A clinical investigation was undertaken to find out the prevalence of craniomandibular signs and symptoms in a group of 11 patients with labiopalatal lesions. The number and distribution of occlusal contacts was evaluated through questionnaire, clinical examination and analysis of mounted casts in partially adjustable articulators in the position of maximum inter cuspation. The most frequent signs and symptoms were articular sounds and lateral pterygoid muscle tenderness to palpation followed by restriction of mouth opening and sensation of tiredness. The number of occlusal contacts was small, mean of 5 contacts per patient, and the site was considered as atypical, 62% of them were on inclined plane surfaces , suggesting occlusal instability. The frequency of signs and symptoms was low and they were of a mild character. The most prevailing signs of craniomandibular dysfunction - articular sounds, muscular tenderness to palpation and restriction of mouth opening - was noticed in only one of the patients

Individuals with spastic cerebral palsy show muscle weakness, difficulties in the control of agonist and antagonist muscles, decreased range of motion and tonus and sensibility alterations, especially in knee joint. These problems can interfere on the performance of functional activities such gait. The aim of this study was to analyze the contribution of knee range of motion on gait of hemiplegic and diplegic children considering their asymmetries. Twelve children, 6 hemiplegics e 6 diplegics from 7 to 12 years of age (age average= 9,5 ± 1,93) took part. Spasticity was assessed by the Ashworth’s Modified Scale and the passive knee range of motion by an eletrogoniometer. The task was to walk on a walkway of 8m long, in their preferred speed, in 6 attempts, been 3 on right and 3 on left sagital planes. Eigth passive markers were bilaterally fixed for the kinematic record. Orthogonally to the walkway, two digital camcorders were assembled on the sagital plane. The fotogrametric procedures were performed by the Dvideow 6.3 software. The Matlab 7.0.1 software was used to filter and to calculate the dependent variables. The U test of Mann- Whitney found differences to the cerebral palsy type for knee extension/hiperextension (U = - 2.943; p= 0.003), knee relative angle at heel contact (U = - 5.992; p= 0.001) and knee range during stride (U = - 4.099; p= 0.001). The Wilcoxon’s test revealed differences according to the asymmetries for the hemiplegics only for the knee relative angle at heel contact (T= - 2.635; p<0.008). The contributions of passive knee range of motion, revealed by the Spearman correlations, for the more afected limb of the diplegics, showed that the knee extension/hiperextension interfere on the cadence, stride duration and step width; the knee relative angle at heel contact change the stride length and duration and cadence; and the...(Complete abstract click electronic access below)

Introdução: A lesão mais comumente encontrada na população ativa é a entorse de tornozelo. Cerca de 40% destes casos continuam a manifestar sensação de instabilidade articular mesmo após a fase aguda da lesão, caracterizando a instabilidade funcional do tornozelo (IFT). A IFT está relacionada com déficits proprioceptivos e redução na capacidade de produção de força e potência muscular. Haja vista que o basquetebol é um esporte com grande incidência de lesões no tornozelo faz-se necessário o entendimento de métodos de avaliação de baixo custo, que possam contribuir para a prevenção deste tipo de lesão em atletas. Objetivo: Nesse sentido, o presente estudo, tem como objetivo identificar os parâmetros de torque, atividade eletromiográfica, senso de reposicionamento articular e a pontuação em teste funcional em jogadoras de basquetebol com tornozelo estável. Correlacionar os valores de torque, atividade eletromiográfica e do senso de reposionamento articular com a pontuação dos testes funcionais em individuos saudáveis. Método: Participaram deste estudo 6 atletas do gênero feminino praticantes do basquetebol com idade entre 18 e 25 anos. As voluntárias realizaram os seguintes testes: torque isocinético concêntrico em inversão (INV) e eversão (EVE) nas velocidades de 60º e 120º, cinco testes funcionais e teste de reposicionamento articular com os ângulos-alvo de 10º e 20º de inversão. Após a verificação da normalidade de distribuição dos dados com o teste Shapiro-Wilk, utilizou-se o teste de Correlação Spearman para verificar a correlação dos testes funcionais com o pico de torque, o sinal EMG e o reposicionamento articular. Articular cartilage is the structure that coats the bone ends in regions where two bones are articulated, allowing movement. It has inefficient intrinsic and extrinsic mechanisms of repair, usually resulting in fibrocartilage formation after injury. Such repair have lower strength, stiffness and usability features when compared to hyaline cartilage. The mesenchymal stem cells have the potential to regenerate tissue without the production of scar, and because of this feature it is well studied. But to have its maximum chondrogenic potential, it is necessary to use scaffolds and growth factors. Biomaterials play the role of scaffold for the cells allowing them to become attached, grow and produce extracellular matrix, leading to formation of repair with hyaline cartilage. In this sense, the purpose of this study is to provide information on the various studies using cell therapy and / or biomaterials to produce hyaline cartilage

The aim of this study was to evaluate the stability of the conservative condylectomy technique and articular disc repositioning as the surgical treatment approach for management of mandibular condylar osteochondroma, with appropriate Orthognathic surgery. Fifteen patients (12 females and 3 males), average age of 32.3 years (range, 13 to 56 years), with unilateral active osteochondroma of the mandibular condyle were analyzed. All patients underwent conservative condylectomy, recontouring of the remaining condylar neck stump and articular disc repositioned and indicated orthognatic surgical procedures. Average post surgical follow-up was 19 months. Each patient's lateral cephalograms were traced at 3 intervals (presurgery, immediate post surgery and long-term follow-up). Immediate after surgery the oclusal plane angle decreased -2.8 ± 4.5o, the maxillomandibular complex rotated counter-clockwise with advancement at menton 5.3 ± 5.6 mm, pogonion 5.0 ± 5.1 mm, B point 3.4 ± 4.2 mm and A point 1.0 ± 1.5 mm. The long-term follow-up showed significant changes in overbite (-0.6 ± 0.5 mm) and SNGoMe (0,93° ± 1,53°). Horizontally and vertically small instabilities occurred in Me (-1.21 ± 1.94 mm) and PNS (-1.48 ± 1.67 mm) respectively. The treatment protocol studied produced counterclockwise rotation and maxillofacial mandibular advancement. The long-term follow-up showed solid dental and skeletal stability with horizontal instability of Me and PNS in the vertical direction.

Temporomandibular joint disorders causing limitation of mouth opening and pain may be related to the restriction of slipping or forward displacement of the mandibular condyle. Temporomandibluar arthrocentesis is the lavage of the joint and is regarded as a simple method that produces good results in patients with these symptoms. This article reports a case of disc displacement and closed lock in a patient who reported limited mouth opening and pain as the main symptoms. The treatment proposed was arthrocentesis and lavage, combining a corticosteroid with a sodium hyaluronate solution. It is concluded that arthrocentesis is a simple, efficient and barely invasive method for particular types of temporomandibular joint disorders and should be considered before opting for more invasive surgical procedures.

Considering the experience taken from the institutional support given by the Humanization’s Brazilian Policy in three health’s regions in São Paulo State, Brazil, this text intends to expose some problems refering to the articulation process of other health’s regional networks mainly composed by cities with less than twenty thousands inhabitants. By problematizing the articulation of inlad health’s networks it is put on the agenda the process of descentraliztion and the interfederaditive relations emphasizing the political and institutional aspects that goes through and modulates these relations, mainly in the small cities. Thus, we are questioning the ways how the current health policies are implemented in these territories and the way how they are articulated, having as a goal the challenge of the process’ ascendance and transversality.

Structural alterations of the bladder induced by detrusor instability. experimental study in rabbits

The aim of this study was to evaluate the histopathological and immunohistochemical alterations induced by detrusor instability in the bladder of rabbits submitted to partial bladder outlet obstruction. Thirty male Norfolk rabbits were divided into 2 groups, a clinical control and a group with detrusor instability. Urine culture, cystometric study, histopathological and immunohistochemical analysis were performed in all animals prior to surgery (M1) and 4 weeks after-surgery (M2). Partial obstruction (G2) resulted in a 2.5 fold increment (p < 0.05) in bladder weight when compared to control (G1). Four weeks after surgery, 93% of animals in G2 developed cystitis. Partial obstruction resulted in detrusor instability at M2 and bladder capacity was significantly increased (p < 0.05) from M1 to M2. The incidence of mild to moderate mucosal and adventitious fibrosis at M2 was higher in G2 (p < 0.05) when compared to G1. Inflammatory reaction at M2 was statistically higher (p < 0.05) in G2. There was no difference in muscular hypertrophy between M1 and M2 in G1. However, 67% of G2 bladders showed a moderate to intense muscular hypertrophy at M2. Hyperplasia of the epithelium was also increased in G2 when M1 and M2 were compared (p < 0.05). Detrusor instability induced by partial bladder outlet obstruction caused significant histopathological and immunohistochemical alterations in the bladder of rabbits.
