OBJECTIVE To compare isoflurane alone or in combination with systemic ketamine and lidocaine for general anaesthesia in horses. STUDY DESIGN Prospective, randomized, blinded clinical trial. ANIMALS Forty horses (ASA I-III) undergoing elective surgery. METHODS Horses were assigned to receive isoflurane anaesthesia alone (ISO) or with ketamine and lidocaine (LKI). After receiving romifidine, diazepam, and ketamine, the isoflurane end-tidal concentration was set at 1.3% and subsequently adjusted by the anaesthetist (unaware of treatments) to maintain a light plane of surgical anaesthesia. Animals in the LKI group received lidocaine (1.5 mg kg(-1) over 10 minutes, followed by 40 microg kg(-1) minute(-1)) and ketamine (60 microg kg(-1) minute(-1)), both reduced to 65% of the initial dose after 50 minutes, and stopped 15 minutes before the end of anaesthesia. Standard clinical cardiovascular and respiratory parameters were monitored. Recovery quality was scored from one (very good) to five (very poor). Differences between ISO and LKI groups were analysed with a two-sample t-test for parametric data or a Fischer's exact test for proportions (p < 0.05 for significance). Results are mean +/- SD. RESULTS Heart rate was lower (p = 0.001) for LKI (29 +/- 4) than for ISO (34 +/- 6). End-tidal concentrations of isoflurane (ISO: 1.57% +/- 0.22; LKI: 0.97% +/- 0.33), the number of horses requiring thiopental (ISO: 10; LKI: 2) or dobutamine (ISO:8; LKI:3), and dobutamine infusion rates (ISO:0.26 +/- 0.09; LKI:0.18 +/- 0.06 microg kg(-1) minute(-1)) were significantly lower in LKI compared to the ISO group (p < 0.001). No other significant differences were found, including recovery scores. CONCLUSIONS AND CLINICAL RELEVANCE These results support the use of lidocaine and ketamine to improve anaesthetic and cardiovascular stability during isoflurane anaesthesia lasting up to 2 hours in mechanically ventilated horses, with comparable quality of recovery.

AIMS The GLOBAL LEADERS trial is a superiority study in patients undergoing percutaneous coronary intervention, with a uniform use of Biolimus A9-eluting stents (BES) and bivalirudin. GLOBAL LEADERS was designed to assess whether a 24-month antithrombotic regimen with ticagrelor and one month of acetylsalicylic acid (ASA), compared to conventional dual antiplatelet therapy (DAPT), improves outcomes. METHODS AND RESULTS Patients (n >16,000) are randomised (1:1 ratio) to ticagrelor 90 mg twice daily for 24 months plus ASA ≤100 mg for one month versus DAPT with either ticagrelor (acute coronary syndrome) or clopidogrel (stable coronary artery disease) for 12 months plus ASA ≤100 mg for 24 months. The primary outcome is a composite of all-cause mortality or non-fatal, new Q-wave myocardial infarction at 24 months. The key safety endpoint is investigator-reported class 3 or 5 bleeding according to the Bleeding Academic Research Consortium (BARC) definitions. Sensitivity analysis will be carried out to explore potential differences in outcome across geographic regions and according to specific angiographic and clinical risk estimates. CONCLUSIONS The GLOBAL LEADERS trial aims to assess the role of ticagrelor as a single antiplatelet agent after a short course of DAPT for the long-term prevention of cardiac adverse events, across a wide spectrum of patients, following BES implantation.

BACKGROUND Physicians traditionally treat ulcerative colitis (UC) using a step-up approach. Given the paucity of data, we aimed to assess the cumulative probability of UC-related need for step-up therapy and to identify escalation-associated risk factors. METHODS Patients with UC enrolled into the Swiss IBD Cohort Study were analyzed. The following steps from the bottom to the top of the therapeutic pyramid were examined: (1) 5-aminosalicylic acid and/or rectal corticosteroids, (2) systemic corticosteroids, (3) immunomodulators (IM) (azathioprine, 6-mercaptopurine, methotrexate), (4) TNF antagonists, (5) calcineurin inhibitors, and (6) colectomy. RESULTS Data on 996 patients with UC with a median disease duration of 9 years were examined. The point estimates of cumulative use of different treatments at years 1, 5, 10, and 20 after UC diagnosis were 91%, 96%, 96%, and 97%, respectively, for 5-ASA and/or rectal corticosteroids, 63%, 69%, 72%, and 79%, respectively, for systemic corticosteroids, 43%, 57%, 59%, and 64%, respectively, for IM, 15%, 28%, and 35% (up to year 10 only), respectively, for TNF antagonists, 5%, 9%, 11%, and 12%, respectively, for calcineurin inhibitors, 1%, 5%, 9%, and 18%, respectively, for colectomy. The presence of extraintestinal manifestations and extended disease location (at least left-sided colitis) were identified as risk factors for step-up in therapy with systemic corticosteroids, IM, TNF antagonists, calcineurin inhibitors, and surgery. Cigarette smoking at diagnosis was protective against surgery. CONCLUSIONS The presence of extraintestinal manifestations, left-sided colitis, and extensive colitis/pancolitis at the time of diagnosis were associated with use of systemic corticosteroids, IM, TNF antagonists, calcineurin inhibitors, and colectomy during the disease course.

BACKGROUND The clinical presentation of spondylsodiscitis/spondylitis are manifold. This commonly leads to a period of several months from initial symptoms to final diagnosis. A standardised treatment is difficult. The purpose of this study is to investigate the treatment carried out for patients with spondylodiscitis or spondylitis to develop an individualised standard care for better treatment. PATIENTS AND METHODS Data of 90 patients were retrospective analysed. In particular documented data of the initial examination and the following treatments concerning identification of causes and systematically control of pathogens were examined. RESULTS In 91 % of patients a diagnostically conclusive MRI was conducted. The degree of spondylidiscitis/spondylitis was mainly ASA criteria I or II (86 %). In 96 % of patients different diagnostic methods for identification of pathogens were conducted and documented. RESULTS confirmed the most common pathogens mentioned in the literature. 75 % of patients were treated by surgery. In 93 % of patients an antibiotic treatment was documented. 50 patients (81 %) were successfully healed. CONCLUSION It is important to identify and treat spondylodiscitis/spondylitis as early as possible. Diagnosis by means of blood culture and MRI and treatment of the infection with antibiotics and possibly surgical interventions seem be very suitable, but need to be individualised to each and every patient.

(beginning of rainbow smelt executive summary) Evidence indicates that anadromous rainbow smelt (Osmerus mordax) populations in Connecticut and elsewhere in the northeast United States have severely declined. Several sampling programs have documented declines in Connecticut’s smelt populations over the last three decades (Marcy 1976a, Marcy 1976b, Millstone Environmental Laboratory 2005). Similar declines have also been documented in the Hudson River (ASA Analysis & Communication 2005) and in Massachusetts (personal communication, Brad Chase, MA Division of Marine Fisheries 2004). Recreational and commercial fisheries in the region for this species have virtually ceased (Blake and Smith 1984). The Connecticut Fish Advisory Committee of the Endangered Species Program has recommended that rainbow smelt be listed as threatened in Connecticut, and the National Marine Fisheries Service (2004) has recently listed rainbow smelt as a Federal Species of Concern. The purpose of this project is to develop an environmental history of rainbow smelt in Connecticut and surrounding regions, and document the current status of populations in Connecticut waters. An environmental history that assesses trends in abundance, environmental threats and historical efforts to ameliorate the threats will contribute to regional efforts to conserve these fish. Comprehensive review of the regional literature and trends associated with rainbow smelt has not been undertaken since Kendall (1926). Assessment of current abundance, distribution, areas of critical habitat, and whether the species is presently reproducing in state waters is critical for clarifying conservation status, designing a monitoring program and developing a recovery or enhancement plan, if this appears to be necessary. (beginning of tomcod executive summary) Atlantic tomcod (Microgadus tomcod) are believed to have declined significantly in Connecticut and other estuaries of the Northeast and Middle Atlantic states. Several monitoring programs indicate that the species is scarce and/or declining in the region’s estuaries (Gottschall and Pacileo 2004, Molnar 2004, Millstone Environmental Laboratory 2005, ASA Analysis and Communication 2005). Once-active recreational (NMFS MRFSS 2005, http://www.st.nmfs.gov) and commercial fisheries for this species in Connecticut are now dormant. For the past 10 years, the Connecticut Fish Advisory Committee of the Endangered Species Program has recommended that studies be undertaken to quantify the status of tomcod populations and to determine if conservation actions should be initiated. The purpose of this project is to develop an environmental history of Atlantic tomcod in Connecticut and surrounding regions, and document the current status of populations in Connecticut waters. An environmental history that assesses trends in abundance, environmental threats and historical efforts to ameliorate the threats will contribute to regional efforts to conserve these fish. Assessment of current abundance, distribution, areas of critical habitat, and whether the species is presently reproducing in state waters is critical for determining conservation status, designing a monitoring program and developing a recovery or enhancement plan, if this appears to be necessary.

La reperfusión, luego de un período de isquemia miocárdica breve, puede desencadenar un daño paradojal, dentro del cual, se destacan las arritmias ventriculares. Existen estudios que reportan un efecto beneficioso del ácido acetilsalicílico (AAS) a nivel cardiovascular, pero se desconocen los efectos electrofisiológicos en el proceso de injuria por isquemia/reperfusión. El objetivo de este estudio es evaluar las propiedades electrofisiológicas del AAS, en especial si puede evitar las arritmias de reperfusión (AR) en forma independiente de su efecto antiplaquetario. Se trabajó con corazones aislados de rata Sprague Dawley según la técnica de Langendorff sometidos a 10 minutos de isquemia regional. Se realizaron 3 series esperimentales: 1) control (C, n=10); 2) , corazones perfundidos durante todo el protocolo con AAS 0.14 mM (AAS, n=10) y 3) corazones que recibieron la misma dosis de AAS sólo en los 3 primeros minutos de la reperfusión (AASR, n=9). Se analizaron la incidencia y severidad de las AR y su relación con el ECG y los potenciales de acción registrados simultáneamente. El 82% del grupo control presentó AR sostenidas, el 30 % con AAS y el 22% con AASR (ambas p<0.05 por χ2). En la reperfusión se observó que luego de los primeros tres minutos la duración del potencial de acción (DPA) fue mayor en el grupo AASR (81,5 ± 23,1) que en el grupo AAS (55,2 ± 10,0) p<0.05 por ANOVA I. Por lo tanto, la menor incidencia de AR en los grupos tratados podría asociarse al efecto de la aspirina sobre la DPA y que la droga estudiada tendría efectos sobre esta variable sólo al momento de reperfusión.

Esta investigación tiene como eje fundamental el estudio de las relaciones actuales sino-rusas y, más particularmente, a la organización de cooperación de Shanghái (en adelante OCS) como un punto de convergencia de intereses Chinos y Rusos en la región de Asía central. Dicha dinámica supone, por un lado, la posibilidad de un proceso de colaboración y de desarrollo conjunto en la región. Pero por otro lado supone un conflicto de intereses ya que tanto China como Rusia buscan expandir su órbita de influencia en el espacio post-sovietico ya señalado. Por lo dicho este estudio intenta acercarse a la comprensión de la dinámica de la OCS en los últimos años. Para ello se aproxima a la temática, partir de dos miradas: La primera podría catalogarse como una mirada que enfatiza cuestiones de carácter acontecimental y cuyo interés reside en comprender los cambios acaecidos en la OCS en los últimos años, un análisis de los acuerdos hacia dentro y fuera del grupo, los marcos e instituciones que componen a la OCS, las distintas instancias de contacto entre los gobiernos ruso y chino, etc. La segunda mirada, a sabiendas de la expansión de las economías asiáticas en los últimos años, profundiza lo expresado anteriormente y lo enmarca en una concepción más amplia que parte del presupuesto teórico de que nos encontramos actualmente en una transición del centro de poder global (Wallerstein, 2006) desde Estados Unidos y Europa Occidental hacia las potencias emergentes asiáticas como la República popular de China, Rusia, India y el continente asiático en general. Dicha mirada funcionara como una guía y un presupuesto para pensar la cuestión desde una óptica de largo plazo

La noción de autonomía ha sido uno de los mayores puntos de tensión teórica del análisis de la política exterior argentina. En este artículo se plantea una revisión etimológica, epistemológica y metodológica de dicha noción con el fin de mostrar la complejidad conceptual de esta discusión y orientar las decisiones prácticas que, derivado de este asunto, pudieran tomarse. Para ello se revisa la relación entre teoría y método, el concepto como basamento de una estructura teórica y las propiedades del mismo. Posteriormente se presenta el origen etimológico de la noción de autonomía y se exponen algunas de las concepciones filosóficas más influyentes de la modernidad occidental para, finalmente, exponer su articulación en el ámbito de las Relaciones Internacionales.
