Eleven cases of involvement of the genital tract in paracoccidioidomycosis were collected in a retrospective study of the clinical records of 683 patients seen in Porto Alegre, Rio Grande do Sul, Brazil. These cases are herein summarily reported. Eighteen similar cases were gathered in review of the Brazilian literature. Obtained data are discussed.

One of the important challenges for multinational organisations is to increase the ability to retain their repatriates. The literature review shows high turnover rates amongst repatriates in the first two years following return from the international assignment. This paper discusses organisation’s (in)ability to retain its repatriates. A diversity of predictable factors of repatriates’ turnover is identified, including factors related to the job, the organisation, qualification, career development, personality, social context and the market. The paper analysis organisational support practices designed to increase the power of retention and decrease the turnover. We concluded that research about retention versus turnover of repatriates is scarce, mainly regarding the role a formal repatriation and retention programme in facilitating the retention of these employees with acquired international knowledge and skills, that is, the role that organisational factors play in voluntary turnover decisions. Further research is suggested at the end of the article.

This investigation reviews literature on human resource management practices that influence the retention of repatriates. The processes of selection and training/preparation before the departure, the role of the mentor and of communication during the international assignment, a program of readjustment to repatriation and a career development plan after return to the home firm are the practices identified in the literature as the main promoters of repatriates’ retention. Evidence suggests that greater responsibility on the part of the firms before, during and after the international assignment allows for more efficiency in the management of their repatriates.

RESUMO - A infecção por VIH é reconhecida mundialmente como um problema de saúde pública, apesar de todos os esforços e recursos despendidos e dos avanços científicos conseguidos nos últimos anos. Portugal, apesar do registo da diminuição de novos casos de infecção VIH, encontra-se no 4.º lugar (dos países da União Europeia) em termos de incidência de infecção por VIH (dados de 2008). Esta situação, juntamente com a crescente constatação dos custos elevados para o tratamento desta infecção, da variabilidade de práticas e a fragmentação dos cuidados, motivou a reflexão sobre modelos alternativos de organização de cuidados, inspirados em modelos de gestão da doença e idealmente centrados nos cuidados de saúde primários. O presente trabalho teve como objectivos analisar os modelos de organização de cuidados prestados à pessoa com VIH, nos países desenvolvidos, a nível dos cuidados de saúde primários e compreender o nível de adequação dos resultados encontrados, para a realidade portuguesa. Para os alcançar foi realizada uma revisão da literatura e a realização de entrevistas a profissionais de saúde. Nos artigos analisados na revisão da literatura, verificou-se consenso em torno de um modelo baseado em cuidados partilhados, entre os prestadores primários e os diferenciados. Foram ainda referenciados múltiplos factores e condições que terão de ser assegurados para uma possível implementação do modelo. Quanto aos entrevistados verificou-se uma opinião favorável em relação ao modelo de cuidados partilhados, mostrando-se expectantes e despertos para a necessidade de mudança, perspectivando uma maior integração do actual estado de organização dos cuidados para a pessoa com VIH. Estes indícios reflectem a relevância de maior investigação sobre o modelo de cuidados partilhados.

There has been a growing interest in the role of children’s literature in language teaching since the 80s, when the communicative approach made it possible to bring stories into the classroom (Garvie, 1990). It is undeniable that storytelling has many benefits. Not only are children naturally drawn to stories, but they are also an effective and enjoyable way to teach and learn. This article presents the findings of a MA project on using stories with children. It shows the importance of stories on language acquisition and concludes with some practical suggestions based on my teaching experience with young learners.

The potentialities of ICT in education bring about changes in the teaching and learning methodologies, in the places where you learn and in the way you learn. This demands a reflection not only on the ways of learning, but also on the support resources, so that learning can take place and, of course, it is indispensable to understand the teachers’ answer to the digital challenges. Thus, the purpose of this analysis is to reflect about technological trends in an educational context and their underlying models by analyzing the role played by digital textbooks in Portugal in an innovating context. This way, we intend to contribute to an educational policy as we plan to relate the teachers’ training to the increasing development of the digital textbooks and we also intend to contribute to the understanding of a didactic resource which is closely related to the learning processes which resort to advanced technology.

RESUMO: Os programas psicoeducacionais para cuidadores de pessoas com esquizofrenia são muitas vezes construídos sem terem em conta as necessidades educacionais destes utilizadores dos serviços de saúde mental. Apresenta-se uma revisão da literatura sobre esquizofrenia, família, intervenções familiares e avaliação de necessidades educacionais. Conduziu-se um estudo transversal descritivo com características exploratórias numa amostra de conveniência de principais cuidadores (N=74) de pessoas com esquizofrenia em tratamento no Departamento de Psiquiatria e Saúde Mental do Hospital de S. Francisco Xavier. A metodologia utilizada é do tipo qualitativo e quantitativo. Objectivos: Descrever as características de uma amostra de principais cuidadores de pessoas com esquizofrenia e as suas necessidades educacionais, bem como contribuir para a validação do Educational Needs Questionnaire (ENQ). Instrumentos: Educational Needs Questionnaire (ENQ) e Inventário Sócio-Demográfico para Familiares (RSDS). Resultados: O doente com esquizofrenia é cuidado sobretudo pela família, sendo as mães os cuidadores por excelência. Trata-se de mulheres idosas que cuidam do doente há muitos anos e que necessitam saber mais sobre como obter ajuda dos serviços de saúde mental. Conclusões: Os cuidadores sentem que os serviços de saúde mental não lhes dão o apoio de que necessitam e estão preocupados sobretudo com o estigma e com o que acontecerá aos seus doentes após a sua morte. A versão portuguesa do ENQ mostrou possuir boa fiabilidade, recomendando-se o desenvolvimento de estudos que dêem continuidade ao esforço de validação aqui iniciado.-------------ABSTRACT: The psychoeducational programs for caregivers of people with schizophrenia are often built without regard for the educational needs of users of mental health services. We present a review of the literature on schizophrenia, family, family interventions and evaluation of educational needs. We conducted a cross-sectional study with exploratory characteristics in a convenience sample of primary caregivers (N = 74) of people with schizophrenia being treated in the Department of Psychiatry and Mental Health at the Hospital of St. Francisco Xavier. We used a qualitative and quantitative methodology. Objectives: To describe the characteristics of a sample of primary caregivers of people with schizophrenia and their educational needs, as well as contribute to the validation of the Educational Needs Questionnaire (ENQ). Instruments: Educational Needs Questionnaire (ENQ) and Inventário Sócio-Demográfico para Familiares (RSDS). Results: Patients with schizophrenia are cared by the family, mothers are the caregivers for excellence. They are older women who take care of the patient for many years and they need to know more about how to get help from mental health services. Conclusions: Caregivers feel that mental health services don‟t give them the support they need and they are especially worried about the stigma and what will happens to their patients after their death. The Portuguese version of the ENQ proved to have good reliability and we recommend the development of studies that give continuity to the validation effort here started.

Tuberculous brain abscesses in AIDS patients are considered rare with only eight cases reported in the literature. We describe the case of a 34-year-old woman with AIDS and previous toxoplasmic encephalitis who was admitted due to headache and seizures. A brain computed tomography scan disclosed a frontal hypodense lesion with a contrast ring enhancement. Brain abscess was suspected and she underwent a lesion puncture through a trepanation. The material extracted was purulent and the acid-fast smear was markedly positive. Timely medical and surgical approaches allowed a good outcome. Tuberculous abscesses should be considered in the differential diagnosis of focal brain lesions in AIDS patients. Surgical excision or stereotactic aspiration, and antituberculous treatment are the mainstay in the management of these uncommon lesions.

Bacteremia due to non-typhi Salmonella is more frequent in patients infected with the human immunodeficiency virus (HIV). However, focal complications have been rarely described. We report a case of liver abscess due to Salmonella enteritidis in an HIV-infected patient who recently returned to Sao Paulo, Brazil, from a trip in the Caribbean. A good clinical and radiological response was seen with both percutaneous catheter drainage and antibiotic treatment. To our knowledge, this is the first culture proven case of non-typhi Salmonellaliver abscess in an HIV-infected patient in Brazil.

ABSTRACT – Background: Primary Health Care (PHC) is usually the first contact with the health system, and health professionals are key mediators for enabling citizens to take care of their health. In Portugal, great improvements have been achieved in the biometric indicators of maternal and child health during the last decades. Nevertheless, scant attention has been paid to the mental health dimension, in spite of the recognition of its importance, being pregnancy and early childhood crucial opportunities in the lifecycle for mental health promotion, especially in the early years of life, with a strong impact in the health of the child. The impact of early attachment between mother and baby on maternal and child health has long been recognized. This attachment can be influenced by some factors, as the mother’s emotional adjustment. Attention to these factors may facilitate implementation of both positive conditions and preventative measures. Family support during the transition to parenthood has been highlighted as an effective measure and PHC professionals are in a privileged position as information sources as well as mediators. Aims: The project we present describes an action-research process developed together among academic researchers and health professionals to embrace these issues. We intend to enable health professionals to support families in the transition to parenthood thereby promoting children’s mental health. Approach: The project is driven by a participatory approach intended to lead to reorganization of health care during pregnancy and early childhood. Effective change happens when those involved are interested and motivated, what makes their participation so important. Reflection about current practices and needs, and knowledge about evidence-based interventions have been guiding the selection of changes to introduce in clinical practice for family support and development of parenthood skills and self-confidence. Development: We summarize the main steps in development: the initial assessment and the picture taken from the community under study; the decision making process; the training programme of PHC professionals in action; the review of the protocols of maternal consultation, home visits and antenatal education; the implementation planning; the plan for evaluation the effectiveness of the changes introduced in the delivery of maternal and child health care units. The already developed work has shown that motivation, leadership and organizational issues are decisive for process development.-------------------------- RESUMO - Os Cuidados de Saúde Primários são habitualmente o primeiro contacto com o sistema de saúde e os profissionais de saúde são mediadores chave na capacitação dos cidadãos para cuidarem da sua saúde. Em Portugal, nas últimas décadas, têm-se alcançado grandes melhorias nos indicadores biométricos de saúde materno-infantil. Contudo, tem-se dedicado pouca atenção à dimensão de saúde mental, apesar do reconhecimento da sua importância. A gravidez e primeira infância têm sido apontadas como uma oportunidade crucial no ciclo de vida para a promoção da saúde mental. É dado especial enfoque aos primeiros tempos de vida, dado o forte impacto na saúde da criança. O impacte da vinculação precoce entre a mãe e o bebé na saúde da mãe e da criança há muito que é reconhecido. Esta vinculação pode ser influenciada por vários factores, nomeadamente pelo ajustamento emocional da mãe. A focalização nestes aspectos pode facilitar a criação de condições favoráveis e a implementação de medidas preventivas. O suporte familiar durante o período de transição para a parentalidade tem sido enfatizado como uma medida eficaz e os Cuidados de Saúde Primários estão numa posição privilegiada como fontes de informação e como mediadores. O projecto que apresentamos descreve um processo de investigação- acção desenvolvido em parceria entre investigadores académicos e profissionais de saúde para abordar os aspectos referidos. Pretende-se capacitar os profissionais de saúde para apoiarem as famílias na transição para a parentalidade, promovendo assim a saúde mental das crianças. O projecto baseia-se numa abordagem participativa, direccionada para a reorganização dos cuidados durante a gravidez e primeiros tempos de vida. A mudança efectiva acontece quando os envolvidos estão interessados e motivados, o que torna a sua participação tão importante. A reflexão acerca das práticas e necessidades actuais e o conhecimento acerca de intervenções baseadas na evidência têm guiado a selecção das alterações a introduzir na prática clínica, no sentido de promover o suporte familiar e o desenvolvimento de competências parentais e auto-confiança. Neste artigo, apresentamos as etapas principais do desenvolvimento do projecto: avaliação inicial da comunidade em estudo; processo de tomada de decisão; programa de formação dos profissionais dos Cuidados de Saúde Primários; revisão dos protocolos da consulta de saúde materna, visita domiciliária e educação pré-natal; planeamento da implementação; plano de avaliação da efectividade das alterações introduzidas na prestação de cuidados. This article describes the standardization and evaluation of an in-house specific IgG avidity ELISA for distinguishing recent primary from long-term human cytomegalovirus (HCMV) infection. The test was standardized with the commercial kit ETI-CYTOK G Plus (Sorin Biomedica, Italy) using 8 M urea in phosphate-buffered saline to dissociate low-avidity antibodies after the antigen-antibody interaction. The performance of the in-house assay was compared to that of the commercial automated VIDAS CMV IgG avidity test (bioMérieux, France). Forty-nine sera, 24 from patients with a recent primary HCMV infection and 25 from patients with a long-term HCMV infection and a sustained persistence of specific IgM antibodies, were tested. Similar results were obtained with the two avidity methods. All 24 sera from patients with recently acquired infection had avidity indices compatible with acute HCMV infection by the VIDAS method, whereas with the in-house method, one serum sample had an equivocal result. In the 25 sera from patients with long-term infection, identical results were obtained with the two methods, with only one serum sample having an incompatible value. These findings suggest that our in-house avidity test could be a potentially useful tool for the immunodiagnosis of HCMV infection.

Cytomegalovirus (CMV) disease in acquired immunodeficiency syndrome (AIDS) patients most commonly presents as chorioretinitis and gastro-intestinal infection. Neurological involvement due to CMV may cause several clinical presentations: polyradiculitis, myelitis, encephalitis, ventriculo-encephalitis, and mononeuritis multiplex. Rarely, cerebral mass lesion is described. We report a 39 year-old woman with AIDS and previous cerebral toxoplasmosis. She presented with fever, seizures, and vulval ulcers. Her chest X-ray showed multiple lung nodules, and a large frontal lobe lesion was seen in a brain computed tomography scan. She underwent a brain biopsy through a frontal craniotomy, but her condition deteriorated and she died in the first postoperative day. Histopathological studies and immunohistochemistry disclosed CMV disease, and there was no evidence of cerebral toxoplasmosis, bacterial, mycobacterial or fungal infection. CMV disease should be considered in the differential diagnosis of cerebral mass lesion in AIDS patients. High suspicion index, timely diagnostic procedures (surgical or minimally invasive), and proper utilization of prophylactic and therapeutic medication could improve outcome of these patients.

Ollier Disease and Maffucci Syndrome are two rare diseases that can cause tumors in several organs, having a special predilection for the hand. However, there have been very few reports in the literature focusing on hand manifestations of these diseases. We report the cases of three female patients: one with Ollier Disease, and two other with Maffucci Syndrome. All patients had hand involvement as their initial primary complaint. The Ollier Disease patient developed chondrosarcomas of two digits and had to have these fingers amputated. One of the Maffucci patients died one year after presentation from a brain glioblastoma. These cases emphasize the importance of early diagnosis of Ollier Disease and Maffucci Syndrome, as these two conditions are associated not only to crippling hand deformity, but also to a significant risk of chondrosarcoma, and other malignant tumors.
