h00, in a forested area, a savannah area, peridomiciles and animal shelters near peridomiciliary areas. Nyssomyia whitmani was the most frequent out of a total of 6,699 specimens collected, belonging to 16 species, followed by Psathyromyia bigeniculata and Lutzomyia longipalpis, found in all the environments investigated, though in their greatest numbers in the animal shelters. Ny. whitmani exhibited its highest frequencies during the dry months, coincident with the fishing season, when the risk of transmission of cutaneous leishmaniasis for tourists and inhabitants increases. Noteworthy was the finding of two species naturally infected by flagellates: Ny. whitmani and Pa. bigeniculata. The local population and visiting tourists should be warned of the threat posed by leishmaniasis and the health authorities alerted to the need for adopting environmental sanitary measures, especially regarding such animal shelters as they seem to provide favorable conditions to the proliferation, maintenance and breeding opportunities of phlebotomines.

Involuntary rhythmic leg movements in childhood is an uncommon condition, the generators of which remain unknown. We report on a male 3 years of age with distinct features providing important clues concerning the location of one of these generators. At the age of 7 months, the previously healthy young male started with low frequency, rhythmic, and continuous (both during wakefulness and sleep) flexion/extension movements of the lower limbs. Movements interfered significantly with gait acquisition, and, despite normal cognitive development, he was able to walk only at age 2 years, 4 months. The neurologic examination revealed the absence of automatic stepping in the neonatal period, but was otherwise normal. A polygraphic electroencephalogram/electromyogram EEG/EMG) recording, at the age of 2 years, 9 months, revealed rhythmic and synchronous legs with EMG activity at 0.5 Hz. A more complete polygraphic recording at the age of 3 years, 10 months, showed a lower frequency (0.35 Hz) for the movements, which were time-locked with the respiratory cycle. Magnetic resonance imaging (MRI) of the brain revealed an increased T2 signal in the upper medulla-lower pons regions. The generator of the rhythmic legs movements is postulated to be the respiratory center, connecting with the reticulospinal projecting neurons through an aberrant pathway.

An increase in the number of new cases of tuberculosis (TB) combined with poor clinical outcome was identified among HIV-infected injecting drug users attending a large HIV unit in central Lisbon. A retrospective epidemiological and laboratory study was conducted to review all newly diagnosed cases of TB from 1995 to 1996 in the HIV unit. Results showed that from 1995 to 1996, 63% (109/173) of the Mycobacterium tuberculosis isolates from HIV-infected patients were resistant to one or more anti-tuberculosis drugs; 89% (95) of these were multidrug-resistant, i.e., resistant to at least isoniazid and rifampicin. Eighty percent of the multidrug-resistant strains (MDR) available for restriction fragment length polymorphism (RFLP) DNA fingerprinting clustered into one of two large clusters. Epidemiological data support the conclusion that the transmission of MDR-TB occurred among HIV-infected injecting drug users exposed to infectious TB cases on open wards in the HIV unit. Improved infection control measures on the HIV unit and the use of empirical therapy with six drugs once patients were suspected to have TB, reduced the incidence of MDR-TB from 42% of TB cases in 1996 to 11% in 1999.

O aumento da incidência do linfoma cerebral primitivo parece ser real em pessoas imunologicamente normais. Contudo, os linfomas do sistema nervoso central têm uma muito maior incidência em doentes imunodeprimidos tais como os submetidos a transplantes cardíacos ou renais, doentes com deficiência de IgA ou síndrome de Wiskott-Aldrich. Assim, não é surpreendente o aumento da predisposição dos doentes com a síndrome de imunodeficiência adquirida para os linfomas do sistema nervoso central, sendo esta a segunda causa mais frequente de lesão ocupando espaço intracerebral nesta população. Os autores descrevem o caso de uma mulher de 36 anos com síndrome de imunodeficiência adquirida e uma lesão ocupando espaço intracerebral que demonstraram tratar-se de um linfoma cerebral.

Objectivo: Determinar os factores de risco, características clínicas e desfecho da queratite por acantamoeba. Métodos: Os autores revêem os casos de queratite por acantamoeba em 6 doentes (3 do sexo masculino e 3 do sexo feminino) com idades compreendidas entre os 18 e os 41 anos, vigiados no Departamento de Córnea do nosso Hospital. As suas características e curso clínicos foram estudados retrospectivamente. Resultados: Todos os doentes eram portadores de lentes de contacto. Quatro dos 6 casos tiveram início do quadro entre final de Maio e Julho. A suspeita inicial de queratite bacteriana, fúngica ou viral precedeu o diagnóstico final de queratite por acantamoeba em todos os doentes. O diagnóstico foi feito através de observação clínica, PCR (polymerase chain reaction) ou microscopia confocal. Em todos os casos a terapêutica dirigida foi iniciada com propamidina, com adição de clorohexidina em um doente. Em 3 doentes verificou-se necessidade de realização de queratoplastia penetrante: um caso por incapacidade de controlo da infecção e por sequelas noutros dois. Conclusão: É necessário um grande nível de suspeita de infecção por acantamoeba em indivíduos utilizadores de lente de contacto e falência inicial à terapêutica antimicrobiana ou antiviral. A microscopia confocal é um auxiliar importante no diagnóstico da queratite por este microrganismo.

Objectivo: Avaliar a incidência de complicações relacionadas com o cateterismo venoso central e a existência de eventuais factores de risco para a sua ocorrência. Material e métodos: Estudo retrospectivo de 305 catéteres venosos centrais colocados pelos médicos da Unidade de Cuidados Intensivos Pediátricos (UCIP) do Hospital Dona Estefânia, durante 5 anos. As variáveis estudadas foram: idade, peso, instituição de ventilação mecânica, alteração da coagulação, local de inserção, número de lúmens e tempo de utilização do catéter, número e tipo de complicações. Na análise estatística foram utilizados o teste do Quiquadrado, o teste exacto de Fisher e o t-teste de Student, considerando-se haver diferenças estatisticamente significativas para valores de p< 0,05. Resultados: Foram submetidos a cateterismo 296 crianças, com idades compreendidas entre 0.08 e 16.00 anos (média=3,6 anos) e pesos entre 2 e 85 Kg (média=16,2 Kg). Os locais de inserção foram, por ordem decrescente de utilização, a veia subclávia (63,3%), a veia femural (29,8%) e veia jugular (6,9%). Os catéteres de duplo lúmen foram os mais utilizados (61,9%), seguidos dos de mono (32,5%) e de triplo lúmen (5,6%). Ocorreram 46 (15,1%) complicações, mas não se verificaram óbitos directamente relacionados com o cateterismo. A remoção do catéter foi electiva em 98 (32,1%) casos, por óbito em 97 (31,8%), por complicações em 39 (12,8%) e por outras causas em 71 (23,2%). O tempo de utilização dos catéteres foi em média de 7,6 dias. Os catéteres com e sem sépsis tiveram uma duração média de utilização de 6,9 dias e 17,0 dias, respectivamente. Conclusões: Este estudo revelou um padrão de complicações do cateterismo venoso central semelhante ao descrito por outros autores, sendo que nenhuma das variáveis analisadas se revelou, por si só, como factor de risco de complicações, excepto o tempo de utilização do catéter na ocorrência de sépsis.

Introdução O sugamadex é uma gama ciclodextrina modificada que forma um complexo com os bloqueadores neuromusculares rocurónio e vecurónio, revertendo o bloqueio neuromuscular (BNM) induzido por estes fármacos1,2,3,4. O sugamadex apresentou valor terapêutico acrescentado em relação aos anticolinesterásicos, nomeadamente à neostigmina, para a reversão do BNM causado pelo rocurónio e vecurónio1,3. No Hospital de São José (HSJ) este medicamento foi introduzido em Outubro de 2010 para a reversão do BNM profundo e nas situações de risco de vida imediato associadas a via aérea difícil com impossibilidade de ventilar e de entubar. A dispensa do sugamadex é efectuada por reposição de stock mediante envio de justificação clínica aos Serviços Farmacêuticos (SF). Objetivo Caracterizar a utilização de sugamadex no HSJ: evolução do consumo, serviços clínicos utilizadores e adequação da utilização clínica face às indicações aprovadas pela Comissão de Farmácia e Terapêutica (CFT). Métodos Pesquisa e análise bibliográfica. Recolha, através da do sistema de gestão integrada do circuito do medicamento, dos dados de consumo desde Outubro de 2010 até Junho de 2015, por semestre e por serviço clínico. Recolha das indicações terapêuticas em que o sugamadex foi administrado, no período acima referido, através da consulta das justificações clínicas. Análise retrospectiva dos dados recolhidos. Resultados Apresentação gráfica dos consumos de sugamadex nos serviços utilizadores no período em estudo. Apresentação gráfica das indicações terapêuticas em que foi administrado, por serviço e no período em estudo. Conclusões O consumo de sugamadex tem um evidente crescimento desde o seu início de utilização. A justificação dominante para a utilização do sugamadex é a curarização residual. Verifica-se um alargamento do âmbito de utilização, face às indicações aprovadas pela CFT. PURPOSE: In this prospective, multicenter, 14-day inception cohort study, we investigated the epidemiology, patterns of infections, and outcome in patients admitted to the intensive care unit (ICU) as a result of severe acute respiratory infections (SARIs). METHODS: All patients admitted to one of 206 participating ICUs during two study weeks, one in November 2013 and the other in January 2014, were screened. SARI was defined as possible, probable, or microbiologically confirmed respiratory tract infection with recent onset dyspnea and/or fever. The primary outcome parameter was in-hospital mortality within 60 days of admission to the ICU. RESULTS: Among the 5550 patients admitted during the study periods, 663 (11.9 %) had SARI. On admission to the ICU, Gram-positive and Gram-negative bacteria were found in 29.6 and 26.2 % of SARI patients but rarely atypical bacteria (1.0 %); viruses were present in 7.7 % of patients. Organ failure occurred in 74.7 % of patients in the ICU, mostly respiratory (53.8 %), cardiovascular (44.5 %), and renal (44.6 %). ICU and in-hospital mortality rates in patients with SARI were 20.2 and 27.2 %, respectively. In multivariable analysis, older age, greater severity scores at ICU admission, and hematologic malignancy or liver disease were independently associated with an increased risk of in-hospital death, whereas influenza vaccination prior to ICU admission and adequate antibiotic administration on ICU admission were associated with a lower risk. CONCLUSIONS: Admission to the ICU for SARI is common and associated with high morbidity and mortality rates. We identified several risk factors for in-hospital death that may be useful for risk stratification in these patients.

BACKGROUND: Chronic respiratory diseases (CRD) are greatly underestimated. The aim of this study was to assess the burden associated with reported CRD and chronic obstructive pulmonary disease, as defined on the basis of various standardized criteria, by estimating their point prevalence in a sample of individuals attending the Primary Health Care (PHC) level and Emergency Room (ER) Departments in Cape Verde (CV) archipelago. The second aim of the study was to identify factors related to airways obstruction and reported CRD in this population. METHODS: A cross-sectional study was carried out in CV during 2 weeks. Outpatients aged more than 20 years seeking care at PHC level and ER answered a standardized questionnaire and were subjected to spirometry, independently of their complaint. Two criteria for airways obstruction were taken into account: forced expiratory volume (FEV(1)) <80% of the predicted value and FEV(1)/forced vital capacity (FVC) ratio <0.70. RESULTS: A total of 274 individuals with a satisfactory spirometry were included. 22% of the individuals had a FEV(1) < 80%. Individuals older than 46 years had a higher risk of having airways obstruction. Asthma diagnosis (11%) had a clear association with airways obstruction. Smoking was a risk factor for a lower FEV(1). Working in a dust place and cooking using an open fire were both related to chronic bronchitis and asthma diagnosis. CONCLUSION: Under-report and underdiagnosis of chronic respiratory conditions seem to be a reality in CV just as in other parts of the world. To improve diagnosis, our results reinforce the need of performing a spirometry

Sheehan's syndrome occurs as a result of ischaemic pituitary necrosis due to severe postpartum haemorrhage. Improvements in obstetrical care have significantly reduced its incidence in developed countries, but postpartum pituitary infarction remains a common cause of hypopituitarism in developing countries. We report a case of severe postpartum haemorrhage followed by headache, central diabetes insipidus and failure to lactate, which prompted us to investigate and identify both anterior and posterior pituitary deficiency compatible with Sheehan's syndrome. A timely diagnosis allowed us to implement an adequate treatment and follow-up plan, which are known to improve clinical status and patient outcome.
