Depuis les années 1990, de nombreuses recherches ont porté sur le dévoilement de l’agression sexuelle chez l’enfant et sur les caractéristiques y étant associées. Outre les facteurs liés à l’enfant et au contexte, le type de questions utilisé par l’intervieweur (notamment les invitations et les questions ouvertes) est une variable déterminante pour le dévoilement de l’enfant et la quantité de détails qui sera fournie entourant l’agression sexuelle. Bien que plusieurs enquêteurs reçoivent une formation sur l’utilisation des questions ouvertes dans les entrevues avec les enfants, peu d’entre eux utilisent un style de question approprié une fois sur le terrain. L’objectif de cette recherche vise à déterminer si certaines caractéristiques personnelles des enquêteurs sont associées à leur adhésion à un protocole d’entrevue structuré pour lequel ils ont été formés, à l’utilisation de questions ouvertes dans des entrevues d’enquête auprès d’enfants soupçonnés d’avoir vécu une agression sexuelle et à la quantité de détails dévoilés par l’enfant lors de ces entrevues. Deux études ont été menées pour répondre à cette question. La première étude a été effectuée à l'École nationale de police du Québec auprès de 24 enquêteurs de police ayant suivi une formation d'une semaine visant l’apprentissage du protocole d’entrevue structuré du National Institute of Child Health and Human Development (NICHD). Le genre féminin, les habiletés cognitives et le trait de personnalité Ouverture à l’expérience sont trois caractéristiques personnelles qui ont été reliées positivement à la performance des enquêteurs lors d’une entrevue simulée avec un comédien jouant le rôle d’un enfant victime d’agression sexuelle, tandis que le nombre d’années d’expérience et la capacité de gestion du stress de ces enquêteurs ont montré une relation négative avec cette performance. Dans la seconde étude effectuée sur le terrain auprès de 13 enquêteurs du Service de police de la Ville de Montréal, 114 entrevues conduites auprès d’enfants ayant dévoilé une agression sexuelle ont été recueillies et cotées pour mesurer l’adhésion au protocole d’entrevue du NICHD, le ratio de questions ouvertes et la quantité de détails dévoilés par l’enfant par question posée par l’enquêteur. L’intelligence émotionnelle et les traits de personnalité Extraversion, Esprit consciencieux et Agréabilité ont été trouvés comme des caractéristiques personnelles positivement associées à l’adhésion au protocole d’entrevue et à l’utilisation de questions ouvertes, tandis que le nombre d’années d’expérience et le trait de personnalité Névrosisme ont été négativement associés à ces deux critères de performance. Le niveau d’habiletés cognitives des enquêteurs a quant à lui montré une association positive avec la quantité de détails dévoilés par l’enfant. La signification et l’interprétation de ces résultats, de même que les implications potentielles pour la sélection et la formation des enquêteurs sont finalement discutées.

Study Design Retrospective study of surgical outcome. Objectives To evaluate quantitatively the changes in trunk surface deformities after scoliosis spinal surgery in Lenke 1A adolescent idiopathic scoliosis (AIS) patients and to compare it with changes in spinal measurements. Summary of Background Data Most studies documenting scoliosis surgical outcome used either radiographs to evaluate changes in the spinal curve or questionnaires to assess patients health-related quality of life. Because improving trunk appearance is a major reason for patients and their parents to seek treatment, this study focuses on postoperative changes in trunk surface deformities. Recently, a novel approach to quantify trunk deformities in a reliable, automatic, and noninvasive way has been proposed. Methods Forty-nine adolescents with Lenke 1A idiopathic scoliosis treated surgically were included. The back surface rotation and trunk lateral shift were computed on trunk surface acquisitions before and at least 6 months after surgery. We analyzed the effect of age, height, weight, curve severity, and flexibility before surgery, length of follow-up, and the surgical technique. For 25 patients with available three-dimensional (3D) spinal reconstructions, we compared changes in trunk deformities with changes in two-dimensional (2D) and 3D spinal measurements. Results The mean correction rates for the back surface rotation and the trunk lateral shift are 18% and 50%, respectively. Only the surgical technique had a significant effect on the correction rate of the back surface rotation. Direct vertebral derotation and reduction by spine translation provide a better correction of the rib hump (22% and 31% respectively) than the classic rod rotation technique (8%). The reductions of the lumbar Cobb angle and the apical vertebrae transverse rotation explain, respectively, up to 17% and 16% the reduction of the back surface rotation. Conclusions Current surgical techniques perform well in realigning the trunk; however, the correction of the deformity in the transverse plane proves to be more challenging. More analysis on the positive effect of vertebral derotation on the rib hump correction is needed. Level of evidence III.

Among the external manifestations of scoliosis, the rib hump, which is associated with the ribs' deformities and rotations, constitutes the most disturbing aspect of the scoliotic deformity for patients. A personalized 3-D model of the rib cage is important for a better evaluation of the deformity, and hence, a better treatment planning. A novel method for the 3-D reconstruction of the rib cage, based only on two standard radiographs, is proposed in this paper. For each rib, two points are extrapolated from the reconstructed spine, and three points are reconstructed by stereo radiography. The reconstruction is then refined using a surface approximation. The method was evaluated using clinical data of 13 patients with scoliosis. A comparison was conducted between the reconstructions obtained with the proposed method and those obtained by using a previous reconstruction method based on two frontal radiographs. A first comparison criterion was the distances between the reconstructed ribs and the surface topography of the trunk, considered as the reference modality. The correlation between ribs axial rotation and back surface rotation was also evaluated. The proposed method successfully reconstructed the ribs of the 6th-12th thoracic levels. The evaluation results showed that the 3-D configuration of the new rib reconstructions is more consistent with the surface topography and provides more accurate measurements of ribs axial rotation.

There are many ways to generate geometrical models for numerical simulation, and most of them start with a segmentation step to extract the boundaries of the regions of interest. This paper presents an algorithm to generate a patient-specific three-dimensional geometric model, based on a tetrahedral mesh, without an initial extraction of contours from the volumetric data. Using the information directly available in the data, such as gray levels, we built a metric to drive a mesh adaptation process. The metric is used to specify the size and orientation of the tetrahedral elements everywhere in the mesh. Our method, which produces anisotropic meshes, gives good results with synthetic and real MRI data. The resulting model quality has been evaluated qualitatively and quantitatively by comparing it with an analytical solution and with a segmentation made by an expert. Results show that our method gives, in 90% of the cases, as good or better meshes as a similar isotropic method, based on the accuracy of the volume reconstruction for a given mesh size. Moreover, a comparison of the Hausdorff distances between adapted meshes of both methods and ground-truth volumes shows that our method decreases reconstruction errors faster. Ziel der vorliegenden Arbeit war es, Lebensmittelqualität am Beispiel von Speisequark aus hofeigener und industrieller Verarbeitung mit natur- und kulturwissenschaftlichen Methoden zu erfassen bzw. zu betrachten. Das Neue dieser interdisziplinären Arbeit lag in der Entwicklung von Verbindungen zwischen hedonischen/sensorischen, ökologischen und kulturellen Qualitäten von Speisequark. Im zweiten Teil der Arbeit wurde eine sensorische Schulung für die hofeigene Milchverarbeitung entwickelt und erprobt, um die Ergebnisse dieser Arbeit in die Praxis umzusetzen. Zuerst musste ein theoretischer Ansatz entwickelt werden, der die Esshandlungen der Verbraucher als Integrationsmoment von objektiven, subjektiven und sozial-kulturellen Qualitäten betrachtet. Bei diesem handlungstheoretischen Ansatz galt es, die Trennung zwischen objektiven und subjektiven Qualitäten zu überwinden, indem über Esshandlungen der Verbraucher integrierte Verbindungen zwischen hedonischen/sensorischen, ökologischen und kulturellen Qualitäten am Beispiel von Speisequark entwickelt wurden. Als empirische Grundlage wurde erstens die sensorische Qualität mit Profilprüfungen erfasst. Zweitens wurde die ökologische Qualität mit einer Verbraucherumfrage und Conjoint-Analyse und drittens die kulturelle Qualität mit einer semiotischen Analyse betrachtet. Die diskursiven Begründungen des Esshandelns, die in ermittelnden Gruppendiskussionen qualitativ erhoben wurden, zeigten deutlich, dass diese drei Qualitäten des Hofquarks untrennbar miteinander verbunden sind. Der komplexe Begriff authentisch, mit dem die Verbraucher Hofquark charakterisierten, beinhaltete vor allem den Geschmack, aber auch die damit verbundene handwerkliche Herstellungsweise, die Verpackung, die regionale Herkunft und die ökologische Anbauweise. Hofquark ist aus Verbrauchersicht ein gutes Lebensmittel, das seine besondere Bedeutung (kulturellen Wert) auch dadurch erhält, was es nicht ist, nämlich ein industrielles Molkereiprodukt. Der neue natur- und kulturwissenschaftliche Ansatz dieser Arbeit stellt gleichzeitig ein Plädoyer für eine mehrdimensionale Qualitätsbetrachtung von Lebensmitteln dar. Aus interdisziplinärer Sicht besteht hierzu noch ein großer Forschungsbedarf.

Introducción: La cirugía laparoscópica ocupa un lugar privilegiado dentro de la cirugía mínimamente invasiva, brindando al paciente y a las instituciones hospitalarias importantes beneficios comparados con la cirugía convencional. Los cirujanos en formación deben contar con un entrenamiento adecuado en cirugía laparoscópica basado en simuladores previo a la práctica con pacientes, disminuyendo la morbimortalidad derivada de la curva de aprendizaje. Este estudio busca describir e identificar los cambios en habilidades y tiempos quirúrgicos antes y después del entrenamiento con simulador de bajo costo y simulador virtual. Metodología: Se realizó un seudoexperimento (antes y después) con 20 residentes de los cuales 18 completaron el estudio, quienes recibieron un entrenamiento dirigido para la realización de procedimientos por vía laparoscópica en simuladores. El análisis estadístico se realiza mediante un análisis uni y bivariado, y se determina la significancia estadística con la medición de X2 y prueba exacta de Fisher así como la prueba T Student para muestras emparejadas y Wilcoxon para las variables numéricas. Resultados: El simulador de bajo costo muestra dependencia en la variable de manejo de tejidos en el ejercicio 3 y 10, con valores de p=0.035, y p=0.028 respectivamente. El 60% de los ejercicios muestra una diferencia estadísticamente significativa en el tiempo empleado en las pruebas. Para simulador virtual, todos los ejercicios mostraron diferencias significativas en al menos una de las variables evaluadas. Conclusiones: El entrenamiento, tanto con el simulador de bajo costo como con el simulador virtual, mejora las habilidades quirúrgicas necesarias para la realización de un procedimiento laparoscópico.
