Estudos recentes mostram que o açaí é rico em polifenóis e uma dieta rica em polifenóis pode estar envolvida na proteção contra o risco cardiovascular. O objetivo deste estudo foi avaliar o efeito do tratamento crônico de animais hipertensos 2R,1C com extrato hidroalcoólico do caroço do açaí (ASE) sobre o desenvolvimento da hipertensão e disfunção endotelial; estresse oxidativo e sobre as alterações vasculares e renais. Ratos Wistar machos foram utilizados para obtenção da hipertensão renovascular 2R,1C e ratos controles 2R (sham) foram somente submetidos à laparotomia e receberam tratamento diário com veículo ou ASE (200 mg/Kg/dia) durante 40 dias. A pressão arterial sistólica (PAS) foi aferida por pletismografia de cauda e os efeitos vasodilatadores da acetilcolina (ACh) e nitroglicerina (NG) foram estudados em leito arterial mesentérico (LAM) perfundido e pré-contraído com norepinefrina. A atividade das enzimas SOD, CAT, GPx, os níveis de MDA, a carbonilação de proteínas e os níveis de nitrito foram avaliados por espectrofotometria. As expressões de enzimas pró e antioxidantes foram avaliadas por western blot. A atividade de MMP-2 foi avaliada por zimografia. Os níveis séricos de creatinina foram avaliados através de kit, por espectrofotometria. As alterações vasculares e renais foram avaliadas por microscopia de luz. A PAS foi maior nos animais 2R,1C, e o tratamento com ASE preveniu o desenvolvimento da hipertensão. O efeito vasodilatador reduzido da ACh em animais 2R,1C foi recuperado pelo ASE. O efeito vasodilatador da NG não foi diferente entre os grupos. O dano oxidativo avaliado pela peroxidação lipídica e carbonilação de proteínas foi maior nos animais 2R,1C, e reduzido pelo tratamento com ASE. As atividades da SOD, CAT e GPx foram menores em amostras de mesentério, plasma, rim e coração de animais 2R,1C e o tratamento com ASE aumentou estas atividades. A produção de NO foi menor no plasma, mesentério e rim dos animais 2R,1C, e o tratamento com ASE aumentou a produção de NO somente no rim e mesentério destes animais. A expressão de SOD-1, 2, eNOS e TIMP-1 foram menores nos animais 2R,1C e o tratamento com ASE aumentou a expressão destas enzimas. A expressão de NOX-4 e MMP-2 e a atividade de MMP-2 foram maiores nos animais 2R,1C e o tratamento com ASE reduziu a expressão destas enzimas e a atividade de MMP-2. Os animais 2R,1C apresentaram um aumento na espessura da camada média da aorta e artéria mesentérica e um aumento na relação média/lúmen da aorta, e estas alterações foram prevenidas pelo ASE. Os níveis séricos aumentados de creatinina nos animais 2R,1C foram reduzidos por ASE. As alterações morfológicas renais nos animais 2R,1C foram prevenidas pelo ASE. Portanto, o tratamento com ASE previne o desenvolvimento da hipertensão, melhora a disfunção endotelial e previne as alterações vasculares e renais em ratos 2R,1C. A redução da atividade antioxidante e o aumento na peroxidação lipídica e carbonilação de proteínas sugerem o envolvimento de um mecanismo deficiente da defesa antioxidante e de um dano oxidativo aumentado, os quais foram revertidos pelo ASE.

Alguns trabalhos demonstraram que a planta, Euterpe oleracea Mart. (açaí) é rica em polifenóis e que uma dieta rica em polifenóis pode estar envolvida na proteção contra o risco cardiovascular. Desta forma, o objetivo deste estudo foi avaliar o efeito do tratamento com o extrato hidroalcoólico do caroço do açaí (ASE) (200mg/Kg/dia), sobre as alterações renais, cardiovasculares, metabólicas, morfológicas e o estresse oxidativo na prole adulta com dezesseis semanas, cujas mães foram submetidas a uma dieta hipoprotéica durante a gestação. Quatro grupos de ratas foram alimentados com dietas experimentais: controle (20% de proteínas); controle + ASE (20% de proteína + ASE); hipoprotéica (6% de proteína); hipoprotéica + ASE (6% de proteína + ASE) durante a gestação. Após o desmame, todos os filhotes passaram a ser alimentados com uma dieta controle e foram sacrificados com 4 meses de idade. A pressão arterial sistólica (PAS) foi medida por pletismografia de cauda e o efeito vasodilatador da acetilcolina (ACh) e nitroglicerina (NG) foi avaliado em leito arterial mesentérico (LAM) perfundido. Foram determinados o peso corporal, níveis séricos de colesterol total, triglicerídeos, lipoproteína de alta densidade (HDL), albumina, uréia, creatinina, glicose, insulina, resistência à insulina (índice de Homa IR), sódio, potássio e a renina plasmática. Foi avaliada a depuração de creatinina, volume de urina, excreção fracionada de sódio (EFNa) e o número de glomérulos renais. O dano oxidativo, níveis de nitrito e a atividade das enzimas antioxidantes: superóxido dismutase, catalase e glutationa peroxidase foram dosados no plasma e homogenato de rim. O peso corporal foi menor no grupo hipoprotéico e recuperado com ASE. A PAS foi aumentada no grupo hipoprotéico e revertida pelo tratamento com ASE. Os níveis de renina plasmática foram aumentados no grupo hipoprotéico e reduzidos com ASE. A resposta vasodilatadora à ACh em LAM estava reduzida no grupo hipoprotéico com ASE houve uma melhora dessa resposta.O efeito vasodilatador da NG não foi diferente entre os grupos. No grupo hipoprotéico também foi observado o aumento dos níveis de triglicerídeos, colesterol total, uréia, creatinina, glicose e insulina, os mesmos foram reduzidos pelo ASE. Não houve diferença nos níveis de HDL, sódio, potássio, depuração de creatinina e volume urinário nos grupos estudados. Os níveis de malondialdeído e carbonilação de proteínas estavam aumentados e os níveis de nitrito reduzidos no grupo hipoprotéico e foram revertidos pelo ASE. As atividades das enzimas antioxidantes no plasma e no rim foram reduzidas no grupo hipoprotéico e aumentadas pelo ASE, com exceção da catalase que não apresentou diferença entre os grupos. Os animais hipoprotéicos apresentaram redução no número de glomérulos renais e aumento da EFNa e o tratamento com ASE preveniu as alterações renais encontradas neste modelo. Em conclusão, o ASE parece proteger a prole, cujas mães foram expostas a uma dieta com pouca proteína durante a gestação, através do efeito anti-hipertensivo, vasodilatador e antioxidante observado neste trabalho.

Circulatory responses of crucian carp injected intraperitoneally with extracted micro-cystins (MCs) were studied at sublethal and lethal doses (150 and 600 mu g MC kg(-1) body mass, respectively). Mean arterial blood pressure (MAP), heart rate, hematocrit (Hct), red blood cell (RBC) counts, and circulating blood volume (BV) were assayed at 0, 1, 3, 12, 24, and 48 h post-toxin administration. MAP decreased significantly in a dose-dependent manner over time. Within the 48-h test period, the lethal dose as well as the sublethal dose resulted in a steady decline of MAP without recovery. Heart rate significantly increased within 24 h post-injection as blood pressure significantly dropped, then showed a terminal decline to the control level. The dose-dependent decreases in BV and Hct were directly related to the drop in MAP. Intraperitoneal injection of a lethal dose of MCs led to hepatic and gill hemorrhage. Consequently, crucian carp given MCs suffered from hypovolemic hypotensive shock. (C) 2009 Elsevier Ltd. All rights reserved.

Increased oxidative stress induced by hyperglycemia may contribute to the pathogenesis of diabetic complications. Urinary 8-hydroxydeoxyguanosine (8-OHdG) has been reported to serve as a sensitive biomarker of oxidative DNA damage and also of oxidative stress. This article studied oxidative DNA damage in patients with diabetic nephropathy and in healthy control subjects by urinary 8-OHdG evaluations. Contents of 8-OHdG in urine were analyzed by capillary electrophoresis with end-column amperometric detection (CE-AD) after a single-step solid-phase extraction (SPE). Levels of urinary 8-OHdG in diabetic nephropathy patients with macroalbuminuria was significant higher than in control subjects (5.72 +/- 6.89 mumol/mol creatinine versus 2.33 +/- 2.83 mumol/mol creatinine, P = 0.018). A significant difference of 24 h urinary 8-OHdG excretions exists between the patients with macroalbuminuria and the patients with nonnoalbuminuria (19.2 +/- 16.8 mug/24 h versus 8.1 +/- 1.7 mug/24 h, P = 0.015). There was a positive correlation between urinary excretion of 8-OHdG and glycosylated hemoglobin (HbA(1)c) (r = 0.287, P = 0.022). A weak correlation exists between the levels of 8-OHdG and triglyceride (r = 0.230, P = 0.074). However, the urinary 8-OHdG contents are not correlated with blood pressure and total cholesterol. The increased excretion of urinary 8-OHdG is seen as indicating an increased systemic level of oxidative DNA damage in diabetic nephropathy patients. (C) 2004 Elsevier B.V. All rights reserved.

Objective: To study the episodic memory, semantic memory, cognitive planning ability and inhibition ability in MHD patients. Method: Neuropsychological research methods such as Action memory of verb-object phrase, Trail Making Test (A and B), Verbal Fluency Test, Go-No/Go test and Stroop Color Naming Task were used to investigate Episodic Memory 、Semantic Memory、Executive Function of 40 MHD and 40 NC. Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale(SDS), Social Support Scale, Life Satisfaction Scale, and biochemical examination were applied and their relationships with cognitive function were analized. The mean age and education level of MHD group and NC group have no significant difference. Result: 1.Action memory of verb-object phrase differed significantly between MHD group and NC group. 2.Two tests of Verbal Fluency differed significantly between MHD group and NC group. 3.Trail Making Test A, Trail Making Test B, the baseline condition of Go-No/Go Test and Stroop Color Naming Test differed significantly between MHD group and NC group. 4.There is no significant difference between MHD group and NC group on the correct rate of No/Go Test and the baseline condition. Both groups showed Stroop Effect in Go-No/Go test, but MHD group performed significantly worse. 5.In Stroop Color Naming Task Test, NC group showed Stroop Effect, significant Repeated Distraction Promotion Effect and significant Negative Priming Effect,while MHD group showed only Stroop Effect and no Repeated Distraction Promotion Effect and no Negative Priming Effect. There is significant difference in Stroop Effect between MHD group and NC group. Conclusion: 1.Comparing with NC group, episodic memory, semantic memory, cognitive planning ability, and inhibition ability of MHD group were impaired significantly. 2.The pathological aging of Executive Function in MHD group showed: executive Function should be a unitary system. 3.Cognitive impairment is negatively correlated with serum creatinine, blood pressure and anxiety score in MHD patients; and is related with hemoglobin, hematocrit, social support and life satisfaction. Introduction: Worldwide, governments are striving to keep people in work to an older age. However, little is known about the effects of work on an older workforce. This thesis aims to investigate the importance of job characteristics to the antecedents and evolution of cardiovascular disease and functional limitations for the older worker (50+ years). Methods: Three studies were used in this thesis. The 5C (Cork Coronary Care Case- Control) Study investigated the association between job strain and a coronary event in males (n=208) 35-74 years old. The Mitchelstown Study examined the association between job characteristics and positive lifestyle behaviours and further, job characteristics and blood pressure for males and females 50-69 years (n=2,047). Finally, the Cork & Kerry Study investigated the physical effects of manual work and reported functional limitations/disabilities in a sample of 60-80 year olds (n=362). Results: Results from the 5C Study show a clear difference between younger (<50 years) and older (≥50 years) workers, with older workers who had a coronary event more likely to have high job strain and low job control. Data from the Mitchelstown Study showed workers with intermediate possibility for development or high quantitative demands (versus low) at work significantly more likely to have co-occurrence of positive lifestyle behaviours. Further, those who had high possibility for development were more likely to have high systolic blood pressure with no indication of recovery from this activation at night. Physically demanding work as reported by the participants of the Cork & Kerry Study was associated with functional limitations and activities of daily living disability for both the paid and unpaid worker. Discussion: The findings from this piece of work highlight the necessity to examine job characteristics and health outcomes in isolation for the over fifties. The challenge is to get this information into the workplace.

BACKGROUND: Many patients with diabetes have poor blood pressure (BP) control. Pharmacological therapy is the cornerstone of effective BP treatment, yet there are high rates both of poor medication adherence and failure to intensify medications. Successful medication management requires an effective partnership between providers who initiate and increase doses of effective medications and patients who adhere to the regimen. METHODS: In this cluster-randomized controlled effectiveness study, primary care teams within sites were randomized to a program led by a clinical pharmacist trained in motivational interviewing-based behavioral counseling approaches and authorized to make BP medication changes or to usual care. This study involved the collection of data during a 14-month intervention period in three Department of Veterans Affairs facilities and two Kaiser Permanente Northern California facilities. The clinical pharmacist was supported by clinical information systems that enabled proactive identification of, and outreach to, eligible patients identified on the basis of poor BP control and either medication refill gaps or lack of recent medication intensification. The primary outcome is the relative change in systolic blood pressure (SBP) measurements over time. Secondary outcomes are changes in Hemoglobin A1c, low-density lipoprotein cholesterol (LDL), medication adherence determined from pharmacy refill data, and medication intensification rates. DISCUSSION: Integration of the three intervention elements--proactive identification, adherence counseling and medication intensification--is essential to achieve optimal levels of control for high-risk patients. Testing the effectiveness of this intervention at the team level allows us to study the program as it would typically be implemented within a clinic setting, including how it integrates with other elements of care. BACKGROUND: Several trials have demonstrated the efficacy of nurse telephone case management for diabetes (DM) and hypertension (HTN) in academic or vertically integrated systems. Little is known about the real-world potency of these interventions. OBJECTIVE: To assess the effectiveness of nurse behavioral management of DM and HTN in community practices among patients with both diseases. DESIGN: The study was designed as a patient-level randomized controlled trial. PARTICIPANTS: Participants included adult patients with both type 2 DM and HTN who were receiving care at one of nine community fee-for-service practices. Subjects were required to have inadequately controlled DM (hemoglobin A1c [A1c] ≥ 7.5%) but could have well-controlled HTN. INTERVENTIONS: All patients received a call from a nurse experienced in DM and HTN management once every two months over a period of two years, for a total of 12 calls. Intervention patients received tailored DM- and HTN- focused behavioral content; control patients received non-tailored, non-interactive information regarding health issues unrelated to DM and HTN (e.g., skin cancer prevention). MAIN OUTCOMES AND MEASURES: Systolic blood pressure (SBP) and A1c were co-primary outcomes, measured at 6, 12, and 24 months; 24 months was the primary time point. RESULTS: Three hundred seventy-seven subjects were enrolled; 193 were randomized to intervention, 184 to control. Subjects were 55% female and 50% white; the mean baseline A1c was 9.1% (SD = 1%) and mean SBP was 142 mmHg (SD = 20). Eighty-two percent of scheduled interviews were conducted; 69% of intervention patients and 70% of control patients reached the 24-month time point. Expressing model estimated differences as (intervention--control), at 24 months, intervention patients had similar A1c [diff = 0.1 %, 95 % CI (-0.3, 0.5), p = 0.51] and SBP [diff = -0.9 mmHg, 95% CI (-5.4, 3.5), p = 0.68] values compared to control patients. Likewise, DBP (diff = 0.4 mmHg, p = 0.76), weight (diff = 0.3 kg, p = 0.80), and physical activity levels (diff = 153 MET-min/week, p = 0.41) were similar between control and intervention patients. Results were also similar at the 6- and 12-month time points. CONCLUSIONS: In nine community fee-for-service practices, telephonic nurse case management did not lead to improvement in A1c or SBP. Gains seen in telephonic behavioral self-management interventions in optimal settings may not translate to the wider range of primary care settings.

Animals have long been noted for their ability to moderate cardiovascular responses to stress. To date, however, little attention has been directed towards the ability of videotapes of animals to buffer people from challenges. This study thus explored the effect of five video conditions (fish, bird, primate, control 1 [humans], control 2 [blank screen]) on the heart rate and blood pressure of 100 volunteers before and after exposure to a cognitive stressor. Twenty participants were randomly assigned to each of the video conditions. Both the heart rate and blood pressure (diastolic and systolic) of the participants were recorded after a 10 minute period of relaxation (phase 1), following 10 minutes of exposure to the appropriate video for that condition (phase 2) and again, following a 10 minute period of reading aloud, i.e. a cognitive stressor (phase 3). The videos encouraged relaxation, with participants in all conditions exhibiting significantly (p < 0.001) lower levels of heart rate and blood pressure in phase 2 than phases 1 or 3. Individuals exposed to the videos of birds, fish and primates showing significantly (p < 0.001) lower levels of heart rate and blood pressure in phase 3 than individuals exposed to the control videos. It is concluded that videotapes of certain animals can reduce cardiovascular responses to psychological stress and may help to buffer viewers from anxiety, at least in the short term.

Background: The aim of the SPHERE study is to design, implement and evaluate tailored practice and personal care plans to improve the process of care and objective clinical outcomes for patients with established coronary heart disease (CHD) in general practice across two different health systems on the island of Ireland.CHD is a common cause of death and a significant cause of morbidity in Ireland. Secondary prevention has been recommended as a key strategy for reducing levels of CHD mortality and general practice has been highlighted as an ideal setting for secondary prevention initiatives. Current indications suggest that there is considerable room for improvement in the provision of secondary prevention for patients with established heart disease on the island of Ireland. The review literature recommends structured programmes with continued support and follow-up of patients; the provision of training, tailored to practice needs of access to evidence of effectiveness of secondary prevention; structured recall programmes that also take account of individual practice needs; and patient-centred consultations accompanied by attention to disease management guidelines.

Methods: SPHERE is a cluster randomised controlled trial, with practice-level randomisation to intervention and control groups, recruiting 960 patients from 48 practices in three study centres (Belfast, Dublin and Galway). Primary outcomes are blood pressure, total cholesterol, physical and mental health status (SF-12) and hospital re-admissions. The intervention takes place over two years and data is collected at baseline, one-year and two-year follow-up. Data is obtained from medical charts, consultations with practitioners, and patient postal questionnaires. The SPHERE intervention involves the implementation of a structured systematic programme of care for patients with CHD attending general practice. It is a multi-faceted intervention that has been developed to respond to barriers and solutions to optimal secondary prevention identified in preliminary qualitative research with practitioners and patients. General practitioners and practice nurses attend training sessions in facilitating behaviour change and medication prescribing guidelines for secondary prevention of CHD. Patients are invited to attend regular four-monthly consultations over two years, during which targets and goals for secondary prevention are set and reviewed. The analysis will be strengthened by economic, policy and qualitative components.

Purpose: To investigate the association of cardiovascular risk factors and inflammatory markers with neovascular age-related macular degeneration (AMD). Design: Cross-sectional case-control study. Participants: Of the 410 of the =65-year-old community sample invited to attend, 205 participated (50% response rate). Of the 215 clinic attendees who were invited to participate, 212 agreed to take part (98% response rate). A diagnosis of neovascular AMD in at least one eye was made in 193 clinic attendees and 2 of the community sample. Methods: Clinic and community participants underwent a detailed ophthalmic examination with fundus imaging, were interviewed for assessment of putative risk factors, and provided a blood sample. Analysis included levels of serum lipids, intercellular adhesion molecule 1 (ICAM), vascular cellular adhesion molecule (VCAM), and C-reactive protein (CRP). All participants were classified by fundus image grading on the basis of the eye with more severe AMD features. Main Outcome Measure: Neovascular AMD. Results: There were 195 participants with choroidal neovascularization in at least one eye, 97 nonneovascular AMD participants, and 115 controls (no drusen or pigmentary irregularities in either eye). In confounder-adjusted logistic regression, a history of cardiovascular disease was strongly associated with neovascular AMD (odds ratio [OR], 7.53; 95% confidence interval [CI], 2.78-20.41). Cigarette smoking (OR, 3.71; 95% CI, 1.25-11.06), being in the highest quartile of body mass index (OR, 3.82; 95% CI, 1.22-12.01), stage 2 hypertension (OR, 3.21; 95% CI, 1.14-8.98), and being in the highest quartile of serum cholesterol (OR, 4.66; 95% CI, 1.35-16.13) were positively associated with neovascular AMD. There was no association between AMD status and serum CRP, ICAM, or VCAM. Conclusions: Our results suggest that cardiovascular disease plays an etiological role in the development of choroidal neovascularization in a proportion of older adults and highlight the importance of control of blood pressure and cholesterol, avoidance of smoking, and maintenance of a normal body weight. © 2008 American Academy of Ophthalmology.
