The aim of this environmental performance review is precisely to help Peru assess its progress towards achieving its environmental goals, enrich and ensure that the necessary policy dialogue continues uninterrupted, promote better accountability and deepen the awareness of all actors involved in the economic and social development of Peru. The 66 recommendations included in this study target some of the country’s main environmental challenges.

No hemisfério norte, o censo de aves é fundamental para gerar informações que auxiliam na compreensão de tendências populacionais. Tais censos, devido à marcada sazonalidade deste hemisfério, são realizados durante dois momentos distintos: na estação reprodutiva (aves residentes) e no inverno (quando as aves migratórias deixam determinadas regiões). Na região neotropical, porém, dependendo da localidade, as aves podem se reproduzir durante qualquer ou vários períodos do ano; podem ou não migrar, e aquelas que o fazem podem apresentar um padrão assincrônico. Em contraste com o hemisfério norte, tendências populacionais são desconhecidas, bem como o impacto das taxas rápidas de urbanização e desmatamento, que também são pouco monitoradas. Para melhor entender padrões temporais de riqueza e abundância de aves, e avaliar como um censo similar pode ser implementado na América tropical, foram utilizados pontos de escuta ao longo de 12 meses em uma localidade no Estado de São Paulo, sudeste do Brasil. Os censos ocorreram duas vezes por dia (manhãs/tardes) em uma floresta semidecidual ao longo de transecções com 10 pontos (20 pontos por dia) distantes 200 m entre si e com raio de detecção limitado em 100 m. Ambas as riquezas e abundâncias de aves foram maiores durante as manhãs, mas as curvas de acumulação sugerem que os censos vespertinos com maior esforço amostral podem fornecer resultados similares aos censos matutinos. Riqueza e abundância das aves não variam de acordo com estações (i.e., sem padrão aparente entre reprodução e migração), enquanto espécies exclusivas foram encontradas todos os meses e relativamente poucas espécies (20%) foram registradas em todos os meses do ano. Durante este ano, 84% de todas as aves florestais da área estudada foram registradas. Sugerimos que a metodologia de pontos de escuta pode ser utilizada à semelhança dos censos do hemisfério norte. Recomendamos ainda que o esforço amostral em transecções deva incluir ao menos 20 pontos, e que o início da contagem das aves deva ser sazonal, utilizando o período de migração das espécies austrais (e os seis meses seguintes) para coordenar pontos de escuta. Por último, sugerimos que os censos no Brasil e até mesmo na América Latina podem ajudar no entendimento de tendências populacionais, mas também demandam maior esforço do que o observado em latitudes temperadas, devido à maior riqueza de espécies e diferenças nas dinâmicas de reprodução e migração. Por meio do uso de censos de aves coordenados poderá ser desenvolvida uma técnica para os trópicos que irá gerar informações que permitam acompanhar tendências populacionais, com benefícios para a conservação das aves, similarmente aos censos realizados em países do hemisfério norte.

Gastrointestinal problems are common, especially in endurance athletes, and often impair performance or subsequent recovery. Generally, studies suggest that 30-50 % of athletes experience such complaints. Most gastrointestinal symptoms during exercise are mild and of no risk to health, but hemorrhagic gastritis, hematochezia, and ischemic bowel can present serious medical challenges. Three main causes of gastrointestinal symptoms have been identified, and these are either physiological, mechanical, or nutritional in nature. During intense exercise, and especially when hypohydrated, mesenteric blood flow is reduced; this is believed to be one of the main contributors to the development of gastrointestinal symptoms. Reduced splanchnic perfusion could result in compromised gut permeability in athletes. However, although evidence exists that this might occur, this has not yet been definitively linked to the prevalence of gastrointestinal symptoms. Nutritional training and appropriate nutrition choices can reduce the risk of gastrointestinal discomfort during exercise by ensuring rapid gastric emptying and the absorption of water and nutrients, and by maintaining adequate perfusion of the splanchnic vasculature. A number of nutritional manipulations have been proposed to minimize gastrointestinal symptoms, including the use of multiple transportable carbohydrates, and potentially the use of nutrients that stimulate the production of nitric oxide in the intestine and thereby improve splanchnic perfusion. However, at this stage, evidence for beneficial effects of such interventions is lacking, and more research needs to be conducted to obtain a better understanding of the etiology of the problems and to improve the recommendations to athletes.

Thirty-three feedlot cattle nutritionists were surveyed to evaluate the management practices and nutritional recommendations adopted by feedlots in Brazil. The web-based survey consisted of 81 questions that included: general information (n = 10); general commodity information (n = 15); use of coproducts (n = 5), roughage source and level (n = 5); adaptation methods (n = 7); feed mixers (n = 6); feeding management (n = 6); cattle management and type of cattle fed (n = 13); formulation practices (n = 9); information resources used for nutritional recommendations (n = 2); and additional questions (n = 3). In total, the 33 nutritionists were responsible for approximately 2,658,000 animals, and moreover, 65.5% of those participants had clients that feed less than 5000 animals yearly. Corn was the primary source of grain used in feedlot diets (87.9%) and cracking was the primary processing method recommended by nutritionists (57.6%). The average concentrate and roughage inclusion in finishing diets was 79.0% and 21.0%, respectively. The main challenges faced by nutritionists are the lack of available and precise equipment and lack of trained employees with respiratory diseases as the main health problem. This survey of nutrition and management practices should aid in the development of research for the feedlot industry in Brazil and similar tropical climates, as well as provide data to facilitate the broader application of future NRC models. A significant infection rate of bovine TB in the deer population of the northeastern lower peninsula poses a potential risk to several important values including public health, United States Department of Agriculture (U.S.D.A.) TB-free accreditation for Michigan cattle, wildlife health, wildlife-related recreation and tourism and economic stability in several sectors. A risk assessment study by the U.S. D.A. Centers for Epidemiology and Animal Health (Fort Collins, CO) predicted that if no changes were made in the management of the affected free-ranging deer population, the TB prevalence (compared to the current prevalence of 2.3%). Although the current annual risk of TB transfer to cattle in the affected area is .I%, the report estimated a 12% cumulative risk that at least one head of cattle would become infected over the next 25 years if no changes are made in deer and/or cattle management.

The lack of data records of electric power consumption of smallphotovoltaic home systems, independently of the method used for sizing them, drives to consider the demand as a constant. However, the existing data reveal the variability of the consumption due to the influences of some social, cultural and psychosocial aspects of the human groups. This paper presents records of consumption data obtainedfrom several solar home systems (SHSs) in Brazil and Peru, and it discusses about the Gamma distribution function that can express to a great extent the behaviour of the demand. By this analysis it was verified that `a lot of people consume little and few people consume a lot`. In that sense, a few recommendations for sizing procedures that can be useful in the implantation of extensive programmes of rural electrification by SHSs are presented. Patients with cleft lip and palate usually present dental anomalies of number, shape, structure and position in the cleft area and the general dentist is frequently asked to restore or extract those teeth. Considering that several anatomic variations are expected in teeth adjacent to cleft areas and that knowledge of these variations by general dentists is required for optimal treatment, the objectives of this paper are: 1) to describe changes in the innervation pattern of anterior teeth and soft tissue caused by the presence of a cleft, 2) to describe a local anesthetic procedure in unilateral and bilateral clefts, and 3) to provide recommendations to improve anesthetic procedures in patients with cleft lip and palate. The cases of 2 patients are presented: one with complete unilateral cleft lip and palate, and the other with complete bilateral cleft lip and palate. The patients underwent local anesthesia in the cleft area in order to extract teeth with poor bone support. The modified anesthetic procedure, respecting the altered course of nerves in the cleft maxilla and soft tissue alterations at the cleft site, was accomplished successfully and the tooth extraction was performed with no pain to the patients. General dentists should be aware of the anatomic variations in nerve courses in the cleft area to offer high quality treatment to patients with cleft lip and palate.

Skeletal muscle is the major deposit of protein molecules. As for any cell or tissue, total muscle protein reflects a dynamic turnover between net protein synthesis and degradation. Noninvasive and invasive techniques have been applied to determine amino acid catabolism and muscle protein building at rest, during exercise and during the recovery period after a single experiment or training sessions. Stable isotopic tracers (13C-lysine, 15N-glycine, ²H5-phenylalanine) and arteriovenous differences have been used in studies of skeletal muscle and collagen tissues under resting and exercise conditions. There are different fractional synthesis rates in skeletal muscle and tendon tissues, but there is no major difference between collagen and myofibrillar protein synthesis. Strenuous exercise provokes increased proteolysis and decreased protein synthesis, the opposite occurring during the recovery period. Individuals who exercise respond differently when resistance and endurance types of contractions are compared. Endurance exercise induces a greater oxidative capacity (enzymes) compared to resistance exercise, which induces fiber hypertrophy (myofibrils). Nitrogen balance (difference between protein intake and protein degradation) for athletes is usually balanced when the intake of protein reaches 1.2 g·kg-1·day-1 compared to 0.8 g·kg-1·day-1 in resting individuals. Muscular activities promote a cascade of signals leading to the stimulation of eukaryotic initiation of myofibrillar protein synthesis. As suggested in several publications, a bolus of 15-20 g protein (from skimmed milk or whey proteins) and carbohydrate (± 30 g maltodextrine) drinks is needed immediately after stopping exercise to stimulate muscle protein and tendon collagen turnover within 1 h.

Because of the development of modern transportation facilities, an ever rising number of individuals including many patients with preexisting diseases visit high-altitude locations (>2500 m). High-altitude exposure triggers a series of physiologic responses intended to maintain an adequate tissue oxygenation. Even in normal subjects, there is enormous interindividual variability in these responses that may be further amplified by environmental factors such as cold temperature, low humidity, exercise, and stress. These adaptive mechanisms, although generally tolerated by most healthy subjects, may induce major problems in patients with preexisting cardiovascular diseases in which the functional reserves are already limited. Preexposure assessment of patients helps to minimize risk and detect contraindications to high-altitude exposure. Moreover, the great variability and nonpredictability of the adaptive response should encourage physicians counseling such patients to adapt a cautionary approach. Here, we will briefly review how high-altitude adjustments may interfere with and aggravate/decompensate preexisting cardiovascular diseases. Moreover, we will provide practical recommendations on how to investigate and counsel patients with cardiovascular disease desiring to travel to high-altitude locations.

The treatment of multiple myeloma has undergone significant changes in the recent past. The arrival of novel agents, especially thalidomide, bortezomib and lenalidomide, has expanded treatment options and patient outcomes are improving significantly. This article summarises the discussions of an expert meeting which was held to debate current treatment practices for multiple myeloma in Switzerland concerning the role of the novel agents and to provide recommendations for their use in different treatment stages based on currently available clinical data. Novel agent combinations for the treatment of newly diagnosed, as well as relapsed multiple myeloma are examined. In addition, the role of novel agents in patients with cytogenetic abnormalities and renal impairment, as well as the management of the most frequent side effects of the novel agents are discussed. The aim of this article is to assist in treatment decisions in daily clinical practice to achieve the best possible outcome for patients with multiple myeloma.
