A pneumonia e a bronquiolite na infância são as principais causas de morbidade e mortalidade no mundo, sendo o Vírus Respiratório Sincicial Humano (VSRH) o principal agente viral. O VSRH está associado a surtos anuais de doenças respiratórias, onde a co-infecção bacteriana tem sido relatada. Este foi o primeiro estudo do VRSH em crianças hospitalizadas com Pneumonia Adquirida na Comunidade (PAC) em Belém, Pará (Norte do Brasil), que teve como objetivo determinar a prevalência da infecção pelo VSRH e avaliar as características clínicas e epidemiológicas dos pacientes. Métodos. Foi realizado um estudo prospectivo em oito hospitais no período de novembro de 2006 a outubro de 2007. Foram testadas 1.050 amostras de aspirado nasofaríngeo para o VRSH, obtidas de crianças hospitalizadas com até três anos de idade com diagnóstico de PAC, pelo método da imunofluorescência direta e da reação em cadeia por polimerase após transcrição reversa (RT-PCR) para identificação do subtipo viral. Foram obtidos resultados da dosagem de proteína C-reativa (PCR) e da cultura bacteriana. Resultados. A infecção pelo VSRH foi diagnosticada em 243 (23,1%) crianças. A idade média do grupo VRSH-positivo foi menor do que a do grupo VRSH-negativo (12,1 meses versus (vs) 15,5 meses, ambos com variância de 1-36 meses, p<0,001), enquanto que a distribuição por genero foi similar. O grupo VRSH-positivo apresentou menor dosagem (PCR) quando comparados ao grupo VRSH-negativo (15,3 vs 24.0 mg/dL, p<0,05). Os achados radiológicos confirmaram que 54,2% do grupo VRSH-positivo e 50,3% do grupo VRSH-negativo apresentavam infiltrado intersticial. A infecção bacteriana foi identificada predominantemente no grupo VRSH-positivo (10% vs 4,5%, p<0,05). Rinorréia e obstrução nasal foram predominantemente observadas no grupo VRSH-positivo. A co-circulação dos subtipos A e B foi observada, com predominância do subtipo B (209/227). A análise multivariada revelou que a idade de 1 ano (p<0,015), os níveis de PCR inferior a 48 mg/dL (p<0,001) e a co-infecção bacteriana (p<0,032) foram independentemente associados com a presença do VRSH em oposição ao grupo VRSH-negativo, e na análise dos sintomas, a obstrução nasal foi independentemente associada com o grupo VRSH-positivo (p<0,001). Conclusão. O presente estudo destaca a relevância da infecção por VSRH em casos hospitalizados de PAC em nossa região; nossos resultados justificam a realização de investigações adicionais que possam ajudar a elaborar estratégias para o controle da doença.

Eighty-two sera were collected from Mekranoiti Indians, yielding 39% of positivity, whereas 11 (20.0%) or the 55 Tiriyo serum samples had antibody to HTLV-I. In addition, positive results occurred in 10 (23.2%) out of 43 sera obtained from patients living in the Belem area, who were suffering from cancer affecting different organs. Five (16.7%) out of 30 Elisa positive specimens were also shown to be positive by either Western blot analysis (WB) or indirect immunogold electron microscopy (IIG-EM).

Examining three bleaching systems, this in vivo clinical trial evaluated the relationship among tooth sensitivity, light activation, and agent concentration, and it correlated dental sensitivity with tooth thickness.Materials and Methods: Eighty-seven volunteer patients were included. Inclusion criteria were the presence of anterior teeth without restorations as well as the absence of a previous bleaching experience and absence of non-carious cervical lesions or dental pain. Exclusion criteria included pregnancy or breastfeeding, a maximum of TF3 hypoplasia, tetracycline-fluorosis stains, malpositioned teeth, orthodontic treatment, periodontal disease, and/or analgesic/anti-inflammatory intake. Patients were randomly assigned to three bleaching groups: Group A (n=25) was treated with 15% H2O2 and nitrogenous-titanium-dioxide and was light activated (Lase Peroxide Lite, DMC, SaoCarlos, Sao Paulo, Brazil); Group B (n=27) was treated with 35% H2O2 and was light activated (Lase Peroxide Sensy, DMC); and Group C (n=35) was treated with 35% H2O2 (White Gold Office, Dentsply, 38West Clark Ave., Milford, USA) without light activation. Tooth sensitivity (TS) was self-reported by the patients using the visual analog scale (VAS) at baseline (TSO), immediately after treatment (TSI), and at seven days after treatment (TS7). In 46 patients, tooth thickness was determined by computed tomography. TSO, TSI, and TS7 were compared between the A and B groups to determine the effect of concentration and between the B and C groups to determine the effect of light using analysis of covariance. The correlation between tooth thickness and TSI was determined by Spearman Rho test (SPSS 15).Results: Eighty-seven patients were evaluated at baseline, and 61 were evaluated at seven days. Separated by groups, tooth sensitivity, expressed as VAS value at the time points TS0, TS1, and TS7, respectively, were as follows: Group A: 13.76 +/- 13.53, 24.40 +/- 25.24, and 5.94 +/- 5.5; Group B: 15.07 +/- 18.14, 42.4 +/- 31.78, and 8.68 +/- 17.99; and Group C: 10.80 +/- 14.83, 31.51 +/- 29.34, and 7.24 +/- 9.2. Group A showed significantly lower tooth sensitivity than group B at TSI (p=0.032). No differences were observed in the tooth sensitivities between groups B and C. No correlation was encountered between tooth thickness and tooth sensitivity immediately after treatment (Rho=-0.088,p=0.563). The median tooth thickness was 2.78 +/- 0.21 mm.Conclusions: Increases in the concentration of bleaching agents directly affect tooth sensitivity, and LED/laser activation and tooth thickness are not correlated with tooth sensitivity after dental bleaching.

Vocal nodules constitute the major cause of dysphonia during childhood. Auditory-perceptual and acoustic vocal analyses have been used to differentiate vocal nodules from normal voice in children.Purpose: To study the value of auditory-perceptual and acoustic vocal analyses in assessments of children with nodules.Design: Diagnostic test study.Patients and interventions: A comparative study was carried out including 100 children with videolaryngoscopic diagnosis of vocal nodules (nodule group-NG); and 100 children without vocal symptoms and with normal videolaryngoscopic exams (control group-CG). The age range of both groups was between 4 and 11 years. All children underwent auditory-perceptual vocal analyses (GRBASI scale); maximum phonation time and s/z ratio were calculated, and acoustic vocal analysis (MDVP software) were carried out.Results: There was no difference in the values of maximum phonation time and s/z ratio between groups. Auditory-perceptual analysis indicated greater compromising of voice parameters for NG, compared to CG: G (79 versus 24), R (53 versus 3), B (67 versus 23) and S (35 versus 1). The values of acoustic parameters jitter, PPQ shimmer, APQ, NHR and SPI were higher for NG for CG. The parameter f0 did not differ between groups.Conclusion: Compromising of auditory-perceptual (G, R, B and S) and acoustic vocal parameters (jitter, PPQ shimmer, APQ, NHR and SPI) was greater for children with nodules than for those of the control group, which makes them important methods for assessing child dysphonia. Rocuronium (ROC) is a neuromuscular blocking agent used in surgical procedures which is eliminated primarily by biliary excretion. A liquid chromatography-tandem mass spectrometry (LC-MS/MS) method was developed and validated for analysis of ROC in human plasma. Separation of ROC and IS (verapamil) was performed using an endcapped C-18 column and a mixture of water:acetonitrile:trifluoracetic acid (50:50:0.1, v/v) as mobile phase. Aliquots of 100 mu L of human plasma were extracted at pH 3, using dichloromethane. The lower limit of quantification of 5 ng/mL shows the high sensitivity of this method. Intra- and inter-assay precision (as relative standard deviation) was all <= 14.2% and accuracy (as relative standard error) did not exceed 10.1%. The validated method was successfully applied to quantify ROC concentrations in patients under surgical procedures up to 6 h after the administration of the 0.4-0.9 mg/kg ROC. The pharmacokinetic parameter estimations of ROC showed AUC/dose of 563 mu g min/mL, total clearance of 2.5 mL/min/kg, volume of distribution at steady state of 190 mL/kg and mean residence time of 83 min.
