We present a 53-year-old man with a vocal cord paralysis observed as a primary manifestation of lung carcinoma. Tc-99m MDP whole body bone scan were performed and resulted a normal scintiscan. The bone scan does not revealed suspicious foci of uptake. The possibility of bone metastasis was taken into consideration. A whole body F18-FDG-PET scan showed intense uptake in the left upper lung corresponding to the primary tumor. A bronchial biopsy confirmed infiltration by small cell lung carcinoma (SCLC). SCLC is composed of poorly differentiated, rapidly growing cells with disease usually occurring centrally rather than peripherally. It metastasizes early. The whole-body F18-FDG-PET scan clearly demonstrated a focus of increased uptake in the second lumbar vertebral body suspicious for osteolytic metastasis. A lytic bone metastasis was confirmed by MRI. The patient then received therapy and underwent follow up abdominal CT. The scan showed blastic changes in the L2 vertebra suggesting response to treatment.

BACKGROUND: Estimating current cancer mortality figures is important for defining priorities for prevention and treatment.Materials and methods:Using logarithmic Poisson count data joinpoint models on mortality and population data from the World Health Organization database, we estimated numbers of deaths and age-standardized rates in 2012 from all cancers and selected cancer sites for the whole European Union (EU) and its six more populated countries. RESULTS: Cancer deaths in the EU in 2012 are estimated to be 1 283 101 (717 398 men and 565 703 women) corresponding to standardized overall cancer death rates of 139/100 000 men and 85/100 000 women. The fall from 2007 was 10% in men and 7% in women. In men, declines are predicted for stomach (-20%), leukemias (-11%), lung and prostate (-10%) and colorectal (-7%) cancers, and for stomach (-23%), leukemias (-12%), uterus and colorectum (-11%) and breast (-9%) in women. Almost stable rates are expected for pancreatic cancer (+2-3%) and increases for female lung cancer (+7%). Younger women show the greatest falls in breast cancer mortality rates in the EU (-17%), and declines are expected in all individual countries, except Poland. CONCLUSION: Apart for lung cancer in women and pancreatic cancer, continuing falls are expected in mortality from major cancers in the EU.

BACKGROUND: Worldwide data for cancer survival are scarce. We aimed to initiate worldwide surveillance of cancer survival by central analysis of population-based registry data, as a metric of the effectiveness of health systems, and to inform global policy on cancer control. METHODS: Individual tumour records were submitted by 279 population-based cancer registries in 67 countries for 25·7 million adults (age 15-99 years) and 75 000 children (age 0-14 years) diagnosed with cancer during 1995-2009 and followed up to Dec 31, 2009, or later. We looked at cancers of the stomach, colon, rectum, liver, lung, breast (women), cervix, ovary, and prostate in adults, and adult and childhood leukaemia. Standardised quality control procedures were applied; errors were corrected by the registry concerned. We estimated 5-year net survival, adjusted for background mortality in every country or region by age (single year), sex, and calendar year, and by race or ethnic origin in some countries. Estimates were age-standardised with the International Cancer Survival Standard weights. FINDINGS: 5-year survival from colon, rectal, and breast cancers has increased steadily in most developed countries. For patients diagnosed during 2005-09, survival for colon and rectal cancer reached 60% or more in 22 countries around the world; for breast cancer, 5-year survival rose to 85% or higher in 17 countries worldwide. Liver and lung cancer remain lethal in all nations: for both cancers, 5-year survival is below 20% everywhere in Europe, in the range 15-19% in North America, and as low as 7-9% in Mongolia and Thailand. Striking rises in 5-year survival from prostate cancer have occurred in many countries: survival rose by 10-20% between 1995-99 and 2005-09 in 22 countries in South America, Asia, and Europe, but survival still varies widely around the world, from less than 60% in Bulgaria and Thailand to 95% or more in Brazil, Puerto Rico, and the USA. For cervical cancer, national estimates of 5-year survival range from less than 50% to more than 70%; regional variations are much wider, and improvements between 1995-99 and 2005-09 have generally been slight. For women diagnosed with ovarian cancer in 2005-09, 5-year survival was 40% or higher only in Ecuador, the USA, and 17 countries in Asia and Europe. 5-year survival for stomach cancer in 2005-09 was high (54-58%) in Japan and South Korea, compared with less than 40% in other countries. By contrast, 5-year survival from adult leukaemia in Japan and South Korea (18-23%) is lower than in most other countries. 5-year survival from childhood acute lymphoblastic leukaemia is less than 60% in several countries, but as high as 90% in Canada and four European countries, which suggests major deficiencies in the management of a largely curable disease. INTERPRETATION: International comparison of survival trends reveals very wide differences that are likely to be attributable to differences in access to early diagnosis and optimum treatment. Continuous worldwide surveillance of cancer survival should become an indispensable source of information for cancer patients and researchers and a stimulus for politicians to improve health policy and health-care systems. BACKGROUND: Current cancer mortality statistics are important for public health decision making and resource allocation. Age standardized rates and numbers of deaths are predicted for 2016 in the European Union. PATIENTS AND METHODS: Population and death certification data for stomach, colorectum, pancreas, lung, breast, uterus, prostate, leukemia and total cancers were obtained from the World Health Organisation database and Eurostat. Figures were derived for the EU, France, Germany, Italy, Poland, Spain and the UK. Projected numbers of deaths by age group were obtained for 2016 by linear regression on estimated numbers of deaths over the most recent time period identified by a joinpoint regression model. RESULTS: Projected total cancer mortality trends for 2016 in the EU are favourable in both sexes with rates of 133.5/100,000 men and 85.2/100,000 women (8% and 3% falls since 2011, due to population ageing) corresponding to 753,600 and 605,900 deaths in men and women for a total number of 1,359,500 projected cancer deaths (+3% compared to 2011). In men lung, colorectal and prostate cancer fell 11%, 5% and 8% since 2011. Breast and colorectal cancer trends in women are favourable (8% and 7% falls, respectively), but lung and Pancreatic cancer rates rose 5% and 4% since 2011 reaching rates of 14.4 and 5.6/100,000 women. Leukemia shows favourable projected mortality for both sexes and all age groups with stronger falls in the younger age groups, rates are 4.0/100,000 men and 2.5/100,000 women, with respectively falls of 14% and 12%. CONCLUSION: The 2016 predictions for EU cancer mortality confirm the favourable trends in rates particularly for men. Lung cancer is likely to remain the leading site for female cancer rates. Continuing falls in mortality, larger in children and young adults, are predicted in leukemia, essentially due to advancements in management and therapy, and their subsequent adoption across Europe.

Abstract Solitary pulmonary nodule corresponds to a common radiographic finding, which is frequently detected incidentally. The investigation of this entity remains complex, since characteristics of benign and malignant processes overlap in the differential diagnosis. Currently, many strategies are available to evaluate solitary pulmonary nodules with the main objective of characterizing benign lesions as best as possible, while avoiding to expose patients to the risks inherent to invasive methods, besides correctly detecting cases of lung cancer so as the potential curative treatment is not delayed. This first part of the study focuses on the epidemiology, the morfological evaluation and the methods to determine the likelihood of cancer in cases of indeterminate solitary pulmonary nodule.

Microtubules are involved in many aspects of cellular biology and represent an important target of anticancer chemotherapeutics. In the past five years, novel natural products such as epothilones, discodermolide, sarcodictyin, eleutherobin, and laulimalide, all of which have biological activities similar to those of paclitaxel (Taxolâ), have been discovered. Taxolâ is an important antitumor drug approved by the FDA for the treatment of ovarian, breast and non-small-cell lung carcinomas and became the first natural product described that stabilized microtubules avoiding the cellular replication. The present article reports new natural products that are able to act on the stabilization of microtubules.

BACKGROUND: The Cancer Fast-track Programme's aim was to reduce the time that elapsed between well-founded suspicion of breast, colorectal and lung cancer and the start of initial treatment in Catalonia (Spain). We sought to analyse its implementation and overall effectiveness. METHODS: A quantitative analysis of the programme was performed using data generated by the hospitals on the basis of seven fast-track monitoring indicators for the period 2006-2009. In addition, we conducted a qualitative study, based on 83 semistructured interviews with primary and specialised health professionals and health administrators, to obtain their perception of the programme's implementation. RESULTS: About half of all new patients with breast, lung or colorectal cancer were diagnosed via the fast track, though the cancer detection rate declined across the period. Mean time from detection of suspected cancer in primary care to start of initial treatment was 32 days for breast, 30 for colorectal and 37 for lung cancer (2009). Professionals associated with the implementation of the programme showed that general practitioners faced with suspicion of cancer had changed their conduct with the aim of preventing lags. Furthermore, hospitals were found to have pursued three specific implementation strategies (top-down, consensus-based and participatory), which made for the cohesion and sustainability of the circuits. CONCLUSION: The programme has contributed to speeding up diagnostic assessment and treatment of patients with suspicion of cancer, and to clarifying the patient pathway between primary and specialised care.

In view of anticancer activity of 7 β-acetoxywithanolide D (2) and 7β-16α-diacetoxywithonide D (3), isolated from the leaves of Acnistus arborescens (Solanaceae), five withanolide derivatives were obtained and their structures were determined by NMR, MS and IV data analysis. The in vitro anticancer activity of these derivatives was evaluated in a panel of cancer cell lines: human breast (BC-1), human lung (Lu1), human colon (Col2) and human oral epidermoid carcinoma (KB). Compounds 2a (acetylation of 2), 3b (oxidation of 3) and 2c (hydrogenation of 2) exhibited the highest anticancer activity against human lung cancer cells, with ED50 values of 0.19, 0.25 and 0.63 μg/mL, respectively.

ErbB receptor tyrosine kinases, epidermal growth factor receptor (EGFR, also known as ErbB1), ErbB2 (HER2 or NEU), ErbB3 (HER3), and ErbB4 (HER4), transduce signals borne by extracellular ligands into central cellular responses such as proliferation, survival, differentiation, and apoptosis. Mutations in ERBB genes are frequently detected in human malignant diseases of epithelial and neural origin, making ErbB receptors important drug targets. Targeting EGFR and ErbB2 has been successful in eg. lung and breast cancer, respectively, and mutations in these genes can be used to select patients that are responsive to the targeted treatment. Although somatic ERBB4 mutations have been found in many high-incidence cancers such as melanoma, lung cancer, and colorectal cancer and germ-line ERBB4 mutations have been linked to neuronal disorders and cancer, ErbB4 has generally been neglected as a potential drug target. Thus, the consequences of ERBB4 mutations on ErbB4 biology are largely unknown. This thesis aimed to elucidate the functional consequences and assess the clinical significance of somatic and germ-line ERBB4 mutations in the context of cancer and amyotrophic lateral sclerosis. The results of this study indicated that cancer-associated ERBB4 mutations can promote aberrant ErbB4 function by activating the receptor or inducing qualitative changes in ErbB4 signaling. ERBB4 mutations increased survival or decreased differentiation in vitro, suggesting that ERBB4 mutations can be oncogenic. Importantly, the potentially oncogenic mutations were located in various subdomains in ErbB4, possibly providing explanation for the characteristic scattered pattern of mutations in ERBB4. This study also demonstrated that hereditary variation in ERBB4 gene can have a significant effect on the prognosis of breast cancer. In addition, it was shown that hereditary or de novo germ-line ERBB4 mutations that predispose to amyotrophic lateral sclerosis inhibit ErbB4 activity. Together, these results suggest that ErbB4 should be considered as a novel drug target in cancer and amyotrophic lateral sclerosis.

The nuclear factor of activated T cells (NFAT) family of transcription factors has been primarily identified in immune cells; however, these proteins have been recently found to be functionally active in several other non-immune cell types. NFAT proteins are activated upon different stimuli that lead to increased intracellular calcium levels. Regardless of their widely known cytokine gene expression properties, NFATs have been shown to regulate other genes related to cell cycle progression, cell differentiation and apoptosis, revealing a broader role for these proteins in normal cell physiology. Several reports have addressed the participation of NFATs in many aspects of malignant cell transformation and tumorigenic processes. In this review, we will discuss the involvement of the different NFAT family members in the regulation of cell cycling, differentiation and tumor formation, and also its implications on oncogenesis. Better understanding the mechanisms by which NFATs regulate cell cycle and tumor-related events should be relevant for the development of rational anti-cancer therapies.

We encapsulated cisplatin into stealth pH-sensitive liposomes and studied their stability, cytotoxicity and accumulation in a human small-cell lung carcinoma cell line (GLC4) and its resistant subline (GLC4/CDDP). Since reduced cellular drug accumulation has been shown to be the main mechanism responsible for resistance in the GLC4/CDDP subline, we evaluated the ability of this new delivery system to improve cellular uptake. The liposomes were composed of dioleoylphosphatidylethanolamine (DOPE), cholesteryl hemisuccinate (CHEMS), and distearoylphosphatidylethanolamine-polyethyleneglycol 2000 (DSPE-PEG2000) and were characterized by determining the encapsulation percentage as a function of lipid concentration. Among the different formulations, DOPE/CHEMS/DSPE-PEG liposomes (lipid concentration equal to 40 mM) encapsulated cisplatin more efficiently than other concentrations of liposomes (about 20.0%, mean diameter of 174 nm). These liposomes presented good stability in mouse plasma which was obtained using a 0.24-M EDTA solution (70% cisplatin was retained inside the liposomes after 30 min of incubation). Concerning cytotoxic effects, they are more effective (1.34-fold) than free cisplatin for growth inhibition of the human lung cancer cell line A549. The study of cytotoxicity to GLC4 and GLC4/CDDP cell lines showed similar IC50 values (approximately 1.4 µM), i.e., cisplatin-resistant cells were sensitive to this cisplatin formulation. Platinum accumulation in both sensitive and resistant cell lines followed the same pattern, i.e., approximately the same intracellular platinum concentration (4.0 x 10-17 mol/cell) yielded the same cytotoxic effect. These results indicate that long-circulating pH-sensitive liposomes, also termed as stealth pH-sensitive liposomes, may present a promising delivery system for cisplatin-based cancer treatment. This liposome system proved to be able to circumvent the cisplatin resistance, whereas it was not observed when using non-long-circulating liposomes composed of phosphatidylcholine, phosphatidylserine, and cholesterol.

The antioxidant effects of Caryocar brasiliense Camb, commonly known as the pequi fruit, have not been evaluated to determine their protective effects against oxidative damage in lung carcinogenesis. In the present study, we evaluated the role of pequi fruit against urethane-induced DNA damage and oxidative stress in forty 8-12 week old male BALB/C mice. An in vivo comet assay was performed to assess DNA damage in lung tissues and changes in lipid peroxidation and redox cycle antioxidants were monitored for oxidative stress. Prior supplementation with pequi oil or its extract (15 µL, 60 days) significantly reduced urethane-induced oxidative stress. A protective effect against DNA damage was associated with the modulation of lipid peroxidation and low protein and gene expression of nitric oxide synthase. These findings suggest that the intake of pequi fruit might protect against in vivo genotoxicity and oxidative stress.

Des études ont observé que le statut socio-économique (SSE) est inversement associé avec le risque de cancer du poumon. Cette thèse approfondit différents aspects de la relation entre le SSE et le risque de cancer du poumon afin de contribuer à la compréhension des facteurs influençant le risque de cette maladie. Son objectif général consiste à étudier les associations entre le SSE et le risque de cancer du poumon en fonction à la fois : 1) de l’indicateur du SSE ; 2) des types d’ajustement pour le tabagisme, et ; 3) des facteurs de risque intermédiares potentiels de l’association entre le SSE et le cancer du poumon. Les données d’une vaste étude cas-témoins menée à Montréal entre 1996 et 2002 et portant sur l’identification de facteurs environnementaux impliqués dans l’étiologie du cancer du poumon ont été utilisées. L’échantillon analysé était constitué de 1203 cas (738 hommes et 465 femmes) diagnostiqués en 1996-1997 dans les 18 principaux hôpitaux du Grand Montréal et 1513 témoins (899 hommes et 614 femmes). Les témoins, recrutés au hasard à partir de la liste électorale, ont été appariés aux cas selon l’âge, le sexe et le district électoral. Les sujets ont été interviewés afin de colliger de l’information entre autres sur les caractéristiques sociodémographiques et économiques, les habitudes de tabagisme, l‘histoire professionnelle et l’alimentation. Une collecte de données additionnelle a été effectuée spécifiquement pour la présente thèse. En effet, l’évaluation foncière de la résidence correspondant à l’adresse de chacun des sujets à l’entrevue a été obtenue à partir des bases de données publiques de la ville de Montréal. Le premier article portait sur la comparaison de différents indices de disponibilité financière chacun en 5 catégories: un indice de valeur résidentielle, le revenu médian issu des données du recensement et le revenu familial auto-rapporté. En comparant au revenu familial auto-rapporté, une très bonne concordance a été obtenue pour 73% et 64% des individus respectivement pour l’indice de valeur résidentielle et le revenu du recensement. Le Kappa pondéré était plus élevé pour la comparaison du revenu familial auto-rapporté avec l’indice de valeur résidentielle (Kappap=0.36, IC 95%: 0.31-0.40) qu’avec le revenu médian issu du recensement (Kappap=0.22, IC 95%: 0.17-0.27). Le choix d’un indice dépend toutefois des objectifs et de la faisabilité de son utilisation dans la population étudiée. Le deuxième article présentait les associations entre les trois indices de disponibilité financière et le risque de cancer du poumon. Les fortes associations observées initialement sont disparues après un ajustement pour plusieurs dimensions du tabagisme. Ce dernier jouait un rôle de grande importance dans la relation entre le SSE et le risque de cancer du poumon. Finalement, les facteurs de risque intermédiares des associations entre trois différentes dimensions du SSE – éducation, revenu, classe professionnelle – ont été étudiés. Les résultats ont suggéré que les 3 facteurs de risque intermédiares potentiels étudiés: tabagisme, consommation de fruits et légumes riches en caroténoïdes, expositions professionnelles avaient un effet médiateur indépendant dans la relation entre le SSE et le cancer du poumon. Le plus important médiateur était le tabagisme, suivi de l’alimentation et à un degré moindre, les expositions professionnelles. Il nous paraît particulièrement pertinent d‘évaluer différents indicateurs du SSE lorsque possible, et d’effectuer un ajustement détaillé pour le tabagisme lorsque la relation entre le SSE et le risque de cancer du poumon est étudiée. Une bonne connaissance et la prise en compte des facteurs de risque intermédiares sont essentielles à la formulation des mesures de prévention du cancer du poumon.

Le cancer occupe le premier rang parmi les causes de décès au Québec. Les taux de mortalité diffèrent cependant à travers les régions du Québec. En considérant les territoires des Centres de santé et de services sociaux (CSSS), ce mémoire propose dans un premier temps d’étudier la distribution des taux de mortalité pour les principaux sièges de cancer, d’identifier les régions où une différence significative du taux avec le reste du Québec est perceptible et d’approfondir des éléments impliqués dans la mortalité, soit l’incidence et la létalité. D’autre part, dans le but d’expliquer les variations des taux de mortalité, des variables socio-économiques liées aux territoires retenus serviront de variables explicatives au modèle de régression utilisé. Les résultats ont permis de constater que c’est pour le cancer du poumon que les écarts significatifs sont les plus nombreux. Quant au cancer colorectal, un seul territoire présentait un écart significatif. Pour ce qui est du cancer de la prostate et du sein, aucun écart n’est perceptible dans les taux de mortalité. Concernant la partie explicative de la mortalité, dans le cas du cancer du poumon, les taux d’incidence sont fortement corrélés aux taux de mortalité. On ne peut cependant faire le même constat pour les cancers colorectal, du sein et de la prostate. De plus, les variables socio-économiques considérées pour les territoires de CSSS ne permettaient pas d’arriver à un modèle explicatif convaincant. Ces résultats montrent la nécessité d’avoir des données longitudinales permettant de suivre les individus dès le moment où le diagnostic de cancer est établi afin d’être en mesure de mieux mesurer les liens entre l’incidence, la survie, le niveau socio-économique et la mortalité.

Objectif : Examiner la relation entre une exposition professionnelle aux fumées de soudage au gaz et à l’arc et le risque de développer un cancer du poumon, chez des travailleurs provenant de différents milieux de travail et exposés à de nombreuses substances de différentes concentrations. Méthodes : Nous avons mené deux études cas-témoins basés sur la population montréalaise (1979-86 et 1996-2001), incluant respectivement 857 et 736 cas, ainsi que des témoins appariés selon la fréquence des cas, le sexe et le lieu de résidence. Un historique détaillé des emplois occupés fut obtenu par entrevue et évalué par une équipe d’experts afin d’estimer l’intensité, la durée ainsi que l’exposition cumulative à un grand nombre de substances. Les fumées de soudage au gaz et à l’arc sont parmi les substances évaluées et nous avons pu calculer un rapport de cote (RC) et son intervalle de confiance de 95% (IC 95%) pour le risque de cancer du poumon en utilisant la régression logistique et en ajustant pour l’historique de tabagisme et plusieurs autres covariables pertinentes. Des analyses subséquentes ont permis d’explorer d’autres voies, comme la modification des effets observés par le tabagisme. Résultats : Les résultats obtenus à partir des deux études étant assez similaires, nous avons donc pu les combiner pour former une seule étude. Aucune association statistiquement significative n’a été trouvée entre le cancer du poumon et les fumées de soudage au gaz (RC=1,13; IC 95%=0,90-1,42) et les fumées de soudage à l’arc (RC=1,01; IC 95%=0,80-1,26). Par contre, dans le sous-groupe des non-fumeurs et fumeurs très légers, nous avons trouvé un risque accru de cancer du poumon en relation avec les fumées de soudage au gaz (RC=2,78; IC 95%=1,66-4,65) et à l’arc (RC=2,20; IC 95%=1,32-3,70). En se concentrant sur ce sous-groupe, nous avons trouvé un risque encore plus élevé de développer un cancer du poumon pour ceux exposé à des doses plus substantielles, pour le soudage au gaz (RC=4,63; IC 95%=2,14-10,03) et à l’arc (RC=3,45; IC 95%=1,59-7,48). Discussion : Globalement, aucun excès de risque causé par une exposition aux fumées de soudage n’a été détecté, mais un excès a été trouvé chez les non-fumeurs et fumeurs légers uniquement et ce, pour les deux types de fumées de soudage. L’effet des fumées de soudage peut être voilé chez les fumeurs, ou bien les non-fumeurs pourraient être plus vulnérables.
