Phenylalanine ammonia-lyase (EC 4.3.1.5) from parsley is posttranslationally modified by dehydrating its Ser-202 to the catalytically essential dehydroalanine prosthetic group. The codon of Ser-202 was changed to those of alanine and threonine by site-directed mutagenesis. These mutants and the recombinant wild-type enzyme, after treatment with sodium borohydride, were virtually inactive with L-phenylalanine as substrate but catalyzed the deamination of L-4-nitrophenylalanine, which is also a substrate for the wild-type enzyme. Although the mutants reacted about 20 times slower with L-4-nitrophenylalanine than the wild-type enzyme, their Vmax for L-4-nitrophenylalanine was two orders of magnitude higher than for L-phenylalanine. In contrast to L-tyrosine, which was a poor substrate, DL-3-hydroxyphenylalanine (DL-m-tyrosine) was converted by phenylalanine ammonia-lyase at a rate comparable to that of L-phenylalanine. These results suggest a mechanism in which the crucial step is an electrophilic attack of the prosthetic group at position 2 or 6 of the phenyl group. In the resulting carbenium ion, the beta-HSi atom is activated in a similar way as it is in the nitro analogue. Subsequent elimination of ammonia, concomitant with restoration of both the aromatic ring and the prosthetic group, completes the catalytic cycle.

A doença valvar crônica de mitral (DVCM) é a principal cardiopatia adquirida dos cães e uma das suas complicações é a hipertensão arterial pulmonar (HAP), o que pode induzir a disfunção do ventriculo direito (VD). Assim, constituíram-se em objetivos do presente estudo identificar e descrever alterações de tamanho do VD, padrão de fluxo na artéria pulmonar (AP) e função sistólica ventricular direita nas diferentes fases da DVCM, além de correlacionar estas variáveis com índices de tamanho, volume, funções sistólica e diastólica do lado esquerdo do coração, bem como com a velocidade da insuficiência tricúspide (IT) e gradiente de pressão entre o ventrículo e átrio direitos nos cães que apresentavam regurgitação da valva tricúspide. Para tanto, foram incluídos 96 cães de diversas raças no estudo, que foram separados em quatro grupos de acordo com o estágio da DVCM: grupos ou estágios A, B1, B2 e C. Os cães com DVCM sintomáticos ou em estágio C apresentaram alterações no fluxo da artéria pulmonar (AP), bem evidenciadas pela redução das suas velocidades máxima e média, além da redução dos tempos de aceleração (TAC) e ejeção (TEJ) do fluxo sistólico da AP e correlação negativa com as variáveis de tamanho e funções sistólica e diastólica do coração esquerdo. O tamanho do VD foi estatisticamente maior nos animais do estágio C em comparação aos do estágio B1 e associou-se, negativamente, com os índices de função sistólica ventricular esquerda (VE). Os índices de função sistólica do VD como índice de excursão sistólica do plano anular tricúspide (iTAPSE) e variação fracional de área (FAC) foram maiores nos estágios mais avançados da DVCM e, juntamente com a velocidade de movimentação miocárdica sistólica do anel valvar tricúspide (onda Sm), correlacionou-se com índices de funções sistólica e diastólica do VE, seguindo o mesmo padrão de aumento de movimentação e estado hipercinético das variáveis do lado esquerdo do coração na evolução da DVCM. O padrão de fluxo sistólico da AP, bem caracterizado pelo TAC e TEJ, e o índice de área doVD foram os índices que mais alteraram com a evolução da hipertensão pulmonar na DVCM, enquanto que os índices de função do VD não apresentaram alterações significativas neste modelo de hipertensão arterial pulmonar em cão

Contexte: La régurgitation mitrale (RM) est une maladie valvulaire nécessitant une intervention dans les cas les plus grave. Une réparation percutanée de la valve mitrale avec le dispositif MitraClip est un traitement sécuritaire et efficace pour les patients à haut risque chirurgical. Nous voulons évaluer les résultats cliniques et l'impact économique de cette thérapie par rapport à la gestion médicale des patients en insuffisance cardiaque avec insuffisance mitrale symptomatique. Méthodes: L'étude a été composée de deux phases; une étude d'observation de patients souffrant d'insuffisance cardiaque et de régurgitation mitrale traitée avec une thérapie médicale ou le MitraClip, et un modèle économique. Les résultats de l'étude observationnelle ont été utilisés pour estimer les paramètres du modèle de décision, qui a estimé les coûts et les avantages d'une cohorte hypothétique de patients atteints d'insuffisance cardiaque et insuffisance mitrale sévère traitée avec soit un traitement médical standard ou MitraClip. Résultats: La cohorte de patients traités avec le système MitraClip était appariée par score de propension à une population de patients atteints d'insuffisance cardiaque, et leurs résultats ont été comparés. Avec un suivi moyen de 22 mois, la mortalité était de 21% dans la cohorte MitraClip et de 42% dans la cohorte de gestion médicale (p = 0,007). Le modèle de décision a démontré que MitraClip augmente l'espérance de vie de 1,87 à 3,60 années et des années de vie pondérées par la qualité (QALY) de 1,13 à 2,76 ans. Le coût marginal était 52.500 $ dollars canadiens, correspondant à un rapport coût-efficacité différentiel (RCED) de 32,300.00 $ par QALY gagné. Les résultats étaient sensibles à l'avantage de survie. Conclusion: Dans cette cohorte de patients atteints d'insuffisance cardiaque symptomatique et d insuffisance mitrale significative, la thérapie avec le MitraClip est associée à une survie supérieure et est rentable par rapport au traitement médical.

To compare intraoperative cerebral microembolic load between minimally invasive extracorporeal circulation (MiECC) and conventional extracorporeal circulation (CECC) during isolated surgical aortic valve replacement (SAVR), we conducted a randomized trial in patients undergoing primary elective SAVR at a tertiary referral hospital. The primary outcome was the procedural phase-related rate of high-intensity transient signals (HITS) on transcranial Doppler ultrasound. HITS rate was used as a surrogate of cerebral microembolism in pre-defined procedural phases in SAVR using MiECC or CECC with (+F) or without (-F) an oxygenator with integrated arterial filter. Forty-eight patients were randomized in a 1:1 ratio to MiECC or CECC. Due to intraprocedural Doppler signal loss (n = 3), 45 patients were included in final analysis. MiECC perfusion regimen showed a significantly increased HITS rate compared to CECC (by a factor of 1.75; 95% confidence interval, 1.19-2.56). This was due to different HITS rates in procedural phases from aortic cross-clamping until declamping [phase 4] (P = 0.01), and from aortic declamping until stop of extracorporeal perfusion [phase 5] (P = 0.05). Post hoc analysis revealed that MiECC-F generated a higher HITS rate than CECC+F (P = 0.005), CECC-F (P = 0.05) in phase 4, and CECC-F (P = 0.03) in phase 5, respectively. In open-heart surgery, MiECC is not superior to CECC with regard to gaseous cerebral microembolism. When using MiECC for SAVR, the use of oxygenators with integrated arterial line filter appears highly advisable. Only with this precaution, MiECC confers a cerebral microembolic load comparable to CECC during this type of open heart surgery.

Within generally calcareous sediment sequences, layers of variable thickness of the giant diatom Ethmodiscus were found in five cores recovered in the Subtropical South Atlantic between 23° and 33°S from both sides of the Mid-Atlantic Ridge. Two types of oozes occur: (almost) monospecific layers of Ethmodiscus and layers dominated by Ethmodiscus, with several accompanying tropical/subtropical, oligotrophic-water diatoms. The two thickest Ethmodiscus layers occur in GeoB3801-6 around 29°S, and accumulated during late MIS 14 and MIS 12, respectively. Downcore concentrations of Ethmodiscus valves range between 3.4 10 4 and 2.3 10 7 valves g -1. We discuss the ooze formation in the context of migration of frontal systems and changes in the thermohaline circulation. The occurrence of Ethmodiscus oozes in sediments underlying the present-day pelagic, low-nutrient waters is associated with a terminal event of the Mid-Pleistocene Transition at around 530 ka, when the ocean circulation rearranged after a period of reduced NADW production.

The Lapeyre-Triflo FURTIVA valve aims at combining the favorable hemodynamics of bioprosthetic heart valves with the durability of mechanical heart valves (MHVs). The pivoting region of MHVs is hemodynamically of special interest as it may be a region of high shear stresses, combined with areas of flow stagnation. Here, platelets can be activated and may form a thrombus which in the most severe case can compromise leaflet mobility. In this study we set up an experiment to replicate the pulsatile flow in the aortic root and to study the flow in the pivoting region under physiological hemodynamic conditions (CO = 4.5 L/min / CO = 3.0 L/min, f = 60 BPM). It was found that the flow velocity in the pivoting region could reach values close to that of the bulk flow during systole. At the onset of diastole the three valve leaflets closed in a very synchronous manner within an average closing time of 55 ms which is much slower than what has been measured for traditional bileaflet MHVs. Hot spots for elevated viscous shear stresses were found at the flanges of the housing and the tips of the leaflet ears. Systolic VSS was maximal during mid-systole and reached levels of up to 40 Pa.
